An Occurrence of Pacheco's Disease in Parrakeet (Neophema bourkii) Reared in Korea (국내 주초앵무의 Pacheco병 발생)

  • Pacheco's disease (PD) is a highly contagious, acute disease of psittacines caused by a herpesvirus. Five 56-day-old dead parakeets (Neophema bourkii) were submitted to the National. Veterinary Research & Quarantine Service for diagnosis. At necropsy, principle lesions were seen in the pancreas, which were hard, whitish, nodular and atrophic resulting in distortion of the duodenal loop. Histologically, lymphocytic pancreatitis with eosinophilic intranuclear inclusions syncytium-formed acinar epithelium was prominent. In the brain, marked focal gliosis and perivascular lymphocytic cuffing were observed. On the electron microscopy examination, various forms of viral particles were found in both the nucleus and cytoplasm of acinar epithelium of pancreas, identified as a herpesvirus. These results were suggested that the cases were diagnosed as PD in the parrakeet.

Biological and molecular characterization of feline caliciviruses isolated from cats in South Korea

  • Feline calicivirus (FCV) infection results in a common upper respiratory disease associated with oral ulceration in cats. Although FCV infection has been reported in cats worldwide, the biologic and genetic features of South Korean FCV are unclear. We aimed to investigate the biological and genetic features of South Korean FCV isolates. Crandell-Rees feline kidney (CRFK) cells were used to isolate FCV from 58 organ homogenate samples. The FCV isolates were confirmed by cytopathic effects, immunofluorescence, electron microscopy, and reverse transcription polymerase chain reaction assays. Viral genetic analysis was carried out with VP2 gene and complete genomes of FCVs. Five viruses propagated in CRFK cells were confirmed to be FCVs. The FCV17D283 isolate showed the highest viral titer of 107.2 TCID50/mL at 36 h post-inoculation. Korean FCV isolates did not grow well in Vero, BHK-21, A72, or Madin-Darby canine kidney cells. The FCV17D03 and FCV17D283 isolates had the highest genetic similarity (80.1% and 86.9%) with the UTCVM-H1 and 14Q315 strains, which were isolated in the United States and South Korea in 1995 and 2014, respectively. We isolated five FCVs from cats and detected important genetic differences among them. FCV isolates did not show any virulent effects in mice.

Comparison of Diagnostic Accuracy and Prediction Rate for between two Syndrome Differentiation Diagnosis Models (중풍 변증 모델에 의한 진단 정확률과 예측률 비교)

  • In spite of abundant clinical resources of stroke patients, the objective and logical data analyses or diagnostic systems were not established in oriental medicine. In the present study we tried to develop the statistical diagnostic tool discriminating the subtypes of oriental medicine diagnostic system, syndrome differentiation (SD). Discriminant analysis was carried out using clinical data collected from 1,478 stroke patients with the same subtypes diagnosed identically by two clinical experts with more than 3 year experiences. Numerical discriminant models were constructed using important 61 symptom and syndrome indices. Diagnostic accuracy and prediction rate of 5 SD subtypes: The overall diagnostic accuracy of 5 SD subtypes using 61 indices was 74.22%. According to subtypes, the diagnostic accuracy of "phlegm-dampness" was highest (82.84%), and followed by "qi-deficiency", "fire/heat", "static blood", and "yin-deficiency". On the other hand, the overall prediction rate was 67.12% and that of qi-deficiency was highest (73.75%). Diagnostic accuracy and prediction rate of 4 SD subtypes: The overall diagnostic accuracy and prediction rate of 4 SD subtypes except "static blood" were 75.06% and 71.63%, respectively. According to subtypes, the diagnostic accuracy and prediction rate was highest in the "phlegm-dampness" (82.84%) and qi-deficiency (81.69%), respectively. The statistical discriminant model of constructed using 4 SD subtypes, and 61 indices can be used in the field of oriental medicine contributing to the objectification of SD.

Meningoencephalitis and pneumonia caused by Pasteurella multocida in rabbits (토끼에서 Pasteurella multocida 감염에 의한 뇌막뇌염과 폐렴 증례보고)

  • Eight rabbits exhibited head tilt and subsequently died. At necropsy, three rabbits had crusty deposits in ears and four had reddish lungs. The main histopathological features were severe diffuse suppurative meningoencephalitis (75.0% of rabbits), fibrinopurulent pneumonia (37.5%), and otitis externa (37.5%). Pasteurella multocida (P. multocida) was isolated from brains, ears, and lungs. The capsular serogroups of the isolates were untypable. Based on histopathological features and bacterial analysis results, the rabbits were diagnosed as P. multocida infection. P. multocida infections might result in considerable economic loss in commercial rabbit production facilities in Korea.

Frozen Section Biopsy to Evaluation of Obscure Lateral Resection Margins during Gastric Endoscopic Submucosal Dissection for Early Gastric Cancer

  • Purpose: To determine the diagnostic utility of a frozen section biopsy in patients undergoing endoscopic submucosal dissection (ESD) for early gastric neoplasms with obscure margins even with chromoendoscopy using acetic acid and indigo carmine (AI chromoendoscopy). Materials and Methods: The lateral spread of early gastric neoplasms was unclear even following AI chromoendoscopy in 38 patients who underwent ESD between June 2007 and May 2011. Frozen section biopsies were obtained by agreement of the degree of lateral spread between two endoscopists. Thus, frozen section biopsies were obtained from 23 patients (FBx group) and not in the other 15 patients (AI group). Results: No significant differences were observed for size, histology, invasive depth, and location of lesions between the AI and FBx groups. No false positive or false negative results were observed in the frozen section diagnoses. Adenocarcinoma was revealed in three patients and tubular adenoma in one, thereby changing the delineation of lesion extent and achieving free lateral margins. The rates of free lateral resection margins and curative resection were significantly higher in the FBx group than those in the AI group. Conclusions: Frozen section biopsy can help endoscopists perform more safe and accurate ESD in patients with early gastric neoplasm.

  • 본 논문에서는 대사증후군관련 의료 빅데이터 표준화 시스템의 환경과 소프트웨어 환경을 설계한 사항에 대해서 기술한다. 이를 위해 임상데이터를 기반으로 의료 빅데이터를 수집하고 국제 표준화인 공통 데이터 모델로 수집된 데이터를 ETL하여 통합 데이터베이스에 저장하였다. 본 연구를 통해 구축된 의료 빅데이터 표준화 시스템은 향후 의사결정 보조시스템 개발과 연계하여 효과적인 검색과 다양한 통계 분석을 지원할 계획이다. 또한 병원의 다양한 임상 연구를 지원하기 위한 주요 시스템으로 자리매김할 것으로 기대한다.

Clinical sign and transmission of foot-and-mouth disease in deer, Review (사슴에서의 구제역 증상과 전파 가능성)

  • Foot-and-mouth disease (FMD) commonly infects cloven-hoofed livestock animals such as cattle, pig, sheep, and goat and its clinical signs are well-known. Besides livestock, FMD can be transmitted among cloven-hoofed animals in the wild. FMD mostly affects livestock animals in farms, but, wild animals are likely to play a pivotal role in spreading the disease due to their way of free living. In the case of deer, the clinical signs of FMD vary widely from subclinical to severe infections. Thus, in some deer species, it may be hard to verify clinical signs of FMD. A deer may carry the virus up to 11 weeks after exposure, shedding the virus during the period. However, deer is not considered as a typical host for persistent infection like buffalo, cattle or sheep. In Korea, small-scale livestock farms which have less than 10 animals make up 63.6% of the entire livestock farms. Considering raising environment in deer farms, it is assumed that the risk of virus excretion and consequent transmission of FMD among deers is relatively lower than other cloven-hoofed animals. However, Sika deer and Elk which are typical deer species in Korea would manifest mild to subclinical symptoms upon FMD infection. Therefore, laboratory testing is necessary to confirm FMD in these animals because of difficulty in verifying clinical signs and the risk of virus shedding during inapparent infection.

Cerebrovascular Events in Pediatric Inflammatory Bowel Disease: A Review of Published Cases

  • Pediatric inflammatory bowel disease (PIBD) is a multisystem disorder characterized by intestinal and extraintestinal manifestations and complications. Cerebrovascular events (CVE) are rare extraintestinal complications in patients with PIBD. Statistics show that 3.3% patients with PIBD and 1.3-6.4% adult patients with inflammatory bowel disease (IBD) experience CVE during the course of the disease. Therefore, this study aimed to review the records of children with IBD who developed CVE during the course of the disease. We retrospectively reviewed 62 cases of PIBD complicated by CVE. The mean patient age at the time of thrombotic events was 12.48±4.13 years. The incidence of ulcerative colitis was significantly higher than that of Crohn's disease (43 [70.5%] vs. 13 [21.3%] patients). Most patients (87.93%) were in the active phase of IBD at the time of CVE. The mean time interval between the onset of IBD and CVE was 20.84 weeks. Overall, 11 (26.83%) patients showed neurological symptoms of CVE at disease onset. The most frequent symptom on admission was persistent and severe headaches (67.85%). The most common site of cerebral venous thrombosis was the transverse sinuses (n=23, 53.48%). The right middle cerebral artery (n=3, 33.34%) was the predominant site of cerebral arterial infarction. Overall, 41 (69.49%) patients who were mostly administered unfractionated heparin or low-molecular-weight heparin (56.09%) recovered completely. Patients with IBD are at a risk of thromboembolism. CVE may be the most common type of thromboembolism. Based on these findings, the most common risk factor for CVE is IBD flares. In patients with CVE, anticoagulant therapy with heparin, followed by warfarin, is necessary.
