Diaphragmatic paralysis can be demonstrated through diaphragmatic elevation on chest X-ray after thoracic lung surgery or the placement of chest tubing. Additional causes of diaphragmatic paralysis are iatrogenic, mass, atelectasis, etc. For the diagnosis of diaphragmatic paralysis, it required some studies (fluoroscopy, computed tomography [CT], magnetic resonance imaging). Diaphragmatic hernia of the liver is a rare clinical entity, usually found after trauma in adults. Congenital diaphragmatic hernia in neonates requires surgery. Non-traumatic diaphragmatic hernia of the liver in an adult is a rare right-sided diaphragmatic hernia. On developing any symptoms, surgery must be performed. When diaphragmatic hernia is incidentally found in adults without trauma, it is placed under observation for a time period. We diagnosed the diaphragmatic herniation of a right hepatic lobe by 16-slice CT scan without surgery.

The motor complications after herpes zoster infection are uncommon, but, the association of muscular paralysis and zoster has been reported for several times. The association between diaphragmatic paralysis and zoster was for the first time reported by Halpern and Covner. The case presented below showed right diaphragmatic paralysis following herpes zoster of C3 and C4 region of dermatome on the right side. A 59-year-old man was found to have a paralysed hemidiaphragm within two months of the appearance of typical herpes zoster rashs involving his right shoulder and neck. Investigations, including bronchoscopy, failed to detect other cause for the diaphragmatic paralysis. We believe that the cervical zoster and diaphragmatic paralysis were causally related.

Bilateral diaphragmatic paralysis is a rare disease. It is caused by trauma, cardiothoracic surgery, neuromuscular disorders, corvical spondylosis, and infection. A 60 year-old male patient developed bilateral diaphragmatic paralysis after an on-bloc resection of thymic carcinoma which invaded the right upper lobe, pericardium, superior vena cava and innominate vein. Severe respiratory difficulty developed and ventilator weaning was impossible. We performed bilateral diaphragmatic plication. After the operation, satisfactorily ventilator weaning and sleeping in supine position were possible; therefore, we report this case.

Herpes zoster is well-known viral disease in immune compromised that produces inflammatory lesions in the posterior root ganglia and is characterized clinically by pain and skin eruptions along the distribution of the affected ganglia. However, motor involvement after a herpes zoster is an uncommon complication. We report a case of diaphragmatic paralysis that occurred after a herpes zoster in 63-year-old woman. The diaphragmatic paralysis occurred one month after the typical herpes zoster eruptions affecting the C3 and C4 dermatomes in the right neck, shoulder and back area.

Diaphragmatic eventration is a rare disease and is caused by congenital etiology. We operated on a patient who had had preexisting left diaphragmatic eventration which was complicated by a right diaphragmatic paralysis and a persistent respiratory insufficiency due to a traffic accident. This was a very rare case and there has not yet been any case reports worldwide. We were able to abtain good surgical results from plication of left diaphragm in this case and thus report it.

Evaluation of diaphragm function is challenging because no single test has a high diagnostic yield. We describe ultrasound findings in three cases with acquired unilateral diaphragmatic elevation. These cases confirm that sonographic evaluation is a valid tool for identifying diaphragm dysfunction. In addition, ultrasound measurements of diaphragm thickness and the contractility can be used to determine if a diaphragm is paralyzed and suggest the duration of paralysis (i.e., acute or chronic).

The conformation of traumatic diaphragmatic rupture is frequently difficult, even if radiologic evaluation has been performed. A 37-year old man with multiple trauma was suspicious with diaphragmatic rupture. The diaphragmatic rupture could not be confirmed with chest CT. We decided thoracoscopic operation for diagnosis. Diaphragm was ruptured about 8 cm length involving entering site of phrenic none into diaphragm and diaphragmatic paralysis was combined. We made 5 cm sized working window additionally. Ruptured diaphragm was repaired by continuous suture and plication of diaphragm was performed. Postoperative result was good at chest radiogram after three monthes.

Diaphragmatic eventration is a rare disease, congenital or acquired, high or elevated position of one leaf of the diaphragm muscle, as a result of paralysis, aplasia or atrophy of varying degree of the muscle fibers of the affected side but with no break in the continuity of the muscle. We experienced 3 cases of the diaphragmetic eventration at the department of thoracic surgery, C.A.F.G.H., from 1980 to 1982, which were treated successfully. Among three cases, one case combinded with hamartoma of the ipsilateral lung. Specific complications were not noticed after surgical repair of diaphramatic eventration with good result.

Phrenic nerve injury has been reported with the use of iced slush for topical cardiac hypothermia. A review of the preoperative and the postoperative chest roentgenograms was performed to detect phrenic nerve injury in patients undergoing cardiac operation with the use of iced slush for topical hypothermia from January, 1985 to June, 1987. The reviewed series included 45 patients who had undergone valve replacement. In this review, the injured site of phrenic nerve was left in 13 cases, right in 1 case and the overall incidence of phrenic nerve paralysis following open heart surgery was 31%. Compared to the incidence of phrenic nerve paralysis in the control group [without pericardial insulation] [406, 12 cases/30 cases], that in the group of patients receiving pericardial insulation [13.3%, 2 cases/15 cases] was lower, but there was no statistical significance. The initial time that diaphragmatic paralysis developed was mean 3.5 days [range 1-8 days] postoperatively and the recovery time of the paralysis was mean 4.7 months [range 0.5-10.5 months] postoperatively.

Unilateral diaphragmatic paralysis due to a phrenic nerve injury is not rare after cardiothoracic surgery and may range from an asymptomatic radiographic abnormality to severe pulmonary dysfunction and even mortality in patients with reduced lung function. The most effective treatment for symptomatic unilateral diaphragmatic paralysis has been known to be a plication of the paralyzed hemidiaphragm. A 38 year-old male patient with asthma received a phrenic nerve reconstruction with a sural nerve for right phrenic nerve injury during resection of the anterior mediastinal tumor. Ten months later, chest PA showed good result and we report this case with literature.
