원위부 위암에서 복강경 D2 림프절 절제술의 수술 성적 (Short-term Surgical Outcomes after Laparoscopic D2 Lymphadenectomy in Patients with Distal Gastric Cancer)

  • 목적: 복강경 기구의 발달과 시술자의 숙련도가 증가함에 따라 점차로 조기위암 이상으로 복강경 위절제술을 확대 적용하려는 노력들이 이루어지고 있다. 본 연구는 복강경 D2 림프절 절제술의 적절성 및 유용성을 평가하고자 하였다. 대상 및 방법: 2007년 2월부터 2007년 8월까지 복강경 D2 림프절 절제술을 시행 받은 22명의 원위부위암 환자들을 대상으로 임상병리학적 특성, 수술 성적 및 수술 후 경과 등에 대하여 같은 시기에 시행된 38명의 개복 D2 림프절 절제술 환자와 비교하였다. 결과: 복강경 D2 림프절 절제술은 평균 수술시간이 오래 소요되었으나($160{\pm}25min$. vs. $135{\pm}21min$., P<0.001), 평균 근위부 절제연 거리($6.1{\pm}2.8cm$ vs. $5.8{\pm}2.5$, P=ns), 평균 림프절 절재 개수($25{\pm}11$ vs. $26{\pm}9$, P=ns)에서 개복 수술과 유의한 차이가 없었다. 수술 후 경과는 복강경 수술군에서 식이개시, 재원일수 등이 유의하게 빨랐다. 복강경 D2 림프절 절제술을 시행 받은 환자 중 개복으로의 전환은 없었으며, 양군에서 유사한 수술 후 합병증 발생빈도를 보여주었다. 결론: 복강경 D2 림프절 절제술은 개복수술과 유사한 수술성적을 가지며, 양호한 수술 후 경과를 보인다. 향후 전향적 연구를 통해 복강경 D2 림프절 절제술의 유용성에 대한 연구가 필요하리라 생각된다.

원위부 진행성 위암에서의 상장간막정맥(14v) 림프절 절제술의 적응증 (Indication of Dissection of the 14v Lymph Node in Advanced Distal Gastric Cancer)

  • 목적: 1998년도에 제정된 일본위암학회의 위암취급규약 영문판 제2판에 따르면 원위부 위암에 있어 2군에 속하는 림프절 중 14v (상장간막정맥) 림프절도 포함되어 있으나 조기 위암에서는 14v 림프절의 절제가 제외되는 등 상황과 술자에 따라 선택적으로 절제되고 있다. 저자들은 원위부 진행성 위암에 있어 14v 림프절의 전이 정도를 조사하여 14v 림프절 절제의 필요성에 대해 알아보고 이 림프절의 절제 적응증을 알아보고자 하였다. 대상 및 방법: 2004년 4월부터 2005년 8월까지 서울아산병원 외과 위암팀에서 원위부 위암으로 수술 받은 환자 중 수술장에서 육안적 소견으로 진행성 위암으로 판단된 환자를 대상으로 일본위암학회의 위암취급규약 영문판 2판에 정의된 D2 림프절 곽청술을 시행하고 림프절의 구획을 표기하였다. 각 림프절 구획의 위암 전이 여부를 확인하여 전이율을 계산하고, 6번 림프절의 양성율과 14v 림프절 양성율의 상관 관계와 14v 림프절 양성군과 음성군을 비교하여 14v 림프절 전이를 예측할 수 있는 인자가 있는지 분석하였다 후향적으로 의무기록을 검토하였고 환자의 예후에 대한 장기 추적은 하지 않았다. 결과: 전체 환자는 50명이었고 평균 연령은 56 ($30{\sim}80$)세, 남녀비는 1.63 : 1이었다. 47명(94%)에서 위원위부 절제술과 위십이지장 문합술을 시행하였고 3명(6%)에서는 위원 위부절제술과 위공장문합술을 시행하였다. D2 림프절 곽청술을 시행 후 림프절의 전이율이 가장 높았던 림프절은 3번(위소만부) 림프절과 6번(유문하림프절)이 54%)로 가장높았고 14v 림프절의 전이율은 10%로 다른 2군 림프절 중 7번 9번 림프절의 전이와 비슷하였다. 14v 림프절 양성군과 음성군을 비교하였을 때 전이된 림프절의 수와(평균 25.4개 vs 4.91개, P<0.001) 6번 림프절의 전이된 림프절 수에서(평균 6.5개 vs 1.42개, P<0.001) 차이를 보였고, 14v 양성군은 UICC 병기 3기 이상으로 음성군에 비해 진행된 위암인 경우가 많았다. 결론: 위암의 상태가 진행되어 있고 6번 림프절이 양성일수록 14v 림프절로의 전이 가능성이 높았다. 따라서 수술장 소견에서 3기 이상, 6번 림프절이 양성일 경우 14v 림프절의 절제가 필수이다. 그러나 14v 림프절 절제의 유용성에 대한 판단은 14v 림프절 절제하지 않은 군과 전향적인 연구를 시행하여 재발과 생존율 분석을 통해 이루어져야 할 것이다.

Profile of HER2 +ve Gastric Cancers in Brunei Darussalam

  • Background: Gastric cancer is the second most common gastrointestinal cancer and is still associated with significant morbidity and mortality due to late presentation and diagnosis at advanced stages. Studies have reported that a variable proportion of gastric cancer is positive for the human epidermal growth factor receptor 2 (HER2) and patients with HER2 positive (HER2 +ve) lesions can benefit from targeted therapy. This study was conducted to assess the prevalence of HER2 +ve gastric cancers in Brunei Darussalam, a developing Southeast Asian nation. Materials and Methods: Patients were identified from the Department of Pathology registry and retrospectively reviewed. HER2 expression was assessed by immunohistochemistry and only those staining 3+were considered positive. Results: Our study included 103 cases (66 males and 37 females) with a mean age of $65.1{\pm}14.8$ years old. There were 14 cases positive for HER2 (10 males and 4 females) giving a prevalence of 13.6%. The HER2 +ve cases were significantly older ($70.6{\pm}19.3$ years old) than the negative cases ($64.2{\pm}13.8$, p=0.041) and had significantly more advanced disease (stages 3 and 4, p=0.026). There were no significant differences in gender distribution, presence of intestinal metaplasia, EBV status, Helicobacter pylori status, tumor location (proximal vs. distal) and degree of tumor differentiation (all p values >0.05). Conclusions: Our study showed that 13.6% of our gastric cancers are positive for HER2, the affected patients being older and having more advanced disease at diagnosis.

Postprandial Asymptomatic Glycemic Fluctuations after Gastrectomy for Gastric Cancer Using Continuous Glucose Monitoring Device

  • Purpose: Although dumping symptoms are thought to involve postprandial glycemic changes, postprandial glycemic variability without dumping symptoms remains poorly understood due to the lack of a method that allows the easy and continuous measurement of blood glucose levels. Materials and Methods: Patients having undergone distal gastrectomy with Billroth-I (DG-BI) or Roux-en-Y reconstruction (DG-RY), total gastrectomy with RY (TG-RY) and pylorus preserving gastrectomy (PPG) for gastric cancer 3 months to 3 years prior, diagnosed as pathological stage I or II, were prospectively enrolled from March 2018 to January 2020. The interstitial tissue glycemic levels were measured every 15 min, up to 14 days by continuous glucose monitoring. Moreover, using a diary recording the diet and symptoms, asymptomatic glucose profiles without sugar supplementation within 3 h postprandially were compared among the four procedures. Results: A total of 40 patients were enrolled, 10 patients for each of the four procedures. There were 47 glucose profiles with DG-BI, 46 profiles with DG-RY, 38 profiles with TG-RY, and 46 profiles with PPG. PPG showed the slowest increase with a subsequent gradual decrease in glucose fluctuations, without hyperglycemia or hypoglycemia, among the four procedures. In contrast, TG-RY and DG-RY showed spike-like glycemic variability, sharp rises during meals, and rapid drops. The glucose profiles of DG-BI were milder than those of RY. Conclusions: The asymptomatic glycemic changes after meals differ among the types of surgical procedures for gastric cancer. Given the mild glycemic fluctuations in PPG and the glucose spikes in TG-RY and DG-RY, pylorus preservation and physiological reconstruction without changes in food pathways may optimize postprandial glucose profiles after gastrectomy.

전국 복강경 위 수술 현황 설문조사 (Nationwide Survey of Laparoscopic Gastric Surgery in Korea)

  • The Information Committee of the Korean Gastric Cancer Association sent questionnaires to 31 laparoscopic gastric surgeons about their personal experiences with laparoscopic gastric surgery from 2001 to 2003. Twenty-four surgeons responded to the questionnaires (response rate: $77.4\%$).The number of laparoscopic gastric surgeries from 2001 to 2003 was 1,130 and increased from 209 in 2001 to 593 in 2003. The number of operations for a gastric adenocarcinoma also increased from 87 cases in 2001 to 403 cases in 2003. Laparoscopic radical procedures, such as a laparoscopyassisted distal gastrectomy or total gastrectomy (LADG or LATG), have increased rapidly during this period. (55 cases in 2001, 150 cases in 2002, and 364 cases in 2003). Laparoscopic function-preserving gastrectomies were not performed until 2003, during which year one pylorus- preserving gastrectomy and six proximal gastrectomies were performed laparoscopically. A wedge resection for a gastric submucosal tumor was performed in 71 cases in 2001, 82 in 2002, and 103 in 2003. Hand-assisted laparoscopic surgery (HALS) was performed in 39 cases in 2001, 55 in 2002, and 49 in 2003. As for personal indications for a LADG, 14 surgeons performed a LADG only for a T1 lesion, and 5 surgeons extended their indications to T2N0 lesions. In the near future, laparoscopic procedures for gastric cancer will be widely adopted in Korea if the medical-insurance obstacle is overcome and the long-term survival results are verified.

Mixed Exocrine and Endocrine Carcinoma in the Stomach: A Case Report

  • We report a rare case of the coexistence of a gastric small cell neuroendocrine carcinoma with a gastric adenocarcinoma. A 62-year-old man presented with epigastric soreness for 1 month. Esophagogastroduodenoscopy revealed a Borrmann type I tumor at the lesser curvature of the lower body of the stomach. The patient underwent a distal gastrectomy with D2 lymph node dissection and the resected specimen exhibited a $3.5{\times}3.5$ cm sized, fungating lesion. Two separated, not intermingling, lesions with non-adenocarcinoma components encircled by well differentiated adenocarcinoma components were identified microscopically. The non-adenocarcinoma component showed neuroendocrine features, such as a solid and trabecular pattern, and the tumor cells showed a high nuclear grade with minimal cytoplasm, indistinct nucleoli, and positive response for synaptophysin, CD56. The final pathological diagnosis was a gastric mixed exocrine-endocrine carcinoma (MEEC) composed of an adenocarcinoma and small cell neuroendocrine carcinoma of the collision type.

A Case of Sudden Onset Septicemia in Recurred Gastric Cancer Following S1 Plus Docetaxel Treatment

  • Pyogenic liver abscess in patients with malignant disease is a fatal state and is easily diagnosed. We presented a rare case of sudden fatal septicemia following anticancer treatment for recurred gastric cancer due to multiple liver abscesses which could not be diagnosed. A 72-year-old male with recurred gastric cancer received anticancer agents. He had a history of distal gastrectomy with right hepatic lobectomy for hepatic metastasis. He received anticancer treatment in the outpatient's service center periodically, and his performance status was preserved with nothing in particular. After administrating docetaxel, he suddenly developed septicemia and multiple organ failure and died 5 days after strong medical supports. Pathological autopsy revealed that multiple minute abscesses of the liver which could not be detected macroscopically were the causes of fatal septicemia. The etiology, therapies and prognosis of rare entity are being discussed.

Risk Factors of Microscopic Invasion in Early Gastric Cancer

  • Purpose: This study aimed to evaluate the clinical significance of microscopic invasion to determine the adequate resection margin in early gastric cancer (EGC). Materials and Methods: A retrospective review was performed that included patients who underwent gastrectomy for clinical early gastric cancer (cEGC) at Seoul National University Hospital between January 2007 and December 2010. After subtracting the microscopic resection margin from the gross resection margin for each proximal or distal resection margin, microscopic invasion was represented by the larger value. Microscopic invasion and its risk factors were analyzed according to the clinicopathologic characteristics. Results: In total, 861 patients were enrolled in the study. Microscopic invasion of cEGC was $6.0{\pm}12.8mm$, and the proportion of patients with microscopic invasion ${\geq}0mm$ was 78.4%. In the risk group, tumor location, pT stage, and differentiation did not significantly discriminate the presence of microscopic invasion. The microscopic invasion of EGC-IIb was $13.9{\pm}16.8mm$, which was significantly greater than that of EGC-I. No linear correlation was observed between the overall tumor size and microscopic invasion (R=0.030). The independent risk factors for microscopic invasion ${\geq}20mm$ were EGC-IIb vs. EGC-I/IIa/IIc/III (odds ratio [OR], 3.103; 95% confidence interval [CI], 1.533-6.282; P=0.002) and male vs. female sex (OR, 1.655; 95% CI, 1.012-2.705; P=0.045). Conclusions: Male sex and EGC-IIb were independent risk factors for microscopic invasion ${\geq}20mm$. Examination of intraoperative frozen sections is highly recommended to avoid resection margin involvement, especially in cases of EGC-IIb.

Mesenteric Fibromatosis Mimicking Recurrence after Distal Gastrectomy for Gastric Cancer

  • Mesenteric fibromatosis is a monoclonal, fibroblastic proliferation arising from musculoaponeurotic structure, and it is distinctive lesions defined as a group of non-metastasizing fibroblastic tumors which has local invasion and has a high recurrence rate after the surgical excision. The main treatment modality is the surgical excision. Radiation therapy, chemotherapy, and hormone therapy are also known as useful treatments. We report our experience of a recent case of Mesenteric fibromatosis. A 62-year old female patient had undergone gastrectomy due to gastric cancer. 18 months after gastrectomy, we detected an abdominal mass. The preoperative radiologic findings were suggestive of recurrence. Exploratory laparotomy was performed and post-operative pathologic diagnosis was confirmed as fibromatosis. We report a patient with mesenteric fibromatosis that mimic recurrence after gastrectomy for gastric cancer.

Is it Beneficial to Utilize an Articulating Instrument in Single-Port Laparoscopic Gastrectomy?

  • Purpose: As the number of gastric cancer survivors is increasing and their quality of life after surgery is being emphasized, single-port surgery is emerging as an alternative to conventional gastrectomy. A novel multi-degree-of-freedom (DOF) articulating device, the ArtiSential® device (LivsMed, Seongnam, Korea), was designed to allow more intuitive and meticulous control for surgeons facing ergonomic difficulties with conventional tools. In this study, we evaluated the feasibility of this new device during single-port laparoscopic distal gastrectomy (SP-LDG) for early gastric cancer (EGC) patients. Materials and Methods: Consecutive patients diagnosed with EGC who underwent SP-LDG with ArtiSential® (LivsMed) graspers between April 2018 and August 2020 were enrolled in the study. The clinical outcomes were compared with those of a control group, in which prebent graspers (Olympus Medical Systems Corp) were used for the same procedures. Results: Seventeen patients were enrolled in the ArtiSential® group. There was no significant difference in operative time (205.4±6.0 vs. 218.1±9.9 minutes, P= 0.270) or the quality of surgery, in terms of the number of retrieved lymph nodes (49.5±3.5 vs. 45.9±4.0, P=0.473), length of hospital stay (15.4±2.0 vs. 12.4±1.3 days, P=0.588), and postoperative complications (40.0% vs. 41.2%, P=0.595), between the ArtiSential® group and the control group. Conclusions: The new multi-DOF articulating grasper is feasible and can be used as an alternative for prebent graspers during SP-LDG.
