프로포폴에 대한 의료인의 지식, 태도, 실천 및 일반인의 인식정도 (Knowledge, Attitudes and Practices in Health Care Professionals and Awareness in the Public toward Propofol)

  • 본 연구는 프로포폴에 대한 의료인의 지식, 태도, 실천 정도를 알아보고, 일반인이 인지하고 있는 프로포폴에 대한 인식 정도를 파악하기 위한 서술적 조사 연구이다. 연구대상은 의사 및 간호사 359명과 외래 및 건강검진실에서 내시경 검사를 받기 위해 방문한 일반인 682명을 대상으로 실시하였다. 수집된 자료는 t-test, ANOVA, 상관분석으로 분석하였고 다중비교방법으로는 Scheffe 검정을 이용하였다. 연구결과 프로포폴에 대한 의료인의 지식은 평균 7.16, 태도는 3.26, 실천은 3.95 등으로 나타났다. 프로포폴에 대한 의료인의 태도는 간호사가 의사보다 높았고(p=.001), 실천은 의사가 간호사보다 높게 나타났으며(p=.025) 일반인의 53.4%가 프로포폴 약물이름을 들어본 적이 없다고 응답하였다. 이상의 연구결과를 통해 프로포폴의 올바른 사용, 현실성 있는 철저한 관리 및 교육을 위한 맞춤형 간호중재를 개발할 수 있을 것으로 사료된다.

치과위생사의 치과 내 소음인식도와 청력관련 특성 및 업무와의 관련성 (The relationship between noise awareness, hearing ability, and dental hygiene performance in dental hygienists)

  • Objectives: The purpose of the study is to investigate the relationship between noise awareness, hearing ability, and dental hygiene performance in dental hygienists. Methods: The subjects were 234 dental hygienists in Seoul, Gyeonggi-do, and Incheon. The questionnaire consisted of 3 questions of general characteristics of the subjects, 1 question of noise awareness, 3 questions of hearing ability, 5 questions of dental hygiene performance, 1 question of noise reduction necessity, and 2 questions of noise control. Noise awareness, dental hygiene performance, and noise reduction necessity were measured by Likert 5 scale. Cronbach's alpha was 0.825 in dental hygiene performance reliability. Results: There was a significant difference in noise recognition by age(p<0.01) and working career(p<0.05). Those who pumped up the higher TV volume accounted for 31.6% and 3.0% had hearing impairment in the regular health checkup. 3.4% of the respondents complained of hearing loss, ear fluid, and equilibrium problem diagnosed by the doctors. The relationship between the type of workplace environment and task performance showed a significant difference. Those who work in the dental hospitals had high score of 3.43, while those who work in dental clinics had 3.20(p<0.05). The increased level of noise recognition affected the task performance and it is necessary to reduce the noise level. More try must be focused on the noise exposure prevention. Conclusions: In order to reduce the problems due to noise and improve their quality of life, it is necessary to change the noise recognition in the work place.

근막통증증후군의 한의 표준임상진료지침 개발을 위한 온라인 설문조사 (Clinical Practice Patterns for Myofascial Pain Syndrome in Korean Medicine: An Online Survey)

  • Objectives This study aimed to develop Korean medicine clinical practice guidelines for myofascial pain syndrome that can actually be applied clinically. Methods A text message containing a link to an online survey was sent to 26,987 Korean medicine doctors registered with the association of Korean medicine. The survey period was from December 29, 2023 to January 19, 2024. Results There were 2,762 Korean medicine doctors who responded to an online survey over a three-week period. Awareness of the standard clinical practice guidelines of Korean medicine was 74.5%, and utilization was 35.8%. The most frequently used diagnostic tool was a numerical evaluation tool, the treatment tool was acupuncture, and the prognosis management guidance was lifestyle guidance. Conclusions If standard Korean medicine clinical practice guidelines for myofascial pain syndrome are developed based on the results of this survey, guidelines that are highly usable in clinical settings will be developed.

의료데이터 관리 및 폐기에 대한 실태 연구 (A Study on the Management and Disposal of Medical Data)

  • In the present age of artificial intelligence and metaverse, research on the importance of data and the amount of data is actively being conducted. Among these data, medical data contains the most sensitive information of individuals, so research on data generation, storage, management, and disposal is urgently needed. This study analyzed the status of medical data management in the United States, Europe, and Korea, and identified and analyzed medical data management laws and implementation status through working-level staff working in medical sites. As a result of the analysis, about 70% of medical professionals were able to identify the absence of recognition and management of medical data. The survey subjects were limited to Gwangju and Jeollanam-do, and 237 medical workers were conducted. More than 54% of the awareness of medical record generation, storage, and management came out, but about 70% of the occupations except doctors, oriental doctors, and dentists did not recognize the medical record management method. As necessary for medical record management, cost and the need for professional managers were 91.4%. Through this study, it was confirmed that the expansion of legal education for medical workers, the enactment of related laws, and the need for sincere fostering of medical record managers were required.

Factors that Influence Awareness of Breast Cancer Screening among Arab Women in Qatar: Results from a Cross Sectional Survey

  • Background: Breast cancer is the most common cancer among women in the State of Qatar. Due to low participation in breast cancer screening (BCS) activities, women in Qatar are often diagnosed with breast cancer at advanced stages of the disease. Findings indicate that low participation rates in BCS activities are significantly related to women's low level of awareness of breast cancer screening. The objectives of this study were to: (1) determine the factors that influence Qatari women's awareness of breast cancer and its screening activities: and (2) to find ways to effectively promote breast cancer screening activities among Arabic speaking women in Qatar. Materials and Methods: A multicenter, cross-sectional quantitative survey of 1,063 (87.5% response rate) female Qatari citizens and non-Qatari Arabic-speaking residents, 35 years of age or older, was conducted in Qatar from March 2011 to July 2011. Outcome measures included participant awareness levels of the most recent national recommended guidelines of BCS, participation rates in BCS activities, and factors related to awareness of BCS activities. Results: While most participants (90.7%) were aware of breast cancer, less than half had awareness of BCS practices (28.9% were aware of breast self-examination and 41.8% of clinical breast exams, while 26.4% knew that mammography was recommended by national screening guidelines. Only 7.6% had knowledge of all three BCS activities). Regarding BCS practice, less than one-third practiced BCS appropriately (13.9% of participants performed breast self-examination (BSE) monthly, 31.3% had a clinical breast exam (CBE) once a year or once every two years, and 26.9% of women 40 years of age or older had a mammogram once every year or two years). Awareness of BCS was significantly related to BCS practice, education level, and receipt of information about breast cancer and/or BCS from a variety of sources, particularly doctors and the media. Conclusions: The low levels of participation rates in BCS among Arab women in this study indicate a strong need to increase awareness of the importance of breast cancer screening in Qatari women. Without this awareness, compliance with the most recent breast cancer screening recommendations in Qatar will remain low. An increased effort to implement mass media and public health campaigns regarding the impact of breast cancer on women's health and the benefits of early detection of breast cancer must be coupled with an enhanced participation of health care providers in delivering this message to Qatar population.

1급 응급구조사의 병원 전 응급환자평가와 응급처치시행에 대한 인식과 실천정도 (The Level of Awareness and Practice in Prehospital Emergency Patient Assessment and Emergency Care of Paramedic in Fire Station)

  • Purpose: The aim of this study is to present the basic data for qualitative improvement of emergency care for emergency patient by paramedic in fire station by understanding the level of awareness and practice in prehospital and transfer step, and understanding the level of emergency care and improvement of clinical knowledge through hospital clinical training. Methods: The researchers explained the objective for 143 persons who completed hospital clinical training from June 2nd, 2006 to October 23rd, 2009 among paramedic in fire station. The questionnaire in this research consisted of 80 questions. In the reliability for the awareness of emergency patient assessment, cronbach's $\alpha$ was 0.95, and in the reliability for emergency care fulfillment, cronbach's $\alpha$ was 0.93. reliability for clinical knowledge improvement is cronbach's $\alpha=.95$, and reliability for emergency care fulfillment is cronbach's $\alpha=.82$. Collected data was analyzed through SPSS 18.0 statistics program for frequency, percentage, average, standard deviation, Paired t-test, t-test, Correlation Coefficient, and internal consistency reliability was analyzed by cronbach's $\alpha$. Results: 1) The paramedic awareness and practice difference for emergency patient is statistically signification for general patient assessment(t=14.159, p=.000), trauma patient assessment(t=11.288, p=.000), internal medicine patient assessment(t=10.898, p=.000), and it shows the level of practice is lower than the level of awareness. 2) The paramedic difference between the level of awareness and practice according to whether or not they have clinical career is not signification on awareness(t=3.119, p=.125), and is high on practice(t=3.119, p=.002). 3) The correlation between paramedic awareness and the level of practice shows positive correlation(r=.61, p=.000). The higher the awareness of emergency patient assessment is, the higher the level of practice is. 4) The difference between paramedic clinical knowledge improvement and the level of emergency care practice is statistically significant(t=3.351, p=.001). 5) 89.6%(128 persons) of paramedic replied hospital clinical training experiences are helpful for field activity. 92.3%(133 persons) replied they apply well for clinical knowledge learned during hospital clinical training and emergency care skills in the field. Conclusion: Paramedic in fire station must evaluate the patient's initial assessment and activate the transfer system to the emergency department. It is necessary to develop and implement the effective education program continuously. The education program should systemize currently operated hospital clinical training. emergency disease and symptoms emergency care method, and practice mainly skill education should be progressed. In the prehospital and transfer management, high quality of medical assessment is required to the emergency medical service system. Medical direction from the doctors can feedback the paramedic continuously and continuing education must be provided to the paramedic in fire station.

Level of Awareness of Cervical and Breast Cancer Risk Factors and Safe Practices among College Teachers of Different States in India: Do Awareness Programmes Have an Impact on Adoption of Safe Practices?

  • Background: Breast and cervical cancers are the most common causes of cancer mortality among women in India, but actually they are largely preventable diseases. Although early detection is the only way to reduce morbidity and mortality, there are limited data on breast and cervical cancer knowledge, safe practices and attitudes of teachers in India. The purpose of this study is to assess the level of awareness and impact of awareness programs in adoption of safe practices in prevention and early detection. Materials and Methods: This assessment was part of a pink chain campaign on cancer awareness. During cancer awareness events in 2011 at various women colleges in different parts in India, a pre-test related to cervical cancer and breast cancer was followed by an awareness program. Post-tests using the same questionnaire were conducted at the end of the interactive session, at 6 months and 1 year. Results: A total of 156 out of 182 teachers participated in the study (overall response rate was 85.7 %). Mean age of the study population was 42.4 years (range- 28-59 yrs). There was a significant increase in level of knowledge regarding cervical and breast cancer at 6 months and this was sustained at 1 year. Adoption of breast self examination (BSE) was significantly more frequent in comparison to CBE, mammography and the Pap test. Magazines and newspapers were sources for knowledge regarding screening tests for breast cancer in more than 60% of teachers where as more than 75% were educated by doctors regarding the Pap test. Post awareness at 6 months and 1 year, there was a significant change in alcohol and smoking habits. Major reasons for not doing screening test were found to be ignorance (50%), lethargic attitude (44.8%) and lack of time (34.6%). Conclusions: Level of knowledge of breast cancer risk factors, symptoms and screening methods was high as compared to cervical cancer. There was a significant increase in level of knowledge regarding cervical and breast cancer at 6 months and this was sustained at 1 year. Adoption of BSE was significantly greater in comparison to CBE, mammography and the Pap test. To inculcate safe practices in lifestyle of people, awareness programmes such as pink chain campaign should be conducted more widely and frequently.

주상병 일치도에 관한 연구 -1개 중소병원을 중심으로- (A Study on the agreement of Principal Diagnosis)

  • Background : The principal diagnosis has been used in many different fields such as hospital statistics, medical research, insurance claim, national health statistics and so on. Some principal diagnoses have a relatively low level of reliability in the medium-sized hospitals. The purpose of this study is to identify the reliability level of principal diagnoses and to suggest ways to improve reliability of the principal diagnosis. Method : Data were collected from a medium-sized hospital located in Pusan. The discharge summaries on 323 patients who were discharged in January, 2008 and the outpatient summaries on 251 patients who visited the hospital on March 28, 2008 were collected, and descriptive analysis was performed using SPSS version 12.0K. Result : The findings are the followings: (1) the diagnostic consistency rate between medical records and doctors' was 92.0%; (2) the diagnostic consistency rate between medical records and insurance claims was 86.1%; (3) the diagnostic consistency rate between doctors' diagnoses and insurance claims was 80.2%. The evidence seems to indicate that some principal diagnoses have reliability problems in the medium-sized hospitals. Conclusion : The results of this study suggest the followings: (1) employees should be trained and supervision of hospital activities are needed; (2) network systems should be constructed for each department; (3) professions need to be fostered (4) doctors' awareness of medical records should be changed.

Evaluation of Managerial Needs for Palliative Care Centers: Perspectives of Medical Directors

  • Background: Palliative therapies have an important role in increasing the quality of healthcare and in dealing with physical and psychosocial problems due to cancer. We here aimed to evaluate the managerial perspectives and opinions of the hospital managers and clinical directors about specialized palliative care centers. Materials and Methods: This study was conducted in two large-scale hospitals in which oncology care is given with medical directors (n:70). A questionnaire developed by the researchers asking about demographic characteristics and professional experience, opinions and suggestions of medical directors about providing and integrating palliative care into healthcare was used and responses were analyzed. Results: Potential barriers in providing palliative care (PC) and integrating PC into health systems were perceived as institutional by most of the doctors (97%) and nurses (96%). Social barriers were reported by 54% of doctors and 82% of nurses. Barriers due to interest and knowledge of health professionals about PC were reported by 76% of doctors and 75% of nurses. Among encouragement ideas to provide PC were dealing with staff educational needs (72%), improved working conditions (77%) and establishing a special PC unit (49)%. An independent PC unit was suggested by 27.7% of participants and there was no difference between the hospitals. To overcome the barriers for integration of PC into health systems, providing education for health professionals and patient relatives, raising awareness in society, financial arrangements and providing infrastructure were suggested. The necessity for planning and programming were emphasized. Conclusions: In our study, the opinions and perspectives of hospital managers and clinical directors were similar to current approaches. Managerial needs for treating cancer in efficient cancer centers, increasing the capacity of health professionals to provide care in every stage of cancer, effective education planning and patient care management were emphasized.

한의사 설문을 통한 미병관리의 제도적 활성화 방향 조사 (A survey for Korean Medicine doctors on the activation of Mibyeong management)

  • Objectives : This study aims to investigate the recognition on Mibyeong among Korean Medicine doctors (KMDs) and to suggest institutional approaches for the active use of Mibyeong. Methods : On-line survey for KMDs who were registered in the Association of Korean Medicine was performed. The survey questionnaires included definitions and categories of Mibyeong, clinical use of Mibyeong, and the need for developing Mibyeong care packages, etc. Results : Of 18,344 KMDs who were received survey e-mail, 884 KMDs replied. As for the awareness of the Mibyeong, over 80% of the KMDs agreed the definitions based on patients' subjective symptoms and Korean Medicine theories. 36.1% of respondents used the term Mibyeong in the medical record, and 38.6% used when they explained the patients' condition to the patients. A majority of respondents agreed with the need for a developing Mibyeong care package. Developing the standard methods for Mibyeong assessment was the primary requisite on Mibyeong research. Conclusions : Currently the level of recognition and use of Mibyeong among Korean Medicine doctors was relatively low. To promote the use of Mibyeong in the clinical field, the standard methods for Mibyeong assessment and Mibyeong care package need to be developed.
