Background: The study aimed to determine the association of individual cognitive ability in late midlife with labor market participation among older workers. Methods: This prospective cohort study estimates the risk of long-term sickness absence, disability pension, early retirement, and unemployment from scores on the Intelligenz-Struktur-Test 2000R by combining data from 5076 workers from the Copenhagen Aging and Midlife Biobank with a register on social transfer payments. Analyses were stepwise adjusted for age, gender, physical and psychosocial work environment, health behaviors, occupational social class, education, and chronic diseases. Results: In the fully adjusted model, low cognitive ability (≥1 standard deviation below the mean for each gender) and high cognitive ability (≥1 standard deviation above the mean for each gender) were not associated with risk of any of the four labor market outcomes. Conclusion: Individual cognitive ability in late midlife was not associated with risk of long-term sickness absence, disability pension, early retirement, and unemployment in the fully adjusted model. Thus, no direct effect of individual cognitive ability in late midlife was observed on the risk of permanently or temporarily leaving the labor market.
Objectives: The purpose of the study is to investigate the change of the salivary flow rate, xerostomia, and oral health-related quality of life in the elderly people after the application of oral massage. Methods: The subjects were 101 elderly people at two senior welfare centers in Mokpo from November, 2012 to January, 2013 by the application of oral massage. The final subjects were 56 elderly people who participated in the massage more than 15 times of 20. They were measured for saliva flow rate, xerostomia, and OHIP. On the first and the last day, a self-reported questionnaire was completed by the elderly people. The questionnaire consisted of general characteristics of the subjects, oral health related knowledge, symptoms and behavior of xerostomia, and OHIP. OHIP included functional limitation, physical pain, psychological discomfort, physical disability, psychological disability, social disability, and handicap, and was measured by Likert 5 point scale. Results: The salivary flow rate of the elderly people increased after the implementation of the program. There was a significant improvement in xerostomia, functional limitation, physical pain, psychological discomfort, and social efficacy (p<0.001). Conclusions : The oral massage program enhanced the oral function of the elderly people, and had an influence on the improvement of oral health-related quality of life.
PURPOSE: The aim of this study was to investigate the effect of scapular dynamic taping on pain, disability, upper body posture, and range of motion (ROM) in the postoperative shoulder. METHODS: Twenty-two patients who underwent acromioplasty and rotator cuff repair surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, n=11) and a control group (CG, n=11). For the EG, exercise therapy, manual therapy, and scapular dynamic taping were applied for 6 weeks; for the CG, only exercise therapy and manual therapy were applied for 6 weeks. Shoulder pain, disability, upper body posture, and ROM were evaluated at baseline, after 3 weeks of intervention, and after 6 weeks of intervention. Assessment tools included quadruple visual analog scale (QVAS) for level of pain; shoulder pain and disability index (SPADI) for functional disability level; forward head angle (FHA), forward shoulder angle (FSA), and pectoralis minor index (PMI) for upper body posture; and ROM testing. RESULTS: Significant differences were observed between the EG and CG in SPADI total scores; internal rotation and external rotation ROM of the glenohumeral joint ; FSA ; and PMI. All groups showed statistically significant improvement in QVAS; SPADI; flexion; abduction; external rotation and internal rotation ROM of the glenohumeral joint; FSA; and PMI. CONCLUSION: These results suggest that, for patients who have undergone acromioplasty and rotator cuff repair surgery, the addition of scapular dynamic taping during therapy is effective for improvement of shoulder disability level, ROM, and upper body posture.
On September 12, 2018, President Jae-In Moon announced the Comprehensive Plan for Lifelong Care for People with Developmental Disabilities, with representatives from the associated government branches (Ministry of Health and Welfare, Ministry of Education, and Ministry of Employment and Labor) in attendance. The goals of this plan are to provide health, medical, rehabilitative, special education, and social welfare services according to the life-stages of the affected individuals; to reduce parental pressure; to promote social interventions; and to enhance community-level participation in order to create a 'welfare society in harmony.' However, in order for the plan to succeed, additional efforts must be made in the following areas. First, an epidemiological survey is needed to understand the scale, prevalence, and incidence of developmental disabilities and to establish an evidence base to support policy development. Second, accurate definitions of developmental disabilities must be established in order to avoid policy discrimination based on impairment type and age. Third, personal evaluations to assess disabled individuals' unmet needs and customized service designs to deliver those needs are required. Fourth, the plan must fulfill the goals of accessibility and fairness that the government intends to provide. Fifth, the government should consider an integrated financial support system and to propose a detailed plan for monetary distributions. Finally, an integrated system that links health, medical, employment, educational, and welfare services must be constructed.
PURPOSE: This study aims to review the FUNfitness program, conducted by physical therapists as part of the Special Olympics Athlete Health Promotion Program, to provide basic data for the health management of individuals with intellectual disabilities. METHODS: This study collected and organized materials related to the Special Olympics Athlete Health Promotion Program, as well as training manuals for FUNfitness. Additionally, it referenced activity reports of the FUNfitness program developed through the collaboration between the American Physical Therapy Association and the Special Olympics. RESULTS: The results of this study present the definition and composition of the Special Olympics Athlete Health Promotion Program, its objectives, the definition and design objectives of the FUNfitness program, the composition and evaluation procedures of the testing and assessment items, and recommendations for referral for additional physical therapy interventions following FUNfitness education. CONCLUSION: This article describes the importance of physical therapists understanding physical therapy screening and management for the health care of individuals with intellectual disabilities, emphasizing the need for their continuous interest in promoting the health of individuals with intellectual disabilities.
The effects of regional health insurance on access to ambulatory care are examined in this paper. Access is measured as use-disability ratios. The data are collected in a household interview survey at Hwachon county before and after the introduction of regional health insurance. Before the introduction of regional health insurance, low-income class has less contacts with physicians than high-income class. This disparity in accessibility among economic classes is reduced with the health insurance coverage, but not removed, even after adjusting for health need.
연구목적 해양자원을 활용한 의미중심-마음챙김 기반의 집단스트레스중재프로그램이 기분, 수면, 삶의 질, 인지 기능의 긍정적인 변화에 미치는 효과를 알아보고자 하였다. 방 법 9명의 중증장애환자 간병가족을 대상으로 경상북도 울진군 해안 일대에서 3박4일간 해양자원을 활용한 집단프로그램을 시행하였다. 프로그램에 참여하기 전과 참여한 후, 그리고 6주 후에 기분, 수면, 삶의질, 인지기능 영역의 변화를 살피기 위해 주관적설문과 객관적평가를 실시하고 이를 통계분석하였다. 결 과 프로그램에 참여한 후 우울증을 비롯한 기분 상태가 유의하게 호전되었고, 이 효과는 6주 후까지 지속되었다. 참여자의 수면의 질과 삶의 질 및 인지기능은 참여 직후보다 프로그램 참여 6주 후에 보다 유의하게 향상되었다. 결 론 중증장애환자 간병가족 대상의 해양자원을 활용한 의미중심-마음챙김 기반의 집단스트레스중재프로그램은 정신적인 건강과 삶에 대한 만족도를 향상시키고, 인지적 효율성을 높인다는 측면에서 향후 유용하게 활용 가능한 프로그램으로 기대된다.
사람들은 노화가 진행되고 신체기능이 저하될수록 집 안에서 머무는 시간이 길어지기 때문에 주택환경의 영향을 더 많이 받게 된다. 그동안 주택환경과 건강의 관련성에 관한 연구는 다수 진행되었으나, 신체기능이 저하된 노인의 장애 발생과 우울에 미치는 주택환경의 영향을 밝히는 연구는 부족하였다. 이에 본 연구는 보행기능제한과 장애, 그로 인한 장애와 우울의 관계에 영향을 미치는 주택환경의 조절 효과를 검증하는 것을 목적으로 한다. 본 연구는 2018년 서울시 노인실태조사 자료를 활용하였으며, 주요 변수에 결측치가 있는 응답자를 제외한 총 3,027명을 대상으로 분석을 수행하였다. 결과분석에는 로지스틱 회귀분석과 다중회귀분석을 사용하였다. 연구결과 첫째, 주택환경에 문제가 있다고 보고한 사람들은 그렇지 않은 사람들에 비해 보행기능제한(OR=1.487, 95% CI: 1.250 - 1.770), IADL장애(OR=1.594, 95% CI: 1.303 - 1.949)와 우울(OR=1.943, 95% CI: 1.553 - 2.430) 수준이 높게 나타났다. 둘째, 주택환경은 보행기능 제한이 도구적 일상생활수행능력(IADL) 장애에 미치는 영향을 조절하는 것으로 나타났다. 즉, 열악한 환경에 거주하며 보행기능제한을 경험하는 노인의 IADL장애 수준이 그렇지 않은 집단보다 높은 것을 알 수 있다. 셋째, 주택환경은 보행기능을 통제한 후에도, IADL장애가 우울에 미치는 영향을 조절하는 것으로 검증되었다. 이는 열악한 환경에 거주하는 IADL장애 노인의 우울 수준이 그렇지 않은 집단보다 높다는 것을 의미한다. 본 연구결과를 기반으로 노년기 장애와 우울을 예방하고, 자신의 집에 오래 머물러 살 수 있도록 지원하기 위한 개입전략으로서 노인의 신체적, 기능적 특성을 반영한 주택환경 개선사업의 필요성과 방향성을 제시하였다.
As the nation is moving into the 21st century, the fundamental challenge facing local health departments in the United States is to improve the quality of peoples lives by preventing disease, injury, and disability through collaboration with public and private partners. PURPOSE: The present study was conducted to improve the understanding of the Korean Standard Functional, Disability, and Health Classification (KCF) and its ease of use in the clinical domain of the musculoskeletal system by comparing, analyzing, and linking the KCF codes with items from patient-reported outcome measures (PROMs), which are currently mainly used to evaluate patients with neck, shoulder, waist, and knee pain. METHODS: The items of the most widely used PROMs, the Neck Disability Index (NDI), Disabilities of the Arm, Shoulder, and Hand (DASH) scores, Oswestry Disability Index (ODI), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and the codes of the KCF were linked by two experts according to the linking rules. RESULTS: The concordance between the items of each of the PROMs and the KCF code linked by the two experts was NDI,86.4%86.4%, DASH 83.3%, ODI 92.0%, and WOMAC 80.7%. The NDI, DASH, and WOMAC indexes were found to comprise items corresponding to physical function, activity, and participation areas, and were linked to 22, 43, and 31 KCF codes, respectively. In addition to these two areas, the ODI included items related to environmental factors and was linked to 25 codes (duplicated codes are treated as one). CONCLUSION: This research can be used by adding the KCF code to the questions of the currently used evaluation tool. This coding can be easily applied and will contribute to the easy understanding of the KCF.
