Cytohistologic Discrepancy of High-Grade Squamous Intraepithelial Lesions in Papanicolaou Smears

  • Objectives: To evaluate the frequency of cytohistologic discrepancy of high-grade squamous intraepithelial lesions (HSILs) in Pap smears and associated factors. Methods: Medical records of 223 women with HSIL Pap smears who were treated at Thammasat University Hospital were reviewed. Data on age, parity, menopausal status, contraceptive use and colposcopic directed biopsy and loop electrosurgical excision procedure (LEEP) pathology results were recorded. Results: Mean (SD) age of patients was 38.0 (9.4) years. The majority were premenopausal (86.5%) and multiparous (83.9%). Cytohistologic discrepancy between the Pap test and colposcopic-directed biopsy histology was 45.7% and that between the Pap test and LEEP histology was 29.5%. Fifty-four (24.2%) women had no high-grade CIN on both colposcopic directed biopsy and LEEP. Nulliparity, postmenopausal status and having no oral contraceptive pills use were factors associated with cytohistologic discrepancy. Conclusion: The exact cytohistologic discrepancy rate was relatively high (24.2%). Factors associated with cytohistologic discrepancy were nulliparity and postmenopausal status and having no oral contraceptive pill use.

Study on the Effect of Discrepancy of Training Sample Population in Neural Network Classification

  • Neural networks have been focused on as a robust classifier for the remotely sensed imagery due to its statistical independency and teaming ability. Also the artificial neural networks have been reported to be more tolerant to noise and missing data. However, unlike the conventional statistical classifiers which use the statistical parameters for the classification, a neural network classifier uses individual training sample in teaming stage. The training performance of a neural network is know to be very sensitive to the discrepancy of the number of the training samples of each class. In this paper, the effect of the population discrepancy of training samples of each class was analyzed with three layered feed forward network. And a method for reducing the effect was proposed and experimented with Landsat TM image. The results showed that the effect of the training sample size discrepancy should be carefully considered for faster and more accurate training of the network. Also, it was found that the proposed method which makes teaming rate as a function of the number of training samples in each class resulted in faster and more accurate training of the network.

대학병원 응급 영상검사에서 영상의학과 전공의 가판독과 전문의 최종 판독 간의 불일치 발생률 분석 (Analysis of the Rate of Discrepancy between Preliminary Reports by Radiology Residents and Final Reports by Certified Radiologists for Emergency Radiology: Studies in a University Hospital)

  • 목적 대학병원 성인응급실에서 영상의학과 전공의 가판독과 전문의 최종 판독 간에 불일치가 발생한 빈도를 조사하고, 이 발생률에 영향을 미칠 수 있는 요인을 살펴보았다. 대상과 방법 2016년 12월부터 2019년 11월까지 성인응급실에서 촬영된 전산화단층촬영과 자기공명영상 검사 중, 전공의 가판독과 전문의 최종 판독 간에 불일치로 인해 환자의 진단이나 치료 계획이 변경될 수 있는 경우를 중대한 불일치로 정의하고, 이를 유형별로 분류하였다. 이후 전공의 근무 시간대, 응급실 근무 경력, 세부 분야별로 중대한 불일치 발생률의 분포를 살펴보았다. 결과 총 72137건의 가판독 중 중대한 불일치를 보인 검사는 총 1348건(1.9%) 이었다. 중대한 불일치의 유형으로는 위음성(72.0%)이 가장 많았고, 오판(26.3%), 위양성(1.7%) 순이었다. 또한 급성 소견의 중대한 불일치(87.2%)가 비급성 소견의 중대한 불일치(12.8%)보다 많았다. 중대한 불일치 발생률은 24시간 교대 근무 후반부로 갈수록 증가하였으며, 새벽 2시부터 4시 사이가 2.9%로 가장 높았다. 전공의의 응급실 근무 경력에 따른 발생률 차이는 없었고, 세부 분야별 발생률은 0.6%-4.5%로 다양했다. 결론 가판독과 최종 판독 간에 중대한 불일치 발생률은 2% 미만이었고, 24시간 교대 근무 후반으로 갈수록 증가하였다.

Do Radiology Residents Perform Well in Preliminary Reporting of Emergency MRIs of Spine?

  • Purpose: To evaluate interpretation errors involving spine MRIs by residents in their second to fourth year of training, classified as minor, intermediate and major discrepancies, as well as the types of commonly discordant lesions with or without clinical significance. Materials and Methods: A staff radiologist evaluated both preliminary and final reports of 582 spine MRIs performed in the emergency room from March 2011 to February 2013, involving (1) the incidence of report discrepancy, classified as minor if there was sufficient description of the main MR findings without ancillary or incidental lesions not influencing the main diagnosis, treatment, or patients' clinical course; intermediate if the correct diagnosis was made with insufficient or inadequate explanation, potentially influencing treatment or clinical course; and major if the discrepancy affected the main diagnosis; and (2) the common causes of discrepancy. We analyzed the differences in the incidence of discrepancy with respect to the training years of residents, age and sex of patients. Results: Interpretation discrepancy occurred in 229 of the 582 cases (229/582, 39.3%), including 146 minor (146/582, 25.1%), 40 intermediate (40/582, 6.9%), and 43 major cases (43/582, 7.4%). The common causes of major discrepancy were: over-diagnosis of fracture (n = 10), missed cord lesion (n = 9), missed signal abnormalities associated with diffuse marrow (n = 5), and failure to provide differential diagnosis of focal abnormal marrow signal intensity (n = 5). No significant difference was found in the incidence of minor, intermediate, and major discrepancies according to the levels of residency, patients' age or sex. Conclusion: A 7.4% rate of major discrepancies was found in preliminary reporting of emergency MRIs of spine interpreted by radiology residents, probably related to a relative lack of clinical experience, indicating the need for additional training, especially involving spine trauma, spinal cord and bone marrow lesions.

C-reactive protein and N-terminal pro-brain natriuretic peptide discrepancy: a differentiation of adenoviral pharyngoconjunctival fever from Kawasaki disease

  • Purpose: To differentiate adenoviral pharyngoconjunctival fever (PCF) from acute Kawasaki disease (KD) using laboratory tests before results of virus-real time polymerase chain reaction and ophthalmologic examination are obtained. Methods: Baseline patient characteristics and laboratory measurements were compared between 40 patients with adenovirus infection and 123 patients with KD. Results: The patients with adenovirus infection were generally older than those with KD (median: 3.9 years vs. 2 years, P=0.000). White blood cell and, platelet count, and aspartate aminotransferase, alanine aminotransferase, and N-terminal pro-brain natriuretic peptide (NT-proBNP) levels showed significant differences between the 2 groups, but the C-reactive protein (CRP) levels did not ($6.8{\pm}3.0mg/dL$ vs. $8.3{\pm}5.8mg/dL$, P=0.126). In the adenovirus infection group, the CRP levels were <1, <3, <10, and ${\geq}10mg/dL$ in 2 (5%), 3 (7.5%), 30 (75%), and 5 patients (12.5%), respectively. The cutoff NT-proBNP level was 265 pg/mL. Discrepancy was defined as CRP and NT-proBNP levels of ${\geq}3$ or <3 mg/dL, and <265 or ${\geq}265pg/mL$, respectively. Among the 35 patients with adenovirus infection whose CRP levels were ${\geq}3mg/dL$, 29 (82.9%) showed a discrepancy. Conversely, of the 103 patients with KD whose CRP levels were ${\geq}3mg/dL$, 83 (80.6%) showed no discrepancy. Between the groups, a significant difference in discrepancy rate was observed (P=0.000). None of the patients with adenovirus infection had CRP and NT-proBNP levels of <3 mg/dL and ${\geq}265pg/mL$, respectively. Conclusion: With a sensitivity of 82.9% and a specificity of 80.6%, CRP and NT-proBNP levels may differentiate between adenoviral PCF and acute KD.

퍼지수를 이용한 공공부문의 서비스 품질 속성분석에 관한 연구 (A study on the analysis of Service Quality attribute using Fuzzy numbers in Public sector)

  • This paper proposed a new method to evaluate service quality attribute of perceived service quality in public sectors, using triangle fuzzy numbers and hamming distance. Our method measured the ratio of the expected and perceived service for the customers' perceived service quality. By using fuzzy numbers, This method not only overcomes linguistic variable problems but also provides more objective and direct information for service quality attributes. The discrepancy rate between expected service and perceived service that is perceived service quality is evaluated by hamming distance. To evaluate the discrepancy rate from hamming distance, we induced general solutions to compute the intersection area between two triangle fuzzy numbers and the weak or strong attributes in public sectors are clarified.

연령과 근시도에 따른 현성굴절검사와 조절마비굴절검사의 비교 (Difference between Manifest Refraction (MR) and Cycloplegic Refraction (CR) with Age and Myopic Value)

  • 목적: 본 연구는 근시안에서 현성굴절검사(manifest refraction: MR)와 조절마비굴절검사(cycloplegic refraction: CR)값을 비교하여 연령과 근시도에 따라 차이가 있는지 알아보고자 하였다. 방법: 근시성 굴절이상을 가진 229명 ($25.3{\pm}11.9$세)을 대상으로 조절마비제(cyclogyl 1%) 점안 전과 후에 자각적 굴절검사를 시행하였다. 결과: 조절마비제 점안 전 후 평균굴절력을 비교 조사하였더니, 근시 0.19 D, 난시 0.02 D 감소하였고 난시방향은 1.85도 차이를 보였다. 근시도의 변화는 30~40대에서 가장 높게 나타났으며, 229명(458안) 중 근시도수에서 0.50 diopter(D) 이상의 차이를 보이는 경우는 73안(16%)으로 근시도가 높은 군일수록 오차율이 높게 나타났다. 결론: 현성굴절검사와 조절마비굴절검사 값을 비교해 본 결과 연령에 따른 근시도의 변화는 유의하지 않았으나, 근시도가 높은 군일수록 오차율이 유의하게 높은 것으로 나타났다.

묘박 중 외력에 의한 선체의 운동 특성 (Characteristic of hull motion due to external forces at anchor)

  • In order to provide basic data to increase the efficiency and stability of seamanship at anchoring, the characteristics of the hull motion including dragging anchor due to external forces were observed at Mokpo and Jinhae anchorage for the avoidance of the typhoon. As a result, it is necessary to check the embedding motion and holding power of the anchor according to at initial position to decrease dragging anchor. Dragging anchor at anchorage seems to have been easily caused according to discrepancy between embedded anchor flukes and the towing direction due to the change in wind direction, rather than the wind speed. This discrepancy, thus, should be considered when anchoring. This test vessel with a small radius of curvature of the stem is relatively vulnerable to the influence of wind direction and wind speed, so it is easy to cause a decrease in the holding power due to an increase in the rate of turn. When the current speed is greater than or equal to 1 knot, the range of the rate of turn is reduced resulting in a relatively increased holding power. In addition, during the swing, the tension of the chain was high according to the angular velocity change of heading at three-quarters of the swing length rather than the left and right ends.

CMC model에 의한 near-extinction methane/air turbulent jet diffusion flame의 수치적 모사 (Numerical Study on Methane/Air Turbulent Jet Diffusion Flames Near-Extinction Using Conditional Moment Closure Model)

  • The first-order conditional moment closure (CMC) model is applied to $CH_4$/Air turbulent jet diffusion flames(Sandia Flame D, E and F). The flow and mixing fields are calculated by fast chemistry assumption and a beta function pdf for mixture fraction. Reacting scalar fields are calculated by elliptic CMC formulation. The results for Flame D show reasonable agreement with the measured conditional mean temperature and mass fractions of major species, although with discrepancy on the fuel rich side. The discrepancy tends to increase as the level of local extinction increases. Second-order CMC may be needed for better prediction of these near-extinction flames.

캐비테이션 벤튜리의 유량제어 성능 (Performance of Flow Rate Control of a Cavitating Venturi)

  • Characteristics of flow rate control has been studied for a cavitating venturi adopted in a liquid rocket propellant feed system. Numerical simulation has been peformed to give about $10\%$ discrepancy of mass flow rate to the experimental data for cavitating flow regime. Mass flow rate is confirmed to be saturated for pressure difference higher than $3\times10^5$pa when the upstream pressure is fixed to $22.8\times10^5$pa and the downstream pressure is varied. The evaporation amount depends substantially to non-condensable gas concentration. However the mass flow rate characteristic is relatively insensitive to the mass fraction of non-condensable gas. So it is reduced by only $2\%$ when the non- condensable gas concentration is increased from 1.5PPM to 150PPM. From the previous comparison the expansions of the non-condensable gas and the evaporation of liquid are verified to have same effect to pressure recovery.
