  • The objective of this study was to determine the effect of propofol and remifentanil combination on hemodynamics and intraocular pressure (IOP), and to compare with those of isoflurane in beagle dogs. Fourteen clinically healthy beagle dogs were divided randomly into 2 groups and each group was consisted with 7 dogs. Anesthetic agents were propofol (0.2 mg/kg/min) plus remifentanil ($0.5{\mu}g/kg/min$, 1% solution in standard saline) in one group (group PRP) and 3% isoflurane in the other group (group ISF). Anesthesia was maintained for 90 min in the both groups. IOP, blood pressure, heart rate and blood gas values (pH, $PaCO_2$, $PaO_2$, $SaO_2$, $tCO_2$, ${HCO_3}^-$) were recorded at 5, 10, 15, 30, 45, 60, 75 and 90 min in the both groups. IOP values in both eyes were significantly decreased in group PRP compared with those in group ISF. but there were no significant differences between two eyes in each group. Systolic, diastolic and mean blood pressures were significantly decreased in group PRP within the normal range. There were no differences between groups in all blood gas parameters. In this study, propofol and remifentanil combination could provide stable IOP and blood pressure compared with isoflurane.

  • The prevalence of diabetes is increasing worldwide, particularly in Asian regions such as Korea. It is estimated that the number of diabetic patients will exceed 5 million by 2030 and that about 30% of people with diabetes are unaware of their condition. A comparative analysis was conducted to determine the relationship between dairy food intake and pre-diabetes risk factors in subjects that visited the health examination center of Sunhan Hospital, Republic of Korea. Subjects were classified into two groups according to fasting blood glucose concentrations: 1) normal (fasting blood glucose <100 mg/dL, n=57) and 2) pre-diabetes (100 mg/dL${\leq}$ fasting blood glucose ${\leq}125mg/dL$, n=56). Total dairy food intake tended to be lower in females and subjects with pre-diabetes compared with males and normal subjects, respectively; however, differences between groups were not significant due to a wide range of variations. HbA1c, systolic and diastolic blood pressures were significantly associated with the prevalence of pre-diabetes. Odds ratios for pre-diabetes were significantly reduced per incremental increases in dairy food intakes after adjusting for confounding factors. Findings in the current study suggest that appropriate intake of dairy foods may need to be considered for the prevention of pre-diabetes.

  • The normal physiological values of Korean mongrel dogs were studied, comparing them with foreign references, on the basis of control physiological data measured on 110 cases out of the dogs submitted to the experiments in our department. The hemodynamic measurements varied widely between the both limits of reported normal control data, with the mean value of heart rate $140.4{\pm}26.6$/min., and both the systolic and diastolic arterial blood pressures $137.2{\pm}31.7$mmHg and $104.7{\pm}14.4$ mmHg, as well as the venous pressure of $9.11{\pm}2.18$ cm.$H_2O$. Hematologically, the number of red blood cells $4,571,000{\pm}767,000$per cu.mm., the amount of hemoglobin $11.57{\pm}3.74$ gm/dl and the hematocrit $37.3{\pm}7.2$ per cent, were equally the values a little lower than the reported normal means. However, the white blood cells were within the reported normal limits both in number, $10,384{\pm}4,877$ per cu. mm and their differential counts with slightly broader ranges of variation. The platelet count was $149,800{\pm}47,000 $per cu. mm and was also far below the normal, while the coagulation time $9.03{\pm}2.69$min. and the prothrombin time $13.17{\pm}6.52$sec were within normal limits, though a little prolonged. The serum electrolytes, Na $146.6{\pm}10.44$mEq/L.,K $4.46{\pm}0.84$mEq/L., CI $118.3{\pm}7.88$mEq/L. and Ca $11.45{\pm}5.62$mg./dl, and the blood glucose level of $94.9{\pm}31.79$mg./dl were essentially not different from the reported normal values. The serum proteins, total protein $7.15{\pm}1.41$gm/dl., albumin $4.09{\pm}0.77$gm./dl. and globulin $3.18{\pm}0.88$gm/dl. were included near the higher limits of the reported normal levels. The thymol turbidity 0.1-3.04 units were normal in 10 cases, and the cephaline flocculation was within normal range except 2 cases out of 26 dogs, showing two positive in 24 hours. And the nitrogen series, NPN $34.61{\pm}10.29$mg/dl. and BUN $12.77{\pm}6.37$mg./dl. were normal. It may be concluded that from the point of view of hereby measured physiological data compared with the foreign references, the Korean mongrel dogs have a compatible laboratory data with only the special regards to their tendency toward anemia in red blood cell series.

  • As the tertiary industry is being developed, many problems are introduced in an industrial area in Korea. This study aims to investigate the residents' health conditions, such as the prevalence of metabolic syndrome and fatty liver diagnosed by ultrasonography, comparing the urban with the industrial areas. The subjects were 378 adults in the urban area and 376 adults in the industrial area. Subject variables accounted for were gender, age, drinking habits, smoking habits, physical activities, waist circumferences and BMI. In blood tests, fasting blood glucose, triglyceride, HDL, blood pressures, total cholesterol, AST, ALT and GGT were examined. Also, the presence and absence of fatty liver, diagnosed using ultrasonography, was investigated. As a result, the prevalence of metabolic syndrome were 43.7% in the urban area and 51.6% in the industrial area, hence it was significantly higher in the industrial area than in the urban area. Fasting blood glucose, triglyceride and waist circumference in the distribution of the individual components were 47.4, 53.7, and 16.1% in the urban area and 54.5, 61.5, and 22.9% in the industrial area respectively, so it was substantially higher in the industrial area than in the urban area. On the other hand, HDL, diastolic blood pressure, and fatty liver were 25.7, 43.4, and 44.7% in the urban area and 21.3, 19.9, and 36.7% in the industrial area respectively and thus it was much higher in the urban area than in the industrial area. In conclusion, this study shows that there were significant differences between the residents' health condition, comparing the urban area to the industrial area. Therefore, the precaution and aggressive management of metabolic syndrome may be necessary, in accordance with the characteristics of the industrial zone.

  • The purpose of this study was to investigate whether there is a relationship between pulmonary function, serum lipid and pulse pressure, which is a risk factor of cardiovascular disease, in subjects with 109 office workers(51 males and 58 females) be at Seoul, Korea. The mean age of the subjects was $43.44{\pm}7.23$ years old (male $43.44{\pm}7.27$, fenale $49.1{\pm}11.5$). The subjects' height, weight, BMI, systolic and diastolic blood pressures were measured and there was statistical difference between men and women. The mean pulse pressure of the subjects was $47.82{\pm}9.02mmHg$, and the mean of FVC was $84.26{\pm}9.91%$. There was no difference between men and women in both. There was a positive correlation between pulse pressure and systolic blood pressure in males (p<0.01) and females (p<0.01), and FVC and HDL in males were positively correlated (p<0.05). Pulse pressure and %FVC were negatively correlated (r =-0.267, P<0.01). Pulse pressure and %FVC were negatively correlated (r =-0.267, P<0.01). Regression analysis showed that PP was the independent affected %FVC. Therefore, pulse pressure and %FVC may be useful as a predictor of cardiovascular disease in middle-aged subjects with normal, suggest a long-term convergence follow-up is warranted to elucidate the incidence of cardiovascular disease.

  • Purpose: Although numerous systematic reviews or meta-analysis have reported the hypotensive effects of garlic, the application of these results in the area of functional food is limited. This is because the trials used various garlic preparations and patients with differing hypertensive intensities. To validate the use of garlic powder as a blood pressure lowering functional food, we performed the current meta-analysis, focusing on the study of prehypertensive subjects. Methods: Literature search was carried out using various database up to July 2020, including PubMed, Cochrane, ScienceDirect and Korean studies Information Service System, and each study was screened by pre-stated inclusion/exclusion criteria. We identified nine trials that met the eligibility, of which two studies with moderate or high risk of bias were excluded. Results: Meta-analysis of the seven studies revealed that an intake of garlic powder significantly lowered the systolic blood pressure (SBP) and diastolic blood pressure (DBP) by -6.0 mmHg (95% confidence interval [CI], -11.2, -0.8; p = 0.025) and -2.7 mmHg (95% CI, -5.3, -0.1; p = 0.046), respectively. Shapes of the funnel plot for both SBP and DBP seemed symmetrical, and the Egger's regression revealed no publication bias. Moreover, duration of the intervention period was inversely associated with the pooled effects of garlic powder on SBP (p = 0.019) and DBP (p = 0.019), and this result was supported by the subgroup-analysis. The daily dose of garlic powder, baseline value of each biomarker, and subject number, did not moderate the effects on SBP and DBP. Conclusion: Results of the present meta-analysis indicate that garlic powder supplements are superior to placebo for improving the BP in prehypertensive individuals.

  • This study investigates the utility of the Triglyceride-glucose(TyG) index and Cardiometabolic Index(CMI) as predictors for diagnosing metabolic syndrome. The study involved 1970 males, 1459 females, totaling 3429 participants who underwent health checkups at P Hospital in Busan between January 2023 and June 2023. Metabolic syndrome diagnosis was based on the presence of 3 or more risk factors out of the 5 criteria outlined by the American Heart Association/National Heart, Lung, and Blood Institute(AHA/NHLBI), and participants with 2 or fewer risk factors were categorized as normal. Statistical analyses included independent sample t-tests, chi-square tests, Pearson's correlation analysis, Receiver Operating Characteristic(ROC) curve analysis, and logistic regression analysis, using the Statistical Package for the Social Sciences(SPSS) program. Significance was established at p<0.05. The comparison revealed that the metabolic syndrome group exhibited attributes such as advanced age, male gender, elevated systolic and diastolic blood pressures, high blood sugar, elevated triglycerides, reduced LDL-C, elevated HDL-C, higher Cardiometabolic Index, Triglyceride-glucose index, and components linked to abdominal obesity. Pearson correlation analysis showed strong positive correlations between waist circumference/height ratio, waist circumference, Cardiometabolic Index, and triglycerides. Weak positive correlations were observed between LDL-C, body mass index, and Cardiometabolic index, while a strong negative correlation was found between Cardiometabolic Index and HDL-C. ROC analysis indicated that the Cardiometabolic Index(CMI), Triglyceride-glucose(TyG) index, and waist circumference demonstrated the highest Area Under the Curve(AUC) values, indicating their efficacy in diagnosing metabolic syndrome. Optimal cut-off values were determined as >1.34, >8.86, and >84.5 for the Cardiometabolic Index, Triglyceride-glucose index, and waist circumference, respectively. Logistic regression analysis revealed significant differences for age(p=0.037), waist circumference(p<0.001), systolic blood pressure(p<0.001), triglycerides(p<0.001), LDL-C(p=0.028), fasting blood sugar(p<0.001), Cardiometabolic Index(p<0.001), and Triglyceride-glucose index (p<0.001). The odds ratios for these variables were 1.015, 1.179, 1.090, 3.03, and 69.16, respectively. In conclusion, the Cardiometabolic Index and Triglyceride-glucose index are robust predictive indicators closely associated with metabolic syndrome diagnosis, and waist circumference is identified as an excellent predictor. Integrating these variables into clinical practice holds the potential for enhancing early diagnosis and prevention of metabolic syndrome.

  • In high sodium societies, the incidence in blood pressure with childhood growth is more abrupt than the rate of rise in low sodium populations. Thus, it appears that a lower level of dietary sodium intake is required to correct established hypertension and to prevent its appearance In present work, an investigation was made to estimate the correlation between urinary sodium, potassium and creatinine excretion, weight, height, upper arm circumference, blood pressures and the number of heart rate. Sixty- four children aged 12-16 years (41 boys and 23 girls) were measured. Twenty -four-hour urinary sodium and potassium excretion averaged 132.8 mEq and 42.1 mEq in boys, 126.4 mEq and 41.3 mEq in girls. Twenty- four -hour urinary creatinine excretion averaged 795.7 mg and 744.3mg in boys and girls, respectively. Systolic and diastolic blood pressure were 117.6mmHg and 49.7mmHg in boys, 95.5mmHg and 58.2mmHg in girls. Systolic blood pressure correlated positively weight, height and urinary creatinine but diastolic blood pressure correlated positively with upper arm circumference and negatively with urinary potassium. It was concluded that urinary sodium does not explain the blood pressure.

  • This study was performed from March to June 1994 in ANsung Medical Center. We investigated serum components of 78 diabetics in the center, in addition to the nutrient intake status and anthrophometric status of 60 diabetics among them. We separated the latter into 2 groups(ⅰ) less tan 5 years of diabetic duration and ⅱ) more than 6 years of diabetic duration) and analyzed the clinical characteristics of each group. The results are as follows : \circled1 The average ages of male and female subjects were 57.1 and 58.9 years, respectively. The average diabetic duration of male subjects was 4.8 years and that of female subjects was 5.9 . The major occupational field of both male and female subjects was agriculture(33.3%, 36.1%). 50% of male and 75% of female subject's monthly incomes were less than 600, 000 won and 75% of male and 94.4% of female subjects had weekly food expense less than 40, 000 won. The nutrient intake status of the subjects was lower than RDA on the whole. There was no big differences between 2 the male groups of different diabetic duration , but calorie, protein , fat carbohydrate, phosphorus, iron and Vit B$_1$intakes of the above 6 year female group were appearently higher than those of the under 5 year female group (p<0.05). There were different intake tendencies between male and female subjects . For females. the longer the diabetic duration, the less intakes of calorie, protein , fat carbohydrate, phosphorus, iron Vit B$_1$, Vit B$_2$, and niacin were observed. Furthermore, the longer the diabetic duration in females , the more intakes of energy and nutrients, except Vit C were observed. Fasting blood glucose levels were 155mg/dl and 164mg/dl and Postprandial glucose levels were 242mg/dl and 247mg/dl for males and females respectively. Hb of both males and females , and Ht of males met the lower limit of normal status, The serum TG level was higher than normal , so that they seemed to suffer from hyperlipidemia ; their high HbAIC status indicated that the management of diabetes mellitus was poor. The total protein level of female subject was higher when compared with that of males(p<0.05). All subjects blood pressure were higher than normal , and the longer the diabetic duration, the higher was the blood pressure. The serum HDL-cholesterol levels of the above 6 year group male subjects were lower(p<0.05) than those of the under 5 year male group subjects, and the former group's systolic and diastolic blood pressures were higher than those of the latter(p<0.05). Levels of serum total protein, albumin and TG of the above 6 year female group were lower than those of the under 5 year female group(p<0.05). As the diabetic duration was extended, HDL-cholesterol and LDL-cholesterol levels showed a tendency to increase, while Hb, Ht, total protein, albumin, and A/G, which represent the nutrients state of iron and protein, were inclined to decrease. This result showed that serum lipid components went bad. To summarize the results, the longer the diabetic duration, the worse were the male subjects nutrient intake status, but those of the female subjects were good. With respect to serum components and serum lipid subjects Fe, protein, nutrition status and serum lipid component got worse as the diabetic duration was extended , implying the possibility of diabetes mellitus complication grows.
