Diaphragm Translocation as Surgical Treatment for Agenesis of the Right Lung and Secondary Tracheal Compression

  • A 12-month-old boy was diagnosed with agenesis of the right lung. Mediastinal deviation progressed to the diseased side as the patient matured; therefore, tracheal distortion developed. As a result, tracheal compression developed between the vertebral body and aorta. The patient was repeatedly admitted to the hospital because of recurrent pulmonary infection and combined severe respiratory distress. Diaphragm translocation was performed to treat the patient. The postoperative course was favorable, and computed tomography scan findings and symptoms had improved at 1 year after surgery.

  • A 4-year-old cat was referred for a suspected pulmonary mass. True diaphragmatic hernia presence was diagnosed via computed tomography (CT). There was a thin membrane covering the diaphragmatic defect. The membrane was thinner than the diaphragm. After contrast injection, the membrane was less enhanced than that of the normal diaphragm. The membrane was identified as a remnant of the parietal pleura. In addition, contrast-enhanced CT images provided clarity in viewing the herniated liver and falciform fat. A thinner membrane, covering the diaphragmatic defect, and attached to the thicker normal diaphragm, is considered a unique CT feature of true diaphragmatic hernia.

  • Silicon diaphragms, 10 and 20 \ulcorner-thick and 1x1 mm\ulcornerarea, have been fabricated by means of the electrochemical P-N junction etch-stop technique. The P-type diffused resistors were formed on the diaphragm, and the piezoresistance effect was examined. It was been found that the fractional variation of the resistance is dependent on the diaphragm thickness, resistor location, and resistor length, etc. The 1.2 k\ulcornerfull-brige pressure sensor with 10\ulcorner-thick diaphragm exhibits a pressure sensitivity of 42 \ulcorner/V\ulcornermHg with a temperature coefficient of 2.3 mmHg/\ulcorner, and shows a good linearity in the pressure range from 0 to 300 mmHg.

Effect of slab stiffness on floor response spectrum and fragility of equipment in nuclear power plant building

  • The floor response spectrum (FRS) is used to evaluate the seismic demand of equipment installed in nuclear power plants. In the conventional design practice of NPP structure, the FRS is simplified using the lumped-mass stick model (LMSM), assuming the floor slab as a rigid diaphragm. In the present study, to study the variation of seismic response in a floor, the FRSs at different locations were generated by 3-D finite element model, and the response was compared to that of the rigid diaphragm model. The result showed that the FRS significantly varied due to the large opening in a floor, which was not captured by the rigid diaphragm model. Based on the result, seismic fragility analysis was performed for the anchorage of a heat exchanger, to investigate the effect of location-dependent FRS disparity on the high confidence low probability of failure (HCLPF).

  • A 2-year-old spayed female British Shorthair cat presented with an increased frequency and duration of cough since infant period. Based on radiographic, ultrasonographic, and computed tomography findings, peritoneopericardial diaphragmatic hernia was considered so that repair surgery was planned. During celiotomy, lax diaphragm was identified instead of defect. Transabdominal diaphragmatic plication was performed to resolve lax diaphragm and to prevent recurrence by overlapping relatively normal part of diaphragm. Diagnosed with diaphragmatic eventration postoperatively, the cat showed improvement in clinical signs and imaging results. Transabdominal diaphragmatic plication is a suitable treatment; the patient maintained normally during a 14-month follow-up period.

  • An optical microphone based on a dual-core fiber collimator and a membrane type micromirror serving as an optical head and a reflective diaphragm respectively was implemented. The micromirror diaphragm is suspended by a thin silicon bar linked with a frame, thus it is subject to a displacement induced by acoustic waves. The optical head incorporating two collimators integrated in a single housing gives light to and receives it from the diaphragm, rendering the optical microphone structure simple and compact. This dual-core collimator having a slowing varying beam profile facilitates the initial alignment of the optical head with the diaphragm, especially the distance between them. For the assembled microphone, the static characteristics were investigated tofind the operation point defined as the optimum distance between the head and the diaphragm, and a frequency response with a variation of about $\pm$5 dB for the range of up to 3kHz was achieved.

  • An optical intensity-type pressure sensor has been fabricated by coupling multimode optical fiber with 100 nm-Au/30 nm-NiCr/150 nm-$Si_3N_4/300 nm-SiO_2/150 nm-Si_3N_4$ optical reflection layer supported by micromachined frame-shape silicon substrate, and its characteristics was investigated. For the application of $Si_3N_4/SiO_2/Si_3N_4$ diaphragm to the optical reflection layer of the sensor, NiCr and Au films were deposited on the backside of the diaphragm by thermal evaporation , respectively, and thus optical low caused by transmission in the reflection layer could be decreased to a few percents. Dielectric diaphragms with uniform thickness were able to be also reproduced because top- and bottom-$Si_3N_4$ layer of the diaphragm could automatically stop silicon anisotropic etching. The respective pressure ranges in which the sensor showed linear optical output power-pressure characteristics were 0~126.64 kPa, 0~79. 98 kPa, and 0~46.66 kPa, and the respective pressure sensitivities of the sensor were about 20.69 nW/kPa, 26.70 nW/kPa, and 39.33 nW/kPa, for the diaphragm sizes of 3$\times$3 $\textrm{mm}^2$, 4$\times$4 $\textrm{mm}^2$, and 5$\times$5 $\textrm{mm}^2$, indicating that the sensitivity increases as diaphragm size increases.

  • The strain characteristics of a fiber optic Fabry-Perot pressure sensor with high sensitivity using a $Si_{3}N_{4}/SiO_{2}/Si_{3}N_{4}$ (N/O/N) diaphragm is experimentally investigated. A 600 nm thick N/O/N diaphragm was fabricated by silicon anisotropic etching technology in 44 wt% KOH solution. An interferometric fiber optic pressure sensor has been manufactured by using a fiber optic Fabry-Perot intereferometer and a N/O/N diaphragm. The 2 cm length fiber optic Fabry-Perot interferometers in the continuous length of single mode fiber were produced with two pieces of single mode fiber coated with $TiO_{2}$ dielectric film utilizing the fusion splicing technique. The one end of the fiber optic Fabry-Perot interferometer was bonded to a N/O/N diaphragm. and the other end was connected to an optical setup through a 3 dB coupler. For the N/O/N diaphragm sized $2{\times}2\;mm^{2}$ and $8{\times}8\;mm^{2}$, the pressure sensitivity was measured 0.11 rad/kPa and 1.57 rad/kPa, respectively, and both of the nonlinearities were less than 0.2% FS.

Long-Term Behavior of Square CFT Columns with Diaphragm (격막이 설치된 각형 CFT 기둥의 장기거동에 관한 연구)

  • This paper presents experimental and analytical studies on long-term behavior of square CFT columns with diaphragm. In order to investigate the effect of the diaphragm on the long~term behavior, experiments for six specimens with two diaphragms and three different column length, and three-dimensional finite element analysis for each specimen have been performed. The finite element models considering the interface behavior between the steel tube and the inner concrete were verified from comparison of the test results with the analysis results. From the test and the analysis results, the following conclusions were obtained. The confinement effect created by the diaphragm does not depends on column length and influences only a part of the whole column that is from the end to the depth which is the same to the width of the column. The shortening of the column with diaphragm which covers more than a half of the cross sectional area of the inner concrete is the same as that of the column under a load applied on the steel tube and the entire section of the inner concrete.
