Factors related to Disturbing Behaviors, Premorbid Personality and Depression in the Pre-demented Elderly and the Mild Demented Elderly (치매 노인의 문제행동에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify premorbid personality, depression and disturbing behaviors to provide information for developing nursing interventions for the pre-demented and mild demented elderly living as residents of their community. Method: The survey was conducted through direct interviews using a structured questionnaire in J city. Among the subjects contacted, 89 made appropriate replies to the survey. Result: The subjects' disturbing behaviors were significantly related to premorbid personality and depression. The predictors to disturbing behaviors of the pre-demented elderly were sex, extroversion, neuroticism and conscientiousness. The predictors to disturbing behaviors of the mild demented elderly were agreeableness, depression, sex, conscientiousness, and extroversion. Conclusion: In conclusion, disturbing behaviors of pre-demented elderly and mild demented elderly are affected not only by their premorbid personality but also by their depression. Therefore, it is necessary to encourage and respond to them with understanding their disturbing, behaviors in 'relation to their premorbid personality. In addition. it is important to maintain positive emotion in order to reduce their disturbing behaviors.

A Study on Disturbing Behaviors and Environment Characteristics in Elderly People with Dementia (시설치매노인의 문제행동과 환경적 특성에 관한 연구)

  • Purpose: This study was conducted to investigate correlations between environmental characteristics and disturbing behaviors in elderly people with dementia living in institutions. Method: A random sample of 107 subjects in three institutions for people with dementia in the Gyeongnam Province was tested between May 29 and June 5, 2002. Data was collected through a questionnaire and analyzed using SPSS/PC Win 10.0 Result: The major findings of the study are as follows. 1) the mean score on physical environment was $3.55{\pm}0.47$, and on psychosocial environment, $3.55{\pm}3.08$. 2) the mean score on disturbing behaviors showed that nonaggressive psychomotor behavior was the highest, and aggressive psychomotor behavior was the lowest. 3) the disturbing behaviors. in 5 the domains correlated with the environment and influencing factors included environment, family, level of dementia and level of ADL. Conclusion: Both the present physical environment and psychosocial environment influence the type of disturbing behavior in elderly people with dementia. Therefore, nursing can reduce the disturbing behaviors through maintenance of both the psychosocial environment and the physical environment. Furthermore, nurses must consider disturbing behaviors of elderly people with dementia by being aware of the level of dementia and ADL.

Study of the Positive and Negative Caregiving Experiences in the family members who care for the psychiatric mentally ill relatives (정신장애인 가족의 긍정적, 부정적 돌봄의 경험에 관한 연구)

  • The caregiving experiences of 100 family menbers of outpatients with schizophrenia and schizoaffective disorders were investigated for the presence of positive(positive family-patient relationship. patient' contribution to the family) and negative caregiving experience(objective and subjective burden) and their predictors. This study attempts to make the analysis of caregiving experience more useful by expanding the focus to incoporate these positive aspects of the experience of family caregiver. Objective burden consists of two elements: 'disruption of family life', 'care'(amount of caregiving related to activity of daily living). Subjective burden is defined as emotional reactions to the care giving and it comprised of 6 emotional subdimensions such as 'stigma', 'grief'. 'worry', 'pity', 'fear', 'despair'. Also we investigate the severity of patients' disturbing behaviors into two categories, positive and negative disturbing behaviors and patient' contribution to the family as a predictors of positive and negative caregiving experiences. This study use Pearson's correlation coefficient, Hierardhical regressions in the SAS Program. The results are as follows: 1. Respondents reported moderate level of objective burden 'disruption of family life' (mean = 2.48, range = 1-4), and 'care' (mean=2.54, range = 1-4), and slightly high level of total subjective burden(mean = 2.19, range = 1-4). Mean scores for the measure of the severity of behavioral disturbance indicated that the caregiver experienced negative disturbing behaviors around almost 'somtimes'(mean=2.28, range = 1-4), and positive disturbing behaviors 'almost not frequent'(mean=2.78. range=1-4). So they reported that they perceived patient's negative disturbing behaviors more than positive disturbing behaviors. Mean scores for the measure of the patient' contributions (mean = 1.99. range = 1-4) indicated that caregivers experienced these contributions a little. It means that there should be a positive aspect of possibilities of patient' family roles that can be developed in the daily life. Mean scores for the measure of the positive family-patient relationship indicated that caregivers experienced moderate level of positive family-patient relationship(mean=2.52, range = 1-4). 2. Hierardhical regression analysis 1) Hierardhical regression of 'disruption of family life' showed that the interaction between positive disturbing behaviors and patient' contributions (B = .20. p = .022) and caregiver's educational level(B=.06. p=.000) were 'significant and Hierardhical regression of 'care' showed that 'negative disturbing behaviors'(B= .35. p= .007). 'patient' contributions'(B= .28, p= .019). 'family income'(B=-.l1. p=.096) were significant. 2) Hierardhical regression of 'total subjective burden', 'stigma', 'grief', 'worry', 'pity'. 'fear', 'dispair' showed that 'positive disturbing behaviors'(B=.51. p=.000). 'negative disturbing behaviors' (B = .17, p = .026), 'caregiver's educational level'(B = .03. p=.036), 'family income'(B=.08. p=.041) were significant predictors of 'total subjective burden': 'positive disturbing behaviors'(B=.32. p=.066). 'negative disturbing behaviors'(B=.24, p=.096) 'durations of illness'(B= .03. p= .079) were significant predictors of 'stigma' 'negative disturbing behaviors'(B=.28. p=.005). 'patient sex'(B=-.32. p=.022). 'positive disturbing behaviors'(B=.28. p=.020), 'patient age'(B=.02. p=.010), 'caregiver age'(B=-01, p= .002) were significant predictors of 'grief' 'negative disturbing behaviors'(B= .28, p= .005). A Study on Disturbing Behaviors of Demented Elderly Staying at Home (재가 치매노인의 문제행동에 관한 연구)

  • This study has a purpose to provide information to help develop nursing intervention for demented elderly staying at home. For this purpose I analysed the relationship of patients' disturbing behaviors with their demographic and social characteristics, premorbid personality, and present environmental characteristics through questionnaire survey on their family members. The survey was performed through direct interview, telephone contact. and mail in the regions of Pusan and Gyeongnam. Among family members contacted. 112 ones made an appropriate response to the survey. The statistical package SAS was utilized for descriptive statistics, correlation coefficients, stepwise multiple regression, and cluster analysis. The results of the study were as follows: 1) Demographic and social characteristics of the patients surveyed are female 81.3%, average age 81.4 years, lack of schooling 83.0%, spouse dead 73.2%, having no other disease 58.9%, average duration of dementia 3.8 years, no medical treatment for dementia 84.8%, good married life 40.2%, and primary care given by daughter-in-law 49.1%. 2) Aggressive Psychomotor Behavior(APB) was observed in a way statistically meaningful in case that primary care was given by daughter-in-law, while Nonaggressive Psychomotor Behavior(NPB) was in case of good married life and primary care given by other than daughter-in-law and spouse. Verbally aggressive behavior (VAB) was observed in groups of female, spouse dead, bad married life, and daughter-in-law's primary care. As for Passive Behavior(PB), it was observed in case that patients had educational background of not less high than middle school and that they were having medical treatment. Functionally Impaired Behavior(FIB) was observed in age group of 60-69 and more than 90, in patients' group having no other disease, and in case that the duration of dementia was not less than 5 years. 3) Premobid Neuroticism(N) showed positive correlation with APB and VAB, while Openness (O) did negative correlation with PB. Agreeableness (A) was proved to have positive correlation with PA and FIB, but to have negative correlation with APB and VAB. In addition, Conscientiousness(C) showed negative correlation with APB and VAB. 4) The worse the psychosocial environment was, the more NPB and VAB were observed. 5) APB was explained 24% by C and primary care-giver, while NPB was explained 28% by psychosocial environment, having other disease or not, and married life. VAB was explained 40% by A. sex, and married life. On the other hand PB was explained 33% by O, A. N, and having medical treatment or not. But any significant factor was not found to explain FIB. 6) A cluster analysis was performed on disturbing behaviors of demented elderly staying at home. It enabled to regroup the demented elderly in 5 patterns: high scored in NPB, high scored in FIB. high scored in NPB and VAB, moderately scored in most disturbing behaviors, and low scored in all areas. In conclusion, disturbing behaviors of demented elderly not only reflect their premorbid personality in the past, but also are affected by their present psychosocial environment. Therefore, it is necessary to encourage and respond them with understanding their disturbing behaviors in relation to their past premorbid personality. In addition, it is important to provide them better psychosocial environment in order to reduce their disturbing behaviors.

Behavioral Symptoms in Nursing Home Residents with Dementia: Developing a Nursing Practice Model

  • Purpose: Behavioral symptoms in dementia (BSD) are one of the most disturbing behaviors to caregivers and a major reason for nursing home placement. Behavioral symptoms are often treated with psychotropic drugs (PD), however, the effect of such drugs for the frail elderly dementia patient is not certain because of their critical adverse effects. Theoretical model applicable to nursing practice for BSD in nursing homes, which is essential in guiding and evaluating such interventions, is absent. This article presents the process of developing a theoretical model of BSD in nursing homes. Method: Using Walker and Avants' theory synthesis method, three behavior models and two system models were incorporated into the proposed model to provide the theoretical and analytical explanation of the relationships between PD usage, its determinants, and BSD. Results: Resident variables and nursing home variables related to the two focal concepts (i.e., PD usage and BSD) were identified. Resident variables include demographical characteristics such as age and gender, and dementia-compromised functions such as cognitive and functional impairment. Nursing home variables include facility characteristics such as ownership type and size, and physical and psychosocial environment. Conclusion: The proposed model suggests that fulfillment of resident unmet needs through improvement of physical and psychosocial environment may produce better health outcomes of nursing home residents with BSD. Assessment and intervening environmental triggers of such behaviors are also suggested to be prior to the PD usage.

Comparison of Sleep Pattern, Factors of Sleep Disturbance and Sleep Enhancement Behaviors between Sleep Disturbance and Non-Sleep Disturbance Aged Patients (수면장애 노인 환자와 일반 노인 환자의 수면양상, 수면장애요인 및 수면증진행위)

  • Purpose: This study was done to compare sleep patterns, factors of sleep disturbance and sleep enhancement behaviors between sleep disturbance and non-sleep disturbance elderly patients. Method: The participants were 117 patients over 65 years old who were hospitalized at 5 general hospitals in Seoul. There were 83 patients in the good sleeper group and 34 in the poor sleeper group. The survey questionnaires included questions on general characteristics, sleep patterns, sleep disturbing factors, and sleep enhancement behaviors of the patients. Results: For sleep patterns and environmental factors of sleep disturbance, the poor sleepers was significantly higher than that of the good sleepers. Significant differences between good sleepers and poor sleepers were also found on the following variables: Environmental factors of sleep disturbance -'Sound of other patient's or care-giver's', 'Discomfort of bed, linen, pillow and patient's gown', 'Light in the room', 'Emergency situations', 'Temperature of patients rooms too hot or too cold'. The significant physical factor of sleep disturbance was 'Hard to breath even without moving' and for sleep enhancement behaviors: 'Use of ear plugs or eye covers' Conclusion: Consequently it is necessary to develop positive and differentiated programs for sleep enhancement for the inactive and poor sleeper among the elderly patients.

A Systemic Review of Positive Behavior Support Intervention Studies on Problem Behaviors of Children With Autism Spectrum Disorders: Focusing on Korean Single-Subject Research Design (자폐스펙트럼장애 아동의 문제행동에 대한 긍정적 행동지원 중재 연구의 체계적 고찰: 단일대상연구 설계를 중심으로)

  • Objective : This study examined the characteristics of the literature involving a single-subject research design among positive behavior support intervention studies to improve problem behavior in children with autism spectrum disorders and assess the quality level. Methods : This is a literature study, and the targets of analysis were nine single-subject research papers published between 2011 and 2020. The subject papers were analyzed by dividing them into general characteristics and the qualitative levels according to the content of the research method. Results : Analysis of analyzing the contents of the study showed that the subjects were preschool and elementary school-age children at the same ratio. Furthermore, the study design involved mostly the middle and multiple baseline designs among the behaviors. All papers presented social validity, intervention fidelity, and observer reliability. Problem behaviors included self-injury and aggression behaviors, disturbing behaviors, and seat break-away behaviors, while the most dependent variables were measured through partial interval recording. As a result of confirming the intervention effect, the effect was confirmed in intervention, maintenance, and generalization. All analysis studies showed high-quality levels. Conclusion : This study confirmed the content and qualitative level of the thesis that applied the single-subject research design among positive behavior support intervention studies for problem behaviors of children with autism spectrum disorders. Positive behavior support intervention, an evidence-based intervention for children with autism spectrum disorders, was confirmed an effective intervention for autism spectrum disorders.

Assessment of Nutritional Status and Food Sources of Significant Nutrients with Picky Eating Behavior in Preschoolers (까다로운 식행동을 가진 유아의 영양섭취 평가와 급원 식품)

  • Picky eating behavior is a significant factor in causing unhealthy eating and disturbing the growth of children. This study examined picky eating behaviors and food intake of 112 preschoolers aged 3~5 year, picky eaters (n=41) and non-picky eaters (n=71), living in Dobong-gu, Seoul metropolitan area, South Korea. Picky eating questionnaires and three consecutive 24-hour dietary recalls were collected from their caregivers. The difference between the two groups was verified using a Chi-square test or t-test and Pearson's correlation. Compared to the non-picky eaters, fussiness, satiety responsiveness, and refusal of food variety were significantly higher in picky eaters (P<0.001). Compared to the non-picky eaters, an assessnent of the preschooler's behavior and the caregivers' perception were positively correlated in picky eaters (r=0.749, P<0.001). The intakes of shellfish (P<0.05), vegetables (P<0.001), fiber, vitamin E (P<0.01), vitamin A, and folate (P<0.05) were significantly lower in picky eaters than the non-picky eaters. The major food sources of vitamin A and folate were vegetables, of which grains were the source of fiber, fats and oils were the source of Vitamin E, and root vegetables were source of vegetables in both picky and non-picky eaters. In conclusion, picky eating behaviors are related to different fussiness and slowness in eating. Therefore, it is suggested to increase the amount of meal and vegetable intake in picky eaters.

As the results, quantitative differences of drivers eye movement, subjective mental workload, steering wheel angle entropy, and cognitive Performance of secondary tasks were compared between straight and curve (right turn) section for each driving condition.

Experimental and Numerical Analysis for Effects of Two Inclined Baffles on Heat Transfer Augmentation in a Rectangular Duct (사각 덕트 내에 설치된 2개의 경사진 배플에 의한 열전달 증진 효과에 관한 실험 수치해석)

  • Baffles enhance heat transfer by disturbing boundary layer and bulk flow, creating impingement, and increasing heat transfer surface area. This study was performed to determine how the two inclined baffles (${\alpha}=5^{\circ}$ perforated models) placed at a rectangular channel affect heat transfer and associated friction characteristics. The parametric effects of perforated baffles (3, 6 and 12 holes) and flow Reynolds number ranging from 28,900 to 61,800 on the heated target surface are explored. Comparisons of the experimental data with the numerical results by commercial code CFX 10.0 are presented. As for the investigation of heat transfer behaviors on local Nusselt number with two baffles placed at $x/D_h=0.8$ and $x/D_h=8.0$ of the edge of baffles, it is evident that the inclined perforated baffles augment overall heat transfer significantly by both jet impingement and boundary layer separation. There exists an optimum perforation density to maximize heat transfer coefficients; i.e., the average Nusselt number increases with increasing number of holes, but the friction factor decreases with an increase in the hole number placed at baffles.
