Evaluation of a Smart After-Care Program for Patients with Lung Cancer: A Prospective, Single-Arm Pilot Study

  • Background: The efficacy of telemedicine among cancer survivors is uncertain. The Smart After-Care Program (SAP), which is an interactive, smartphone-based remote health monitoring system, was developed to help patients manage their health after leaving the hospital. This study was designed to evaluate the efficacy of our remote health care program for lung cancer patients. Methods: We enrolled 50 patients with lung cancer. Self-monitoring devices were supplied to all patients, who were instructed to enter their daily vital signs and subjective symptoms to the Smart After-Care app. The app also provided information about rehabilitation exercises and a healthy diet for lung cancer patients. All patients received health counseling via telephone once a week and visited an outpatient clinic during weeks 6 and 12 to assess satisfaction with the SAP and changes in quality of life and physical performance. Results: Overall satisfaction with the SAP was very high (very good, 61.9%; good, 26.2%). In the multivariate analysis to identify factors affecting satisfaction, the distance between the patient's residence and the hospital was the only significant independent factor (p=0.013). Quality of life improved along all functional scales (p<0.05). Muscle strength significantly improved in the lower limbs (p=0.012). Two-minute walk distance also significantly improved (p=0.028). Conclusion: This study demonstrated that the SAP was acceptable for and supportive of patients with reduced pulmonary function after lung cancer treatment. The SAP was found to be particularly useful for patients living far from the hospital.

Dietary supplementation with Korean pine nut oil decreases body fat accumulation and dysregulation of the appetite-suppressing pathway in the hypothalamus of high-fat diet-induced obese mice

  • BACKGROUND/OBJECTIVES: Korean pine nut oil (PNO) has been reported to suppress appetite by increasing satiety hormone release. However, previous studies have rendered inconsistent results and there is lack of information on whether dietary Korean PNO affects the expression of satiety hormone receptors and hypothalamic neuropeptides. Therefore, our study sought to evaluate the chronic effects of Korean PNO on the long-term regulation of energy balance. MATERIALS/METHODS: Five-week-old male C57BL/6 mice were fed with control diets containing 10% kcal fat from Korean PNO or soybean oil (SBO) (PC or SC) or high-fat diets (HFDs) containing 35% kcal fat from lard and 10% kcal fat from Korean PNO or SBO (PHFD or SHFD) for 12 weeks. The expression of gastrointestinal satiety hormone receptors, hypothalamic neuropeptides, and genes related to intestinal lipid absorption and adipose lipid metabolism was then measured. RESULTS: There was no difference in the daily food intake between PNO- and SBO-fed mice; however, the PC and PHFD groups accumulated 30% and 18% less fat compared to SC and SHFD, respectively. Korean PNO-fed mice exhibited higher messenger RNA (mRNA) expression of Ghsr (ghrelin receptor) and Agrp (agouti-related peptide) (P < 0.05), which are expressed when energy consumption is low to induce appetite as well as the appetitesuppressing neuropeptides Pomc and Cartpt (P = 0.079 and 0.056, respectively). Korean PNO downregulated jejunal Cd36 and epididymal Lpl mRNA expressions, which could suppress intestinal fatty acid absorption and fat storage in white adipose tissue. Consistent with these findings, Korean PNO-fed mice had higher levels of fecal non-esterified fatty acid excretion. Korean PNO also tended to downregulate jejunal Apoa4 and upregulate epididymal Adrb3 mRNA levels, suggesting that PNO may decrease chylomicron synthesis and induce lipolysis. CONCLUSIONS: In summary, Korean PNO attenuated body fat accumulation, and appeared to prevent HFD-induced dysregulation of the hypothalamic appetite-suppressing pathway.

Towards water-efficient food systems: assessing the impact of dietary change and food waste reduction on water footprint in Korea

  • Globally, agriculture is one of the largest consumers and polluters of water resources, contributing to the unsustainable use of limited water resources. To reduce the resource use and environmental footprints associated with current and future food systems, researchers and policy makers have recommended the transition to sustainable and healthier diets and the reduction of food loss and waste along the food supply chain. However, there is limited information on the synergistic effects and trade-offs of adopting the two measures. In this study, we assessed the water-saving potential of the two measures in South Korea using environmentally extended input-output relying on the EXIOBASE database for the reference year 2020, along with scenario analysis to model the potential outcomes. Specifically, we analyzed scenarios where meat consumption was reduced by 30% and 50% and in combination with a 50% reduction in food waste at the consumption stage for each scenario. According to our findings, by considering individual measures of dietary change and food waste reduction, shifting to a diet with 30% and 50% less meat consumption could lead to reduction in water footprint by 6.9% and 7.5%, respectively, while 50% reduction in food waste at the consumption stage could save about 14% of water footprint. However, the synergistic effects of the two measures such as 30% less meat consumption and 50% food waste reduction, and 50% less meat consumption and 50% food waste reduction result to 20% and 24% reductions in water footprint, respectively. Moreover, our findings also showed that increasing food consumption with high environmental impacts could promote resources use inefficiency when waste occurs. Thus, policy strategies that address synergistic effects of both dietary change and food waste reduction should be strengthened to achieve sustainable food system. International and national policies can increase resource efficiency by utilizing all available reduction potentials while considering strategies interactions.

우리나라 소화기암 환자들의 대체의료이용에 관한 연구 (Use of Complementary and Alternative Medicine among Cancer Patients in Korea)

  • Complementary and alternative medicine (CAM) has gained in popularity among cancer patients in recent years. The use of CAM in cancer patients is common with about one third of patients using some form of CAM in Western countries. The purpose of this study was to analyze the use of CAM and determine what factors affect to use CAM among cancer patients to provide CAM therapy information and assist therapy selection among various CAM therapies to cancer patients. The design of the study was descriptive cross-sectional, and data were collected using a 16-item questionnaire. This study was conducted in subjects with confirmed diagnosis of stomach, colon, liver, or pancreas cancer, in both out-patients clinics and inpatients setting in a tertiary hospital in Seoul Korea. As a result, among the participants, past or current CAM use was reported by 75%, which shows a statistically significant difference in income groups(P<0.05), but no difference in age and religion groups. The most common therapies use by cancer patients included traditional Korean medicine (32.1%), folk remedies (26.6%), exercise (14%), dietary supplements (11.6%), physical therapy (9.9%), diet therapy (5%), and meditation (4%). 77.8% of patients show satisfaction and 64.4% shows perceived effectiveness of CAM. Male patients with higher income, and previous treatment were more likely to use CAM. The main benefits from CAM reported by cancer patients were psychological improvement and symptom improvement. Of the cancer patients used CAM, 30.9% were dissatisfied, 25.8% did not have benefits from the use, and 7.6% experience side effects. Cancer patients who prefer CAM (more than 3 kinds) used it to cure cancer, on the contrary, the one who do not prefer CAM used to improve symptoms and psychological stability. The main sources of information about CAM were family and friends(54.4%), and media(24.5%), doctor and nurse(18.3%), and religion group(2.6%). Findings suggest that due to the relatively high use of CAM among cancer patients in Korea, this topic should be taken into account in the development of a holistic approach to cancer patients and efficient cancer patients management system and proactive and consistent management of CAM is necessary in the health care system in Korea.

패밀리 레스토랑의 영양표시제도 시행에 따른 대학생들의 영양정보에 관한 인식 연구 (Perception of University Students on Nutrition Information According to Food & Nutrition Labeling Systems in Family Restaurant)

  • 본 연구에서 외식 시 식품영양표시에 대한 이용실태, 인지여부 및 활용도를 조사하고 이들 요인간의 관련성을 분석한 결과는 다음과 같다. 조사대상자는 영양교육을 받은 경험이 없는 대학생들로 패밀리 레스토랑의 자율영양표시제도가 활성화되기 전인 2006년과 활성화된 2011년 설문조사를 통해 자료를 수집하였다. 2006년 110명, 2011년 176명으로 총 286명이 조사에 참여하였으며 남자는 108명, 여자 178명으로 연령은 20살 전후였다. 건강에 대한 인식은 2006년 63.6%가 '건강하지 못하다'고 하였고 6.4%는 '건강한 편'이라고 답하였으며, 2011년 조사결과 54%가 '건강하지 못하다'고 하였고 13.6%가 '건강한 편'이라고 답하였다. 체중 관리에 대한 관심도는 2006년 10%, 2011년 9.1%가 '매우 관심이 많다'라고 하였다. 하루 평균 외식 횟수는 2006년 82.7%, 2011년 87.4%가 하루 1회 이상 외식을 한다고 하였다. 패밀리 레스토랑 이용실태를 살펴보면 2006년에는 패밀리 레스토랑 이용 시 75.5%가 주변의 권유로 방문하였으나 2011년 57.4%로 감소하였으며, 과거 경험에 의해 방문하는 경우는 12.7%에서 23.9%로 약 2배 증가하였고 컴퓨터나 스마트폰을 이용하는 비율이 6.4%에서 15.3%로 2배이상 증가하였다. 매장에서 보내는 시간을 살펴보면 2006년 2시간 이상 시간을 보내는 비율이 17.3%인 반면 2011년에는 35.8%로 패밀리 레스토랑에서 머무는 시간이 더 길어졌으며, 외식비용과 패밀리 레스토랑 1회 이용 시 1인당 평균 외식비용도 증가하였다. 메뉴 이용 시 관심도를 살펴보면 맛, 영양, 신메뉴, 외형에 관한 관심도는 유의적인 차이가 없었으나 가격에 있어서는 2006년 $3.94{\pm}0.71$, 2011년 $4.10{\pm}0.66$으로 유의적인 차이를 보여 메뉴 선택 시 가격을 중요하게 생각하는 것으로 나타났다. 패밀리 레스토랑의 충분한 영양정보 제공 여부에 대해 2006년 $2.52{\pm}0.93$, 2011년 $3.30{\pm}0.77$로 유의적으로 증가하였으며, 패밀리 레스토랑의 메뉴판에 제시된 영양정보 활용에 관한 질문에 2006년에 비해 2011년에 활용도가 증가했다고 응답하였다. 맛이 좋은 음식이 영양가가 풍부하다고 생각하느냐는 문항 또한 2006년 $2.45{\pm}0.82$, 2011년 $3.51{\pm}0.91$로 유의적으로 증가한 것을 볼 수 있었다. 메뉴 영양표시 활용도에 관한 인식에서는 2006년, 2011년 각각 26.1%, 23.9%가 메뉴의 영양가를 중요하게 생각한다고 답했으며 고려하는 영양성분표시로는 총열량을 전후 각각 39.4%, 35.6%로 가장 많이 고려하였다. 고려하지 않는다는 응답자 중 각각 68.2%, 59.7%가 영양성분 이외의 다른 요인을 더 중요시하기 때문이라고 답했다. 영양정보가 지속적으로 제공되길 바라냐는 질문에 2006년, 2011년 각각 82.4%, 86.9%가 지속되길 바란다고 답했으며, 지속적으로 제공되길 바라는 영양성분표시로는 총 열량이 2006년, 2011년 각각 46.6%, 34.2%로 가장 높았고 나트륨의 경우 2006년 6.8%에서 2011년 17.3%로 요구도가 가장 증가하였다. 메뉴 영양표시의 만족도는 현재의 영양표시제를 이해하느냐는 질문에 2006년엔 $2.87{\pm}0.99$이며, 2011년엔 $3.35{\pm}0.84$로 영양표시 이해도는 유의적으로 증가함을 보였다. 영양성분표시가 필요하다고 생각하는가에 대한 질문에는 2006년에는 $3.68{\pm}0.9$, 2011년도엔 $4.32{\pm}0.61$로 필요성에 대한 인식도 유의적으로 상승함을 알 수 있었다. 영양정보 요구에 대한 충족도는 2006년 $2.85{\pm}0.66$, 2011년 $3.06{\pm}0.65$로 유의적으로 증가함을 보였다. 영양표시에 대한 이해도, 필요도, 유용도, 충족도 간의 상관계수는 이해도와 필요도의 상관계수는 0.513, 이해도와 유용도의 상관계수는 0.420으로 상관이 있는 것으로 나타났으며 필요도와 유용도 사이의 상관계수는 0.641로 높은 상관을 보였다. 이해도와 만족도의 상관계수는 0.190, 필요도와 만족도의 상관계수는 0.173으로 상관이 매우 낮았으며, 유용도와 만족도의 상관계수는 0.335로 상관이 낮은 편이었다. 연구 결과, 패밀리 레스토랑의 자율영양표시제도가 적극적으로 시행된 후에도 메뉴 선택 시 영양적인 면에 대한 고려는 낮게 나타났다. 그러나 2006년에 비해 외식업체에서 충분한 영양정보를 제공하고 있으며, 이를 사용하는 횟수도 증가하고 있다. 소비자들의 인식 또한 지속적으로 영양정보가 제공되길 바라고 있으며 특히 만성질환과 관련이 깊은 열량, 지방, 나트륨에 대한 요구도가 증가한 것으로 나타났다. 그리고 영양정보를 활용하는 소비자들의 만족도도 증가하고 있으며 이에 대한 필요성도 높아지고 있다. 따라서 지속적인 자율영양표시제도가 확대되어야 하며 영양권장량에 대한 비율도 함께 제시함으로써 지속적인 건강식을 할 수 있도록 해야 한다. 또한 영양표시에 대한 올바른 이해는 이에 대한 필요도를 증가시키고 유용도도 함께 증가시키므로 식생활이 형성되는 시기부터 영양표시를 접할 수 있도록 해야 한다. 향후 영양교육을 받은 경험이 있는 대상자의 영양표시에 대한 인식과 활용도에 대한 연구가 이루어진다면 영양표시제도에 대한 영양교육의 필요성을 알아볼 수 있으므로 이에 대한 연구가 수행되어야할 것으로 생각된다.

Quali-Quantitative Analysis of Flavonoids for Mulberry Leaf and Fruit of 'Suhyang'

  • BACKGROUND: Globally, mulberry (Morus sp.) is exploited for feeding leaf to silkworms in order to obtain silk fiber or for animal feedstock production. Also, mulberry fruit is known to a by-product that was produced from mulberry tree after harvesting leaves for silkworm rearing, as a yield and consumption of mulberry fruit was increased, it has been fixing to a newincome crop. Mulberry leaves and fruits are used for the health benefits of human beings. Mulberry contains various bioactive components, such as alkaloids and flavonoids. Mulberry flavonoids are an important part of the diet because of their effects on human nutrition. The flavonoids in mulberry leaf and fruit of 'Suhyang'(Morus alba L.) were determined. METHODS AND RESULTS: Flavonoids for mulberry leaf and fruit of 'Suhyang' were analysed using ultrahigh performance liquid chromatography coupled with diode array detection and quadrupole time-of-flight mass spectrometry (UPLC-DAD-QTOF/MS)technique. An UPLC-DAD-QTOF/MS system was used, and identification of mulberry leaves constituents was carried out on the basis of the complementary information obtained from LC spectra, MS ions, and MS/MS fragments. The mulberry leaf (16 flavonoids) and fruit (9 flavonoids) were isolated and analyzed from Suhyang using UPLC-DAD-QTOF/MS chromatogram. To the best of our knowledge, Quercetin 3-O-(6"-O-malonyl) glucoside and quercetin 3-O-rutinoside (rutin) was detected on the highest content in leaf and fruit, respectively and further research will be devoted to evaluate their biological activity. CONCLUSION: Obtaining information about the concentration of functional materials in mulberry leaves could contribute to the development and promotion of processed, functional products and offer possible industrial use of 'Suhyang', holding promises to enhance the overall profitability of sericulture.

일본국립국회도서관의 한국 관련 주제명 전거데이터 분석 (An Analysis of Subject Authorities Related to Korea in the National Diet Library of Japan)

  • NDL 전거시스템을 고찰하고 이를 바탕으로 NDL 전거데이터 가운데 한국 관련 주제명 전거데이터를 추출하여 주제명의 현황 및 특성을 분석하였으며, 결과를 요약하면 아래와 같다. 첫째, NDL의 한국 관련 주제명 전거데이터는 전거표목 2,205건, 참조 938건을 포함하여 전체 3,143건이다. 이 가운데 사회과학이 52.4%로 절반 이상을 차지하고 있으며, 개별 학문별로 경제학 전거데이터가 552건 17.6%로 가장 많았다. 둘째, 일본에 의해 유발되었거나 직접적인 연관이 있는 역사적 사건의 주제명은 대부분 일본의 시각에서 전거데이터를 기술하고 있으며, 한국에 익숙한 용어는 주로 전거표목이 아니라 참조에 기재되어 있다. 셋째, 한국적인 특성이나 역사적 사건을 나타내는 주제명이 매우 부족하며, 대부분이 일반적인 성격의 주제명으로 표현되어 있다. 넷째, 국가명에 있어 남한과 북한을 함께 통칭하는 용어를 '조선(朝鮮)'으로 하고 있어 여전히 우리나라를 조선시대에 머물게 하고 있다. 시대세목에 있어 '대한민국(大韓民国)'과 '조선민주주의인민공화국(朝鮮民主主義人民共和国)' 시대인 1948년 이후의 역사를 세분할 필요가 있으며, '발해(渤海)'는 중국 역사로 간주하고 있으나, 엄연히 한국 역사의 일부이므로 한국의 시대세목에 추가되어야 한다.

한국 다문화 가정 학생의 문제행동 관련 요인에 관한 체계적 문헌고찰 (Related Factors of Problem Behaviors among Students of Multicultural Family in Korea: A Systematic Review)

  • 본 연구는 한국 다문화 가정 학생의 문제행동 관련요인을 분석한 체계적 문헌고찰 연구이다. 체계적 문헌고찰은 국내 데이터베이스인 한국연구정보서비스, 한국학술정보서비스와 국회전자도서관의 온라인 검색을 통해 2018년 7월 4일까지 검색된 국내문헌을 대상으로 하였으며, 자료 검색어는 다문화와 문제행동 혹은 행동문제로 하였다. 그 결과, 총 2,084편이 검색되었고, 최종 21편이 분석에 이용되었다. 분석결과, 다문화 가정 학생의 문제행동 관련 요인으로 개인적 요인(성별, 학업성적, 자아존중감 등), 가족적 요인(가족의 유형, 부모지지, 어머니의 문화적응스트레스 등), 환경적 요인(친구지지, 교사지지, 사회적지지)의 변수들이 관련요인으로 파악되었다. 또한, 한국 다문화 가정 학생의 문제행동을 측정한 변수는 학교생활적응이 10편으로 가장 많았고, 심리적, 사회적 적응 또는 부적응이 4편, 문제행동 또는 위험행동과 정서적 행동문제가 각각 3편으로 나타났다. 이를 근거로 다문화 가정 학생의 문제행동과 관련된 개인적, 가족적, 환경적 요인에 대한 중재를 통해, 문제행동 발생을 줄일 필요가 있으며, 이를 위한 사회적, 정책적 노력이 필요함을 알 수 있다. 또한 문제행동에 대해 보다 구체적이고 일관성 있는 이론적 정의를 근거로 도구를 개발하여, 문제행동을 정확히 측정하고, 이를 예방할 수 있는 교육적, 사회적 전략이 마련되어야 할 것이다.

Evaluation of a Nutrition Model in Predicting Performance of Vietnamese Cattle

  • The objective of this study was to evaluate the predictions of dry matter intake (DMI) and average daily gain (ADG) of Vietnamese Yellow (Vang) purebred and crossbred (Vang with Red Sindhi or Brahman) bulls fed under Vietnamese conditions using two levels of solution (1 and 2) of the large ruminant nutrition system (LRNS) model. Animal information and feed chemical characterization were obtained from five studies. The initial mean body weight (BW) of the animals was 186, with standard deviation ${\pm}33.2$ kg. Animals were fed ad libitum commonly available feedstuffs, including cassava powder, corn grain, Napier grass, rice straw and bran, and minerals and vitamins, for 50 to 80 d. Adequacy of the predictions was assessed with the Model Evaluation System using the root of mean square error of prediction (RMSEP), accuracy (Cb), coefficient of determination ($r^2$), and mean bias (MB). When all treatment means were used, both levels of solution predicted DMI similarly with low precision ($r^2$ of 0.389 and 0.45 for level 1 and 2, respectively) and medium accuracy (Cb of 0.827 and 0.859, respectively). The LRNS clearly over-predicted the intake of one study. When this study was removed from the comparison, the precision and accuracy considerably increased for the level 1 solution. Metabolisable protein was limiting ADG for more than 68% of the treatment averages. Both levels differed regarding precision and accuracy. While level 1 solution had the least MB compared with level 2 (0.058 and 0.159 kg/d, respectively), the precision was greater for level 2 than level 1 (0.89 and 0.70, respectively). The accuracy (Cb) was similar between level 1 and level 2 (p = 0.8997; 0.977 and 0.871, respectively). The RMSEP indicated that both levels were on average under-or over-predicted by about 190 g/d, suggesting that even though the accuracy (Cb) was greater for level 1 compared to level 2, both levels are likely to wrongly predict ADG by the same amount. Our analyses indicated that the level 1 solution can predict DMI reasonably well for this type of animal, but it was not entirely clear if animals consumed at their voluntary intake and/or if the roughness of the diet decreased DMI. A deficit of ruminally-undegradable protein and/or a lack of microbial protein may have limited the performance of these animals. Based on these evaluations, the LRNS level 1 solution may be an alternative to predict animal performance when, under specific circumstances, the fractional degradation rates of the carbohydrate and protein fractions are not known.

  • Aging is a senescence and defined as a normal physiologic and structural alterations in almost all organ systems with age. As Leonard Hayflick, one of the first gerontologists to propose a theory of biologic aging, indicated that a theory of aging or longevity satisfies the changes of above conditions to be universal, progressive, intrinsic and deleterious. Although a number of theories have been proposed, it is now clear that cell aging (cell senescence) is multifactorial. No single mechanism can account for the many varied manifestations of biological aging. Many theories have been proposed in attempt to understand and explain the process of aging. Aging is effected in individual by genetic factors, diet, social conditions, and the occurrence of age-related diseases as diabetes, hypertension, and arthritis. It involves an endogenous molecular program of cellular senescence as well as continuous exposure throughout life to adverse exogenous influences, leading to progressive infringement on the cell's survivability so called wear and tear. So we could say the basic mechanism of aging depends on the irreversible and universal processes at cellular and molecular level. The immediate cause of these changes is probably an interference in the function of cell's macromolecules-DNA, RNA, and cell proteins-and in the flow of information between these macromolecules.
