Evaluation of the heart and lung dosimetric parameters in deep inspiration breath hold using 3D Slicer

  • Purpose: The present study was conducted to compare dosimetric parameters for the heart and left lung between free breathing (FB) and deep inspiration breath hold (DIBH) and determine the most important potential factors associated with increasing the lung dose for left-sided breast radiotherapy using image analysis with 3D Slicer software. Materials and Methods: Computed tomography-simulation scans in FB and DIBH were obtained from 17 patients with left-sided breast cancer. After contouring, three-dimensional conformal plans were generated for them. The prescribed dose was 50 Gy to the clinical target volume. In addition to the dosimetric parameters, the irradiated volumes and both displacement magnitudes and vectors for the heart and left lung were assessed using 3D Slicer software. Results: The average of the heart mean dose (Dmean) decreased from 5.97 to 3.83 Gy and V25 from 7.60% to 3.29% using DIBH (p < 0.001). Furthermore, the average of Dmean for the left lung was changed from 8.67 to 8.95 Gy (p = 0.389) and V20 from 14.84% to 15.44% (p = 0.387). Both of the absolute and relative irradiated heart volumes decreased from 42.12 to 15.82 mL and 8.16% to 3.17%, respectively (p < 0.001); however, these parameters for the left lung increased from 124.32 to 223.27 mL (p < 0.001) and 13.33% to 13.99% (p = 0.350). In addition, the average of heart and left lung displacement magnitudes were calculated at 7.32 and 20.91 mm, respectively. Conclusion: The DIBH is an effective technique in the reduction of the heart dose for tangentially treated left sided-breast cancer patients, without a detrimental effect on the left lung.

Modeling the Flushing Effect of Multi-purpose Weir Operation on Algae Removal in Yeongsan River (영산강 다기능보 운영에 따른 플러싱 및 조류 배제 효과 모델링)

  • The purpose of this study was to model the effect of flushing discharge on algae removal by multi-purpose weir operation in Yeongsan River (Seungchon Weir) using a 3-dimensional (3D) model. The chlorophyceae Eudorina sp. formed bloom in May 2013. Flushing discharge was conducted in two different ways for algal bloom reduction. To elucidate the spatial variability, a high-resolution 3D model, ELCOM-CAEDYM, was used to simulate the spatial variations of water quality and chl-a over a month. The results showed that ELCOM-CAEDYM could reproduce highly spatially resolved field data at low cost, and showed very good performance in simulating the pattern of algal bloom occurrence. The effect of each flushing discharge operation was analyzed with the results of modeling. The results of case 1, flushing discharge using an open movable weir, showed that the algal bloom between the Seochang Bridge and the Hwangryong River junction is rapidly flushed after operating the movable weir, but the residual algae remained in the weir pool as the discharge decreased. However, the results of case 2, fixed weir overflow with a small hydropower stop, showed that most of the algae was removed after flushing discharge and the effect of algae removal was much bigger than that in case 1, as per modeling results and observed data.

Shear Strength and One-dimensional Compression Characteristics of Granitic Gneiss Rockfill Dam Material (화강편마암 댐 축조재료의 전단강도 및 일차원 압축특성)

  • In this study, a rockfill-dam material was investigated on its shear strength and compressibility by performing large-scaled triaxial and oedometer tests. The rockfill material was compacted at two different compaction levels and sheared in triaxial compression at three different confining stresses. Also, rockfill samples were prepared to have three different grain size distributions but the same dry density. Each sample with a given grain size distribution was then compressed one-dimensionally in a large-scaled oedometer cell with and without soaking. The rockfill samples exhibited slightly different shear behaviors with the varying compaction and confining stress levels. The increase in the compaction level changed the behavior from contractive to dilative. Dilation decreased gradually with increasing confining stress, resulting in reduction in the peak shear strength. The large-scaled oedometer test results showed that particle breakages increased with increasing average particle sizes of the samples. Comparing the samples with different gradations, a relatively well-graded sample exhibited lower compressibility. For saturated samples, slightly higher deformations were observed, compared to dry samples. The values of tangent constrained modulis for the dry samples were larger by about 10 to 20$\%$, on the average, than those for the saturated samples.

  • This paper treats optimal route safety assessment system at seaway based on weather forecasting data through INMARSAT. Since early times, captain have been sailing to select the optimum route considering the weather, ship loading status condition and operational scheduling empirically. However, it is rare to find digitalized onboard route support system whereas weather facsimile or wave and swell chart are utilized for the officer, based on captain's experience. In this paper, optimal route safety assessment system which is composed of voyage efficiency and safety component is introduced. Optimum route minimized ETA(estimated time of arrival) and fuel consumption that shipping company. and captain are requiring to evaluate for efficient voyage considering speed loss and power increase based on wave added resistance of ship. In the view point of safety, seakeeping prediction is performed based on 3 dimensional panel method Basically, the weather forecast is assumed to be prepared previously in order to operate this system.

  • Micronucleus test is genotoxicity assay for detection of micronuclei in the cytoplasm of interphase cells. The reduction and replacement of in vivo toxicity testing on animals require the development of in vitro models to predict the genotoxicity or other tests for cosmetic products. In this study, we evaluated a genotoxicity assay for topically applied chemicals using a three-dimensional human reconstructed skin model, KeraSkin$^{TM}$. Two genotoxins, mitomycin C (MMC) and methyl methanesulfonate (MMS), induced significant dose-related increases in cytotoxicity and micronuclei induction in the skin model. In contrast, two non-genotoxins, 4-nitrophenol (4-NP) and trichloroethylene (TCE), induced cytotoxicity but not micronucleus formation. In conclusion, micronucleus test using human skin model may be useful for predicting in vitro genotoxic potentials of cosmetic products.

  • There have been great attentions on polymer electrolyte membrane fuel cell (PEMFC) due to their advantageous characteristics such as zero emission of hazardous pollutant and high energy density. In this work, we evaluated degradation phenomena and stability of single cell performance via one dimensional single cell modeling. Here, CO poisoning on anode on anode was considered for cell performance degradation. Modeling results showed that the performance and stability were highly degraded with CO concentration in fuel gas. In addition, cell performance was reduced by slow oxygen reduction on cathode in long term operation. In order to overcome, it is required to increase ratio o#hydrogen in the fuel gas of anode and high Pt loading contained in the cathodic catalyst layer.

The Change of Facial Contouring after Unilateral Injection of Botulinum Toxin in Unilateral Masseter Hypertrophy Patients (편측성 교근비대 환자에서 편측 보툴리눔 독소 주사 후 안모의 변화)

  • Botulinum toxin type A (BoNT-A) is used for treating bilateral masseter hypertrophy since 1994. Recently there have been more clinical studies in this area, with some authors reporting that BoNT-A can reduce the size of the masseter muscle, as documented by photography, ultrasonography, computed tomography, and 3D(three dimensional) laser scan. However, earlier studies were only for bilateral masseter hypertrophy cases, not for unilateral masseter hypertrophy cases. The aim of this study was to use 3D laser scanning to evaluate changes in the external facial contour induced by unilateral BoNT-A injection. BoNT-A was injected into hypertrophic masseter muscle unilaterally in 10 patients with asymmetric masseter hypertrophy. The clinical effects of unilaterally injected BoNT-A were evaluated before the injection and 4, 8, and 12weeks after the injection using 3D laser scan. And the mean values of both sides (injection and non-injection sides) were compared with. At injection side, mean values of the volume and the bulkiest height at each time point diminished significantly between pre-injection and 4, 8, and 12weeks post-injection. At non-injection side, in contrast, mean values of the volume and the bulkiest height diminished also but less than that of injected side, and there was no statistical significance. In this limited study, we concluded that the unilaterally BoNT-A injection side showed greater mean values of the reduction of muscle volume than non-injection side at 4, 8, and 12 weeks after the injection.

  • In ubiquitous environment where various kinds of computers are embedded in persons, objects and environment and they are interconnected and can be used in my place as necessary, different types of data need to be exchanged between heterogeneous machines through home network. In the environment, the efficient processing, transmission and monitoring of image data are essential technologies. We need to make research not only on traditional image processing such as spatial and visual resolution, color expression and methods of measuring image quality but also on transmission rate on home network that has a limited bandwidth. The present study proposes a new motion vector estimation algorithm for transmitting, processing and controlling image data, which is the core part of contents in home network situation and, using algorithm, implements a real time monitoring system of multi dimensional images transmitted from multiple cameras. Image data of stereo cameras to be transmitted in different environment in angle, distance, etc. are preprocessed through reduction, magnification, shift or correction, and compressed and sent using the proposed metamorphosis hierarchical motion vector estimation algorithm for the correction of motion. The proposed algorithm adopts advantages and complements disadvantages of existing motion vector estimation algorithms such as whole range search, three stage search and hierarchical search, and estimates efficiently the motion of images with high variation of brightness using an atypical small size macro block. The proposed metamorphosis hierarchical motion vector estimation algorithm and implemented image systems can be utilized in various ways in ubiquitous environment.

  • Recently, due to global warming and climate change in Korea, local heavy storm occurs frequently. In this study, the risky areas for flooding in urban areas are analyzed for flood inundation based on two-dimensional urban flood runoff model (XP-SWMM) focusing on coastal high flood-risk urban areas. In addition, the MCDM (Multi-Criteria Decision Making) technique is utilized in order to establish the flood defense structural measures. The alternative flood reduction method are compared and the optimum flood defense measures are selected. A simulation model was used with three structural flood prevention measures (drainage pipe construction, water detention, flood pumping station). In order to decrease the flooding area, flood assessment criteria are suggested (flooded area, maximum inundation depth, damaged residential area, construction cost). Priorities of alternatives are determined by using compromise programming. As a result, the optimal flood defence alternative suggested for Janghang Zone 1 is flood pumping station and for Janghang Zone 2, 3 are drainage pipe construction.

Volumetric modulated arc therapy for carotid sparing in the management of early glottic cancer

  • Purpose: Radiotherapy of the neck is known to cause carotid artery stenosis. We compared the carotid artery dose received between volumetric modulated arc therapy (VMAT) and conventional fixed-field intensity-modulated radiotherapy (IMRT) plans in patients with early glottic cancer. Materials and Methods: Twenty-one early glottic cancer patients who previously underwent definitive radiotherapy were selected for this study. For each patient, double arc VMAT, 8-field IMRT, 3-dimensional conformal radiotherapy (3DCRT), and lateral parallel-opposed photon field radiotherapy (LPRT) plans were created. The 3DCRT plan was generated using lateral parallel-opposed photon fields plus an anterior photon field. VMAT and IMRT treatment plan optimization was performed under standardized conditions to obtain adequate target volume coverage and spare the carotid artery. Dose-volume specifications for the VMAT, IMRT, 3DCRT, and LPRT plans were calculated with radiotherapy planning system. Monitor units (MUs) and delivery time were measured to evaluate treatment efficiency. Results: Target volume coverage and homogeneity results were comparable between VMAT and IMRT; however, VMAT was superior to IMRT for carotid artery dose sparing. The mean dose to the carotid arteries in double arc VMAT was reduced by 6.8% compared to fixed-field IMRT (p < 0.001). The MUs for VMAT and IMRT were not significantly different (p = 0.089). VMAT allowed an approximately two-fold reduction in treatment delivery time in comparison to IMRT (3 to 5 minutes vs. 5 to 10 minutes). Conclusion: VMAT resulted in a lower carotid artery dose compared to conventional fixed-field IMRT, and maintained good target coverage in patients with early glottic cancer.
