Early dropout predictive factors in obesity treatment

  • Diet attrition and failure of long term treatment are very frequent in obese patients. This study aimed to identify pre-treatment variables determining dropout and to customise the characteristics of those most likely to abandon the program before treatment, thus making it possible to modify the therapy to increase compliance. A total of 146 outpatients were consecutively enrolled; 73 patients followed a prescriptive diet while 73 followed a novel brief group Cognitive Behavioural Treatment (CBT) in addition to prescriptive diet. The two interventions lasted for six months. Anthropometric, demographic, psychological parameters and feeding behaviour were assessed, the last two with the Italian instrument VCAO Ansisa; than, a semi-structured interview was performed on motivation to lose weight. To identify the baseline dropout risk factors among these parameters, univariate and multivariate logistic models were used. Comparison of the results in the two different treatments showed a higher attrition rate in CBT group, despite no statistically significant difference between the two treatment arms (P = 0.127). Dropout patients did not differ significantly from those who did not dropout with regards to sex, age, Body Mass Index (BMI), history of cycling, education, work and marriage. Regardless of weight loss, the most important factor that determines the dropout appears to be a high level of stress revealed by General Health Questionnaire-28 items (GHQ-28) score within VCAO test. The identification of hindering factors during the assessment is fundamental to reduce the dropout risk. For subjects at risk, it would be useful to dedicate a stress management program before beginning a dietary restriction.

A Study on Use of Alternative Therapy in Chronic Ill Patients (만성 질환자의 대체요법 이용 실태 조사연구)

  • This retrospective descriptive study was conducted to survey the use of alternative therapy by chronicly ill patients. The subject of this study were 205 chronicly ill patients at Pusan National University Hospital. The data for this study were patient record. The data were gathered from August, 1 to September, 30 1998 and analyzed through SPSSWIN program for frequency, percentile and $x^2-test$. The important results of this study are as followings. Of population-sociological characteristics on studied objects, sexual distribution showed a similar percentage male 47.8% and the female 52.2%. Age distribution showed that the 60-69 year old group made up 31.5%, and was the highest. Of religion Buddism was the most frequest. The diagnosis distribution showed cancer at 28.8% and coronary artery disease at 16.1%. According to the results of the study, 51.2% of subjects had used alternative therapy. The most common type of alternative therapy was dietary and nutritional therapy, 40.2%, the place of use, home, 44.0%, duration, less than 3 months, 51.5%. The most common motive was a recommendation by friends or family. The degree of satisfaction after the use of alternative therapy was high for 21.0%, and slight for 53.3%. The reaction after the use of alternative therapy was increasing power for patients with coronary artery disease and cancer, sugar control for those with DM, and pain control for those with chronic arthrits. The study showed that for chronicly ill patients, age, religion, diagnosis name have an important effect on the use of alternative therapy. In conclusion, chronicly ill patients showed high-use of alternative therapy but they did not used alternative therapy as a healing method. Therefore we must provide an education program nurses and physicians. And we must provide more information or healing method's and support chronicly ill patients.

How single amount of the intake food is for patients with gastrectomy? (위절제술 환자가 한 번에 식사할 양은 어떻게 결정하는가?)

  • Background : Many gastrectomy patients have experienced poor intake and it was cause of the decreasing nutritional status. The purpose of this study was to identify the amount of the intake food and the influencing factors among gastrectomy patients. Methods : A cross-sectional study was conducted with 163 patients who underwent gastrectomy. Data were collected through self-report questionnaires and chart review about demographic, clinical, nutritional status and dietary information and were analyzed with the SPSS 20.0 WIN program. Result : Among 163 patients, the group with over 3/4 intake food was 115(70.5%). The group with 1/2 intatke was 30(18.4%) and the group with the lowest intake of 1/4 was 18(11.1%). There were significant differences in single amount of intake food by patient's subjective feeling(P<.001). Single amount of intake food among three groups were no significant difference in type of gastrectomy, hospital length, body mass index and albumin level. Conclusion : The findings of the study showed that single amount of the intake food was correlated with subjective feeling of patients. Therefore, subjective feelings should be considered important factors in developing diet program for patients with gastrectomy.

Eating Behaviors, Perception of Body Image, Hematological Indices and Nutrient Intake of Adolescent Female Athletes in Incheon (인천지역 청소년 여자 운동선수의 식행동, 체형에 관한 인식 및 영양상태평가)

  • The purpose of this study was to investigate the eating behaviors, the self-perception of body images, the hematological indices and the nutrient intake of adolescent female athletes in Incheon. The subjects were 112 female athletes(track and fleld: n=32, target shooting: n=27, fencing n=29, swimming: n=14, badminton: n=10) from middle and high schools in Incheon. This cross-sectional study was conducted by means of a sol(-administered questionnaire. Fasting blood samples were obtained and analyzed for hemoglobin (Hb), hematocrit (Hct), ferritin, serum iron, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration(MCHC), unsaturated iron binding capacity (UIBC), total iron binding capacity (TIBC) and transferrin saturation (TS). Nutrient intakes obtained by means of the 3 day-recall method were analyzed using the Computer Aided Nutritional Analysis Program. Statistical analysis was conducted using the SPSS 10.0 program. Most of the female athletes had dietary problems such as eating unbalanced meals, skipping meals, and preferences for processed foods. More than 60% of the female athletes skipped breakfast. As for perception of their body images, track and field athletes in particular, controlled their weights significantly better than the other athletes (p<0.05). With regard to their physical exertion during exercise, 56.3% of the swimmers and 31.3% of the track and field athletes answered “very hard”, which was a significant difference (p< 0.001). More than 80% of the female athletes experienced vertigo during exercise (p< 0.01). Also more than 50% of the female athletes, with the exception of the badminton players, had experienced irregular menstruation (p<0.05). The average serum iron levels (p<0.05), the serum ferritin levels (p<0.05) and TS (p<0.05) of the track and field athletes was significantly lower as compared to that of the other athletes. The nutrient intakes of the female athletes, with the exception of Vitamin B$_{6}$, niacin and phosphorus were lower than the Korean Recommended Daily Allowances (RDA). In particular, the calcium and iron intakes of the female athletes were under 50% of the Korean RDA. Therefore, proper nutritional education and supplementation are required for female athletes to encourage desirable eating habits, as well as to improve their nutritional status and exercise performances.s.

Effect of Nutrition Support Program for the Elderly in a Rural Community (일 농촌지역에서 수행된 노인 영양공급 프로그램의 효과)

  • Objectives: This study was performed to assess the effects of nutrition support for the elderly in rural communities. Methods: The data for this study were obtained from 148 aged over 65 years in G district(70 intervention group and 78 control group). General characteristics, physical health status, food intake, dietary habits, knowledge of nutrition and nutrition risk factors of the subjects were examined by individual interview. The participants were divided into two groups through the first questionnaire of nutrition: nutritional intervention group(nutritional intervention and education of nutrition) and control group(education of nutrition alone). Results: Nutritional intervention group showed significant increase of energy intake to 87.4% from 71.0% and of most nutrients except vitamin A and niacin after intervention. After nutritional intervention program, depression index was significantly decreased and changes of food habits, self-efficacy and conviction indices were significantly increased. Conclusions: This study showed that the nutritional intervention serving foods for short-term intervention period was very effective in improving the nutritional status. In addition, it suggested that it was not enough for nutritional improvement of the elderly to provide public health education or counseling alone, therefore, for achieving its goals, it should be needed proper nutritional supply to them.

Evaluation of the dietary quality and nutritional status of elderly people using the Nutrition Quotient for Elderly (NQ-E) in Seoul (노인 영양지수 (NQ-E)를 이용한 서울 일부지역 노인의 식생활 및 영양 상태 평가)

  • Purpose: This study evaluated the dietary quality and nutritional status of elderly people using the Nutrition Quotient for Elderly (NQ-E). Methods: The participants were 204 elderly people over 65 years of age (38 men and, 166 women) in Seoul. The dietary information was analyzed using a questionnaire of NQ-E, which consisted of 19 checklist items, and 24-recall test data. The NQ-E scores and its four factors, including 'balance', 'moderation', 'diversity', and 'dietary behavior' factors, were calculated according to general characteristics of the subjects. The subjects were divided into the 'monitoring needed group' (62 > NQ-E score) and the 'good group' (62 ≤ NQ-E score) according to their NQ-E score. Results: The mean NQ-E score of the total subjects was 61.9, which was within the mediumhigh grade. The scores of balance, moderation, and dietary behavior factors were within the medium-high grade, while the score of the diversity factor was within the medium-low grade. The NQ-E score was 54.8 in the monitoring needed group and 69.3 in the good group. For the score of the diversity factor, the elderly living alone had a significantly lower score than the score for the elderly living with a spouse. The female subjects showed significantly higher scores of moderation and dietary behavior factors than did the male subjects. The daily intakes of vitamin A, thiamin, riboflavin, vitamin C and calcium in the monitoring needed group were significantly lower than those in the good group. The nutrient adequacy ratios (NAR) of vitamin A, thiamin, riboflavin, vitamin C and calcium in the monitoring needed group were significantly lower compared to the scores of the good group. The indexes of nutritional quality (INQ) of vitamin A, thiamin, riboflavin, vitamin C, calcium and potassium were less than 1 for all the subjects. The monitoring needed group had a significantly lower consumption of total foods, vegetables and mushrooms than the good group. As a result, the nutritional status of the monitoring needed group was significantly lower than that of the good group. Conclusion: The results of this study show that NQ-E would be a useful tool for assessing the dietary quality of the elderly. In conclusion, a focused-nutrition education program and a useful guideline are needed for promoting the health and nutritional status in elderly people.

Health-related Dietary Behaviors and Lifestyle Factors associated with Sodium Hyperingestion in Korean Adults (한국 성인 나트륨 과다섭취군의 남녀별 건강관련 식이행태와 생활양식 특성요인)

  • This study was performed to determine health-related dietary and lifestyle of characteristic factors associated with sodium hyperingestion of korean adults by sexual difference. Study subjects were 30~69years (n=1,622) adults who participated in the 2010 Korean National Health and Nutrition Examination Survey (KNHANES V-1). Subjects were divided into the hyperingestion group (HG, n=1,494) and normal ingestion group (NG, n=128) by 'WHO growth standard sodium intakes(2,000mg/day)' and the relationships between Comparison of demographics and physical measurement, eating behaviors and health-related factors were analyzed. By logistic regression method, risk factors for sodium hyperingestion group revealed in this study were gender(female), number of family', 'breakfast', 'lunch', 'dinner', 'nutrition labeling use'. and In case of male, risk factors for sodium hyperingestion group were 'lunch', 'dinner', 'self-rated body image' of eating behaviors. In case of female, Which were number of family' of demographics, 'breakfast', 'lunch', 'dinner' of eating behaviors. In these results, according to different sexual aspect of sodium hyperingestion, we suggest that nutrition label use and proper nutrition intake criteria among sexual, recognition promotion campaign along with education program for repressed sodium hyperingestion related to hypertension prevention of korean adults should include eating habits improvement and demographics lifestyle modification.

A Study on the Health and Nutritional Status and Nutrient Intakes in Elderly Korean Female (서울 일개 교회 여자 노인의 영양상태, 건강상태와 영양소 섭취에 관한 연구)

  • Purpose: The purpose of this study was to investigate the health status, nutritional status, and nutrient intakes of Korean female elderly who are members of a local church in Seoul, Korea. Methods: The subjects were 59 female who are aged >65 yr. We investigated the nutritional status of the subjects using anthropometric and biochemical measurements. Interview was conducted to obtain the information regarding the health status. We also obtained dietary intake of these subjects to figure out the nutrient intakes. Results: The mean age of the subject was 74.0 yr old. Based on the WHO standard (obesity, body mass index, BMI ${\qeq}25$), we categorized the subjects to the obese and the non obese groups. The number of subjects in the obese group was 34 (57.6%). The mean Diastolic BP (p=.002), BMI (p<.001), waist circumference (p<.001), hip circumference (p<.001), blood level of transferrin (p=.038), and nutrient intakes of calcium (p=.026), potassium (p=.046), folate (p=.038) and vitamin E (p=.031) were higher in the obese group than in the non obese group. The subjects with hypertension was 67.5%. The presence of hypertension was correlated with BMI (p<.05). The number of subjects with hypercholesterolemia was 17 (28.8%). The level of cholesterol was correlated with BMI (p<.05) and wasit/hip ratio (p<.05). Conclusion: The female elderly in our study had higher prevalence of obesity which is a risk factor for hypertension and hypercholesterolemia. The nutrient intake of high sodium, high cholesterol, and low dietary fiber of these subjects suggests that there is an urgent need to develop an well planned nutritional education program for female elderly.

A Study on Serum Lipid Levels and Dietary Fat and Fatty Acid Intakes in Primary School Children (초등학교 아동의 혈청 지질 상태 및 지방 섭취에 관한 연구)

  • The purpose of this study was no investigate serum lipid levels of primary school children and to estimate their intakes of total fat and fatty acids. Subjects fasting blood samples were obtained and analyzed for serum triglycerides(TG), total cholesterol(Chol) and high density lipoprotein-cholesterol (HDL-Chol). Low density lipoprotein cholesterol (LDL-Chol), LDL-Chol/HDL-Chol ratio(LPH), and atherogenic index(AI) were calculated,. Dietary intake of nutrients was assessed by means of a 24-hour recall method using food models and other measuring tools . The serum levels of TG, Chol and LDL-Chol in girls were higher than those in boys, but the serum HDL-Chol level of girls was lower than that of boys. As the degree of obesity increased, the serum TG level of girls was lower than that of boys. As the degree of oesity increased , the serum TG level of girls increased. The serum LDL-Chol level was higher in obese boys than in normal ones. Percentage s of subjects at risk of cardiovascular disease based on corresponding criteria of TG, Chol,HDL-CHol and LDL-Chol were 25.9%, 7.6%, 20.7% and 10.1%, respectively. The serum TG level of children provided with the national school lunch program(NSLP) was lower than that of children without NLSP. The total fat intake of boys was higher than that of girls, but calorie-adjusted fat intake became similar between the two groups. Intakes of polyunsaturated fatty acids(PUFA), monounsaturated fatty acids(MUFA) and saturated fatty acids(SFA) were 13.3$\pm$9.5g, 16.1$\pm$9.3g and 21.4$\pm$14.2g in boys, respecitively , and 12.6$\pm$11.3g, 15.3$\pm$9.7g and 19.9$\pm$13.1g in girls, respectively . The ratios of polyunsaturated /monoun-saturated /saturated /saturated fatty acids(P/M/S) in boys and girls were 0.7/0.8/1.0 and 0.8/0.8/1.0 respectively. The ratios of $\omega$6/$\omega$3 fatty acids in boys and girls were found to be 12.1 /1.0 and 8.6/1.0 , respectively. These results indicated the urgent need of nutritional education in primary schools to prevent further increase in childhood obesity and hyperlipidemia . Therefore, this study will contribute to the establishment of dietary guidelines and health recommendation for school children.

A Study on the Nutrient Intake and Food Habits of College Students in Chung-Nam Area (충남 일부지역 대학생의 식이섭취 및 식습관 실태조사)

  • The study examined nutritional conditions to enhance the quality of meals and to provide the basic data for more proper dietary life of college students. Our analysis was based on the body measurement, food intake and food habits for 200 students of Chung-Nam. The nutrient intake was examined using the 24-hour recall method. The data were analyzed by the Computer Aided Nutritional Analysis Program and then the diet quality was estimated using the Mean Adequacy Ratio(MAR), the Nutrient Adequacy Ratio(NAR) and the Index of Nutritional Quality(INQ). The average weight and height of male students were $67.57{\pm}8.66kg,\;174.64{\pm}5.87cm$, while those of female students were $53.46{\pm}6.86kg,\;159.10{\pm}4.19cm$, respectively. The total energy intake of the male students was $3221.71{\pm}200.39kcal$, while that of the female students was $1884.67{\pm}100.93kcal$. The male students are higher than female in % of RDA. The percentages of carbohydrate, protein and lipid for male students were $57.18{\pm}7.83%,\;15.15{\jpm}3.36%\;and\; 27.67{\pm}6.68%$, while those of female students were $58.77{\pm}7.68%,\;15.51{\pm}4.55%\;and\;25.72{\pm}6.20%$, respectively. There was no significant difference in calcium intakes for both groups, however they are less than the RDA. In terms of iron, the male students took $16.98{\pm}5.57mg$ and the female students took $11.35{\pm}2.68mg$, with significant difference (p<0.001). The male and female students showed sufficient intake of vitamins, with significant difference between two groups except for vitamin A. The mean MAR, an index of overall dietary quality, was significantly higher in the male students than female students(p<0.01). Male and female students enjoying two meals or more a day were 59%, and those having enough time for one meal or less a day were 36.5%. Male and female students having no breakfast were 39%. Male and female students having vegetables intake were 53.5%, and having milk or milk products every day were 29.5% of them. In conclusion, nutritional education is recommended to increase the calcium intake and fruit and vegetable consumption for dietary fiber, and to emphasize regularity of meal time.
