Influence of Co-Habitation on a Family Line Resemblance in Nutrient and Food-Group Intake among Three Generations of Japanese Women

  • Although a generation-related difference in nutrient and food-group intake has been broadly recognized in the Japanese population, few studies have examined the difference and correlation of intakes with consideration of a family line and co-habitation status. We conducted a dietary survey using female college students in dietetic course in Aichi, Japan, and their mothers and grandmothers. A validated self-administered diet history questionnaire was used for assessment of intakes. Data from 110 families were included in the analysis. The means of 13 nutrients and 10 food groups (of the 15 and 14, respectively) showed significant differences among the three generations. A significant difference was observed in the carbohydrate, protein, saturated fatty acid, cholesterol, and fish intakes between the students living with their mothers and those apart. In the correlation analyses between generations, moderate correlation was observed for most nutrients and food groups between the students and their mothers living together (correlation range = 0.30-0.61 for nutrients, and = 0.21 -0.56 for food groups). A wide variation with no consistency was observed for the correlation between the students and their grandmothers (-0.18 -0.59 and -0.33 -0.65, respectively), No meaningful correlation was observed between any two generations living apart. Among the food groups examined, pulses, fish, and vegetables showed relatively large differences for the correlation between two groups with different living conditions. When living together, the correlation coefficients for nutrients and food groups between the students and their mothers decreased according to the increase in frequency of eating out by the students. These results suggest that living together and eating together were an important factor for the resemblance in dietary habits between generations in the population with a marked generation-related difference in intakes. 투석전 만성신부전 환자의 영양상태 평가 및 식습관 조사 (Assessment of Nutritional Status and Survey of Dietary Habits in Predialysis Patients of Chronic Renal Failure)

  • The purpose of this study was to analyze the nutritional status and dietary habits in predialysis patients of chronic renal failure(CRF). The patients group was composed of total 35 persons with chronic renal failure(diabetes exclusion), male 20, female 15 who were treated in the kidney internal department and the control group also composed of 35 persons, male 18, female 17 who were classified as normal by the medical examination. Their dietary habits, nutritional status and nutritional knowledge were investigated from two general hospitals in Inchon, middle of this year 2002. There were 31.4% of low weight patients (BMI below 20), 77.1% of anemia patients (serum hemoglobin below 12g/㎗), 6S.6% of hypertension patients with diastolic blood pressure over 90mmHg, 80% with systolic blood pressure over 140mmHg, 20% of hypercholesterolemia patients (serum cholesterol over 230mg/㎗), and 22.9% of hyperlipemia patients (serum triglyceride over 200mg/㎗). The cardiovascular disease seemed to be caused by the abnormality of lipid metabolism. The possibility of the bone disease was shown from patients of hyperphosphatemia (serum phosphorus over 4.7mg/㎗, 22.9%) and hypocalcemia (serum calcium below 8.4mg/㎗, 25.7%). Intake of insufficient calories which was caused by the lack of appetite affected on the nutritional status. The intake of most nutrients was not significantly different from the RDA for Koreans. Consequently, the patient groups took a lot of salt even after the diagnosis of CRF. But patients ate 6.lg of salt which were more than the recommended amount 2∼4g for patients with CRF. The patient groups, who had the experiences of nutritional counselling, had significantly higher nutritional knowledge related to CRF than control group. Unfortunately, patients could not have enough chances for nutritional counselling by the nutritionist even though they needed the nutritional informations and dietetic treatments. The continuous research is expected with regard to the detail plan for the improvement of nutritional support and the nutritional counselling because it is important to decide the requirements of nutrients for patients with kidney disease, considering the kidney function and status of nutrition.

개별실습을 강화한 당뇨교육이 제2형 당뇨병 환자의 혈당, 자가 관리 및 자각증상에 미치는 효과 (Effects of Diabetic Education Fortified with Individual Practice on Plasma Glucose, Self-care, and Self Reported Physical Symptom in Type 2 Diabetic Patients)

  • Purpose: This study was attempted to estimate the effects of diabetic education fortified with individual practice on plasma glucose, self-care, and self reported physical symptoms in type 2 diabetes patients Method: The subjects consisted of 46 patients with type 2 diabetes patients, who took the hospitalization diabetes education program from July 2003 until February 2004 at Seoul C university hospital. The experimental group was assessed at preand post intervention. The diabetes education was provided for one week. The education consisted of diabetes education videos for the diabetic introduction, group education for medication therapy, dietetic treatment and diabetes complication education. Also individual education for nurses examination of glycemia and insulin injection practice. Results: The HbA1c values significantly decreased from 9.6% on the time of hospitalization to 7.4% 3 months after discharge. In respect to the number of days of self-care, medication, diet, exercise, cleansing feet, and carrying sweets to prepare for hypoglycemia all significantly increased 3 months after discharge compared to the values at the time of hospitalization. Self reported physical symptoms were also significantly increased 3 months after discharge compared to the time of hospitalization. Conclusion: The diabetic education fortified with individual practice can be practically used as a plan for managing glycemia, self-care, and self reported physical symptom of diabetes patients.

한국 병원 영양사의 지위 및 업무내용에 관한 실태조사 (A Survey on the Status of Hospital Dititians and their Job Analysis)

  • Since the first dietitian was produced in Korean about 15 years ago the Korena Dietetic Association has grown up to have its 8,500 members as of March, 1979. This study was undertaken to survey the status of hospital dietitians and their bob analysis. current practices were learned and the problems identified mainly in such aspects as : 1. Do the hospitals have enough dietitians? 2.What is the administrative position of the dietetics in the hospital? 3. What is the salary level of the dietitians? 4. How professional are the dietitian's daily tasks? How appropriately are the jobs distributed among the dietary employees? and 5. Do the hospital dietetics have their own diet manuals to go by? The findings are : 1. The severly lacking number of dietitians are employed by the hospitals where one dietitian is responsible for the feeding and nutrition education of 171 in-patients on the average. 2. The administrative position of hospital dietetics appears to be low showing administrative position of hospital dietetics appears to be low showing only 45.7% of the sample hospitals recognize their dietetics as the independent department or section. 3. The starting salary of the 4-year college graduate dietitians is 151,450 won which is 89.23-87.26% of the average starting salary for the 4-year college graduates I 1979. The starting salary for the 4-year college graduates in 1979. the starting salary of the 2-year college graduate dietitians is 148,000 won which is 113.9% of the average standard salary for the 2-year college graduates in 1979. 4. The hospital dietitians spend most of their time doing clerical jobs rather than the jobs utilizing higher priority professional skills they ought to perform. the most obvious cause of the problem can be found in the lack of man power for the general clerical jobs in the dietetics which can be proven from the small number of dietary clerks hired by the sample hospitals not even one person (0.45) per hospital on the average. 5. 68.6% of the sample dietetics have some form of compiled diet manuals or guidelines. 11.4% of the samples have diet guide lines ready for the important diets only. 14.3% of the samples do not have any form of diet guideline prepared. 5.7% of the samples use reference diet manuals from other hospitals when need.

고혈압 환자의 체격지수, 식습관 및 영양소 섭취상태에 관한 연구 (A Study on Physical Indices, Food Habits and Nutrient Intakes in Patients with Hypertension)

  • This study analyzed the physical indices, food habits and nutrient intakes of patients with hypertension, to provide better research data for the prevention and the cure for disease as well as basic data for nutrition education. The subjects were 44 patients who visited the general hospital, located in Daegu, to receive hypertension treatment The lifestyle, food habit, nutrient intakes, anthropometric measurements, body fat and blood pressure were measured as main indices. By the BMI, $72.8\%$ of the patients were over weight or obesity $(>23\;kg/m^2)$. In the male group, the WHR tended to increase according to the BMI (p<0.05). In the female group, the body fat tended to increase according to the BMI (p<0.05). The group with more monthly income showed lower systolic blood pressure than the group with smaller monthly income (p<0.05). The scores about food habits concerning balanced diet and preference for salt were low, which implied that food habits of subjects were not desirable. The intakes of riboflavin and calcium didn't meet the RDA requirements. Especially, the lack of calcium was more ap~nt in the female patients than the male ones. Despite this, they had more than enough protein, vitamin C, and iron. The physical indices according to blood pressure revealed that the group of higher blood pressure showed greater WHR (p<0.001). The male group with higher systolic blood pressure showed greater BMI than one with lower systolic blood pressure (p<0.05). As for nutrient intakes according to blood pressure, the group with higher blood pressure had more sodium and iron than one with lower blood pressure (p<0.05). The patients with hypertension tended to be more overweight or obese, preferred salty foods, and were in an unbalanced state of nutrient intakes. Thus more systematic and ongoing measures should be pursued to take care of patients with hypertension.

병원 급식.영양 서비스 향상을 위한 질 관리 기준 및 질 평가 도구의 개발 (Development of Quality Management Standards and Evaluation Tool for the Quality Improvement of Hospital Food and Nutrition Services)

  • The purpose of this study were to : a) establish quality managements standards and performance indicators in order to define the concept of the hospital food and nutrition services, b) develop a self evaluation tool for quality management applicable to dietetic department's CQI program on the basis of the estabilished quality management standers, c) investigate the factual quality of hospital food and nutrition services throught the result of the application study of the quality management tool and the perceived quality by the patients servery, and d) verify the validity of the quality management tool through analysing the correlation between the factual quality and the preceived quality. The quality management standards were classifie dinto 3 parts, 'Nutrition Care', 'Foodservice Operation', and 'Management Function'. There consisted of 72 quality management standards and 293 performance indicators. The developed quality evaluation tool using 5-Likert scale was performance at 45 general hospitals over 400 beds in Seoul and Kyungki-do. Also, 1,573 patients in 42 hospitals were participated in the patient survey. The result of quality evaluation tool application study showed that 17 standards were relatively high(more than 4.5) and 22 relatively low(less than 4.0) in score. The categorical mean scores of $\ulcorner$Nutrition Care$\lrcorner$, $\ulcorner$Foodservice Operation$\lrcorner$and $\ulcorner$Management Function$\lrcorner$ were respectively 60.7, 162.8, and 73.5 Each mean score was conversed to 81.0, 83.5, and 81.7 on the basis of 100 point scale, but the range of score was very wide between hospitals. The Cronbach's $\alpha$ was more than 0.6 in 59 quality management standards, this result verified reliability of the quality management tool was proved. The factual quality showed positive correlation with the perceived quality. Therefore, this result verified the criterion validity of the quality management tool.

고지혈증인 폐경 후 여성에서 이소플라본 보충이 혈청 지질 농도 및 항산화능에 미치는 영향 (The Effect of Isoflavone Supplement on Plasma Lipids & Antioxidant Status in Hypercholesterolemic Postmenopausal Women)

  • Postmenopausal women are at an increased risk of developing coronary artery disease. This is due to primarily dyslipidemia accompanying the loss of estrogen secretion. Soy isoflavones are known to have weak estrogenic effects. The purpose of this study is to investigate whether isoflavone supplement improves the risk of cardiovascular disease in hypercholesterolemic postmenopausal women. Subjects consisted of 39 Korean postmenopausal women with hypercholesterolemia (total cholesterol $\geq$ 200 mg/㎗ or LDL cholesterol $\geq$ 130 mg/㎗). Subjects were divided into 2 groups; placebo group (PG), isoflavone supplement group (IG). During 12 weeks, subjects were given placebo and 80mg isoflavone daily. Anthropometric measurement, blood sample analysis and dietary intake measurement were taken at baseline and after 12 weeks. After 12 weeks, systolic blood pressure was decreased significantly (p < 0.01) and plasma HDL cholesterol level was increased significantly (p < 0.05) in IG. But there were no significant changes in plasma total cholesterol, LDL cholesterol and triglyceride levels after isoflavone supplementation. There was a negative correlation between initial plasma HDL cholesterol level and the extent of plasma HDL cholesterol reduction in IG (r=-0.572, p=0.012). Atherogenic index (AI), total-/LDL- cholesterol ratio and LDL/HDL cholesterol ratio were improved significantly after isoflavone supplementation. In subjects whose initial plasma LDL cholesterol level were above 160 mg/㎗, plasma malondialdehyde (MDA) level were decreased and total antioxidant status (TAS) were increased significantly after isoflavone supplement (p < 0.05). However there were no significant changes in flow-mediated dilator (FMD), the marker of endothelium-dependent vasodilation and nitroglycerine-mediated dilator (NMD), the marker of endothelium-independent vasodilation and the extent of DNA damage after isoflavone supplement. In conclusion, these results indicate that isoflavone supplement may decrease the risk of cardiovascular disease via improving blood pressure, HDL cholesterol level and AI in hypercholesterolemic postmenopausal women. Futhermore, in case of subjects with elevated LDL cholesterol level, isoflavone supplementation may have more antiatherogenic effects via improving antioxidant status.

Individualized Medical Nutrition Therapy Improved Nutritional Status and Quality of Life in Hemodialysis Patients

  • A case-controlled, 12 week follow-up, study was designed to investigate the effect of dietician-delivered medical nutrition therapy (MNT) on the nutritional status and quality of life in hemodialysis patients. Subjects were recruited at Kyung-Hee Medical Center and were randomly assigned to two groups : the control and the MNT group. The MNT group received individualized MNT for 12 weeks. The results were as follows: 1) The mean ages of the control (n = 20) and MNT (n = 24) groups were 50.6 $\pm$ 14.8 and 45.7 $\pm$ 14.0 years, and the mean durations of dialysis were 2.3 $\pm$ 2.3 and 1.7 $\pm$ 1.9 years, respectively. The interdialysis weight gain of the subjects was higher than that recommended. 2) The indicators of the subjects nutritional status showed that S to 25% of the subjects had some degree of malnutrition, with most of them in the mild malnutrition category. 3) After 12 weeks of the experiment, the percentage of the ideal body weight (% IBW) of the control group decreased, but that of the MNT group increased. Changes in other anthropometric parameters in both groups during the study period were not significantly different. 4) At the beginning of the study, the 54% of the MNT group consumed more than 28 kcal/kg body weight/day and 50% consumed more than 1.0g protein/kg body weight/day. However, these percentages rose to 71% and 75%, respectively, after 12 weeks of the individualized MNT. 5) The serum albumin and blood urea nitrogen (BUN) levels of the control group decreased significantly artier 12 weeks of the experiment, while those of the MNT group did not change. 6) After 12 weeks of individualized MNT, the mean score of nutrition knowledge and total mean score of quality of life (QL) of the MNT group were significantly higher than that of th\ulcorner control group. Body pain and social functioning scores of the MNT group were significantly higher than those of the control group. The positive effect of individualized MNT on the hemodialysis patients consisted of their improved nutritional status, nutritional knowledge, and the quality of life. These results suggest that individualized MNT continuously performed by a dietitian can be helpful for hemodialysis patients. However, larger and longer term studies are needed to confirm these positive effects of MNT. In addition, the development of nutritional education programs for MNT is needed to increase the positive impact of MNT.

체중조절 프로그램이 중년기 비만여성의 식습관 및 혈액성분에 미치는 영향 (Effects of Weight Control Program on Dietary Habits and Blood Composition in Obese Middle-Aged Women)

  • 본 연구에서는 12주간 중년 비만여성을 대상으로 체중조절 프로그램 (영양교육 및 상담, 운동요법)을 실시하여 식습관, 혈액성분, 체성분의 변화를 살펴보고자 하였다. 영양 교육실시 후 식습관에서는 식사의 규칙성, 간식섭취횟수, 외식횟수 및 식사속도 등이 긍정적으로 개선되었다. 혈액지질 성분과 혈압에서는 총콜레스테롤, 중성지방, LDL-콜레스테롤, 수축기 혈압이 유의적으로 감소하였으며, 동맥경화지수와 이완기 혈압도 유의적인 차이는 없었지만 감소하였다. 비만지표인 BMI와 WHR (허리-엉덩이 둘레비)이 유의적으로 감소되어 체중이 감량됨을 알 수 있었고 체지방률, 지방량도 유의적으로 감소되었으며, 근육량은 유의적인 차이는 없었지만 약간의 증가를 가져와서 장기적으로 체성분의 긍정적인 변화를 기대할 수 있었다. 이상의 결과를 미루어 효율적인 비만관리를 위해서는 획일적인 방법보다는 대상자들의 특성을 고려하여 식행동 개선에 초점을 맞춘 영양교육 및 상담과 아울러 운동요법을 병행하여관리가 이루어져야 할 것으로 생각된다. 그러기 위해서는 각 영역의 전문가들의 긴밀한 협조와 노력으로 실질적인 건강향상을 위한 체계적인 비만관리 프로그램이 이루어져 비만을 예방하고 건강지표 향상을 가져와 국민건강 증진에 도움이 될 것으로 본다.

New Directions in Communicating Better Nutrition to Older Adults

  • Nutrition education should be an important component of ongoing health promotion for older adults and their caregivers. This is because prevention through sound nutrition and food hygiene practices and regular excercise is the most cost-effective way to reduce risks for and deal with their major health problems. nutritions education services should effectively promote optimum intake and successful self-care. Unfortunately, however relative to other vulnerable groups, nutrition education for older adults has not been systematically developed or evaluated. Usually oder adults care a lot about their health, so this should be a relatively easy group to teach - but their increasing numbers, longevity and great diversity with respect to health, physical, and economic status and educational level present challenges. Some older adults may not perceive they would benefit from nutrition education, so interesting and motivating them is a challenge. The food and nutrition knowledge of older people has been acquired through a lifetime of experience. For most older adults in the Asian region, their sources are restricted by their restricted education, so that their major sources of information have been informal sources, such as television, radio, friends, family, and perhaps newspapers and magazines if they are literature. Nonetheless, dietary advice for older people should build on their existing knowledge and ingrained values. It should provide information useful in daily food selection, and focus on food, not nutrients - the same foods and groups considered appropriate for younger people, with consistent messages as given throughout the population. Attention must also be paid to discovering learning styles in older people. When we teach in schools, the young students are a captive audience resigned to their learning role. Learning by an older adult, however, reflects an effort to meet his or her perceived needs. Therefore, nutrition education should be a positive experience in a non-threatening environment, relaxed and non-competitive, and perhaps even social environment. The messages also need to be practical and achievable. A needs assessment is essential, because our ability to provide the most effective nutrition education will depend on our success in matching the needs, both perceived and unperceived. of this vulnerable group. Therefore, go to the potential older learners to assess their interest and preferences. Nutrition education activities for older adults are widespread, but few have been evaluated. Evaluation is therefore also recommended, particularly when new methods are used. Tips from other countries for giving successful nutrition education will be given, including some examples of applications as attempted in Hong Kong. Research needs will also be described. In conclusion, successful nutrition education for older adults depends on positive needs-based messages. This is may be hard to do, as few good examples are available to illustrate these principles.
