The present study explored the influence of temporomandibular disorders(TMD) with anterior open bite on swallowing. Fifteen subjects with anterior open bite(group A), 9 subjects with anterior open bite and TMD(group B), and 24 subjects without malocclusion or TMD symptoms (group C) were included. BioPAK system(Bioresearch Inc., Milwaukee, WI, USA) was used to record the muscle activities of anterior temporal, masseter, sternocleidomastoid(SCM) and anterior digastric muscles during maximum clenching and swallowing. Positional change of the mandible during swallowing was also recorded using the same system. The obtained results were as follows: 1. Group A, B, and C did not show significant differences each other in the muscle activity of resting position in most of head and neck muscles. 2. Group B showed significantly lower muscle activity in maximum clenching than group C in anterior temporal(p<0.01), masseter(p<0.05), SCM(p<0.05) and digastric muscles(p<0.05). 3. Group A showed significantly lower muscle activity during swallowing than group C in anterior temporal and masseter muscles(p<0.01). Group B showed significantly lower muscle activity during swallowing than group C in anterior temporal(p<0.01), masseter(p<0.01), and SCM muscles(p<0.05). 4. Group A and B showed increased positional change of the mandible during swallowing measured from vertical, anteroposterior and lateral aspects, and prolonged swallowing(p<0.05). 5. After given instructions for normal swallowing pattern, group A and B showed increased muscle activity during swallowing in anterior temporal, masseter and SCM muscles(p<0.01). Positional change of the mandible and time elapsed for swallowing also decreased after the instruction(p<0.01). 6. Occlusal splint did not change the muscle activity during swallowing. However, vertical change(p<0.01) and swallowing time(p<0.05) were decreased with splint.
The purpose of this study was finding the pain inhibitory effect of acupuncture based on rataining time at LI -4. The pain at dentes incisor was evoked by noxious electric stimulation and digastric electromyogram(dEMG) changes based on time interval were measured. To do this, the opioid antagonist was administered intraperitoneally and four groups were made for convenience. Without naloxone, dEMG was changed by either retaining the needle for 40 minutes (Group I) or by lifting and thrusting the needle (Group II). With naloxone administration, dEMG was changed by either retaining the needle for 40 minutes (Group III) or by lifting and thrusting the needle (Group IV). The results are as following 1. The pain inhibitory effect of acupuncture at LI -4 was expressed best in Group I. 2. The pain inhibitory effect was somewhat expressed in Group II but the effect was smaller than Group I. 3 .In Groups III and IV, the pain inhibitory effect was not expressed. The overall result should be the foundation for the further studies to figure out the underlying mechanism of acupuncture. In addition, it is assumed that the results will be useful for optimal retaining time of acupucture for its maximal effect.

Background Botulinum toxin (BT) chemodenervation and anterior belly of digastric muscle (ABD) transfer are both treatment options in the management of an isolated marginal mandibular branch of the facial nerve (MMB) palsy. We compare the patient satisfaction following either BT injections or ABD transfer in the management of their isolated MMB palsy. Methods Patients in the ABD-arm of the study were identified retrospectively from September 2007 to July 2014. The patients in the BT-arm of the study were identified prospectively from those attending the clinic. Both groups of patients completed a validated patient satisfaction survey. Statistical analysis was performed and a P-value <0.05 was considered statistically significant. Results Seven patients were in the ABD-arm and 11 patients in the BT-arm of the study. The patient satisfaction in both groups was high with 45% of ABD-arm patients and 40% of BT-arm patients rating their overall outcome as 'better' or 'much better', which was significantly more than the proportion rating their outcome as 'worse' or 'much worse' (P<0.001), although there was a significant trend towards those in the ABD-arm being more likely to be dissatisfied with their outcome (P=0.01). Conclusions BT therapy is a good first-line intervention in the management of isolated MMB palsy. We have, however, shown that the overall satisfaction in both groups is high. Therefore, in patients who would prefer a more permanent solution to manage their facial asymmetry, ABD transfer remains a satisfactory treatment option with a good level of patient satisfaction.
Objectives : The pain was induced on upper and lower incisor of the rat based on the theory of 'connections of upper incisor pain with stomach meridian and lower incisor pain with large intestine meridian'. Such acupoints as LI4 and ST36 were used for alleviation of upper and lower incisor pain. Methods : The digastric myogram (dEMG) was utilized for the pain measurement. Results : The ST36 acupuncture after induction of upper incisor pain was gradually decreased or increased the dEMG. The LI4 acupuncture after induction of upper incisor pain was gradually decreased the dEMG. The ST36+LI4 acupuncture after induction of upper incisor pain was gradually decreased the dEMG. We knew this thing which the ST36+LI4 acupuncture decreased the dEMG most greatly and kept long compared to ST36 acupuncture, LI4 acupuncture. Conclusions : Acupuncture treatment at the loci of not only ST36 acupuncture but LI4 acupuncture were relieved the upper incisor pain. It was well suitable to the theory 'connections of upper incisor pain with stomach meridian' that ST36 acupuncture decreased the upper incisor pain. But there was not to decrease the dEMG for the ST36 acupuncture. We are considered as tracing study continuously about ST36 acupuncture. It was not suitable to the theory 'connections of upper incisor pain with stomach meridian' that LI4 acupuncture decreased the upper incisor pain. These results was considered as the function by the characteristic of the LI4 acupoint.
Purpose: To evaluate the usefulness of shear wave elastography in the diagnosis of oral and maxillofacial diseases. Materials and Methods: Ten patients with oral and maxillofacial diseases and 28 volunteers drawn from our student doctors were examined by shear wave elastography with a 14-MHz linear transducer using an Aplio 300 apparatus (Canon Medical Systems, Otawara, Japan). A statistical analysis of the shear elastic modulus(kPa) of healthy tissue (the sublingual gland, submandibular gland, anterior belly of the digastric muscle, and geniohyoid muscle) in the 28 volunteers was performed using 1-way repeated measures analysis of variance with the Tukey honest significant difference test. The maximum shear elastic modulus(kPa) of 8 patients with squamous cell carcinoma (SCC) and 2 patients with benign lesions was evaluated with the Mann-Whitney U test. The analysis used a 5% significance level. Results: The mean shear elastic modulus of the sublingual gland ($9.4{\pm}3.7kPa$) was lower than that of the geniohyoid muscle ($19.2{\pm}9.2kPa$, P=.000) and the anterior belly of the digastric muscle ($15.3{\pm}6.1kPa$, P=.004). The maximum shear elastic modulus of the SCCs($109.6{\pm}14.4kPa$) was higher than that of the benign lesions($46.4{\pm}26.8kPa$, P=.044). Conclusion: Our results demonstrated the usefulness of shear wave elastography in the diagnosis of oral and maxillofacial diseases. Shear wave elastography has the potential to be an effective technique for the objective and quantitative diagnosis of oral and maxillofacial diseases.

The lower incisor pain was gradually decreased during LI4 stimulation with significant differences at 15, 20 and 40 minute point. In conclusion, the upper incisor pain was relieved with ST36 and LI4 acupuncture while LI4 was effective on the lower incisor pain alleviation based on the theory of 'connections of upper incisor pain with stomach meridian and lower incisor pain with large intestine meridian'.
This study was performed to investigate the mechanism of central analgesic effects of antidepressants. Thirty four male rats were anesthetized with pentobarbital sodium (40 mg/kg, ip). A stainless steel guide cannula and a PE tube (PE10) were implanted into the lateral ventricle and cisterna magna area. Stimulating and recording electrodes were implanted into the incisor pulp and anterior digastric muscle. Electrodes were led subcutaneously to the miniature cranial connector sealed on the top of the skull with acrylic resin. The jaw opening reflex was used in freely moving rats, and antidepressants were administered intracisternally in order to eliminate the effects of anesthetic agents on the pain assessment and evaluate the importance of the central action site of antidepressants. After 48 hours of recovery from surgery, digastric electromyogram (dEMG) of freely moving rats was recorded. Electrical shocks (200 ${\mu}sec$ duration, 0.5-2 mA intensity) were delivered at 0.5 Hz to the dental pulp every 2 minute. Intracisternal administration of $15\;{\mu}g$ imipramine suppressed dEMG elicited by noxious electrical stimulation in the tooth pulp to $76{\pm}6%$ control. Intracisternal administration of $30\;{\mu}g$ desipramine, nortriptyline, or imipramine suppressed dEMG remarkably to $48{\pm}2,\;27{\pm}8,\;or\;25{\pm}5%$ of the control, respectively. Naloxone, methysergide, and phentolamine blocked the suppression of dEMG produced by intracisternal antidepressants from $23{\pm}2\;to\;69{\pm}4%,\;from\;32{\pm}5\;to\;80{\pm}9%,\;and\;from\;24{\pm}6\;to\;77{\pm}5%$ of the control, respectively. These results indicate that antidepressants produce antinociception through central mechanisms in the orofacial area. Antinociception of intracisternal antidepressants seems to be mediated by an augmentation of descending pain inhibitory influences on nociceptive pathways.
Rehabilitation of the paralyzed face as a result of trauma or surgery remains a daunting task. Complete restoration of emotionally driven symmetric facial motion is still unobtainable, but current techniques have enhanced our ability to improve this emotionally traumatic deficit. Problems of mass movement and synkinesis still plague even the best reconstructions. The reconstructive techniques used still represent a compromise between obtainable symmetry and motion at the expense of donor site deficits, but current techniques continue to refine and limit this morbidity. In chronically paralyzed face, direct nerve anastomosis, nerve graft, or microvascular-muscle graft is not always possible. In this case, regional muscle transposition is tried to reanimate the eyelid and lower face. Regional muscle includes maseeter muscle, temporalis muscle and anterior belly of the digastric muscle. Temporalis muscle is preferred because it is long, flat, pliable and wide-motion of excursion. In order to reanimate the upper and lower eyelid, Upper eyelid Gold weight implantion and lower eyelid shortening and tightening is mainly used recently, because this method is very simple, easy and reliable.
Identification and protection of the facial nerve is very important in the proper operation of the parotid tumor. Posterior approach which finds main trunk of the facial nerve by surgical landmark such as tragal pointer, tympanomastoid suture, and posterior belly of digastric muscle is most commonly used. In case of posterior located tumor, inferior approach may be used, in which the retromandibular vein is followed from the neck and inferior branch of the facial nerve is located. In general, the facial nerve lies superficial to the retromandibular vein. But we experienced the anomalous relationship of the facial nerve and the retromandibular vien. We report this case with a literature review.

Objective : The purpose of this study was conducted to find treatment of forward head posture and shoulder instability with proprioceptive neuromuscular facilitation concept. Methods : This is literature study with books, seminar note and international course. Results : Forward head posture and shoulder instability has related each other. Forward head posture will make muscle instability, weakness and stiffness on neck and shoulder girdle. It will make pain also. Important muscle are suboccipital muscle, omohyoid muscle, sternoclaidomastoid muscle, scaleni, pectoralis minor, levator scapular and digastric. Conclusion : Treatment of the forward head posture and shoulder instability is provided. It is that treatment of stiff muscle with eccentric muscle work, muscle elongation, muscle strengthening at the structure level and at the functional level for daily activities.
