A 45 year old male officer was admitted due to upper substernal pain for 1 month, which was aggravated by swallowing. On past and family history, there was no specific history except heavy drinking. Simple chest x-ray revealed no specific abnormal findings. Preoperative esophagofiberscopy and Barium study showed midesophageal diverticulum, pulsion type, at about 2 cm below the left main bronchus. The opening of the diverticulum was located at the left posterolateral aspect of esophagus. Midesophageal false diverticulum, measuring 2 x 2 x 1 .S cm in size, was noted at about 5 cm under the aortic arch protruding through a slit-like muscular defect. After inversion of diverticular sac, interrupted sutures with 3-0 silks were done on muscular defect site, and mediastinal pleura was reinforced on the lesion with interrupted sutures. On 4th postoperative day, esophagography revealed no diverticulum or stenosis. Also esophagofiberscopy showed smooth mucosal tag without disturbance of passage. On 14th postoperative day, the patient was discharged uneventfully, and follow-up for 3 months after discharge revealed nothing abnormal symptoms. The authors report one case of midesophageal, pulsion type, false diverticulum.
Efficiency of a centrifugal pump is known to drop rapidly with a decrease of specific speed $n_s$. However, below $n_s=60\;[min^{-1},\;m^3/min,\;m]$, the pump characteristics are not yet clear. Therefore, present study is aimed to investigate the influence of large change of specific speed on the performance of a very low specific speed centrifugal pump. Moreover, influence of impeller configuration on the performance of very low specific speed pump is investigated. The results show that very low specific speed can be accomplished by reducing volute throat sectional area using circular spacer. Influence of the spacer's location and configuration in the discharge passage on the pump performance is very small. Best efficiency of very low specific speed centrifugal pump decreases proportionally to the specific speed but the best efficiency decreases on a large scale in the range of $n_s<40$. Influence of impeller configuration on the pump performance and radial thrust of centrifugal pump are considerably small in the range of extremely low specific speed $(n_s=25)$.
In general, CT and Shunt have been traditionally used as a sensor for detecting the partial discharges in order to diagnose the present insulation state of the electric power apparatus. The former is very convenient for the practical application since it is not only non-contact method but its frequency bandwidth and resonance frequency could be designed for its specific application. However, it has been proved to have poor linearity and low sensitivity. For the latter, even though it is an ideal sensor, noise from the power source and the ground could flow into the system. Furthermore, the surge current could be easily come into the measuring systems giving rise to a severe breakdown. In this respect, a hybrid sensor has been designed and fabricated in order to overcome the shortcoming of these two types of sensors. For this purpose, the experimental comparison with commercialized products has been also carried out. In this concept of the hybrid sensor, two different impedances could provide the passage of the signals. In this way, the discrimination of the noise could be accomplished very effectively with high ratio of signal over noise(S/N) under the little influence from the external noises and the breakdown.
Purpose : To evaluate the factors which contribute to the time of the first stool and the first urine passage. Methods : We retrospectively reviewed a chart of 1,221 infants ${\geq}34$ weeks of gestational age admitted to the normal newborn nursery of Il Sin Christian Hospital, Busan, from November 2004 to April 2005. We compared the time to first stool and urine according to maternal factors(maternal age, parity, mode of delivery, meconium-stained amniotic fluid, and maternal diabetes) and infant factors (gender, Apgar score, gestational age, type of feeding during first 24 hours, age at the first feeding, number of feeds during the first 12 hours and age at discharge). Results : In total, 95.3 percent of our infants had passed their first stool by 24 hours and 99.8 percent of them had a stool by 36 hours. A total of 95.8 percent of our infants had passed urine by 24 hours of age and 98.3 percent of them by 36 hours. Comparing preterm and term infants, the time to first urine is $6.5{\pm}5.8$ hours and $12.1{\pm}6.6$ hours, respectively(P=0.000). The time to first stool is $20.7{\pm}13.5$ hours and $10.0{\pm}6.3$ hours, respectively(P=0.000) Early-fed infants were significantly earlier in time to first urine(P=0.023) and first stool(P=0.012). There was no statistically significant relationship between the number of feeds in 0-12 hours, mode of delivery, Apgar score, parity, gender, type of feeding, maternal diabetes and the time of the first urine and first stool. Conclusion : Gestational age, birth weight and age at first feeding were significantly related to the time of the first urine and first stool passage. When there is delayed passage of the first urine and first stool, we should consider close observation of other associated symptoms and other factors previously mentioned, to avoid extensive evaluation and intervention.
Seventy four Holstein heifers were randomly assigned over three trials to PRID-7+PG-6 and Synchromate B-9 regimens to synchronize estrus cycle for embryo transfer. Sexual behaviors; moounting, standing, orientation, chin-resting, sniffing, licking, rubbing and butting, vaginal swelling and mucus discharge were observed between 06-08, 12-14 and 18-20 h on 1st day and 00-02, 06-08, 12-14 and 18-20 h on 2nd day after removal of hormones. Synchromate-B treatment (81.6%) showed higher synchronized estrus rate than PRID treatment (77.8%) during observation period. Standing estrus was observed within 74 h after PG injection in PRID and within 52 h after removal of implant in Synchromate-B. About 68% of heifers in PRID and 74% of heifers in Synchromate-B showed standing estrus between 0-14 h on 2nd day after removal of the hormones. Synchromate-B resulted in a tighter synchrony of standing estrus than PRID. Incidence of average mounting and standing per head during observation period was 22.3 and 16.6 in PRID and 28.1 and 13.6 in Synchromate-B. The PRID showed peak in active mounting at 18-20 h on 1st day, however, the Synchromate-B showed at 0-2 h on 2nd day after removal of hormone. Active standing was shown between 18 h on 1st day to 20h on 2nd day in PRID, however, between 0-14 h on 2nd day after removal of hormone is Synchromate-B. There was slight difference in pattern of active mounting and standing during estrus between PRID and Synchromate-B. Conception rate of synchronized heifers transferred with fresh and frozen embryos by non-surgical and surgical methods was higher in synchromate-B (62.5%) than in PRID (38.5%). Chin-resting showed highest incidence among 6 sexual behavioral components in the both treatments. Synchromate-B showed higher incidence of chin-resting (16.6) than PRID (10.7). Synchromate-B group showed also higher incidence of orientation, sniffing and butting than PRID group. Synchromate-B resulted in more active sexual behaviors than PRID. The pattern of incidence of chin-resting, licking and butting was almost symmetrical in PRID with their peak values at 6-8 h on 2nd day, however in Synchromate-B chin-resting and sniffing was symmetrical with their peak values at 12-14 h on 2nd day after removal of hormone. There was tendency to increase vaginal swelling according to time passage of synchronized estrus in the both treatments. Incidence of mucus discharge in Synchromate-B was slightly higher than in PRID. Twenty to 40% was false negative in conception rate by tall painting before re-estrus day as judged by rectal palpation.

Rapid industrialization and urbanization have resulted in an increase in impervious areas and an increase in runoff, therefore, this causes more flooding and damage in urban areas. This study has analyzed the effects of improvements to the roughness coefficient in storm sewer pipes on flood runoff and outflow through rainfall-runoff simulations. The simulations are implemented by three scenarios to evaluate effects of improvements to the roughness coefficient for the improved length ratio to the total length, diameters and mainlines of sewer pipes. The size and length of the sewer mains are large and long to effectively increase the flow rate to the outlet, secure the passage discharge capacity of the pipe and reduce the overflow. It is effective for flood reduction that the improvement to roughness coefficient is first conducted in mainlines with longer lengths and larger diameters. The results from this study can provide a guideline for prioritizing of the sewer pipe replacement.

The section of this study is the geological vulnerable zone where groundwater leakage occurred through the tunnel barrier during excavation of the shield tunnel boring machine(TBM) for the construction of the electric power unit. Therefore, a Three D imensions(3D) numerical analysis was performed to analyze the actual situation from before construction to the time when the change in groundwater level occurred, and to reflect the surrounding ground conditions based on the observed change in groundwater level during construction. As a result of the study, the correlation between groundwater level change and tunnel construction around the site was identified. Therefore, it was similar to the measurement result of groundwater level at the target ground. The amount of groundwater discharge to the entrance of the tunnel construction was also similar to the actual measured result, and the numerical analysis method and modeling in this study were analyzed to reflect the site conditions.
Esophagel atresia and tracheoesophageal fistula may occur as separate entities but usually occur in combination. First described by Durston in 1670, esophageal atresia was not successfully treated until 1939 when Ladd in Boston and Leven in St. Paul obtained the first survivors utilizing the methods of gastrostomy, esophagostomy and extrapleural ligation of the tracheoesophageal fistula as multiple operations which required months of hospitalization. Two years later Cameron Haight performed the first successful primary repair and afterward about 2000 cases of esophageal atresia with distal tracheoesophageal fistula reported in the world. In Korea, there appeared about 27 cases in the literature and 8 successful repaired cases noted in these year. Anther report two cases of esophageal atresia, of which one case was successfully treated with Haight`s method. Case 1.: Normal full term delivered boy with chief complaints of respiratory difficulty and persistent drooling with chocking, 3. lkg, was admitted with emergency 5 hours after delivery. Physical findings revealed no specific abnormal signs except distended abdomen and grunting respiration. Esophagograrn and bronchogram revealed proximal esophageal atresia and distal tracheoesophageal fistula proximal to the carina. Parent refused operative therapy and patient died 24 hours after discharge. Case 2. :3. lkg. normal full term delivered girl was admitted 4 days after delivery with chief complaints of regurgitation after feeding, chocking, cyanotic spell and fever since the day after delivery. Physical examination revealed persistent drooling, grunting respiration, and fever with moderate dehydration. Tracheoesophageal suction and fluid therapy with antibiotics improved her condition and subsided ]pneumonic condition. Esophagogram revealed markedly dilated proximal esophagus as blind loop and stomach distended with gas, and repairing operation as Haight`s method was performed on the 7th day after delivery. Patient tolerated all the operative procedure well and recovered uneventfully. Esophagogram on the 7th postoperative day showed passage of the lipiodol through the anastomotic side with moderate stricture,and feeding permitted. Patient tolerated all the feeding amount well and discharged on the 11th postoperative day. Followup revealed intermittent regurgitation after feeding and corrected with bougination.
History of Instream Flow Incremental Methodology (IFIM) Following the large reservoir and water development era of the mid-twentieth century in North America, resource agencies became concerned over the loss of many miles of riverine fish and wildlife resources in the arid western United States. Consequently, several western states began issuing rules for protecting existing stream resources from future depletions caused by accelerated water development. Many assessment methods appeared during the 1960's and early 1970's. These techniques were based on hydrologic analysis of the water supply and hydraulic considerations of critical stream channel segments, coupled with empirical observations of habitat quality and an understanding of riverine fish ecology. Following enactment of the National Environmental Policy Act (NEPA) of 1970, attention was shifted from minimum flows to the evaluation of alternative designs and operations of federally funded water projects. Methods capable of quantifying the effect of incremental changes in stream flow to evaluate a series of possible alternative development schemes were needed. This need led to the development of habitat versus discharge functions developed from life stage-specific relations for selected species, that is, fish passage, spawning, and rearing habitat versus flow for trout or salmon. During the late 1970's and early 1980's, an era of small hydropower development began. Hundreds of proposed hydropower sites in the Pacific Northwest and New England regions of the United States came under intensive examination by state and federal fishery management interests. During this transition period from evaluating large federal reservoirs to evaluating license applications for small hydropower, the Instream Flow Incremental Methodology (IFIM) was developed under the guidance of the U.S. Fish and Wildlife Service (USFWS).

Bathymetric changes were studied in the southern sea off the Jinwoo-do Island, which is one of the deltaic barrier islands surrounding the Nakddong river estuary. In this study, 16 bathymetry data sets were obtained from June 2006 to April 2015. Two narrow channels, the one lying between Jinwoo-do and Shinja-do, and the other one lying between Nulcha-do and Jinwoo-do extended into the eastern and western parts of the study area, respectively. The eastern extension of the channel contained a passage of mixed estuarine waters of seawater and river water discharged from the Nakdong river barrier and the west Nakdong River. The western channel connected the Nakdong River estuary with the Busan New Port via a connecting pier. Total volumetric changes of sediments in study area and discharge flow of the Nakdong river barrier were analyzed. Bottom topographical changes occurred mainly in the eastern extension of the channel. These changes were initially characterized by gradual erosion or deposition followed by rapid restoration. The total volume of sediment gradually increased from June 2006 to March 2013, but experienced a sudden decrease in October 2013 because of typhoon Danas. Few fluctuations were observed from October 2013 to April 2015. Analysis of the cross-sectional bathymetry of the north-south direction showed that the deepest point of the eastern channel moved 100-130 m westward and 200 m northward between June 2006 and April 2015.
