  • Background: Promoting patients' safe return home at discharge and reducing length of stay in hospital is key for Restorative Rehabilitation Institution (RMI). Objects: This study was designed to identify the factors influencing the return to home and length of stay among various factors. Methods: A total of 120 stroke patients (76 males and 44 females) who were hospitalized in an adult inpatient unit of a RMI for more than 2 months were retrospectively analyzed for this study (multivariate logistic regression analyses, p < 0.001). As predictor variables for assessing the return to home and length of stay, demographic data (sex, age, duration between onset and admission, length of stay, caregiver after discharge, occupation after discharge, reason for discharge, and household type after discharge) were collected. Additionally, following measurements were selectively collected from patient's medical records: scores of Mini-Mental State Examination Korean version (K-MMSE), modified Barthel Index Korean version (K-MBI), Berg Balance Scale and Functional Ambulation Category were obtained at admission and discharge. Results: The K-MMSE at admission and K-MBI at discharge were found to be the predictors of return to home. Additionally, K-MBI at admission influenced the length of stay. Conclusion: This study suggests cognitive functioning at admission and the level of activities of daily living at discharge predicted the return to home and length of stay.

  • The effect of rotation on the discharge coefficient of orifices with various length-to-diameter ratios and two different inlet corner radii was studied. Length-to-diameter ratios of the orifices range from 0.2 to 10, while the inlet shapes are square edged, or round edges of radius-to-diameter ratio of 0.5. From the experiment, we found that rotational discharge coefficient and Rotation number, when based on ideal exit velocity of the orifice considering momentum transfer from the rotor, describe the effect of rotation very well. In this study, the discharge coefficients of rotating orifices are shown to behave similar to those of the well-known non-rotating orifices. For both rotating and non-rotating orifices, the discharge coefficients increase with the length-to-diameter ratio until a maximum is reached. The flow reattachments in the relatively short orifices are responsible for the increase. The coefficient then decreases with the length-to-diameter ratio due to the friction loss along the orifice bore. The length-to-diameter ratio that yields maximum discharge coefficient, however, increases with the Rotation number because the increased flow-approaching angle requires larger length-to-diameter ratio for complete reattachment. The length-to-diameter ratio for complete reattachment is shorter for round edged orifices than that of square edged orifices by about a unit length-to-diameter ratio.

  • Pressure coefficient in rotating discharge hole was measured to gain insight into the influence of rotation to the discharge characteristics of rotating discharge hole. Pressure measurements were done by the telemetry system that had been developed by the authors. The telemetry system measures static pressure using piezoresistive pressure sensors. Pressure coefficients in rotating discharge hole were measured in longitudinal direction and circumferential direction with various rotating speed and 3 pressure ratios. From the results, the pressure coefficient, and therefore the discharge coefficient, is known to decrease with the increase of Ro number owing to the increase of flow approaching angle to the discharge hole inlet. However, there exists critical Ro number where the decrease rate of discharge coefficient with the increase of Ro number changes abruptly; flow separation occurs from the discharge hole exit at this critical Ro number. Critical Ro number increases with the increase of length-to-diameter ratio, but the increase is small where the length-to-diameter ratio is higher than 3. The decrease rate of discharge coefficient with the increase of Ro number depends on the pressure recovery at the discharge hole, and the rate is different from each length-to-diameter ratio; it has tendency that the short discharge hole shows higher decrease rate of discharge coefficient.

  • This study was attempt to encourage for developing on rehabilitation delivery system and programs as a substitute service instead of hospitalization for accident patients at work, such as hospital based home health care nursing service. It needs vary substitutes service of hospitalization to curtail the length of stay for inpatients who were hospitalized with workers compensation insurance. It focused on developing an estimation of early discharge day of accident inpatients based on a detail statement of treatment for 115 inpatients who were hospitalized at General Hospital in 1997. This study has four specific purpose as follows. First, to find out the status of health service utilization. Second, to estimate the early discharge days and income increasing effect based on the early discharge for those patients. Third, to identify the factors to affect total medical expenditure and the length of stay for those inpatients. Forth, to figure out the need of utilizing home health care nursing service for accident patients. In order to analyze of the length of stay and medical expenditure for inpatients who were hospitalized due to the accident, the authors conducted with micro-analysis and macroanalysis from medical records and medical expenditure records. Micro-analysis was done by nominal group discussion of 4 expertise with the critical criteria. such as a decrease in the amount of treatment after surgery, treatments, tests, drugs and changes in the tests consistency, drug methods, vital signs, start of ROM exercise, doctor's order, patient's outside visiting ability, stable conditions. In addition to identify affected variables for medical expenditure. the length of stay and income effect due to early discharge day, the data was analyzed with multiple regression analysis and linear regression analysis model by SPSS-PC for windows and Excell program. Results of this study as follows. First, the mean length of stay was 37.1 days, whereas the mean length of stay due to early discharge was 28.2 days at the hospital. The estimation of early discharge days were shown that depends on the length of stay. The longer length of stay, the longer length of early discharge days, such as under 7 days length of stay patients was to estimated the mean length of stay was 4.9 days and early discharge days was 4.6. whereas the mean length of stay was 122.6 days and early discharge days was 92.0 respectively. The mean medical expenditure per day were found to be 133.409 Won. whereas the mean medical expenditure per day was shown negative linear trends according to the length of stay at the hospital. The estimation results of the income effect due to 11 early discharge days per bed was around 2,150,000 won. However, it means not the real benefits from early discharge, but the income increasing amount without considering medical prime cost in general hospital. Therefore, it needs further analysis on the cost containments and benefits under the considering as well turn over rates per bed as the medical prime costs. The length of stay was most significant and the sign was positive to the total medical expenditure, as expected. Surgery and patient's residential area also an important variable in explaining medical expenditure. The level of complications was most significant variable in explaining the length of stay. The level of the needs on horne health care nursing service which can be used for early discharge accident patients were shown very high. The needs distribution varied from 65.5% of patients and 88.9% of caregivers, to 96.4% of doctors, and 99.1% of nurses. In addition horne health nurse responded that they can be managed the accident inpatients from early discharge. From these research findings. the following suggestions has been drawn it needs to develop strategies on rehabilitation delivery system in order to focused on consumer's side which is planned for 21 century health policy in Korea. Vary intermediate facilities and horne health care would have been developed in the community based for comprehensive rehabilitation services as a substitutes of hospitalization for shortening the length of stay of hospitalizations. In hospital based horne health care nursing service, it's available immediately to utilize for the patients who wanted rehabilitation services as a substitutes of hospitalization under the cooperations with workers compensation insurance company.

  • Electrostatic precipitators(EPs) have low pressure drop and high dust collection efficiency and are widely used for industrial dust collectors. The current-voltage characteristics, which are important to maintain high dust collection efficiency, depend on several factor: discharge electrode shape, gas flow property, dust loading etc. In this study, experiments are performed to investigate the current-voltage characteristics of the corona discharge of various electrode geometries and an empirical model is proposed to predict current-voltage characteristics of the corona discharge. The corona onset voltage correction coefficient$(\alpha)$ and the geometry correction coefficient$(k_g)$ are used to the conventional equation for wire-plate type discharge electrode. The corona onset voltages are -6.3kV and almost constant when the numbers of discharge pins are varied from 3 to 9. The length of discharge pins has very sensitive effects on the corona onset voltage. They are increased from -6.3 to -7.8kV when the discharge pin length are 8.5 and 4.5mm, respectively. The empirical model shows good agreement with experimental results and can predict the effects of discharge pin length and number.

  • This study attempts to encourage the development of a rehabilitation delivery system as a substitute service for hospitalization such as a community based intermediate facility or home health care. We need substitute services for hospitalization to curtail the length of stay for inpatients due to car accidents. It focused on developing an estimation for early discharge based on a detailed statement of treatment from medical records of 109 inpatients who were hospitalized at General Hospital in 1997. This study has three specific purposes: First, to find the mean length of stay and mean medical expenditure. Second, to estimate the mean of early discharge from the mean length of stay. Third, to analyize the income effect per bed from early discharge. In order to analyze the length of stay and medical expenditure of inpatients the author conducted a micro and macro-analysis with medical expenditure records. To estimate the early discharge we examined with a group of 4 experts decreases in the amount of treatment after surgery, in treatments, in tests, in drug methods. We also looked their vital signs, the start of ROM exercise, the time removel, a patient's visitations, and possible stable conditions. In addition to identifing the income effect due to an early discharge, the data was analyzed by an SPSS-PC for windows and Excell program with a regression analysis model. The research findings are as follows: First, the mean length of stay was 47.56 days, but the mean length of stay due to early discharge was 32.26 days. The estimation of early discharge days was shown to depend on the length of stay. The longer the length of stay, the longer the length before discharge. For example, if the patient stayed under 14 days the mean length of stay was 7.09 while an early discharge was 6.39, whereas if the mean length of stay was 155.73, the early discharge time was 107.43. The mean medical expenditure per day of car accident patients was found to be 169,085 Won, whereas the mean medical expenditure per day was shown to be in a negative linear form according to the length of stay. That is the mean expenditure for under 14 days of stay was 303,015 Won and the period of the hospitalization of 15 days to 29 days was 170,338 Won and those of 30 days to 59 days was 113,333 Won. The estimation of the income effect due to being discharged 16 days was around 2,350,000 Won with a regression analysis model. However, this does not show the real benefits from an early discharge, but only the income increasing amount without considering prime medical cost at a general hospital. Therefore, we need further analysis on cost containments and benefits incending turn over rates and medical prime costs. From these research findings, the following suggestions have been drawn, we need to develop strategies on a rehabilitation delivery system focused on consumers for the 21st century. Varions intermediate facilities and home health care should be developed in the community as a substitute for shortening the length of stay in hospitals. In home health care cases, patients who want rehabilitation services as a substitute for hospitalization in cooperation with private health insurance companies might be available immediately.

  • The discharge coefficients of critical sonic nozzles were obtained in a high pressure gas flow standard system, which was a gravitational weighing system. The discharge coefficients of critical sonic nozzle farbricated according to ISO specifications are in good agreement with ISO correlation. The discharge coefficients for small inlet radius decrease significantly as the inlet length become short due3 to separation at the sharp-edged inlet. For nozzles having long inlet radius, the effects of inlet length on the discharge coefficients were relatively small, but the effects become significant at the short inlet length. The effect of separation at the sharp-edged inlet is stronger than that of the boundary layer growth. The experimental results support that the shape of critical sonic nozzles suggested by ISO specifications is excellent.

Results of this study were as follows. First. the mean length of stay was 50.3 days. whereas the mean length of stay due to early discharge was 34.3 days at the hospital. The estimation of time of early discharge depended on the length of stay. The longer the length of stay, the longer the length of time of early discharge : for instance a length of stay under 10 days was estimated as correlating to a mean length of stay of 6.6 days and early discharge of 6.5. The mean length of stay was 217.4 days and the time of early discharge was 110.1 respectively. The mean medical expenditure per day was found to be 169.085 Won and the mean medical expenditure per day showed negative linear trends according to the length of stay at the hospital. The estimation results of the income effect due to being discharged 16 days early was around 2,244,000 won per bed. However. this sum does not represent the real benefits resulting from early discharge, but rather the income increasing amount without considering medical prime cost in the general hospital. Therefore, further analysis is required on the cost containments and benefits as turn over rate per bed as the medical prime costs. • Estuarine dams are a recent and global phenomenon. While estuarine dams can provide the benefit of improved freshwater resources, they can also alter estuarine processes. Due to the wide range of estuarine types and estuarine dam configurations, the effect of estuarine dams on estuaries is not well understood in general. To develop a systematic understanding of the effect of estuarine dam location and freshwater discharge interval on a range of estuarine types (strongly stratified, partially mixed, periodically stratified, and well-mixed), this study used a coupled hydrodynamic-sediment dynamic numerical model (COAWST) and compared flow, sediment transport, and morphological conditions in the pre- and post-dam estuaries. For each estuarine type, scenarios with dam locations at 20, 55 and 90 km from the mouth and discharge intervals of a discharge every 0.5, 3, and 7 days were investigated. The results were analyzed in terms of change in tide, river discharge, estuarine classification, and sediment flux mechanism. The estuarine dam location primarily affected the tide-dominated estuaries, and the resonance length was an important length scale affecting the tidal currents and Stokes return flow. When the location was less than the resonance length, the tidal currents and Stokes return flow were most reduced due to the loss of tidal prism, the dead-end channel, and the shift from mixed to standing tides. The discharge interval primarily affected the river-dominated estuaries, and the tidal cycle period was an important time scale. When the interval was greater than the tidal cycle period, notable seaward discharge pulses and freshwater fronts occurred. Dams located near the mouth with large discharge interval differed the most from their pre-dam condition based on the estuarine classification. Greater discharge intervals, associated with large discharge magnitudes, resulted in scour and seaward sediment flux in the river-dominated estuaries, and the dam located near the resonance length resulted in the greatest landward tidal pumping sediment flux and deposition in the tide-dominated estuaries.

  • In this study, the partial discharge degradation properties for 2-core PVC cable($2cores{\times}1.5mm^2$ cross section, length of 10 cm, 20 cm, 30 cm) following immersion with the salt water that the 2%, 4%, 8% of NaCl is dissolved in 100 g of distilled water for 48 and 96 hours has been measured. The results of this study are as follows. When the degradation time in salt water of 2% NaCl is 48 hours, it found that the number of partial discharge increased as about 40 pps, 50 pps, 90 pps with increasing the length of cable to 10 cm, 20 cm, 30 cm. In case the concentration and degradation time is same, the inception and extinction voltage decreased with increasing the length of cable. When the degradation time in salt water is 96 hours and the length of cable is 20 cm, it found that the number of partial discharge decreased as 3,000 pps, 500 pps, 100 pps with increasing the concentration of NaCl to 2%, 4%, 8%.
