Comparison of histopathological and molecular diagnostic methods in Helicobacter suis infection in Korea (국내 도축돈의 위병변과 Helicobacter suis의 검출과의 상관관계)

  • Helicobacter suis is a gram negative bacterium and colonizes in porcine stomach. It causes gastric diseases in the stomach and plays a significant role in daily weight gains in pigs. Recent studies about one of potential sources of human gastric diseases. Therefore, this study was conducted to compare histopathological lesions and molecular detection of Helicobacter suis in the pyloric mucosa of porcine stomachs transferred from slaughterhouses, based on gross and histological examinations and a PCR assay. A total 90 stomach samples were investigated to record gastric lesion scores by characteristic gastric lesions, followed by routine H & E and Warthin-Starry silver staining to detect Helicobacter-like organisms. For PCR assay, H. suis specific primers and conditions are used. Sixty-one samples (67.8%) showed gross gastric lesions, of which 38 samples (40.2%) presented grade 1, 12 samples (13.3%) presented grade 2, and 11 samples (12.2%) presented grade 3, respectively. In Warthin-Starry silver stain, Helicobacter-like organisms were detected from 11 samples (12.2%) with 4 samples (4.4%) for grade 0, 5 samples (5.6%) for grade 1, 1 sample (1.1%) for grade 2 and 1 sample (1.1%) for grade 3, respectively. The PCR resulted positive in 37 samples (41.1%) with 14 samples (15.6%) for grade 0, 14 samples (15.6%) for grade 1, 3 samples (3.3%) for grade 2 and 6 samples (6.7%) for grade 3, respectively. Positive samples for both examinations were 5 samples (5.6%). The result suggested that it should be considered as one of factors causing a gastric disease in pigs. Also, it could be acknowledged to research fundamental aspects of Helicobacter-induced gastritis in human as an animal model.

  • This study aims to empirically examine the effect of feedback on attention recovery. The role of feedback has been proven to be positive in particular to extend the limitation of attention resource. We studied the impact of feedback on attention by varying its type and modality. An experimental system was developed to observe how accurately the participants performed the pattern-matching task with differential feedback provided on a real-time basis based on the ADHD diagnostic model. Six university students participated in this study with 6 different feedback conditions and controlled conditions. The participants experienced the feedbacks before experiment. They was asked two hundred tasks in four feedback conditions. The difference of feedback effect according to different modality is to find within a subject. The results indicated that the combined feedback of cognitive with perceptual stimulus led better performance than the combined of perceptual feedbacks.

Full mouth rehabilitation with vertical dimension increase in patient with loss of anterior guidance due to maxillary anterior teeth wear: A case report (상악 전치부 마모로 전방 유도가 상실된 환자에서 수직고경 거상을 동반한 전악구강회복 증례)

  • Severely worn dentition is frequently multifactorial. It is crucial that the etiology of excessive wear be determined, but accurately diagnosing the factors responsible for tooth wear is often confusing. Before initiating the treatment of these cases, meticulous examination and determining vertical dimension are essential. A 69-year-old male patient had the chief complaint that he has worn dentition and functional and esthetic discomfort. Based on model analysis and diagnostic wax up, new vertical dimension had been determined. Provisional restorations were cemented and after 5 months permanent prostheses were fabricated. This case reports a satisfactory functional and esthetic clinical outcome achieved by restoring the vertical dimension.

  • Multiple tooth loss and excessive occlusal wear can result in damage to occlusal disharmony, functional disorders and esthetic problems, requiring comprehensive prosthetic treatments. Changing vertical dimension harmonized with surrounding muscle tissue is important. In this case, the patient with loss of vertical dimension caused by severe tooth loss and tooth wear was treated with the analysis of vertical dimension, such as diagnostic model, radiography and various clinical exams. the patient was satisfied with favorable functions and esthetics for 1 years of follow-up.

  • Flat panel based digital radiography (DR) systems have recently become useful and important in the field of diagnostic radiology. For DRs with amorphous silicon photosensors, CsI(TI) is normally used as the scintillator, which produces visible light corresponding to the absorbed energy. The visible light photons are converted into an electric signal in the amorphous silicon photodiode. In order to produce good quality images, we need to understand the detailed behavior of DR detectors in radiation. We, therefore, investigated the relationship between DR outputs and X -ray in terms of absorbed energy, using the SPEC-78, X-ray energy spectrum model. We calculated the total filtration of X-ray equipment measuring air exposure and this value was used in the calculation of absorbed energy. The relationship between DR output and the absorbed energy of the X-ray was obtained by matching the absorbed energy with pixel values of real images under various conditions. It was found that the relationship between these two values was almost linear. The results were verified using phantoms made of water and aluminium. The pixel value of the phantom image was estimated and compared with previous results under various conditions. The estimated pixel value coincided with the results, although the effect of scattered photons introduced some errors.

Convergent Research of Korean-Western Medicine: the Concept, Types and Suggestions for Promoting the Research (동서의학 융합연구: 개념, 유형, 활성화를 위한 제언)

  • In this paper, we try to define and classify the concepts of Convergent Research of Korean-Western Medicine (CRKWM) to clarify the goals of research. In addition, some suggestions that are necessary to promote the CRKWM were made. The results are as follows. First, the CRKWM should be used as a concept that refers to 'interdisciplinary research' jointly conducted by experts in both Korean and Western medicines beyond their own fields. Second, True CRKWM is an "eclectic combination of East-West" that combines the advantages of traditional Korean medicine and western medicine to create a new field; it is necessary to redefine the concept of 'Integrative medicine', which is used in different context by the traditional Korean medicine and western fields in Korea, as an "eclectic combination of East-West" type. Third, In order to promote the CRKWM, it is necessary to convert the current dual medical system of relativism to one of pluralism. Forth, In order for CRKWM to be promoted, methodologies, theories, and models that become the backbones of convergence should be developed. As of now, systems medicine and network science are the best fit. Fifthly, In order to promote convergent clinical research, the development of Korean-Western diagnostic model that integrates the pathologies of Western medicine and symptoms of Korean medicine is urgent. Lastly, Mutual understanding between Korean and Western medicine fields should be preceded in order for CRKWM to be stimulated.

Effect of Sleep Quality on Fatigue and Quality of Life : a Sasang Constitutional Medicine Perspective (사상체질에 따른 수면의 질과 피로 및 삶의 질 관련성)

  • Previous studies have reported an association between poor sleep and various symptoms and diseases, such as fatigue, obesity, depression, and anxiety. The effects of poor sleep may differ by age and sex. In addition, sleep characteristics and their effects may vary according to Sasang constitutional type. The aim of this study was to investigate the associations between sleep quality, fatigue, and quality of life and to assess whether these differ by constitutional type. Participants were individuals aged 40-69 years living in two Korean communities in 2012-2014. Sleep quality, fatigue, and quality of life were measured using the Pittsburgh Sleep Quality Index (PSQI), the Fatigue Severity Scale, and the 12-item Short Form Health Survey, respectively. The effects of total PSQI score and PSQI component scores were analyzed using a generalized additive model. A Korean Sasang constitutional diagnostic questionnaire was used to assess Sasang constitution. Data for 5,793 participants were analyzed. Poor sleep quality was related to greater fatigue, and lower physical and mental quality of life. The PSQI components including subjective sleep quality, sleep latency, sleep disturbances, use of sleep medications, and daytime dysfunction were associated with fatigue and physical and mental quality of life. Sleep quality was significantly lower in So-Eum compared to So-Yang and Tae-Eum. PSQI component scores for fatigue and quality of life differed significantly by Sasang constitution: for Tae-Eum, sleep latency and use of sleep medications; for So-Eum, daytime dysfunction; and for So-Yang, use of sleep medications and daytime dysfunction. The effects of different aspects of sleep quality differ by Sasang constitution. To improve sleep quality, interventions need to be tailored to constitutional type.

  • Diagnostic problem solving is a major application area of knowledge-based systems research. However, most of the current approaches, both heuristic and model-based, are designed to identify single faults, and do not generalize easily to multiple fault diagnosis without exhibiting exponential behavior in the amount of computation required. In this paper, we employ a decomposition approach based on system configuration to generate an efficient algorithm for multiple fault diagnosis. The basic idea of the algorithm is to reduce the inherent combinatorial explosion that occurs in generating multiple faults by partitioning the circuit into groups that correspond to output measurement points. Development of a Nutrition Quotient (NQ) equation modeling for children and the evaluation of its construct validity (어린이 영양지수 (NQ, Nutrition Quotient) 모형 개발과 구성타당도 평가)

  • The objective of this study was to develop and evaluate the construct validity of a Nutrition Quotient (NQ) for children. In a previous report (Kang, et al., 2012), the food behavior checklist for children's NQ, consisting of 19 items, was grouped into a 5-factor structure according to the exploratory factor analysis: balance, diversity, moderation, regularity, and practice. In this study, the construct validity of the NQ was assessed using a confirmatory factor analysis. Elementary school students (n = 1,393) from six large cities completed the NQ test. Indicator tests suggested an adequate model fit (goodness of fit index = 0.9613; adjusted GFI = 0.95; standardized root mean square residual = 0.0464; chi-square test statistics of < 0.001 p-value, 82.1), and item loadings were significant for all subscales (p < 0.05). The standardized path coefficients were used as the weights of the items. The NQ and the 5 factor scores of the student were calculated by the obtained weights of the questionnaire items. Logistic regression was applied to find the significant factors in order to affect a specific nutrient status. The receiver operation characteristic curve analyses were performed in order to find diagnostic cut-off points of the five factors. The food behavior checklist for children's NQ would be a handy and suitable instrument for evaluating dietary behaviors of Korean children.

A Proposal of Risk Management Framework for Design as a Secure Power Control System (안전한 전력 제어시스템 설계를 위한 위험관리 프레임워크 제안)

  • In smart grid, enhancement of efficiency and interoperability of electric power system is achieved through the connection with outer network, and this induces that power grid system is threatened increasingly, becomes the main target of cyber terrorism, and is sincerely required to design the secure power system. Although SSDLC(Secure System Development Life Cycle) is used for risk management from the design phase, traditional development life cycle is somewhat limited for satisfaction of information security indicator of power control system. Despite that power control system should reflect control entities of information security considering its own characteristics, validation elements are insufficient to apply into real tasks based on existing compliance. To make design of diagnostic model and assessment process for power control system possible and to give a direction for information security and present related indicator, we propose the new risk management framework of power control system which is applied operational security controls and standard architecture presented by IEC 62351 TC 57 with enterprise risk management framework.
