Systematic Review about Occupational Therapy Interventions Applied to the Improvement of Activities of Daily Living in Dementia Patients (치매 환자의 일상생활활동 능력향상에 적용된 작업치료 중재에 관한 체계적 고찰)

  • Purpose: This study analyzes occupational therapy interventions to improve activities of daily living (ADL) in dementia patients and the instruments used to verify their effects through a systematic review and attempts to use the results as preliminary data in selecting further interventions and instruments. Method: The databases searched included NDSL, DBpia, RISS, KISS and National Assembly Library with search words including 'Alzheimer's disease', 'Alzheimer', 'daily living' and 'ADL.' The subjects of analysis were a total of 7 studies, and a frequency analysis was used for the usage count of the interventions used in each study. In order to provide evidence, PICO Method was used for sorting. Result: As a result of this study, there were 7 occupational therapy interventions applied to improve ADL in dementia patients, which were used 7 times total. As for the instruments used to validate the effects of the interventions for the ADL, it turned out that '3 studies used AMPS (42.9%),' which was the most, followed by 'Allen Cognitive Level Screen' (ACLS) and Functional Independence Measure' (FIM), respectively used in 2 studies (28.6%); and 'Modified Barthel Index' (MBI) and 'Philadelphia Geriatric Center IADL' (PGC IADL), respectively used in 1 study. Regarding the qualitative level of evidence, it turned out that 4 studies were Level III (57.1%), followed by 2 studies at Level IV (28.6%) and 1 study at Level I (14.3%). Conclusion: This study suggested the kinds and frequencies of usage of the interventions and instruments of occupational therapy for the improvement of ADL in dementia patients, and the studies of evidence were presented by the PICO Method. It is judged that the results of this study can be used as preliminary data in selecting interventions and instruments to improve the ADL in dementia patients. In the future, studies should be carried out on the ADL in other areas related to dementia.

Study on effect of the herbs that invigorate and dispel blood stasis on Angiogenic inhibition (활혈거어약물(活血祛瘀藥物)이 항혈관신생(抗血管新生)에 미치는 영향(影響)에 관(關)한 연구(硏究))

  • Object When angiogenesis is excessive, Cancer, RA, Blindness, Psoriasis, Hemangioma, Diabetic retinopathy, Granulation, etc are induced. On the contrary, when it is insufficient, Stroke, Heart disease, Ulcer, Infertility, Scleroderma, artherosclerosis, delay of the wound recovery, etc occur. In recently, the methods which is control of abnormal angiogenesis are researching actively in relathion to anticancer research. This study is search for effective drugs which suppress this angiogenesis, in the ingredients of the herbs that invigorate and dispel blood stasis using to treat intravascular coagulation in the oriental herbal medicine. Methods We maked 80 % methanole extracts of Cnidii Rhizoma, Olibanum, Myrrha, Corydalidis Tuber, Curcumae Radix, Curcumae longe Rhizoma, Zedoariae Rhizoma, Salviae miltiorrhizae Radix, Polygoni cuspidati Rhizoama, Leonuri Herba, Persicae Semen, Carthami Flos, Trogopterorum Faeces, Achyranthis Bidentatae Radix, Manitis Squama, Eupolyphaga, Hirudo, Tabanus, Lycopi Herba, Artemisiae anomalae herba, Vaccariae Semen, Sappan Lignum, Gleditsiae Spina, Draconis Resina, Leonunari Semen, Selaginelliae Folium, Spatholobi Caulis, and these extracts were tested for MTT viabilaty test, BrdU incorporation, Tube foramtion assay on ECV304(immotalized human umbilical vein endothelial cell) at the concentration of $50{\mu}g/ml$, $100{\mu}g/ml$, $200{\mu}g/ml$, $400{\mu}g/ml$ Results All extracts except Draconis Resina have no cytotoxicity at the $100{\mu}g/ml$, and in BrdU incorporation test, proliferation rate were reduced below 60% at the concentaraion of $100{\mu}g/ml$ by Zedoariae Rhizoma, Sappan, Lignum Gleditsiae, Spina Draconis Resina Vaccariae Semen. Zedoariae Rhizoma Sappan Lignum Gleditsiae Spina Draconis Resina Vaccariae Semen Olibanum, Achyranthis Bidentatae Radix showed inhibition effects on tube formation of ECV304 at the concentration of $100{\mu}g/ml$. Conclusion At the concentration of $100{\mu}g/ml$ in which cytotoxicity is not found, Zedoariae Rhizoma, Sappan Lignum, Gleditsiae Spina, Vaccariae Semen showed the inhibition effect on proliferation and tubeformation of ECV304.

A Literature Review of Effectiveness on the Gongjin-dan (Gongchen-dan) (공진단의 효과에 대한 문헌적 고찰)

  • Objectives The purpose of this study was to investigate the effectiveness of the Gongjin-dan (Gongchen-dan, here in after GJD) in order to obtain the evidence for clinical application. Methods The GJD-related articles published from 1990 to 2013 were searched using "Korean Traditional Knowledge Portal", "Oriental Medicine Advanced Searching Integrated System (OASIS)", "Korean Association of Medical Journal Edition (Koreamed)", "Research Information Services (RISS4U)", "Korean Medicine Database (KMbase)", "National Discovery for Science Leader (NDSL)", "PubMed", "China National Knowledge Infrastructure (CNKI)". The search keywords were "Gongjin-dan", "Gongchen-dan". Thirty-nine articles were obtained. After excluding the eighteen article which did not meet inclusion criteria, finally twenty-one articles were included; five clinical articles and sixteen experimental articles. Results In clinical studies, GJD has the various effectiveness in cardiovascular diseases, alcoholic hepatitis, mild dementia, anemia. Also experimental studies related to the GJD show a variety of effects, such as anti-oxidative activity, neuroprotective activity, hepatoprotective activity, anti-inflammatory activity, immunological activity, reproductive recovery activity with fewer side-effects. Conclusions It has been suggested that there are various effects of GJD in treating a wide-range disease. However, in order to put GJD to use for many kinds of diseases in more reasonable ways, it is needed to publish well-design clinical trial based on the variety of results of experimental studies.

A Study on Stroke Patients' ADL, Depression, Self-Efficacy and Quality of Life (재가 뇌졸중 환자의 일상활동 수행능력, 우울, 자기효능감 및 삶의 질과의 관계)

  • This study was a descriptive research in search of a nursing intervention scheme to improve stroke patients' quality of life by understanding the relationship of stroke patients' ADL, depression, self-efficacy and quality of life with various variables and identifying factors that affect their quality of life. Each subject was interviewed one to one based on a structured questionnaire. With regard to research tools, Modified Barthel Index (MBI: Fortinsky et al., 1981), which was translated by Choi, Hye-sook (1996), was used in measuring ADL, Randloff's (1977) tool, which was translated by Choi, Soon-hee, was used in measuring depression, and the tool developed by Sherer et al. (1982), which is to measures self-efficacy under general conditions not limited to specific conditions, and modified by O, Bok-ja (1994) was used in measuring self-efficacy. The quality of life was measured using the scale of satisfaction of life developed by Diener et al. (1985). The results of this study were as follows: 1. The means of ADL of the subjects was $79.5{\pm}31.9$, depression $26.8{\pm}10.4$, self-efficacy $47.1{\pm}25.7$, and the quality of life $12.3{\pm}4.9$. 2. The subjects' quality of life showed a statistically significant difference according to gender (t=7.9, p= .006), satisfaction with income (F=5.8, p= .004), the burden of medical fee (F=3.7, p= .028) and the period of disease (F=2.8, p= .042). 3. With regard to relationship among ADL, depression, self-efficacy and the quality of life, ADL was in a relatively low positive correlation (r= .293, p= .003) with and the quality of life, depression in a high negative correlation (r=- .634, p= .000) with the quality of life, and self-efficacy in a positive correlation with the quality of life (r= .388, p= .000). 4. Factors that made a significant influence on the quality of life were depression (B=- .309, p= .001) and satisfaction with income (B=-2.611, p= .001). Based on these results, this study made following suggestions: 1. It is necessary to run rehabilitation programs to improve stroke patients' ADL, depression and self-efficacy. 2. It is necessary to perform research of monitoring stroke patients' quality of life in various areas using measuring tools.

  • In the course of search antagonistic fungi from soil in green house, four kind of fungi (AF1, AF2, AF3, AF4) were isolated, which have activities against Phytophthora capsici, Botrytis cinera, Rhizoctonia solani, Pythium ultimum and Fusarium oxysporum. The AF2 was identified according to the morphological description of Aspergillus terreus. This antagonistic fungus inhibiting various plant pathogens was effective to reduce disease incidence of cucumber seedlings caused by mixed inoculum of Rhizoctonia solani, Pythium ultimum and Fusarium oxysporum. Antifungal compound I was isolated and purified by fresh chromatography from A. terreus. The $^1H$ and $^{13}C$ assignment of compound I was achieved from two-dimensional $^1H-^1H\;COSY$, HMQC, HMBC with the add of homonuclear and heteronuclear double resonance experiment. The compound I was identified butyrolactone I (${\alpha}$-oxo-${\beta}$-(p-hydroxyphenyl)-${\gamma}$-(p-hydroxy-m-3,3-dimethyl-allylbenzyl)-${\gamma}$-methoxycarbonyl-${\gamma}$-butyrolactone, $C_{24}H_{24}O_7$, M.W.=424).

Research of recognition factors of folk medicine using statistical testing and data mining (통계적 검정과 데이터마이닝기법의 융합을 통한 민간요법 인식 요인 탐색조사)

  • Nowaday, beyond the time of wellbeing and LOHAS, many people have great interest in self therapy, so it is called healing era. As the folk medicine fields are actively industrialized and the interest in health improvement, not disease cure, is increased, many researches about the alternative medicine and therapy in various fields are being performed. In the times of the interest in health improvement and spontaneous, natural healing ability of human body is getting increase, it is very meaningful to search the factors which consist of recognition to folk medicine. So in this study, we developed the questionaries on the basis of previous studies, researched the factors affecting the recognition to folk medicine using factor analysis, and tested statistically the difference of recognition character according to demo-statistical traits. As the result, the twenty-four measurable variables related to folk medicine are sorted to four factors, ie, health improvement factor, safety factor, psycholocial factor, and substitutional factor. And overall, the middle and senior ages, the forties to sixties, and higher-educated peoples have more experiences in folk medicine than the younger ages, below thirties and lower-educated peoples. The distiction of sex makes little differences.

Search of Experimental Studies(35) on Anti aging and Anti oxidant in Korea (국내문헌(國內文獻)(35)에 나타난 항노화(抗老化) 및 항산화(抗酸化)의 실험적(實驗的) 연구(硏究)에 대(對)한 검색(檢索))

  • 35 theses conducted in Korea on the topic of senescence or anti-oxidant were classified on the basis of research methods, animals used for the experiments, and research items. Evaluating these research works with respect to the Free Radical Theory, the following conclusions were reached. 1. Of the 17 theses written in the Oriental medicine aspect, three theses used a single herb, nine theses used a complex prescription, four theses concentrated on the usage of the medicinal acupuncture, and one research paper focused on using scientific components. Common objection of these papers were on the verification of the efficacy of herbs. 2. Of the 18 these written in the Western medicine aspect, five theses used a single drug, seven theses conducted a research on the changes due to senescence, and 6 papers were on variety of topics. The main focus of these works were on the mechanism and pathology related to the senescence rather than on the suppression of senescence. 3. Among the theses written in the Oriental medicine perspective, a total of 48 herbs were utilized. 26 of these herbs has a tonification function on the Kidney. Six out of nine complex prescription mentioned above has a function of tonifying the Kidney. 4. With respect to the research subjects used on the experiments, 8 theses have used Senescence-Accelerated mice, 13 theses have used Sprague-Dawley mice, and remaining 7 papers have used human or other animals. 5. These are the categorization of the research items used: the weight (11) and weight changes of the visceral organs (9), the measurement of the content of peroxide-disease (9), the measurement of enzyme vitality (21), the blood and urine test (10), the experiment concerning immune system (3), the influence on the hepatic capability of metabolizing foreign substance (3), the effect on hepatic cell protection (3), the measurement of both the suppression of Free Radical and ability to create Free Radical (2), the measurement of effect of suppresing MDA(malondialdehyde) (4), the effect of eliminating DPPH Radical (2), and experiements about the functions (2). 6. The rate of vitalization of well known anti-oxidants such as Superoxide dismutase (SOD), Protein-bound SH, Nonprotein-bound SH, Glutathione(GSH), Catalase, and etc. were tested in 17 theses. Considering the conclusions mentioned above, the theses related to the senescence published in Korea elected different animals used for experiments, research items and the methods of research, the end result seems to be a lack of objectivity. Thus, I would argue that research methods to overcome such a deficiency need to be developed systematically.

Assessment of the Quality of Postherpetic Neuralgia Related Korean Internet Sites (대상포진후신경통에 관한 인터넷 사이트 평가)

  • Background: There is no assessment of internet sites that carry information on chronic pain disease. So we assessed the quality of information about postherpetic neuralgia available on Korean internet sites. Methods: The keywords 'postherpetic neuralgia', 'herpes zoster, neuropathic pain', 'herpes zoster, pain', 'herpes zoster' were searched in Korean on four search engines in Korea between the 1st to the 15th of May, 2009. We evaluated the outcome on two factors; the aspect of the contents which is subdivided into two categories, the content and authorship, and the technical aspect including web design, and efficiency. Results: A total of 26 internet sites were found. Among these sites, 6 (23%) informed by anesthesiologist. The average score of the 26 internet sites was only $37.4{\pm}20.1$ out of a total of 100. A mean score of the contents was $13.3{\pm}8.3$ out of 40 points, the authorship was $10.0{\pm}6.7$ out of 20 points, the design was $9.2{\pm}5.3$ out of 20 points, the efficiency was $6.8{\pm}4.3$ out of 20 points. When comparing the score between anesthesiologist and non-anesthesiologist, the contents was $18.7{\pm}7.4$ vs. $11.7{\pm}7.9$, the authorship was $13.4{\pm}4.7$ vs. $9.0{\pm}6.8$, the design was $12.5{\pm}4.2$ vs. $8.3{\pm}5.2$ and the efficiency was $6.8{\pm}4.5$ vs. $4.3{\pm}4.0$ (P < 0.05). Conclusions: There is a need for more accurate information about postherpetic neuralgia on the Korean internet by anesthesiologists.

MOLECULAR CLONING AND SEQUENCE ANALYSIS OF THE GENE FOR THE HEMIN-BINDING PROTEIN FROM Prevotella intermedia (Prevotella intermedia에서의 Hemin 결합 단백질 유전자의 분리 및 염기서열 분석)

  • Prevotella intermedia is one of the most frequently implicated pathogens in human periodontal disease and has a requirement for hemin for growth. This study has identified a hemin-binding P. intermedia protein by expression of a P. intermedia genomic library in Escherichia coli, a bacterium which does not require or transport exogenous hemin. The genomic library of P. intermedia was constructed into plasmid pUC18, transformed into Escherichia coli strain $DH5{\alpha}$, and screened for recombinant clones using heminbinding activity by plating onto hemin-containing agar. Approximately 5,000 recombinant E. coli colonies were screened onto LB-amp-hemin agar, single clone(pHem1) was exhibited a clearly pigmented phonotype. The 2.5kb insert DNA of pHem1 was determined by restriction enzyme mapping. Southern blot analysis of BamHI, BglII, EcoRI, HindIII and PstI-digested P. intermedia DNA indicated that single copy of the gene was present in the genome. Northern blot analysis revealed that the size of transcript was approximately 1.8 kb. The cloned gene contained a single ORF, consisting of approximately 850-residue amino acids. A BLAST search of the Institute for Genomic Research genes with similar nucleotide sequence revealed no significant similarity It needs further investigation to clarify the mechanisms of heme uptake in P. intermedia.

The Review of a Standardized Method in Retention-Enema: a Clinical Study (보류관장의 표준화된 방법을 위한 고찰: 임상논문을 중심으로)

  • Objectives This study was aimed to review a standardized method of retention enema by extracting elements associated with methods of retention enema from papers involved. Methods 1) Data sources : Retrieval was made, using 'Enema' or 'Retention Enema' as search words in MEDLINE, The Cochrane Library, KISS, RISS, and NDSL DB. 2) Study selection : Two authors excluded irrelevant papers and chose qualified abstracts in the rest of the papers. The chosen studies were reviewed fully by the authors so that they could screen the significant papers based on the exclusion criteria. 3) Data extraction : Data on the total number of subjects, the target disease, the type, quality, length, diameter of catheter, the insertion length of catheter, the sample, the sample volume, the process of retention enema and the number of performing retention enema were extracted from the selected studies. Results & Conclusions The retention enema can be applied to gastrointestinal diseases, genitourinary diseases, metabolic diseases such as hypertension and diabetes, headaches and neurological disorders. The standardized treatment method is as following: 1) inserting a rental tube of 18-28 Fr that measures 20 to 30 cm into the patient's anus, 2) injecting herbal medicines which have purgative action or benefit on stomach and intestinal tract at the rate of 4 ml/min, and 3) holding them for 10 to 15 minutes.
