Accidental electrocution in two cattle confined in stanchions

  • On a farm with 6 Korean beef cattle, an adult cow and a calf suddenly died while eating. At that time, hair was pressed upon the stanchion facilities where the adult cow was fed. Two dead cattle were submitted to the Animal and Plant Quarantine Agency for disease diagnosis. Grossly, the mouth of the adult cow was full of feed that had been eaten before death, and there was a foamy fluid in the calf's trachea. Both hemorrhages were observed in the muscles, as well as in the internal organs such as the liver, lungs, and heart. Ruminal mucosa was easily peeled off. Histopathologically, both muscles and internal organs showed severe hemorrhages. In particular, affected muscles were shown segmental and band-like patterns of myonecrosis, disappeared some myocytes, burned hair follicles, and irregular surface of skin. Severe hemorrhages of muscles and internal organs were consistent with previous reports. Easily separated mucosa of rumen and muscle lesions were very characteristic. We determined electrocution according to the pathological findings, laboratory results, and traces of accidents. This report will help to understand of pathological lesions of electrocution. Furthermore, this case suggests that the electrical wires and devices must be installed and handled carefully during the construction of the cowsheds care.

자동현상기 지능화에 필요한 연산처리 기법의 개발을 위한 방사선 필름의 영상 지수의 분석 (Analysis of Image Factors of X-ray Films: Study for the Intelligent Replenishment System of Automatic Film Processor)

  • We analyzed image factors to determine the characteristic factors that need for intelligent replenishment system of the auto film processor. We processed the serial 300 sheets of radiographic films of chest phantom without replenishment of developing and fixation replenisher. We took the digital data by using film digitizer which scaned the films and automatically summed up the pixel values of the films. We analyzed characteristic curves, average gradients and relative speeds of individual film using densitometer and step densitometry. We also evaluated the pH of developer, fixer, and washer fluid with digital pH meter. Fixer residual rate and washing effect were measured by densitometer using the reagent methods. There was no significant reduction of the digital density numbers of the serial films without replenishment of developer and fixer. The average gradients were gradually decreased by 0.02 and relative speeds were also gradually decreased by 6.96% relative to initial standard step-densitometric measurement. The pHs of developer and fixer were reflected the inactivation of each fluid. The fixer residual rates and washing effects after processing each 25 sheets of films were in the normal range. We suggest that the digital data are not reliable due to limitation of the hardware and software of the film digitizer. We conclude that average gradient and relative speed which mean the film's contrast and sensitivity respectively are reliable factors for determining the need for the replenishment of the auto film processor. We need more study of simpler equations and programming for more intelligent replenishment system of the auto film processor.

폐결핵에 있어서 기관지폐포세척액 결핵균검사 및 PCR의 진단적 가치 (The Diagnostic Value of Bronchoalveolar lavage fluid microscopic study and PCR in Pulmonary tuberculosis)

  • 연구배경: 폐결핵은 고전적인 방법인 객담 도말 검사 및 배양검사로 전단할 수 있지만 객담 도말 민감도가 낮고 배양검사는 시일이 오래걸리는 단점이 있고 효과적인 객담배출이 되지 않는 환자에서는 검사가 불가능하기도 하다. 또한 최근에 개발된 PCR법은 신속하기는 하지만 위양성과 위음성이 문제가 되고 있다. 그래서 기관지내시경을 통한 기관지폐포세척액을 이용하여 세균학적 검사 및 PCR을 시행하여 폐결핵진단에 있어서 민감도와 예민도를 높이고 보다 신속한 진단을 할 수 있는지 연구하였다. 방법: 폐결핵 67예와 폐결핵이외의 폐질환을 가진 43예를 대상으로 객담 도말검사 및 배양검사를 시행하고, 기관지내시경을 시행하여 흉부 X-선상 병변이 보이는 엽상기관지에 기관지내시경을 위치하고 생리식염수 5cc로 3-5회에 걸쳐서 세척을 시행하여 세척액을 채취한 후 세균학적검사 및 PCR을 시행하였다. 결과: 1) 폐결핵환자의 기관지폐포세척액 결핵균 도말 검사 및 배양검사의 민감도는 각각 47.8% 및 80.6%로서, 객담 도말검사 및 배양검사의 민감도인 32.8% 및 57.4%보다 유의하게 높았다. 2) 폐결핵환자의 기관지폐포세척액 PCR의 민감도는 80.6%로서 배양검사의 민감도와 같았고, PCR의 배양검사에 대한 양성예측율은 81.5%였다. 3) 객담 도말검사 및 배양검사상 음성을 보인 폐결핵환자에서 시행한 기관지내시경을 통한 기관지폐포세척액 도말검사의 민감도는 23.1%였으며, 배양검사의 민감도는 100%였고, PCR의 민감도는 88.5%였으며, PCR의 배양검사에 대한 양성예측율 82.4%였다. 4) 기관지폐포세척액 PCR의 특이도는 77.0%였다. 5) 폐결핵의 최초 진단 후 치료시작까지의 기간은 객담 도말검사로 최초 진단이 된 경우가 평균 5일로 가장 빨랐고, 기관지폐포세척액 도말검사가 평균 9일, 기관지폐포세척액 PCR이 평균 26일이었으며, 객담배양검사는 평균 32일이었고, 기관지 폐포세척액 배양검사는 평균 56일로 가장 길었다. 결론: 폐결핵환자에서 기관지내시경을 통한 기관지폐포세척액의 결핵균 도말검사 및 배양검사는 객담을 이용한 검사보다 민감도가 높다. 그러므로, 객담을 배출하지 못하는 환자나 객담의 세균학적 검사가 음성인 환자에서 기관지내시경이 고려되어야 하며, 기관지폐포세척액의 PCR법까지 병행할 때에는 보다 신속하고 높은 진단율을 나타내므로, 조기에 적절한 항결핵제 투여에 도움이 될 것으로 기대된다.

  • 저자들은 4세에 신증후군으로 진단받고 스테로이드를 투여하며 재발을 수 회 반복하던 환자가 갑작스런 복통과 구토를 주소로 입원하여 시행한 검사 상 급성 무결석 담낭염으로 진단받고 금식, 비 위관 배액, 충분한 수액 공급, 그리고 항생제치료로 완치된 경우를 경험하였기에 이를 보고하는 바이다.

  • 가역성 대뇌 혈관 수축 증후군(reversible cerebral vasoconstriction syndrome)은 벼락두통과 함께 3개월 안에 소실되는 뇌혈관의 다발성 협착을 특징으로 하는 임상적 영상의학적 증후군이다. 해당 환자 중 일부에서 혈관 벽 조영증강이 보고되고 있으나 그 병태생리학적 의미와 진단적 가치는 불분명하다. 이에 혈관 벽 조영증강을 동반한 가역성 대뇌 혈관 수축 증후군의 증례를 보고하고 병태생리학적 의미와 진단적 가치를 고찰하고자 한다.

Fluid Accumulation in Canine Tympanic Bulla: Radiography, CT and MRI Examinations

  • Fluid accumulation within the tympanic bulla is an important diagnostic indicator of canine otitis media although its identification can be a challenge using currently available imaging techniques. The purpose of this study was to compare radiography, computed tomography (CT) and magnetic resonance imaging (MRI) in the identification of fluid accumulation within canine tympanic bulla. Unilateral tympanic bulla in 10 beagles were experimentally filled with blood or saline. Quantitative analysis of CT images were obtained by using Hounsfield unit (HU). MR signal intensity was obtained by using region of interesting (ROI) and compared with those of gray matter. On the CT image, the presence of blood or saline produced a fluid opacity occupying the tympanic bulla. On the MR image, the appearance of blood in the tympanic bulla was isointense in T1-weighted images and hyperintense in T2-weighted images. However, the appearance of saline in the tympanic bulla was hypointense in T1-weighted images and hyperintense in T2-weighted images. This study suggest that CT and MR imaging are useful methods for detection and differentiation of fluid in canine tympanic bulla.

  • Numerical solutions for spin-up problem of a thermally stratified fluid in a cylinder with an insulating sidewall and time-dependent rotation rate are presented. Detailed results are given for aspect ratio of O(1), fixed Ekman number $10-^{4}$, Rossby number 0.05 and Prandtl number O(1). Angular velocity of a cylinder wall changes with following formula, $\Omega_f=\Omega_i+\Delta\Omega[1-\exp(-t/t_c)]$. Here, this $t_c$, value, which is very significant in present study, represents that how fast/slow the angular velocity of the cylinder wall reaches final angular velocity. The normalized azimuthal velocity and meridional flow plots for several tc value which cover ranges of the stratification parameter S(1 ~ 10) are presented. The role of viscous-diffusion and Coriolis term in present study is examined by diagnostic analysis of the azimuthal velocity equation.

개에 발생한 간내 담관상피암의 진단예 (Diagnosis of Cholangiocellular Carcinoma in a Dog)

  • A 5-years-old female pointer dog was submittedto the hospital with the disease history of gradual distension of abdomen and emaciation for two months. An applesized hard mass was palpated on the right costal arch on physical examination. On blood chemical analysis, the marked elevation of SALT and decrease in serum albumin were detected. On paracentesis, a large volume of blood tinged fluid was detected. The peritoneal fluid contained tumor cells and the numerous blood cells. At autopsy the tumor mass located in the left medial lobe of liver was whitish and firm, and was consisted of many cysts. Histologically the tumor was identified as typical cholangiocellular carcinoma. Metastatic lesions of the tumor were detected in the lung, stomach, spleen, diaphragm, and lymph glands. This observation indicates that the cytological examination of the abdominal fluid have a diagnostic significance in the clinical examination of patients with abdominal tumors.

Barotrauma-Induced Pneumocephalus Experienced by a High Risk Patient after Commercial Air Travel

  • A 49-year-old female with a history of several neurosurgical and otolaryngologic procedures for occipital meningioma and cerebrospinal fluid leaks was diagnosed with pneumocephalus after a one hour flight on a domestic jet airliner. Despite multiple operations, the air appeared to enter the cranium through a weak portion of the skull base due to the low atmospheric pressure in the cabin. The intracranial air was absorbed with conservative management. The patient was recommended not to fly before a definite diagnostic work up and a sealing procedure for the cerebrospinal fluid leak site had been performed. Recent advances in aviation technology have enabled many people to travel by air, including individuals with medical conditions. Low cabin pressure is not dangerous to healthy individuals; however, practicing consultant neurosurgeons should understand the cabin environment and prepare high risk patients for safe air travel.

  • In order to obtain monoclonal antibody from ascites fluid at sufficiently high purity using a single hydroxylapatite chromatography (HA) a further optimization on its operating variables was carried out. By adjusting the pH of the eluent, the sodium phosphate buffer, to 6.0 from 6.8 and adding CaCl$_2$to 1 mM at the column inlet, the elution molarities (M$_{elu}$) for the desired monoclonal antibody and contaminating proteins can be distinguished from each other with enough resolution. Previously these two groups of proteins co-eluted at the same time at pH 6.8 and without CaCl$_2$. This sin81e step hydroxylapatite chromatography yields the desired antibody pure enough for diagnostic use.
