A Study on the Determinants of the Incidence and the Transition of Older Adult Disability: Findings from the Korea Longitudinal Study of Aging(KLOSA) (노년기 장애발생과 장애정도의 변화에 미치는 영향요인 연구: KLOSA 1차와 2차 자료를 중심으로)

  • The purpose of this study is to investigate the factors associated with the incidence and the transition of disability among Korean older adults. Samples consist of 1,454(42.7%) men and 2,032(58.3%) women aged 65 and over who participated in the 1st and 2nd wave of the Korea Longitudinal Study of Aging: KLOSA. To estimate the level of disability, ADL and IADL disability indexes are used. As the results, major risk factors for ADL/IADL disability incidence include injury, vision problem, cognitive function, depression, health behavior, socioeconomic characteristics and age. Among the normal older adults, the odds ratio of having dementia symptoms at 2nd wave(2008) are 2.0 times greater for the older adults who have less cognitive function than those who don't have at 1st wave(2006). Among the older adults with chronic diseases, the odds ratio of having disability at 2nd wave are 1.8 times greater for the older persons who have depression than those who don't have at 1st wave. Secondly, concerning the predictors affecting the disability transition among the disabled older adults at 1st wave, the likelihood of remaining at the same level or deteriorating the level of IADL disability, as compared with improving the level, is associated with having less instrumental support or being older. These results indicate that it is necessary to prefer multilevel intervention in order not only to prevent the incidence of disability, but also to prolong the deterioration of disability in the older adults.

  • Angelman syndrome(AS) is a rare genetic neurological disorder. The main clinical characteristics of this syndrome are delayed neuropsychological development, intellectual disability, speech impairment, jerky movements especially hand-lapping, frequent laughter or smiling. AS is a classic example of genetic imprinting in that it is usually caused by deletion or inactivation of genes on the maternally inherited chromosome 15. The syndrome has oral manifestations such as diastemas, tongue thrusting, sucking/swallowing disorder, mandibular prognathism, frequent drooling, and excessive chewing behavior. The purpose of this paper is to describe the interesting aspects of the dental treatment of a childe with AS.

A Case Report of an Intervention Strategy that Applied an ICF Tool to Improve the Walking Ability of Stroke Patients (뇌졸중 환자의 보행능력 증진을 위한 ICF(International Classification of Functioning, Disability and Health) Tool을 적용한 중재전략의 증례)

  • Purpose: The objective of this study was to identify functional problems, including walking ability, of patients with strokes using the International Classification of Functioning, Disability, and Health (ICF) and to present a method that could solve functional problems, thereby determining the applicability of the ICF to increase the quality of evaluation and intervention in clinical fields in the future. Methods: Information on stroke patients who were admitted and treated in a hospital was collected. The authors conducted evaluations, interventions, and measurements of the results of the ICF tool in order to improve gait abilities of patients. The subjects were trained using proprioceptive neuromuscular facilitation (PNF) one hour a day and five times a week for four weeks. The result measurement variables were a six-minute gait test, 10 m velocity test, gait instability test, and measurements using the ICF sheet. Results: In the six-minute gait test, gait distance increased by 48 m, from 102 m to 150 m. The gait velocity test result showed an improvement from 0.36m/s to 0.44m/s. The subjects performed a gait instabilitytestwithacupfilledwith50mmwater. In the gait instability test, the amount of water was 38 mm before the intervention; however, it was 50 mm after the intervention. The gait velocity with a cup filled with water improved from 0.25m/s to 0.31m/s. Conclusion: An evaluation and intervention were designed with the ICF tool for stroke patients. Gait abilities improved when the PNF technique was used. The IFC method can be used for evaluation and intervention, and it could help improve gait abilities of stroke patients.

Current smoking rate and related factors of people with disability (장애유무에 따른 현재흡연율과 관련요인)

  • Objective : Smoking can increase the risk of secondary conditions for people with disabilities, which can in turn counteract primary disorders and exacerbate existing disorders. More active interventions for smoking cessation are necessary for people with disabilities because the physical condition of people with disabilities is more vulnerable, and risk factors such as smoking, drinking, obesity, and stress can have a decisive influence on people with disabilities compared to non-disabled people. Methods : In this study, the subjects who participated in National Health and Nutrition Survey of 2013 were those aged over 19 and smoker. The final 299 people with disability and 4,260 people without disability were analyzed. Results : Current smoking rate was 21.5% of non-disabled people and 21.4% of disabled people respectively. Factors influencing the current smoking rate of the disabled people were analyzed by type of household, economic status, home ownership, drinking, binge drinking, activity limit. Conclusions : In order to reduce socioeconomic inequalities of smoking and to actively intervene with the disabled, there need a new approach strategy focusing on people with disabilities.

COMPARISON OF DENTAL STAFF'S ATTITUDE TOWARD THE SPECIAL HEALTH CARE NEEDS PATIENTS BETWEEN THE SPECIAL DENTAL CLINIC AND THE GENERAL DENTAL CLINIC (장애인전문치과와 일반치과 의료진들의 장애인에 대한 태도 비교)

  • The aim of this research is to compare the attitude toward the special needs of dental staffs between the special dental clinic (SDC) and the general dental clinic (GDC). Surveys were done on 21 dental staffs (7 dentists and 14 dental hygienists) of SDC and 47 dental staffs (10 dentists and 37 dental hygienists) of GDC. Attitudes were rated on a questionnaire with the five-point Likert scale. Mann-Whitney test was utilized for statistical analysis. In SDC, more female dentists and dental staffs who have relatives with disability were working. Dental staffs of SDC showed that the more experience they had, the more positive attitudes toward the communication with the patients with special needs (p<0.05). Dental staffs of SDC represented more active attitudes toward the oral health promotion, treatment, research and volunteering (p<0.05). In conclusion, dental staffs who have personal motivation were working in SDC and the experience of dental treatment to the patients with special needs influenced the dental staffs' attitudes toward them. In future, a research about the attitude toward the disabled with more exact scale should be followed.

PATTERN OF CHECK UP AND DENTAL MANAGEMENT OF SPECIAL CARE NEEDS PATIENTS ACCORDING TO TYPES OF DISABILITIES (장애 유형별 치과 검진 및 관리에 대한 비교연구)

  • Regular dental visit of disabled patients is an important strategy for maintenance of oral health because of the lack of awareness of oral care. But there is limited information about follow up period in disabled patients after dental treatment. The aim of this study was to investigate the pattern of dental visit and dental management of special care needs patients according to the types of disabilities. A total of 140 patients who received dental treatment at Seoul National University Dental Hospital from 2010 to 2012 were reviewed. Patients were divided 3 groups according to the types of disabilities (disorder of external physical function, disorder of internal organ, psychic disability). Patient's characteristic, dental treatment performed, follow up period and frequency were investigated. The mean follow up period was 28.2 months and average frequency of visit was 3.69 times per year. There was no difference in period and annual visit between 3 groups, but significantly difference in cooperation with medical hospital in affiliation. Regular check-up was the most common dental treatment in all groups. In psychic disorder group, the proportion of sedative treatment was significantly higher than other groups. The results of this study provide information necessary for treatment planning and dental management of disabled patients.

The Status of Health-related Lifestyle in Massage Therapists with Visual Impairment (시각장애인 안마사의 건강관련 생활양식에 관한 연구)

  • The purpose of this study was to examine the status of health-related lifestyle according to the degree of disability, length of work, and type of work in massage therapists with visual impairment. A total of 236 visually impaired massage therapists who participated in refresher training in 2015 were participated in this study. Health-related lifestyle in this population was evaluated using validated health-related lifestyle questionnaire. Collected data were analyzed by $x^2$-testing using SPSS version 22.0. There were differences in regular breakfast and snack consumption according to the degree of disability. Massage therapists with shorter length of work more smoked and less consumed breakfast than massage therapists with longer length of work. Lastly, massage therapists who are working at massage parlor were more smoked and less regularly slept, consumed breakfast, and regularly rested than massage therapists who are working in other places. Collectively, degree of disability, length of work, and type of work impact health-related life-style in massage therapists with visual impairment.

Relationship between oral health quality of life and oral health-related factors in patients with successful dental implants (임플란트 완성 환자의 구강건강관련 삶의 질과 관련 요인)

  • Objectives : This study was purposed to estimate the level of oral health quality of life and the relationship between health-related factors and oral health quality of life in patients with successful dental implants. Methods : This questionnaire survey was conducted during the period from December 2009 to March 2010 with 126 patients confirmed successful dental implants in Busan. The oral health related quality of life and oral health-related factors were estimated with OHIP-14 and 8 oral health characteristics, respectively. Data analysis was performed with descriptive analysis, t-test, ANOVA, multiple regression analysis using SAS(ver 9.1) program. Results : The score of oral health quality of life in patients with successful dental implants was $12.25{\pm}7.82$. The scores of subscale of the oral health quality of life were $1.24{\pm}1.35$ in social disability, $1.60{\pm}1.37$ in handicap, $1.60{\pm}1.44$ in psychological disability, $1.70{\pm}1.38$ in functional limitation, $1.81{\pm}1.35$ in physical disability, $2.14{\pm}1.46$ in physical pain, $2.17{\pm}1.46$ in psychological discomfort, respectively. The related factors of oral health quality of life in patients with successful dental implants were absence of other type prosthesis and experience of tooth brushing education. Conclusions : The oral health related quality of life in patients with successful dental implants was relatively good condition. Social supports and chances for high quality denture and tooth brushing educations are needed to improve oral health related quality of life in patients with dental implants.

CURRENT STATUS OF SPECIAL CARE DENTISTRY EDUCATION OF DENTAL SCHOOLS IN KOREA (한국 치과대학 및 치의학 전문대학원의 장애인치과학 교육 현황)

  • Special care dentistry, also known as special needs dentistry, is concerned with the oral health of people who have intellectual disability, or who are affected by other medical, physical, or psychiatric issues. Dental schools must educate dental students for the competency in managing and treating individuals with special health care needs. The purpose of this study is to identify the current status of special care dentistry education in Korean dental schools. A questionnaire relating to the education of special care dentistry was sent to eleven dental schools in Korea via email. The result turned out that eight out of eleven schools had classroom teaching of special care dentistry, while only four had practical teaching programs. Nine schools had dental clinics for the disabled, in which two of them had special care clinics within the Pediatric dentistry clinic, and other seven in separate clinical sites. Despite the increased interest in special care dentistry, education including classroom teaching and practical teaching was still insufficient and needed more development. This study implicated a small but valuable understanding of special needs dentistry education in Korea.

Mediating Effect of Self-esteem on the Relationship between Economic, Health Characteristics and Life Satisfaction of Elderly People with Disability (고령장애인의 경제적 및 건강 특성과 삶의 만족도 관계에 대한 자아존중감의 매개효과)

  • This study is aimed at finding plans to enhance life satisfaction of elderly people with disability. For this, it analyzed the influence of economic characteristics and health characteristics upon life satisfaction, as well as a mediating effect of self-esteem. This study utilized data from the $7^th$ Panel Survey of Employment for the Disabled(PSED). The results are as follows, First, life satisfaction was high in the case of being employed and when old-age preparation was ready, current status of health, whether or not they need help in their daily life, chronic disease. Second, the mediate effect of self-esteem was analyzed for the influence of the plan for old age, good health condition and other's help in daily life upon life satisfaction. That is, those enhanced the self-esteem and it played an important role to improve life satisfaction. Based on such study results, this study recommends a number of the policy suggestions.
