뇌척수액의 세포병리 (Cytologic Findings of Cerebrospinal Fluid)

  • Cerebrospinal fluid (CSF) cytology is based on the cytopathologic findings of other body fluids. However, CSF's cytologic features are less familiar to physicians than are those of the other body fluid's cytology because of the small number of cases. The low overall diagnostic accuracy and the presence of false positivity still remain as problems. The incidence of lymphoreticular malignancies and metastatic carcinomas are rather higher than that of primary brain tumors. In this review, the characteristic cytologic findings of conventional CSF cytology are reviewed along with a brief note on the technical preparation and diagnostic pitfalls.

Computed tomographic features of gastric and esophageal content in dogs undergoing CT myelography and factors influencing the presence of esophageal fluid

  • Background: Gastroesophageal reflux (GER) has been reported to be a common finding in dogs under general anesthesia. Objectives: The aim of this retrospective study was to assess the esophageal and gastric contents in a population of dogs undergoing computed tomographic myelography (myeloCT) examination and to evaluate the factors influencing the presence of esophageal fluid (gastric content, duration of anesthesia, body position, and intrinsic factors). Methods: Esophageal and gastric contents of 83 non-brachycephalic dogs were retrospectively assessed based on plain and myelo-CT scans. Age, weight, breed, sex, and the time between the 2 computed tomography [CT] scans were included. Results: Esophageal fluid was present in 19% (16/83) of the animals, and 14% (12/83) and 46% (37/83), respectively, had fluid or food material in their stomachs. The frequency of observing esophageal fluid on myelo-CT scans was significantly increased compared with plain CT scans (p = 0.006). The presence of gastric fluid was significantly associated with an increased frequency of observing esophageal fluid compared to other gastric contents (p = 0.049; odds ratio, 3.1). The presence of esophageal fluid was not correlated with alimentary gastric contents (p = 0.17). Increased body weight and duration of anesthesia were significantly associated with an increased frequency of observing esophageal fluid (p = 0.022, p = 0.021). Conclusions: Unlike alimentary gastric contents, fluid gastric contents were correlated with the presence of esophageal fluid upon myelo-CT. The observation of fluid in the esophagus may be consistent with GER. This study provides data additional to pH monitoring studies of GER and may support previous studies recommending shorter pre-anesthetic fasting periods in dogs.

환자안전을 고려한 횡와위 유지시간에 따른 영상변화에 관한 연구 (흉부 측와위 촬영 시) (A Study on Change Image According to Recumbent Position Holding Time for Patient Safety (In Chest Lateral Decubitus Examination))

  • Chest lateral decubitus is a chest examination to determine the persence of pleural fluid in thorax. In this study, we prepare recumbent holding position time standard of chest lateral decubitus. The records of 15 patients with chest lateral decubitus between May and Jun. Recumbent holding time is 30, 60, 90, 120, 180, 210, 240 seconds. The result is fluid level change between 0.88mm to 9.63. Fluid heigh change between 9.9 percent to 42.5 percent. We can confirm fluid level change with chest decubitus image. The proper time for fluid level change is 180 seconds.

  • Although exfoliative colonic cytology for the diagnosis of colorectal cancer has been largely abandoned due to the widespread use of colonoscopy, some authors still insiston the usefulness of colon lavage fluid. We tried evaluating the diagnostic feasibility of colon lavage fluid cytology using an orally administered balanced electrolyte solution. We collected colon lavage fluids in 106 patients prior to colonoscopy and reviewed the slides. Cytologic examination revealed neoplastic cells in 7 of 16(44%) cases of endoscopically proven adenocarcinoma patients. Therefore, we thin cytologic study of colon lavage fluid may be considered as one of the noninvasive diagnostic tools in colorectal cancer.

체강액의 세포학 (The Cytopathology of Body Cavity Fluid)

  • Cytologic examination of the body cavity fluid is very important because the specimens represent a significant percentage of nongynecologic samples and this cytologic examination may be the first, best or only chance for making the diagnosis of an underlying malignancy. The purposes of body cavity fluid examination are to correctly identify cancer cells and if possible, to identify the tumor types and primary sites when presented with unknown primary tumor sites. The most important basic differential diagnosis is that of benign and reactive disease vs malignant disease. Reactive mesothelial cells are a consistent population in body cavity fluid, and these are the most versatile cells in the body. Due to the specific environment of the body cavity, the exfoliated reactive mesothelial cells may show significant morphologic overlap with the morphology of cancer cells. With a focus on the differential points between reactive mesothelial cells and metastatic adenocarcinoma cells, the practical diagnostic approaches, the diagnostic clues and the pitfalls to achieve a correct diagnosis are presented in this review.

간수흉 1예 (A Case of Hepatic Hydrothorax)

  • Hepatic hydrothorax is defined as the presence, in a cirrhotic patient, of a large pleural effusion in the absence of primary pulmonary or cardiac disease. Pleural effusions occur in 5% to 10% of patients with cirrhosis of the liver. The effusions are usually right-sided, but may be bilateral or left-sided. The precise mechanism of fluid accumulation is not clear. We reported a case of right hepatic hydrothorax occuring in the liver cirrhosis with ascites. Diagnosis was confirmed by the intraperitoneal and intrapleural injection of radioisotope $^{99m}Tc-tin$ colloid that demonstrated the one-way transdiaphragmatic flow of fluid from the peritoneal to pleural cavities.

  • Air-operated valve is one of principal valves that are using to control fluid flow in nuclear power plants. A period diagnosis lot safety of power plants is necessary. But there are many difficulties such as economic loss caused by income of high cost devices and a matter hard to deal with users. In this study, we developed the diagnostic system that users of power plants are easy to handle. The diagnostic system is composed of database module, diagnosis test module and evaluation module.

유압실린더 힘 제어계의 인-프로세스 서보밸브 마모진단에 관한 연구 (In-Process Diagnosis of Servovalve wear in Hydraulic Force Control Systems)

  • An in-process method of diagnosing the spool wear of hydraulic servovalves was explored. The diagnostic method discussed in this paper is for force-control hydraulic servo systems. The key principle used is that pressure sensitivity of a servovalve drops as the valve spool wears out so that it is possible to determine the spool condition by monitoring pressure sensitivity. A diagnostic algorithm was developed and evaluated through numerical simulation and experiments. Two major steps of diagnosis are the evaluation of null bias of the servovalve and the approximation of pressure sensitivity, both of which could be successfully done during normal operation of a servo system. The difference between a new servovalve and a worn valve could be clearly detected in-process, and the diagnostic test was found to be repeatable.

Validation of Synovial Fluid Clinical Samples for Molecular Detection of Pathogens Causing Prosthetic Joint Infection Using GAPDH Housekeeping Gene as Internal Control

  • Identification of the pathogens causing infection is important in terms of patient's health management and infection control. Synovial fluids could be used as clinical samples to detect causative pathogens of prosthetic joint infections (PJIs) using molecular diagnostic assays, therefore, normalization and validation of clinical samples are necessary. Microbial culture is considered the gold standard for all infections, including PJIs. Recently, molecular diagnostic methods have been developed to overcome the limitation of microbial culture. Therefore, guideline for validating clinical samples to provide reliable results of molecular diagnostic assays for infectious diseases is required in clinical field. The present study aimed to develop an accurate validating method of synovial fluid clinical samples using GAPDH gene as an internal control to perform the quantitative PCR TaqMan probe assay to detect pathogens causing PJIs.
