Treatment of Combat-related Gunshot and Explosive Injuries to the Extremities (전투 상황에서 발생한 사지 총상 및 폭발창의 치료)

  • Purpose: We should prepare proper medical service for disaster control as South Korea is not free from terrorism and war, as we experienced through the two naval battles of the Yeonpyeong, one in 1999 and the other in 2002, the sinking of Cheonan in 2010, and the attack against the border island of Yeonpyeong in 2010. Moreover, North Korea's increasingly bellicose rhetoric and mounting military threats against the world demand instant action to address the issue. The aim of this article is to describe our experience with three patients with combat-related gunshot and explosive injuries to their extremities and to establish useful methods for the management of patients with combat-related injuries. Methods: Three personnel who had been injured by gunshot or explosion during either the second naval battle of the Yeonpyeong in 2002 or the attack against the border island of Yeonpyeong in 2010 were included in our retrospective analysis. There were one case of gunshot injury and two cases of explosive injuries to the extremities, and the injured regions were the left hand, the right foot, and the right humerus. In one case, the patient had accompanying abdominal injuries, and his vital signs were unstable. He recovered after early initial management and appropriate emergency surgery. Results: All patients underwent emergent surgical debridement and temporary fixation surgery in the same military hospital immediately after their evacuations from the combat area. After that, continuous administration of antibiotics and wound care were performed, and definite reconstructions were carried out in a delayed manner. In the two cases in which flap operations for soft tissue coverage were required, one operation was performed 5 weeks after the injury, and the other operation was performed 7 weeks after the injury. Definite procedures for osteosynthesis were performed at 3 months in all cases. Complete union and adequate functional recovery were achieved in all cases. Conclusion: The patient should be stabilized and any life-threatening injuries must first be evaluated and treated with damage control surgery. Staged treatment and strict adherence to traditional principles for open fractures are recommended for combat-related gunshot and explosive injuries to the extremities.

The Relationship between Somatic Symptoms and the Trauma-Related Characteristics of Patients with PTSD : A Korean Multicenter Study (다기관 모집 외상 후 스트레스장애 환자에서 외상의 특성과 신체적 증상과의 관계)

  • Objective : The aim of this study is to examine the characteristics of somatic symptoms in patients with PTSD according to trauma type. Methods : The subjects of this study were 84 patients diagnosed with PTSD according to the DSM-IVTR and CAPS criteria. The subjects ranged in age from 18 to 76 years, and they were recruited from 18 hospitals across the nation. All participants were asked about their trauma history, and they all completed the Davidson Trauma Scale (DTS), Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), Impact of Event Scale-Revised (IES-R) and Korean-Physical Somatic Symptom Inventory (K-PSI). The items of the K-PSI were categorized into 5 groups according to organ system, and the participants were classified into 6 groups according to trauma type. We compared the results of the social demographical scale, DTS, BDI, STAI, IES-R and each recategorized subscale of the K-PSI among the six groups. Results : There were significant differences between the 6 groups in terms of the Korean-Physical Somatic Symptom Inventory scores for each organ system, except for the cardiopulmonary system, as well as the BDI scores. Post hoc analysis revealed differences between the combat-related trauma group and all other groups but not among any of the other groups. Conclusion : Our result showed that there were significant differences in the somatic symptom scores among the 6 trauma groups. However, patient age and the time elapsed since the traumatic event may have hada crucial influence on the result of this study.

Contents Analysis of First Aid in Elementary, Middle and High School Textbooks (응급처치에 대한 초.중.고등학교 교과서 내용 분석)

  • Purpose: The purpose of this study was to provide basic data to develop first aid education programs for elementary, middle and high school students. Method: Forty nine textbooks adopted by first to six grade elementary schools and first to third grade of both middle and high schools were analyzed for content on first aid. Results: 1) First aid content was covered only in courses on and 2) Content on first aid was included in the categories, "General of first aid", "Rescue & moving", "Fire & Burns", "Cardiopulmonary resuscitation", "Musculoskeletal injuries", "Thermal injuries", "Wounds", "Removing foreign bodies", "Disaster & Accidents" and "Others". Categories that were not included were "Poisoning" and "Bites". The content did not follow a progression related to the level of intellectual and physical development of the students. Conclusion: These results suggest that the content on first aid should be revised to be practical and rational for the benefits of student safety.

  • 현대의 건축 재료는 각종 고분자 화합물의 비중이 커지고 있어 건축 재료가 연소할 때 발생하는 가스의 성분비가 천연재료를 이용하던 과거의 건축물과 차이를 보인다. 실제로 근래의 건축재료 연소가스의 정성분석결과에서도 천연재료의 연소가스에서 발견되는 CO, $CO_2$ 외에도 HF, HCN, HBr, HCl, NO, $NO_2$, $SO_2$등의 화학종이 발견되고 있다. 이러한 화학종들은 고분자 화합물에서 발생하는 것으로 분석되며, 건축 재료에서 화합물의 비중이 높아지는 만큼 향후 이러한 화학종들이 건축물의 화재에서 대피의 주요 방해요인으로 작용할 수 있다. 이에 본 연구에서는 HF를 대상으로 동물 노출실험을 통하여 얻은 결과와 HF에 대한 기존연구를 비교 분석하여 HF의 독성에 대한 기초 연구를 수행하였다.

Improving the disease control ratings to manage livestock contagious animal disease in farms (가축 전염병 관리를 위한 농장 질병관리등급제 개선 방안)

  • The emergence of highly contagious viral diseases of livestock and poultry such as foot-and-mouth disease (FMD), African swine fever (ASF) and highly pathogenic influenza (HPAI) have necessitated the formulation of policies and regulatory frameworks for preventing ingress of exotic diseases and controlling dissemination of endemic diseases within the country. The disease control ratings to manage contagious livestock disease in farms have not being widely used because the evaluation parameters were not being considered inappropriate. The purpose of this study was to reinforce the validity of the parameters for evaluation factors and the objectivity of the scoring criteria in the application of the current disease management rating system. The diseases subject to intensive management were improved by reflecting farm situation. For the evaluation based on the disease grading system of farms, the diseases subject to intensive management were realized, the list of diseases for evaluating the vaccination rate was revised, and the quarantine and sanitation management standards were revised. The effectiveness of the improved livestock disease management system was confirmed through the application of actual pig farms. Therefore, this system will be used as a means to evaluation the biosecurity at the farm units. Through this, it will contribute to establish effective biosecurity measures for each farm by improving and supplementing the present disease grading system.

Analysis of the bachelor's degree course curricula of paramedicine departments in colleges (전문대학 응급구조학과 전공심화 교과과정 운영현황 및 분석)

  • Purpose: The study aims to provide basic data for understanding the department of paramedicines' bachelor's degree courses to enable curriculum development and reorganization. Methods: The 2023 academic curricula for all eleven universities with active departments of paramedicine offering bachelor's degrees were investigated from April 25 to May 10, 2023. Results: The average courseload was 22.0 units. There were 7.73 major subjects, with 19.45 units on average. The culture subject was available in 7 colleges and was 2.55 units on the average 1.18 subject. Grouping the major subjects showed that 15.3% of subjects were related to advanced paramedicine (the most common course), followed by 11.8% related to research methodology and seminar, and 10.8% related to radiology and simulations. Related subjects in statistics, disaster, and forensic science were also offered. Conclusion: This study found that the curricula of the departments of paramedicine differed from that of the general bachelor's degree. Going forward, university leaders should organize the paramedicine curriculum considering the environmental changes in emergency medicine and the scalability of EMT-Paramedic jobs.

A case report overlapped vaccine and COVID-19 in disseminated atherosclerosis

  • Coronavirus disease 2019 (COVID-19) has become a part of our lives now and we have no more effective way of coping than a vaccine. COVID-19 is a disease that causes severe thrombosis outside the respiratory tract. Vaccines also protect us in this respect, but in some rare cases, thrombosis has been found to develop after vaccination (much less frequently than COVID-19). What was interesting in our case was that it showed how a disaster could happen under three factors that predispose to thrombosis. A 65-year-old female patient with disseminated atherosclerosis was admitted to the intensive care unit with complaints of dyspnea and dysphasia. In the evening of the day, the patient had the vaccination 2 weeks ago, she had active COVID-19. On examination, lower extremity pulses could not be detected. The patient's imaging and blood tests were performed. Multiple complications such as embolic stroke, venous and arterial thrombosis, pulmonary embolism, and pericarditis were observed in the patient. This case may give consideration to anticoagulant therapy studies. We give effective anticoagulant therapy in the presence of COVID-19 in patients at risk of thrombosis. Can anticoagulant therapy be considered after vaccination in patients at risk of thrombosis such as disseminated atherosclerosis?

Job analysis of paramedic on the developing a curriculum method (데이컴 기법에 의한 1급 응급구조사 직무분석)

  • Purpose: The purpose of this study was to investigate the extended job of paramedic after the 1st job description in 2000 by the National Health Personnel Licensing Examination Board. Methods: This study was carried out from May 2012 to July 2013 on the Developing A Curriculum (DACUM) method to analysts 330 responses among 400 questionnaires (response rate 82.5%) after analysing 30 pilot questionnaires. In order to enhance validity and reliability of this job description, 2 job analysts in universities and 10 EMT-Paramedics in hospitals and fire stations were recruited. Results: There were differences between 166 (year 2000) and 240 (2012) in task element. There were higher answering at triaging patient(2.64 frequency, 4.15 importance), calling for help(2.68 frequency, 4.07 importance) in disaster emergency care. There were higher answering at measuring vital sign (3.95 frequency, 4.22 importance), measuring ECG(3.84 frequency, 4.14 importance), wound care(3.48 frequency, 3.86 importance), bleeding control & wound care(3.46 frequency, 3.86 importance), applying $O_2$(3.40 frequency, 3.94 importance) in physician assistant. Conclusion: Scope of extended practice as a physician assistant in hospital setting recommended us to replace the present curriculum and national exam with new curriculum and national written & skill test.

Improvement for the Job Safety in the Scene Activities of Fire Fighters (소방공무원의 현장활동 안전성 확보를 위한 개선방안)

  • This study was performed to propose measures to improve and secure safety in the scene activities of firefighters. First, the majority of firefighters thought that working environment and equipment in fire station were inadequate for perforeming work tasks safely. Second, influential factors threatening safe scene activities mainly arose externally such as insecure working conditions of firefighting or emergency scene. Third, improvement in working environments was pointed out as the most urgent issue to secure safety. Firefighters recognized the expansion of field workforce in system, personnel management and the foundation of hospitals for firefighters hospital in working environment, and the development and distribution of safe equipment and change and inspection of air respirator facepiece on regular basis in equipment as urgent issues to be improved.
