한국재래산양의 후지골격에 관한 해부학적 연구 (Anatomical Studies on the Skeleton of Pelvic Limb of Korean Native Goat)

  • The anatomical structure of pelvic limb, of thirty-one adult Korean native goats (Body weight: 14~17kg) was observed after skeletal preparation, and the osteometry was performed in each bone. The results were as follows: 1. The pelvic limb of the Korean native goat was composed of the hip bone, femur, patella, tibia, fibula, tarsal bones, metatarsal bone, phalanges and sesamoid bones. 2. The hip bone consisted of the ilium, ischium and pubis which fused each other, The gluteal surface of the ilium was directed dorsolaterally. The tuber sacrale and tuber coxae were formed at the dorsal apex and ventrolateral part of the iliac wing, respectively. The lesser ischiatic notch was deeper than the greater one. The ischiatic tubercles were triangular form and consisted of the dorsal, lateral and caudal ischiatic tubercles. The left and right hip bone indexes were 67.08, 66.20, the acetabular indexes were 93.78 and 92.10 and the obturator foramen. indexes were 53.84 and 54.77, respectively. 3. In femur, both of the greater and lesser trochanter were well developed but the third trochanter was not observed. The left and right femur indexes were 26.55 and 26.14, head indexes were 81.66 and 81.49 and the trochlear-epicondyle indexes were 42.47 and 41.63, respectively. 4. The patella was observed as an isosceles triangle with base lying proximal and the cranial surface was more convex. 5. The tibial shaft was sigmoid form and the popliteal notch was deep. There was a large nutrient foramen at the cranial aspect of the cranial intercondylar area. The tibial indexes were 22.09 in left and 21.10 in right. 6. The proximal extremity of the fibula was fused with the lateral condyle of the tibia but the distal one was observed independently as the malleolar bone. 7. The tarsal bones were five in number; the talus, calcaneus, centroquartal tarsal bone, first tarsal bone, and second-third tarsal bone. 8. The metatarsal bone was composed of a large metatarsal bone. resulted from the fusion of the third and fourth metatarsal bones, The structure of metatarsal bone was similar to the metacarpal bone but longer about 7mm. 9. The phalanges and sesamoid bones were similar to these of the thoratic limb. 10. The ratios of the lengths among the hip bone, femur, tibia and metafarsal bone were 1.71 : 1.54 : 1.73 : 1.00 in left and 1.68 : 1.53 : 1.72 : 1.00 in right, respectively.

제4 족지 근위 지골에 발생한 골연골종(1예보고) (Osteochondroma of the Proximal Phalanx of the Fourth Toe (A Case Report))

  • Osteochondroma is the most common benign bone tumor which commonly occurs in the metaphysis of the long bones such as proximal humerus, tibia, and distal femur. It is rarely found in bones of the foot. Although they are benign lesions, when they occur in the foot, they are typically identified earlier than other regions because of the low proliferation of subcutaneous tissue in the region and may cause symptoms. We experienced a rare case of osteochondroma in a 60-year-old male which cause pain and swelling of the fourth toe.

전방십자인대 손상의 진단 (Diagnosis of Anterior Cruciate Ligament Injury)

  • Recently, the incidence of anterior cruciate ligament injury is rapidly increasing because many young Korean are actively participating in sports activities including basketball, skiing and soccer. Sometimes acute ACL injuries arc misdiagnosed as sprain because of pain and muscle spasm. An accurate physical examination and careful history taking are very important to diagnose the ACL injuries. Therefore author described the skill of history taking and the technique of physical examination to diagnose ACL injuries. The most sensitive test is the Lachman's test, which is performed with the knee in 200 of flexion, the femur stabilized with full flexed examiner's knee placed behind patient's distal thigh and one hand of examiner, and the proximal tibia grasped with the other. The pivot shift test is a passive motion test that attempts to subluxate the tibia anteriorly, then have it reduce rapidly. The pivot shift test is very sensitive test to diagnose the chronic ACL injuries.

골스캔에서 냉소로 보인 급성골수염 (Acute Osteomyelitis Shown as a Cold Lesion on Bone Scan)

  • Acute osteomyelitis is usually shown as a localized area of increased activity (hot uptake) in the infectious focus on bone scintigraphy. In our patient, absence of radioactivity (cold lesion) was noted in the distal metaphyseal and diaphyseal regions of his right femur. Initial x-ray was interpreted as normal except for mild soft tissue swelling in the right thigh. The lesion was confirmed as an acute osteomyelitis with subperiosteal abscess on surgery. Staphylococcus aureus was the etiologic organism. We describe a case of acute osteomyelitis in a one-year-old boy shown as a cold lesion on bone scan.

슬관절부위의 골종양에서 시행한 하지 분절 절제 및 회전 재접합술 (Segmental Resection and Rotationplasty for Bone Tumors about the Knee)

  • A segmental resection and rotationplasty was performed in 23 patients with malignant and aggressive boning tumor of the distal femur or proximal tibia between February 1988 and September 1992 at the Severance Hospital. There were 13 male and 10 female patients. The yongest was 5 years old and the oldest was 37 years old. Mean age at operation was 22 years. Of the 23 cases 14 were osteosarcoma, 7 were giant cell tumor and 2 were synovial sarcoma. After an average observation period of 32.5months, there was no evidence of local recurrence but 6 distant metastasis developed. According to Shriner's rating scale, the functional result were excellent or good in all cases. All patient were convinced that they chosen the appropriate operative procedure and the ability to achieve high level of activity was considered as the main benefit.

소아 슬관절 상부 절단단 연장 성형술 - 증례 보고 2례 - (Lengthening of the Above-knee Amputation Stump - Reports of 2 cases -)

  • Limb-salvage surgery has become more popular than amputation for the treatment of malignant bone tumor because no differences in local recurrence and the 5-year survival rate have been found. However for young patients with sarcoma, skeletal immaturity may be a contraindication to limb-salvage surgery due to the expected leg length discrepancy. If limb-sparing procedure should be impossible for skeletally immature patients, amputation has to be given first consideration. To minimize the functional difficulty from short amputation stump of above knee amputation, we performed lengthening of the amputation stump using ipsilateral tibia. One patient was lengthened 17cm using ipsilateral tibia and the other, 12cm. Two patients and their families were satisfied both clinically and psychologically.

역분화 방골성 골육종 - 증례 보고 1 례 - (Dedifferentiated Parosteal Osteosarcoma - A case report -)

  • Parosteal osteosarcoma is characterized as a densely ossifying lesion, usually occurring on the surface near the metaphyses of a long bone. The histological pattern is a well- differentiated mature bone trabeculae with a hypocellular spindle-cell stroma. The cytological details are those of a low-grade malignant lesion. The natural history of this lesion is indolent local growth, late invasion of the underlying bone, and infrequently, distant metastasis. However, there is a significant risk of eventual dedifferentiation into a high-grade lesion. We report here-a case of parosteal osteosarcoma dedifferentiated into a high-grade lesion, which occurred in the left distal femur of a 40-years-old woman, and discuss the experience in detail.

종골에 발생한 원발성 골육종 - 1예 보고 - (Primary Ostoegenic Sarcoma on Calcaneus - One Case Report -)

  • Osteogenic sarcoma is the most common primary malignant tumor of bone in which tumor cells form neoplastic osteoid or bone or both. Classic osteogenic sarcoma usually involves the metaphysis of the more rapidly growing long bones (distal femur, proximal tibia). Osteogenic sarcoma of the foot is rarely noted and only a few well documented cases have been reported. Osteogenic sarcoma of foot can clinically, radiographically, and histologically mimic several benign lesions and tumor-like lesions, so it sometimes leads to late diagnosis and delayed treatment. We experienced a case of primary osteogenic sarcoma on left calcaneus in 66-years-old female and report it with a review of references.

Three Newly Recorded Species of the Genus Dohrniphora (Diptera: Phoridae) from Korea

  • Three species of the genus Dohrniphora, viz., D. cornuta, D. malaysiae, and D. prescherweberae were newly recorded from Korea. These newly recorded species can be easily identified by the characteristic that inner face of male hind femur equipped with a group of peg-like stout setae on basal sensory area. Peg-like setae are arranged perpendicularly in D. cornuta whereas linear in D. malaysiae and somewhat arched in D. prescherweberae. Also, concave area near the peg-like setae is also well developed in male which is smooth in D. cornuta, on the other hand, there is well developed distal carina in D. malaysiae and with groups of microtrichia in D. prescherweberae. In this study, detailed descriptions with photographs of three newly recorded species and a key to males of all Korean Dohrniphora species are provided.

Descriptions of Clubiona bakurovi and Otacilia ansula sp. nov. (Araneae: Clubionidae and Phrurolithidae) from South Korea

  • The spider fauna of mountains, including national parks, in Korea was intensively surveyed in 2018-2019. During the seasonal surveys, males of Clubiona bakurovi Mikhailov, 1990 (family Clubionidae Wagner, 1887) and Otacilia ansula sp. nov. (family Phrurolithidae Banks, 1892) were collected from shrub foliage and leaf litter, respectively, in mountainous mixed forests. The males of Otacilia ansula sp. nov. can be easily distinguished from those of other its congeners by the slender and hook-shaped retrolateral tibial apophysis, ovoid distal tegular apophysis, and one dorsal spine on the femur with seven pairs of ventral spines on the tibia of leg I. This study provides descriptions of these two species with diagnosis, measurements, and morphological photographs.
