장애 유형별 의료서비스 이용의 차이 (Differences in Utilization of Health Care Services by the Type of Disability)

  • The disabled population is a vulnerable group, having very complex medical conditions, but little is known about differences in the level of access by type of disability. This study was performed to investigate the differences of health care utilization by the type of disability. The database was constructed from registry of the disabled and health insurance and medical aid claims data submitted to the Korea Health Insurance Cooperation during in the year 2003. The disability classified three groups according to the Disabled Welfare Act; physically disability with external dysfunction, physically disability with organic disease, and mentally disability. There were huge differences in health care utilization by the type of disability. For the inpatient care, those with a mental disability were more likely to utilize health care services in terms of average visit number of medical facilities and visit days per case, but the treatment amount per case was the highest in physically disabled with organic disease. For the outpatient care, those who the physically disabled with organic disease were more likely to utilize health care services in terms of average visit number of medical facilities, treatment amount per case, and the treatment days per case. Also, those who physically disabled with organic disease were more likely to utilize general hospital for both inpatient and outpatient care, and spent more out-of-pocket expenditure. As the number of persons with disabilities rises, the need to consider new approaches to protecting their health grows increasingly. Especially, Korean health care system should be refined to be more responsive to the needs of the type of disability.

한국인 질병의 장애가중치 측정에 관한 연구 : 호주 장애가중치와의 측정 결과 비교를 중심으로 - (Disability Weights for the Korean Burden of Disease Study : Focused on Comparison with Disability Weights in the Australian Burden of Disease Study)

  • Objectives: This study aimed to measure the disability weights for the Korean Burden of Disease study, and to compare them with those adopted in the Australian study to examine the validity and describe the distinctive features. Methods : The standardized valuation protocol was developed from the Global Burden of Disease (GBD) study and the Dutch Disability Weights study. Disability weights were measured for 123 diseases of the Korean version of Disease Classification by three panels of 10 medical doctors each. Then, overall distribution, correlation coefficients, difference by each disease, and mean of differences by disease group were analyzed for comparison of disability weights between the Korean and Australian studies. Results : Korean disability weights ranged from 0.037 to 0.927. While the rank correlation coefficient was moderate to high ($r_s$=0.68), Korean disability weights were higher than the corresponding Australian ones in 79.7% of the 118 diseases. Of these, war, leprosy, and most injuries showed the biggest differences. On the contrary, many infectious and parasitic diseases comprised the greater part of diseases of which Korean disability weights were lower. The mean of the differ ences was the highest in injuries of GBD disease groups, and in cardiovascular disease, injuries, and malignant neoplasm of the Korean disease category. Conclusions : Korean disability weights were found to be valid on the basis of overall distribution pattern and correlation, and are expected to be used as basic data for broadening the scope of burden of disease study. However, some distinctive features still remain to be explored in following studies.

Mainstreaming of Students with Intellectual Disability in the Kingdom of Saudi Arabia: Special Education Teachers' Perceptions

  • Educators have been making strides in the research into and practices supporting the policy of mainstreaming students with disability. A move towards including students with intellectual disability in community schools with all the other students can be seen in many countries' education systems, including that of the Kingdom of Saudi Arabia. The 'rights of the child' has been the main argument put forward by advocates of this policy in an attempt to move from the medical to the social model. This study argues that, although mainstreaming can be viewed as a positive trend toward effective education, its implementation remains somewhat problematic. It is believed that more investigative research into professionals' attitudes is needed to improve service provision and inform the administration of mainstreaming practices. The attitudes of special education teachers on the policy of mainstreaming are examined and emerging key themes discussed. Furthermore, challenges that continue to inhibit mainstreaming practices in Saudi Arabia are identified.

장애인 건강검진 접근성 저해요인과 개선방안 도출에 대한 질적 연구 (Accessibility Factors to Health Check-Ups for People with Disability: A Qualitative Study)

  • Background: The purpose of this study was to identify factors inhibiting access of people with disability to health check-ups as well as identify pertinent solutions for improvement. Methods: Twenty-three people with disability older than the age of 19 who took respective health check-ups within the last 3 years were selected as participants. For the data collection, the 1:1 intensive interview was used. The data were analyzed by the grounded theory by Corbin and Strauss. Results: The results comprised nine categories, 23 subcategories, and 179 concepts. The central phenomenon was 'failure to obtain check-ups.' Causal conditions were observed as a 'lack of communication method,' 'physical difficulties,' and 'staff unfamiliar with people with disability,' Interventional conditions comprised 'physical accessibility,' 'staffs' competency,' and 'assistant manpower.' The active strategy was included 'to investigate the professional medical institution,' 'to find the medical institution of convenient traffic accessibility,' 'to overcome communication difficulties through equipment,' and 'to overcome linguistic barriers through sufficient communication.' Whereas, 'utilization of ancillary equipment,' 'the education of staffs on people with disability,' 'universal design manual,' and 'customized check-ups' were included in the passive strategy. Such processes arose in the contextual conditions of 'lack of expectations for daily lives' and 'lack of government support.' As a consequence, the subjects participated experienced the 'disadvantages,' 'discrimination,' and 'reduced reliability of the health check-ups.' Conclusion: The subjects who participated in this study emphasized 'staffs familiar with people with disability' and 'systems customized for people with disability' are mandatory to secure complete health check-ups for people with disability.

일부 지체장애인의 미치료 경험에 영향을 주는 결정요인 분석 (Determinants of untreated experiences among persons with physical disability)

  • Objective: This study is examined the factors affecting forms of untreated experiences in persons with physical disability. Method: The data collected from 461 persons with physical disability in community. Based on the Behavioral Model of Andersen, predisposing, enabling, and need factors are hypothesized to affect persons with physical disability's untreated experiences. The data were analyzed by statistical methods such as frequency and multiple logistic regression analysis. Result: Participation rates of untreated experiences were 26.7%. The significant factors of persons with physical disability's untreated experiences are predisposing factors (gender, partner, and religion), enabling factors (income, private insurance, information of assistive device, disability discrimination, and subjective discrimination), and need factors(subjective health status health screening and chronic disease). Also untreated experiences related to gender, subjective health status, health screening, and chronic disease factors using multiple logistic regression analysis. Conclusion: Implications of the findings were discussed and the recommendations for the improvement of health care utilization, subjective health statue. Especially, development of health education and program should be needed persons with physical disability.

The Influences of Disability of the New Disabled on Economical, Social and Cultural Exclusion

  • 본 연구는 장애 발생이 경제·사회·문화적 배제에 어떠한 영향을 미치는가를 살펴보고, 이들 변수 간의 관계에 있어서 사회환경이 조절변수로서 역할을 하는지에 대해 경험적으로 검증해 봄으로써 신규 장애인을 위한 이론적 정책적 시사점을 도출하는데 연구 목적을 두고 있다. 이를 위해 이 연구에서는 장애인들을 표본추출하여 신규 장애인의 경제·사회·문화적 배제와 사회환경의 조절효과를 분석하였다. 분석결과는 다음과 같다. 첫째, 신규장애자의 장애 발생은 경제적 영역에서 배제효과가 있는 것으로 나타났으며, 둘째, 사회문화적 영역에서도 배제효과가 큰 것으로 나타났다. 셋째, 장애 발생에 따른 경제·사회문화적 배제효과에 있어서 사회적 지지의 조절효과가 큰 것으로 나타나, 사회적 지지는 장애 발생이 장애인의 사회적 배제에 미치는 부정적인 영향을 줄이는데 중요한 역할을 하는 것으로 나타났다. 이 연구에서는 이러한 분석결과를 토대로 장애인에 대한 사회적 지지를 제고시켜주기 위한 정책적 시사점을 제시하였다. 첫째, 장애인에 대한 인식개선 프로그램을 개발하여 어린 시기부터 실천되어야 할 것이다. 둘째, 장애인에 대한 인식 개선을 위해 민간부문을 통한 사회적 캠페인이 적극적으로 추진되어야 할 것이다. 셋째, 장애인들을 위한 공공정책이 물질적 지원뿐만 아니라 정서적 지원으로까지 더욱 확대·강화되어야 할 것이다.

미국 연방 장애법과 동법이 장애인의 의료서비스에 미친 영향 (FEDERAL DISABILITY LAW AND ITS IMPACT ON HEALTH CARE FOR PERSONS WITH DISABILITIES IN THE UNITED STATES)

  • Federal disability law has evolved from several laws geared to protect people with disabilities since the late 1960s and early 1970s. When U.S. Congress passed the Americans with Disabilities Act (ADA) in 1990, no federal statute prohibited the majority of employers, program administrators, owners and managers of places of public accommodation and others from discriminating against people with disabilities. Toward the ends to assure equality of opportunity, full participation, independent living, and economic self-sufficiency for individuals with the disabilities, the ADA pursues three major strategies: Title I addresses inequality in employment, Title II, inequality in public services, and Title III, inequality in services and accommodations offered by private entities. The purposes of the study were to analyze the impact of the ADA on health care for persons with disabilities and to review the ongoing health policy reforms at the federal and state governments. Essential remedies that the ADA contemplates are based on two principles, simple discrimination and reasonable accommodation, which significantly improved access to quality care, especially long-term care, by persons with disabilities. However, the ongoing Medicaid policy reforms to control rising health care costs in the U.S. could threaten the access to care by persons with disabilities in optional groups and to optional care services by persons with disabilities in mandatory groups.
