A 59-year-old patient with a history of hepatocellular carcinoma presented with decreased consciousness and left hemiparesis. A rim-enhanced mass lesion without diffusion restriction was observed in contrast-enhanced MRI including diffusion-weighted imaging. Based on these findings, metastatic brain tumor was suspected. However, brain abscess (BA) was diagnosed after multiple bacterial colonies were observed in aspiration biopsy. Initial conventional antibiotic treatment including vancomycin had failed, so linezolid was used as second-line therapy. As a result, infection signs and clinical symptoms were resolved. We report a case with atypical imaging features and antibiotic susceptibility of a BA in an immunocompromised patient undergoing chemotherapy.
Purpose: The purpose of this study was to determine the relation between quantitative magnetic resonance imaging biomarkers, and clinical performances in chronic phase of carbon monoxide intoxication. Materials and Methods: Eighteen magnetic resonance scans and cognitive evaluations were performed, on patients with carbon monoxide intoxication in chronic phase. Apparent diffusion coefficient (ADC) ratios of affected versus unaffected centrum semiovale, and corpus callosum were obtained. Signal intensity (SI) ratios between affected centrum semiovale, and normal pons in T2-FLAIR (fluid-attenuated inversion recovery) images were obtained. The Mini-Mental State Exam, and clinical outcome scores were assessed. Correlation coefficients were calculated, between MRI and clinical markers. Patients were further classified into poor-outcome and good-outcome groups based on clinical performance, and imaging parameters were compared. T2-SI ratio of centrum semiovale was compared, with that of 18 sex-matched and age-matched controls. Results: T2-SI ratio of centrum semiovale was significantly higher in the poor-outcome group, than that in the good-outcome group and was strongly inversely correlated, with results from the Mini-Mental State Exam. ADC ratios of centrum semiovale were significantly lower in the poor outcome group than in the good outcome group, and were moderately correlated with the Mini-Mental State Exam score. Conclusion: A higher T2-SI and a lower ratio of ADC values in the centrum semiovale, may indicate presence of more severe white matter injury and clinical impairment. T2-SI ratio and ADC values in the centrum semiovale, are useful quantitative imaging biomarkers for correlation with clinical performance in individuals with carbon monoxide intoxication.
Objective : To investigate the efficiency of diffusion tensor imaging (DTI) fiber-tracking based neuronavigation and assess its usefulness in the preoperative surgical planning, prognostic prediction, intraoperative course and outcome improvement. Methods : Seventeen patients with cerebral masses adjacent to corticospinal tract (CST) were given standard magnetic resonance imaging and DTI examination. By incorporation of DTI data, the relation between tumor and adjacent white matter tracts was reconstructed and assessed in the neuronavigation system. Distance from tumor border to CST was measured. Results : The sub-portion of CST in closest proximity to tumor was found displaced in all patients. The chief disruptive changes were classified as follows : complete interruption, partial interruption, or simple displacement. Partial interruption was evident in seven patients (41.2%) whose lesions were close to cortex. In the other 10 patients (58.8%), delineated CSTs were intact but distorted. No complete CST interruption was identified. Overall, the mean distance from resection border to CST was 6.12 mm (range, 0-21), as opposed to 8.18 mm (range, 2-21) with simple displacement and 2.33 mm (range, 0-5) with partial interruption. The clinical outcomes were analyzed in groups stratified by intervening distances (close, <5 mm; moderated, 5-10 mm; far, >10 mm). For the primary brain tumor patients, the proportion of completely resected tumors increased progressively from close to far grouping (42.9%, 50%, and 100%, respectively). Five patients out of seven (71.4%) experienced new neurologic deficits postoperatively in the close group. At meantime, motor deterioration was found in six cases in the close group. All patients in the far and moderate groups received excellent (modified Rankin Scale [mRS] score, 0-1) or good (mRS score, 2-3) rankings, but only 57.1% of patients in the close group earned good outcome scores. Conclusion : DTI fiber tracking based neuronavigation has merit in assessing the relation between lesions and adjacent white matter tracts, allowing prediction of patient outcomes based on lesion-CST distance. It has also proven beneficial in formulating surgical strategies.
Resting-state Functional Magnetic Resonance Imaging(fMRI) data detects the temporal correlations in Blood Oxygen Level Dependent(BOLD) signal and these temporal correlations are regarded to reflect intrinsic cortical connectivity, which is deactivated during attention demanding, non-self referential tasks, called Default Mode Network(DMN). The relationship between fMRI and anatomical connectivity has not been studied in detail, however, the preceded studies have tried to clarify this relationship using Diffusion Tensor Imaging(DTI) and fMRI. These studies use method that fMRI data assists DTI data or vice versa and it is used as guider to perform DTI tractography on the brain image. In this study, we hypothesized that functional connectivity in resting state would reflect anatomical connectivity of DMN and the combined images include information of fMRI and DTI showed visible connection between brain regions related in DMN. In the previous study, functional connectivity was determined by subjective region of interest method. However, in this study, functional connectivity was determined by objective and advanced method through Independent Component Analysis. There was a stronger connection between Posterior Congulate Cortex(PCC) and PHG(Parahippocampa Gyrus) than Anterior Cingulate Cortex(ACC) and PCC. This technique might be used in several clinical field and will be the basis for future studies related to aging and the brain diseases, which are needed to be translated not only functional connectivity, but structural connectivity.
Anatomical imaging is the basis of the diagnosis and treatment response assessment of brain tumors. Among the existing imaging techniques currently available in clinical practice, diffusion-weighted imaging and perfusion imaging provide additional information. Recently, with the increasing importance of evaluation of the genomic variation and heterogeneity of tumors, clinical application of imaging techniques using radiomics and deep learning is expected. In this review, we will describe recommendations for magnetic resonance imaging protocols focusing on anatomical images that are still important in the clinical application of brain tumor imaging, and the basic principles of diffusion-weighted imaging and perfusion imaging among the advanced imaging techniques, as well as their pathophysiological background and clinical application. Finally, we will review the future perspectives of radiomics and deep learning applications in brain tumor imaging, which have been studied to a great extent due to the development of computer technology.

Objective: To investigate roles of dynamic contrast enhanced magnetic resonance (DCE MR) and diffusion-weighted (DW) imaging in preoperative prediction of underestimation of ductal carcinoma in situ (DCIS) ${\geq}2cm$ on US guided core needle biopsy. Materials and Methods: Twenty two patients with DCIS on US-guided 14 gauge core needle biopsy were included. Patients were divided into a group with and without DCIS underestimation based on histopathology. MR images including DCE and DW imaging were obtained with a 3.0-T MR. The lesion type (mass or non-mass), enhancement pattern, peak enhancement, and apparent diffusion coefficient (ADC) values of proven malignant masses were generated using software of CADstream and compared between two groups using Fisher's exact test and Mann Whitney test. Results: Eight patients were in the group with underestimation and 14 patients were in the group without underestimation. The lesion type and enhancement pattern were not different between two groups (P values = 1.000 and 0.613, respectively). The median peak enhancement of lesions with underestimation was 159.5%, higher than 133.5% of those without underestimation, but not significant (P value = 0.413). The median ADC value of lesions with underestimation was $1.26{\times}10^{-3}mm^2/sec$, substantially lower than $1.35{\times}10^{-3}mm^2/sec$ of those without underestimation (P value = 0.094). Conclusion: ADC values had the potential to preoperatively predict DCIS underestimation on US-guided core needle biopsy, although a large prospective series study should be conducted to confirm these results.
본 논문에서는 인체의 머리 부분을 촬영한 자기 공명 촬영 영상(MRI: Magnetic Resonance Imaging)에서 뇌 영역의 대뇌 피질 만을 분할하는 방법에 대해 제시하고자 한다. 뇌의 해부학적 구조를 파악할 경우에 영상 내에 함께 보여지는 두개골과 뇌척수액 등을 제외한 대뇌피질 영역을 분할하면 보다 효과적인 정보 분석 및 진단이 가능하게 된다. 본 논문에서는 3단계 알고리즘을 제시한다. 첫번째 단계에서는 Speckle reducing anisotropic diffusion (SRAD)를 이용하여 영상 내에 존재하는 잡음을 제거하기 위한 필터링이다. 두번째 단계에서는 필터링된 결과를 이용하여 추출된 임계값과 사용자의 인터렉션인 씨드 포인트를 통해 영상분할을 수행하고, 세 번째 단계에서는 후 처리를 통해 분할 결과를 보완한다. 영상분할 결과의 정확성을 측정하기 위하여 현재 병원의 의료진들이 사용하고 있는 Mayo clinic사의 Analyze를 이용하여 분할된 결과와의 오류를 측정하였다. 또한 최종 결과에 대해 ultravis를 이용한 볼륨 렌더링으로 영상분할의 최종 결과를 제시하였다.
Cerebral ischemia results in neuronal changes in remote areas that have fiber connections with the ischemic area. The aim of this study was to investigate the nigral changes by examining the correlation between the apparent diffusion coefficient (ADC) and the tissue structure. Sprague-Dawley rats were subjected to middle cerebral artery occlusion. Four days after the occlusion, when T2-weighted images revealed the presence of an area of high signal intensity in the ipsilateral substantia nigra, and the ADCs were calculated and imaged. Histopathologic examination by both light and electron microscopy was performed on day 4 after surgery. This finding was consistent with the high signal intensity seen on T2-weighted and diffusion-weighted images, as well as with the ADC reduction, but we did not expect to observe uniform ADC reduction attributable mainly to astrocytic swelling in the perivascular end-feet.
To exhibit our clinical experience of diffusion-weighted (DW) MR imaging for various brain pathologies and to determine its role in characterizing brain pathologies in children. DW images in 177 children (M:F=96:81, mean age, 4.7 years) with various brain pathologies were retrospectively collected over past 3 years. DW images (b value: 1000 s/mm) were reviewed along with corresponding apparent diffusion coefficient (ADC) maps. Brain pathologies included cystic or solid brain tumor (n = 55), cerebral infarct (n = 32), cerebritis with or without brain abscess (n = 21), metabolic or toxic brain disorder (n = 19), demyelinating disease (n = 16), hypoxic-ischemic encephalopathy (n = 16), intracerebral hemorrhage including traumatic brain lesion (n = 15), and posterior reversible leukoencephalopathy (n = 3). We reviewed whether DW images and ADCmaps contribute to further characterization of brain pathologies by defining a chronological age of lesions, the presence of cytotoxic edema in lesions, and the nature of cystic lesions.
Transient global amnesia is a syndrome of temporary loss of short-term memory and is not accompanied by any other neurological deficit. Diffusion-weighted imaging is useful to improve the diagnostic accuracy of transient global amnesia. We report a 68-year-old woman with multiple lesions on diffusion-weighted imaging in the right corpus callosum and left hippocampus. To the best of our knowledge, this is the first case of a diffusion-weighted imaging lesion in the body portion of the corpus callosum.
