The Effect of Action Observation Training on Balance in Patients with Chronic Stroke (동작관찰훈련이 만성 뇌졸중 환자의 균형능력에 미치는 영향)

  • This study examined the effects of action observations on the balance of patients with chronic stroke. The study period was between July 8, 2013 and August 10 2013. The participants were divided randomly into either the landscape picture observation group (n=20) and the action observation group (n=20). Each group observed the landscape pictures and video-recorded action-related balance for 5 minutes, respectively. Both groups had a balance training session for 10 minutes after the picture and video observations. The balance training sessions were repeated twice a day, three times a week for 6 weeks. As a result, significant improvement was noted in the action observation group than the landscape picture observation group for static balance and dynamic balance (p<.05). Consequently, the combination of action observation and balance training is believed to help improve the balance recovery for patients with chronic stroke.

  • Purpose: The purpose of this study is to report the effect of Traditional Korean Medicine(TKM) on cold sensitivity of hands and feet. And we wanted that cold sensitivity of hands and feet was measured objectively and diagnosed definitely using Digital Infrared Thermographic Imaging(DITI). Methods: The patient in this case was a 29-year-old female. The chief complaint was cold sensitivity of hands and feet. We treated her by TKM and evaluated the progress of symptoms by DITI. And then we compared DITI before treatment and after treatment. Results: After TKM treatment, the differences of temperature of hands and feet were decreased on DITI, respectively. Conclusion: TKM treatment is effective on cold sensitivity of hands and feet. And DITI would be valid to measure for treatment's effect of cold sensitivity of hands and feet.

  • Objective : The purpose of this study is to evaluate clinical effect of oriental treatment for lumbago patients by comparing of Improvement index, Visual analog scale, DITI gap between common acupuncture with Dong-Si acupuncture treatment group and common acupuncture treatment group. Methods : The 38 patients who had a diagnosis of lumbago were observed from June 2002 to February 2003, were divided into two classes ; the A group was 19 cases practised Dong-Si acupuncture with common acupuncture, B group 19 cases only common acupuncture. Then the time of 2 weaks after, I compared Improvement index, Visual analog scale, DITI gap of two groups. Results : In Improvement Index, Group A is $0.4442{\pm}0.1165$ and Group B is $0.3061{\pm}0.1402$. So Group A is thought to be significant(P=0.002). In VAS(visual analogue scale), Group A is $7.4737{\pm}1.1239$ and Group B is $6.3684{\pm}1.0116$. So Group A is thought to be significant(P=0.003). In DITI, temperature differance of Group A is lower than Group B after treatment.

Effects of McKenzie Exercise on the Functional Recovery and Forward Head Posture of Chronic Neck Pain Patients. (만성경부통증 환자에 대한 McKenzie 운동이 기능 회복과 두부전방자세에 미치는 영향)

  • Purpose : To evaluate effects of McKenzie exercise on the functional recovery and forward head posture of chronic neck pain patients. Methods : The subjects were consisted of fifteen patients who had abnormal neck posture, mild neck pain (28 males, 17 females; mean aged 21.9) from 19 to 33 years of age(mean age 21.9). All subjects were received McKenzie exercise for 35 minutes with clinical massage per day three times a week during 4 weeks period. Neck disability index was used to measure functional disability level. Visual analogue scale(VAS) was used to measure subjective pain level. craniovetebral angle(CVA), cranial rotation angle(CRA) was used to measure forward head posture with digital camera. All measurements of each patients were measured at pre-treatment and after 2 week, after 4 week, post-treatment on 2 week. Result : The VAS of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The Neck Disability Index of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CVA of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CRA of McKenzie exercise was not significantly reduced between pre-treatment and post-treatment (p<.05). Conclusion : McKenzie exercise improved pain and function of Chronic neck pain patients.

Electromyographic Analysis of Upper and Lower Limb Muscles during Gardening Tasks

  • Movements of the upper and lower limb muscles during five common gardening tasks were analyzed by using electromyography (EMG). Twenty adults aged in their twenties (mean age, $24.8{\pm}2.4$ years) were recruited. On two separate occasions, subjects visited a garden plot to perform digging, raking, troweling, weeding, and hoeing; all tasks were performed three times with 20 s intervals for each trial. To measure muscle activation during the five gardening tasks, surface EMG was used. Bipolar surface EMG electrodes were attached to eight upper limb muscles (bilateral anterior deltoid, biceps brachialis, brachioradialis, and flexor carpi ulnaris) or eight lower limb muscles (bilateral vastus lateralis, vastus medialis, biceps femoris, and gastrocnemius) on both sides of the body, for a total of 16 muscles. During the five tasks, photographs were taken of movement phases using a digital video camera. The right flexor carpi ulnaris and brachioradialis showed higher activation than the other upper and lower limb muscles measured during the tasks. All 16 upper and lower limb muscles were actively used only during digging. According to movement analysis of each activity, digging was classified into four movement phases, whereas raking, troweling, weeding, and hoeing each were divided into three movement phases. In each activity, there were high-impact phases in terms of muscle activation; the flexor carpi ulnaris and brachioradialis were identified as major muscles in each impact phase. This analysis may be used to generate biomechanical profiles of gardening tasks for practitioners when designing efficient gardening interventions for physical health or rehabilitation.

  • 치주질환이 진행되는 동안이나 치주처치후 치유되는 과정에서 치조골의 변화가 야기되는데 방사선 사진은 치조골 변화를 감지하는 유일한 비외과적인 방법이다. 미묘한 치조골 변화의 진단은 치료시나 유지관리기 환자의 평가시 중요한 바, 최근에는 규격화시킨 디지털 영상을 이용하여 정량적인 골변화 측정이 가능하게 되었다. 본 연구의 목적은 중등도의 치주질환을 지닌 환자에서 국소마취하에 초기 치주처치를 시행한후 참조체와 함께 구내 방사선 사진을 촬영하고 디지털화 한 다음 참조체 당량치를 이용하여 치조골의 밀도변화를 평가하기 위한 것이다. 이 연구를 위하여 치주질환에 이환된 환자 5명(남자 3명, 여자 2명 : 평균 47.4세)에서 탐침깊이가 5㎜ 이상이고 골내낭이 있는 제 $1{\cdot}2$ 소구치, 제 $1{\cdot}2$ 대구치 40개(상악 24개, 하악 16개)를 대상으로 구강위생교육과 치석제거술, 치근면활택술을 시행하였다. 임상지수는 술전과 술후 8주째에 측정하였고, 방사선 사진은 술전, 술후 2주, 4주, 6주, 8주째에 촬영하였고, 구리 스텝웨지를 사용하여 규격화 하였다. 촬영된 영상은 NIH image program(U.S.A)에 의해 분석되어졌고 이들 자료를 통해 다음과 같은 결과를 얻었다. 1. 치조골의 밀도는 초기치료후 2주째 까지는 감소된 양상을 보이다가 4주 이후로는 점차적으로 증가하는 양상을 보였다. 2. 치조골의 밀도는 초기치료전과 비교시 초기치료수 6주째와 8주째에서 유의한 차이를 보였다.(p<0.01) 3. 상하악 간의 치조골 밀도는 유의한 차이를 보이지 않았다(p>0.05). 이상과 같은 결과를 통하여 볼때 초기 치주처치만으로도 치조골의 밀도가 증가됨을 확인할 수 있었다.

A STUDY ON THE EFFECTS OF SCATTERING DOSE ON EYES AND THYROID FOR PANORAMAGRAPHY (Focus on TLD and PLD)

  • This study concerning the surface dose of eye and thyroid from panoramagraphy used thermoluminescent dosimeter (TLD) and photoluminescent dosimeter (PLD) to take measurements at ten hospitals in the Gwangju metropolitan area. The recommendations from ICRP 60 and ICRP 73 on the allowance standard for eye are 15 mSv and for thyroid is 1 mSv. The left eye TLD and PLD values are 0.19 mSv and 0.24 mSv respectively. The right eye TLD and PLD values are 0.23 mSv and 0.25 mSv respectively. Thyroid TLD and PLD values are 0.08 mSv and 0.25 mSv respectively and did not exceed the allowance standards(p<0.001). Also comparisons are made between TLD and PLD for each organ and PLD has higher dose measurements than TLD. There are statistically significant differences in left eye measurements and thyroid measurements (p<0.01). There is no significant difference in measurements for the right eye (p>0.05). The TLD and PLD measured dose from panoramagraphy instruments on eyes and thyroid from each hospital did not exceed the recommended dose from ICRP 60 for surface dose measurements. However, due to the probability of influence, consideration should be made for all levels of dose.

Intraarterial Nimodipine Infusion to Treat Symptomatic Cerebral Vasospasm after Aneurysmal Subarachnoid Hemorrhage

  • Objective : Cerebral vasospasm leading to cerebral ischemic infarction is a major cause of morbidity and mortality in the patients who suffer with aneurysmal subarachnoid hemorrhage. Despite adequate treatment, some patients deteriorate and they develop symptomatic vasospasm. The objective of the present study was to investigate the efficacy and clinical outcome of intraarterial nimodipine infusion on symptomatic vasospasm that is refractory to hemodynamic therapy. Methods : We retrospectively reviewed the procedure reports, the clinical charts and the transcranial doppler, computed tomography and digital subtraction angiography results for the patients who underwent endovascular treatment for symptomatic cerebral vasospasm due to aneurysmal SAH. During the 36 months between Jan. 2005 and Dec. 2007, 19 patients were identified who had undergone a total of 53 procedures. We assessed the difference in the arterial vessel diameter, the blood flow velocity and the clinical outcome before and after these procedures. Results : Vascular dilatation was observed in 42 of 53 procedures. The velocities of the affected vessels before and after procedures were available in 33 of 53 procedures. Twenty-nine procedures exhibited a mean decrease of 84.1 cm/s. We observed clinical improvement and an improved level of consciousness with an improved GCS score after 23 procedures. Conclusion : Based on our results, the use of intraarterial nimodipine is effective and safe in selected cases of vasospasm following aneurysmal SAH. Prospective, randomized studies are needed to confirm these results.

Effect of Acupressure Massage on Temperatures of Acupoints, Severity of Facial Paralysis, Subjective Symptoms, and Depression in Bell's Palsy Patients

  • Purpose: Bell's palsy is a nerve paralysis disease that causes functional impairments and affects psychological and aesthetical parts. This study aimed to examine whether acupressure massage had positive effects on facial paralysis, subjective symptoms, and depression in Bell's palsy patients. Methods: This study was conducted by a nonequivalent control group pretest-posttest design. Participants were 60 patients with Bell's palsy. 30 patients were assigned to the experimental group and the remaining 30 patients were assigned to the control group. The period of the study was from October 1, 2008 to July 30, 2009. Acupressure massage was offered to the experimental group for 20 minutes per day for two weeks (a total of six times). A SPSS/Win 12.0 program was used for data analysis. Results: A difference in Digital Infrared Thermographic Imaging (DITI) between affected and unaffected sides was less in the experimental group having acupressure massage than in the control group and the score of the recovery of facial paralysis was also increased in the experimental group. The Facial Nerve Grade Systems by Brackmann score that is a more objective index showed a significant difference between two groups (F=26.81, p<.001). Subjective symptom and depression scores were more decreased in the acupressure massage group than in the control group. Conclusion: Based on the results, it is considered that acupressure massage can be applied to Bell's palsy patients as an alternative therapy. It can be used as an evidence-based East-West nursing intervention to improve patients' physical and mental functions.

Extraction of Deep Neck Flexors from Cervical Utrasound Images using Enhanced Fuzzy Techniques (개선된 퍼지 기법을 이용한 경추 초음파 영상에서의 경부심굴곡근 추출)

  • 본 논문에서는 경추 초음파 DICOM 영상에서 개선된 퍼지 시그마 기법을 이용하여 경부심굴곡근을 추출하고 두께를 측정하는 방법을 제안한다. 제안된 방법은 ROI 영역에서 Ends-In Search Stretching을 적용하여 명암 대비를 강조한다. Stretching된 ROI 영역에서 평균 이진화를 적용한 후, Blob 알고리즘을 적용하여 흉쇄유돌근과 경부심굴곡근의 후보 영역을 추출한다. 추출된 경부심굴곡근 후보 영역에서 경추의 위치 정보를 이용하여, 경추의 경계 영역을 검출한 후, Cubic Spline 보간법 알고리즘을 적용하여 스플라인 곡선을 추출한다. 스플라인 곡선 영상에서 상/하 탐색 알고리즘을 적용하여, 최대/최소 범위 영역을 설정한다. Stretching된 ROI 영역에서 최대/최소 범위에 해당하는 영역에 대해 개선된 퍼지 시그마 이진화를 적용한다. 적용된 영역을 Blob 알고리즘을 이용하여 잡음을 제거하고 Morphology 알고리즘을 이용하여 초음파 영상의 첫 번째 경추 기준점의 좌표 정보를 추출한다. 경추 기준점을 기준으로 두께 측정에 필요한 경부심굴곡근 후보 영역을 추출하고 개선된 퍼지 시그마 이진화 알고리즘을 적용한다. 개선된 퍼지 시그마 이진화 알고리즘이 적용된 영상에서 근막의 위치 정보를 이용하여 경부심굴곡근상단 경계선을 추출한다. 추출된 각 경추 객체에 DDA(Digital Differential Analyzer) 알고리즘과 Cubic Spline 보간법 알고리즘을 적용하여 경부심굴곡근의 하단 경계선을 추출한다. 추출된 경부심 굴곡근의 상/하단 경계선의 위치 정보를 이용하여, 측정에 필요한 경부심굴곡근을 추출한다. 제안된 방법을 경추 초음파 영상에 적용하여 경부심굴곡근을 추출한 결과, 기존의 경부심굴곡근추출 방법보다 효율적으로 경부심굴곡근을 추출하는 것을 확인할 수 있었다.
