  • The Authors have observed a case of hypopituitary patient in 15 year-old Korean child for 10 months. Since he was discharged, he was taken the hormonal therapy with corticosteroid and T₃. As the result of it, his body weight increased by 1.2kg and height grew by 3cm. However it is not clear whether this growth is due to the child's natural growth or due to the above hormonal therapy.

  • In patient with lung cancer, the resection margin of right main bronchus was invaded by tumor intraoperatively. So we performed tracheal reconstruction with autologous pericardium after resection of lower trachea including carina. Postoperatively, the patient discharged well and followed up for 5 months without any evidence of tumor recurrence or restenosis.

  • This is a report of a migrated polyethylene catheter from the arm vein cutdown site into the heart. The patient was a 48 years old male who was operated on for mitral stenosis complicated with pulmonary hypertension. On the 8th postoperative day after open mitral commissurotomy, polyethylene catheter embolization occurred. The catheter was removed successfully with an incision on the proximal basilic vein. The patient recovered uneventfully and discharged without complications.

The Relationship between Patient Characteristics and Satisfaction with Hospital Care (환자특성에 따른 의료이용에 대한 환자만족도 비교)

  • Purpose: The purpose of this study was to identify the relationship between patient characteristics and patient satisfaction. Methods: A cross-sectional questionnaire survey was conducted in an acute care hospital. The subjects were 317 patients discharged from general medical and surgical nursing care units during September, 2005. Patient satisfaction was measured using the short-form satisfaction scale of Hwang and Park(2001). Additional information about patient characteristics, including general demographics and health care utilization variables, was collected from the hospital information systems. Multiple regression analysis was performed to determine patient characteristics influencing patient satisfaction. Results: Patients were satisfied with hospital care with an average of 4.10 on a five-point Likert scale. Patient characteristics explained 13.5% of the variance of patient satisfaction. The significant factors influencing patient satisfaction were patients' age and perceived health status. There was no significant relationship between structural variables and patient satisfaction. Conclusion: This study showed that patients' characteristics were significant factors explaining patient satisfaction. Therefore, these characteristics should be adjusted in reporting patient satisfaction as an indicator for hospital-level or department-level rating.

  • Objectives : The purpose of this study was to investigate the effect of Sojukjeongwon-san (이하 SJJWS) on a rectal cancer patient. Methods : This clinical case was analyzed on a patient who was admitted on August 11th and discharged on September 7th, 2006. We recordedhis abdominal pain, urine, anal pain, bleeding, mucous stools, frequency and hardness and amount of the stools, weight, lab follow-ups and so on. Results : After the treatment, the chief complaints of the patient, such as rectal pain, bloody excrement, loss of weight, etc., had improved. Conclusion : In this study we suggest that SJJWS reduced symptoms for a rectal cancer patient and improved the patient's quality of life.

  • A 26 year old male patient had admitted to the department of plastic surgery for the treatment of skin defect of forearm and spastic contracture of right hand, attributable to burn injury following carbon monoxide intoxication. After receiving skin graft the patients tenotomy of flexor tendons, the patients was consulted to pain clinic for further evaluation and treatment of allodynia, hyperalgesia, and hyperpathia with marked emotional insufficiency. The patient was treated with stellate ganglion blocks, intermittent or continuous epidural blocks, and intermittent brachial plexus blocks for 3 months. with this treatment the patient's pain level improved to(VAS 10 to 4~5) and was discharged. The patient was readmitted 3 months later, due to the aggrzvation of pain. Brachial plexus blocks were given again by interscalene, supraclavicular, or axillary route, sometimes using a catheter, together with cervical epidural blocks. Tricyclic antidepressant was also prescribed. The results were remarkably good(VAS 2~3) and the patient did not require any further analgesic medication.

  • A male patient weighing 2.5 kg was admitted for respiratory difficulty, and a large ventricular septal defect (VSD) was diagnosed. During care, sudden right leg swelling with a femur shaft fracture occurred. The patient's father had a history of recurrent lower extremity fractures; thus, osteogenesis imperfecta was considered. The patient's respiratory difficulty became aggravated, and VSD repair in the neonatal period was therefore performed with gentle sternal traction and great vessel manipulation under total intravenous anesthesia to prevent malignant hyperthermia. The patient was discharged without notable problems, except minor wound dehiscence. Outpatient genetic testing revealed that the patient had a COL1A1/COL1A2 mutation.

Caregiver or Family Doses due to Discharged $^{131}I$ Administrated Patient from the Hospital (고용량 $^{131}I$ 투여환자 퇴원이후 환자 간병인과 환자 가옥의 피폭선량 측정)

  • Exposed doses to the patient's caregiver and their house due to the 131I from patients discharged from the hospital were measured using OSL dosimeters. Usually, 3.37-5.55 GBq (100-150 mCi) of $^{131}I$ administrated patients are discharged from the hospital after 3 or 4 days of hospitalization in Korea. In addition, after 5 to 8 days, the accumulated doses of the patient's caregiver and house after hospitalization of the patient were measured using OSL dosimeters. The results of the measured average accumulated doses were 0.1 mSv, which is 10% of 1 mSv, the public dose limit in the Korean Atomic Energy Law. And it's standard deviation was 0.087 mSv. Based on the results of this study, we anticipate that we could assure the compliance of the regulation requirement 5 mSv of MEST (Ministry of Education, Science and Technology) Notice No. 2008-45 for the patient's caregiver or family, even if we reduce the 3-4 days of hospitalization to 1-2 days or less.

Ebstein's Anomaly -A Case Report- (엡스타인 심기형 -1례 보고-)

  • Ebstein`s anomaly, a rare congenital cardiac anomaly, is characterized by downward displacement of abnormal tricuspid valve. Indication for surgical repair and the optimal surgical approach are still controversy. Recently, we experience a case of Ebstein anomaly, which was treated by atrilized right ventricular plication and annuloplasty. The patient was discharged with good result on 17th post-operative day.

A Study on the Characteristics of Prematurely Discharged Patients and the Model for Predicting Premature Discharge (환자이탈군 특성요인과 이탈환자 예측모형에 관한 연구)

  • Background : We developed a model for predicting premature discharge and identifying related factors. Methods : Prediction model was developed by data mining techniques. Basic data were collected from the total discharge data base of a university hospital in Chungnam Province during the period from July 1, 1999 to June 30, 2000. Results : 1. Among 22,873 patients, the number of patients discharged with usual discharge orders were 21,695 or 94.8%. The number of the prematurely discharged patients were 1,178 or 5.2%. 2. The primary reason for unusual discharge was transfer to other hospital. Move to a local hospital closer to their home and burdensome medical expenses were main reasons. 3. Predictability of each model was tested using the top 10 percent of patients with the highest probabilities of premature discharge. The neural network model was chosen as the most appropriate model for predicting prematurely discharged patients. 4. Ten percent of the total number of patients had been selected randomly to test the effectiveness of the neural network model. We have chosen the threshold of the neural network model as 0.7. The number of patients who were expected to discharge prematurely was 312. Among them, 241 had been discharged prematurely (77.2%). Conclusion : Of the several data mining techniques used, the neural network model was the most effective, It can be used to identify and manage the patients who are expected to discharge prematurely.
