Objectives : Trigeminal neuralgia and hemifacial spasm are caused by vascular compression of the REZ(root entry or exit zone) of the 5th and the 7th cranial nerve. Preoperative detection of neurovascular compression is essential for accurate diagnosis, appropriate treatment, and the good operative results. Three dimensional Fourier Transformation-Constructive Interference in Steady State(3DFT-CISS) images are known to give good contrast between CSF, nerve, and vessels. We applied a 3DFT-CISS imaging technique for the preoperative evaluation of patients with these diseases and estimated the diagnostic accuracy and usefulness of this study. Methods : A series of 71 patients with trigeminal neuralgia and hemifacial spasm were treated by microvascular decompression. Among them 34 patients with trigeminal neuralgia and 24 patients with hemifacial spasm had preoperative CISS images. We compared the radiologic finding with the operative finding, and analysed the diagnostic usefulness of 3DFT-CISS imaging. Results : The sensitivity of CISS images of detecting the neurovascular compression was 90.3% in trigeminal neuralgia and 100% in hemifacial spasm. There were one false-positive and three false-negative cases in trigeminal neuralgia, and one false-positive case in hemifacial spasm. The accuracy in diagnosing the causative vessel was 73.5% in trigeminal neuralgia and 83.3% in hemifacial spasm. Conclusion : CISS image is very useful diagnostic tool for preoperative evaluation of neurovascular compression in patients with trigeminal neuralgia and hemifacial spasm. No additional neuroradiologic examination other than CISS image and MRA is needed for preoperative evaluation of patients with trigeminal neuralgia and hemifacial spasm.
Clinical lymphadenopathies are subjected to fine needle aspiration cytology(FNAC) for diagnosing not only benign lesions but also malignant ones, as the first diagnostic procedure. While the diagnostic reliability in metastatic carcinoma is high, it is difficult to differentiate malignant lymphoma from reactive conditions. We evaluated the diagnostic reliability of FNAC in lymphadenopathy, and discuss the diagnostic limitation and its place in clinical practice in this study, Over 8 years from January 1988, FNAC of 1,216 lymphadenopathies were analyzed and among them 170 cases were compared with histopathology. The results are as follows. 1. Of ail the cases, 890 cases(73.2%) were diagnosed cytologically as benign, 312 cases(25.7%) as malignant, and 14 cases(1.1%) as unsatisfactory material. Reactive hyperplasia was diagnosed in 585 cases(65.7%) of the benign lesions, and among the malignant diseases, metastatic carcinoma was diagnosed in 248 cases(79.5%), and malignant lymphoma in 62 cases(19.9%). 2. The overall diagnostic accuracy was 89.2%, and no false positive case and 9 false negative results were observed among 170 cases which were proven by histopathology. Six cases of sampling error of false negative diagnoses included 3 of metastatic carcinomas and 3 of malignant lymphomas. The causes were difference between aspiration and biopsy site, poor fixation, or scanty cellularity with bloody smear. All 3 cases of misinterpretation error were malignant lympliomas, one of mixed type on biopsy which was diagnosed as reactive hyperplasia cytologically. In summary, FNAC technique is thought to be useful in the initial diagnosis of lymphadenopathies as well as in the follow-up of patients with known malignancy. Although the results of malignant lymphoma was less accurate than other malignant lesions, the application of strict cytologic criteria or lymphoid marker studies of aspiration material will reduce the false negative rate.
FCM(Fuzzy Cognitive Map) is a fuzzy signed directed graph for representing causal reasoning which has fuzziness between causal concepts. Authors have already proposed FCM-based fault diagnostic scheme and verified its usefulness. However, the previously proposed scheme has the problem of lower diagnostic resolution as in the case of other qualitative approaches. In order to improve the diagnostic resolution, a concept of fuzzy number is introduced into the basic FCM-based fault diagnostic algorithm. By incorporation the fuzzy number into fault FCM models, quantitative information such as the transfer gain between the state variables can be effectively utilized for better diagnostic resolution. Furthermore, an enhanced TAM(Temporal Associative Memory) recall procedure and modified and modified pattern matching scheme are also proposed.
The aim of this study was to assess the Clinical Usefulness of Helicobacter pylori Stool Antigen (HpSA) immunochromatographic assay for the diagnosis of H. pylori infection. In this study, we had compared HpSA-immunochromatographic assay with CLO test and UBT test. From a total of 140 patients (M:F=88:52) with upper endoscopy, biopsy specimens were obtained for CLO test. Stool specimens was collected from all patients and tested using a HpSA-immunochromatic assay. H. pylori infection status was defined as infected if the results of both CLO test and UBT test were positive. CLO test and UBT test findings showed that 92 patients were H. pylori positive and 48 patients were H. pylori negative. According to this definition, the sensitivity, specificity, and positive or negative predictive value (PPV, NPV) of HpSA-immunochromatographic assay were 97.8%, 100%, 100%, and 96%, respectively. Cross reactivity test of HpSA-immunochromatographic assay were performed with 10 enteric bacteria strains in fecal habitat, and there were no false positive reaction. We evaluated the usefulness of HpSA assay for eradication therapy with 10 of 92 H. pylori positive patients, positive results of them at pre-eradication therapy were converted to negative at post-eradication. The HpSA-immunochromatographic assay is a highly sensitive and specific non-invasive diagnostic method for detection of H. pylori infection, a useful diagnostic method for H. pylori in post eradication stage.
Analysis of intraepidermal nerve fibers using skin biopsy is a recently developed technique, providing diagnostic information on small fiber neuropathies. The specimens are obtained by 3 mm punch biopsy, which is safe and minimally invasive. Immunohistochemical staining by Protein gene product (PGP) 9.5 demonstrate not only intraepidermal nerve fibers but dermal structures, such as sweat gland and erector papillae. Up to now, many studies agree that intraepidermal nerve fiber density is dramatically reduced in various sensory neuropathies. The utility of density measure was confirmed with high sensitivity in the diagnosis of sensory neuropathy, comparable to sural nerve biopsy or quantitative sensory testing. Besides quantitative methods, morphological changes like axonal swelling and fragmentation can be used as predegenerative markers. This article reviews the technique of skin biopsy and clinical and experimental usefulness of skin biopsy in diagnosing and monitoring peripheral neuropathies.
Objectives: The liver fibroscan has been developed as a noninvasive and convenient method to assess hepatic fibrosis. This study aimed to review this device in terms of its technological concept and clinical application in traditional Korean medicine (TKM). Methods: The technological background, diagnostic evaluation, and clinical usefulness of fibroscan were reviewed using various literature and clinical studies. One clinical example-a patient with hepatofibrosis who had been treated with TKM-is presented. Results: The liver fibroscan was approved as a medical device for noninvasive measurement of the hepatic fibrosis level by the Korean Food and Drug Administration (KFDA). Numerous clinical studies have confirmed that its sensitivity and specificity allow it to serve as a substitute for liver biopsy, the present gold standard diagnostic method. The accuracy and reproducibility and lack of technical risks are strong points of the fibroscan; however, it has some limitations for application, especially in patients with obesity or severe hepatitis. One clinical example showed the applicability of the liver fibroscan in herbal medicine-based treatments. Conclusions: The requirement for diagnostic medical devices is an important issue in TKM; therefore, this study provides valuable information for practitioners of TKM.
Since the successful advent of IVF-ET, reproductive surgeons have been forced to compare pregnancy outcomes of surgical procedures for tubal infertility with those of IVF-ET. The current study was designed in an effort to determine the usefulness of pre-operative infertility work-up, especially diagnostic laparoscopy, in predicting the pregnancy rate and in assessing the reversal feasibility. 109 patients who underwent a microsurgical tubal reversal were reviewed. The patients were followed over one year. The total intrauterine pregnancy, spontaneous abortion, and ectopic pregnancy rates were 66.4%, 3.7%, and 7.3%, respectively. The result showed that the use of loupe might increase the chance of the ectopic pregnancy. I found that the final tubal length, especially the longest tubal length, the site of reanastomosis, and the methods of sterilization affected the pregnancy rates. A 85% intrauterine pregnancy rate was achieved in the patients with good preoperative assessments(Group II-1). There was only 10% incidence of intrauterine pregnancy in the patients with bad assessments(Group II-2). It is concluded that the pre-operative infertility work-up, especially diagnostic laparoscopy, is a good diagnostic tool in assessing the reversal feasibility.
We have developed and applied a diagnostic Multi-Leaf Collimator (MLC) to optimized the X-ray field in medical imaging and the usefulness evaluated through the fusion of infrared image and X-ray image acquired by infrared camera. The hand and skull radiography with multi-leaf collimator(MLC) showed significant area dose reductions of 22.9% and 31.3% compared to ARC and leakage dose was compliant with KS A 4732. Also scattering doses of 50 cm and 100 cm showed a significant decrease to confirm the usefulness of MLC. It was confirmed that the fusion of infrared images with an adjustable degree of transparency was possible in the X-ray images. Therefore, fusion of anatomical information with physiological convergence is expected to contribute and improvement of diagnostic ability. In addition, the feasibility of convergence X-ray imaging and DITI devices and the possibility of driving MLC with infrared images were confirmed.
1986년부터 1995년까지의 10년간 영남대학교 의과대학 부속병원에서 절제술이나 세침 생검을 시행하여 조직 병리학적으로 확진된 신경아세포종 23례 중 전산화단층촬영을 시행한 16례를 대상으로 후향적으로 분석한 결과 신경아세포종은 소아에서 많이 발생하며, 부신에서 호발하고, 임상적인 주 증상은 만져지는 종괴였다. 전산화단층촬영소견은 과반수 이상에서 작고 고밀도이거나 곡선 모양의 석회화를 보이며, 중앙선을 침범하며, 분엽상의 모양과 분명한 경계를 보이고, 내부에 낭성 변화를 보이며, 불균등한 조영 양상을 보이고, 주위 중요 혈관과 림프절을 침범한 소견을 보였다. 이와 같은 전산화 단층촬영소견들은 소아 복부 종양에서 신경아세포종의 진단과 감별에 도움을 줄것으로 생각된다.
