Individual Variations in the Code of the International Classification of Disease for Similar Outpatient Conditions among General Practitioners (동일 질환에 대한 상병분류기호의 의료기관별 변이에 관한 연구)

  • The code of the International Classification of Disease(ICD) is seriously questioned on its effectiveness in identifing an independent disease entity from similar conditions at general practitioner's offices. This study has attempted to show individual coding variations in ICD for similar ambulatory care conditions. It has been assumed that a following outpatient visit is regarded as the sane kind of visit owing to the same disease if a visit to the different source of care would be mad within an interval of less than two days. The 'D' health insurance association was selected for this analysis. The 'D' association had 153,298 members and made claims of 642,605 outpatient care in 1990. Out of the total outpatient claims, 8.6%(55,102 claims) were counted as the same disease which could meet the above assumption. Percent of conditions classified as the 10 leading causes of frequent visits which were matched accurately to the subsequent ICD diagnostic code found to be 15.8% on the average. The URI was noted for the highest concurrence rate of 20.4%. This proportion was even decreased to 11.6% on the case of chronic disease. Despite the fact that the assumption underlying the definition of the above same disease is rather rough and inappropriate, this study reveals that the code of ICD currently in use has weaknesses in seperating a certain independent disease from similar conditions at the outpatient setting. Thus, efforts need to be elaborated to meet the need of a new system of classification for conditions and diseases encountering at ambulatory care.

  • Data is explosively growing, but many companies are still using data analysis only for descriptive analysis or diagnostic analysis, and not appropriately for predictive analysis or enterprise technology strategy analysis. In this study, we analyze the structured & unstructured patent data such as IPC code, inventor, filing date and so on by using big data analysis techniques such as network analysis and TF-IDF. Through this analysis, we propose analysis process to understand the core technology and technology distribution of competitors and prove it through data analysis.

  • Electronic control units (ECUs) are currently popular, and have evolved further towards the high-end application of autonomous vehicles in the automotive industry. Such digital technologies have also become widespread, in agriculture and construction equipment. Likewise, transmission control of high-speed tracked vehicles is based on the transmission control unit (TCU), performing complex gear change control functions, and diagnostic algorithms (a TCU's self-diagnostic and reporting capability of malfunction data through CAN communication). Since all functions of TCU are implemented by embedded-software, it is hardly possible to analyze specifications by reverse engineering. In this paper a real-time transmission simulator adaptable to TCU is presented, for analysis of diagnosis algorithm and standards. Signal simulation circuits are deliberately designed considering electrical characteristics of TCU inputs and various analysis tools, such as analog input auto scan function, and global output enable switch, are implemented in software. Test results from hardware-in-the-loop simulator verify tolerance time for each error, as well as cause of fault, error reset conditions.

The Performance Test of Anti-scattering X-ray Grid with Inclined Shielding Material by MCNP Code Simulation

  • Background: The scattered photons cause reduction of the contrast of radiographic image and it results in the degradation of the quality of the image. In order to acquire better quality image, an anti-scattering x-ray gird should be equipped in radiography system. Materials and Methods: The X-ray anti-scattering grid of the inclined type based on the hybrid concept for that of parallel and focused type was tested by MCNP code. The MCNPX 2.7.0 was used for the simulation based test. The geometry for the test was based on the IEC 60627 which was an international standard for diagnostic X-ray imaging equipment-Characteristics of general purpose and mammographic anti-scatter grids. Results and Discussion: The performance of grids with four inclined shielding material types was compared with that of the parallel type. The grid with completely tapered type the best performance where there were little performance difference according to the degree of inclination. Conclusion: It was shown that the grid of inclined type had better performance than that of parallel one.

  • PACS has been run at the Kyung Hee University Medical Center since 2001, and the installation and operation of PACS have contributed to automation and quantification of center's medical environment. In order to classify tile annual number of trouble cases processed by PACS, the authorshave made a classification code system which enabled detailed statistical processes for each section. Such process method has not only shown the management efficiency to trouble management of PACS, but also raised the interests in frequently occurring troubles, and enabled the prediction of troubles that may occur hereafter. Predictable troubles lead to preventive check, and this has direct effects on medical treatment and the hospital administration. The authors intend to arouse the necessity of preventive check of PACS by analyzing trouble management processes for the last 1 year.

  • Recently operation and maintenance cost of gas turbine engines has been issued as a major parameter in terms of designing and manufacturing. Accordingly, the conception that the maintenance and repair of an engine has to be conducted in assembled condition has been spreaded out. However, it is possible only if the prediction of the engine performance is clearly identified. In this study, therefore, a diagnostic code of the engine performance has been developed by using GPA(Gas Path Analysis) and Fuzzy Logic which can analyze the engine performance and estimate the health parameters. The prediction of the quantitative performance deterioration of the established model of the turbo-shaft engine for SUAV has been achieved in a satisfied level compared to that obtained by GSP code.

  • Purpose: This study was aimed at evaluating the effects of Music Therapy for the autistic children. Method: The subjects of this study consisted of 3 autistic children who were trained in an Attachment Promotion Therapy Program for 6 months. The Children were all males and 4years 9months, 3years 1 month, and 3years 8month each, and diagnosed with Autism by Child and Adolescent Psychiatrists. Data was collected by using video-taping methods(ADOS, Fagot's Interactive Behavior Code), an interview and observational methods(SMS). Music Therapy intervention was done once a week for 6 months. Data was gathered by quantitative and qualitative analysis. Result: This study showed that the Autism Diagnostic Observation Scale and Social Maturity Quotient(SQ) much improved after the Music Therapy After the Music Therapy, the Interactive Behavior Code : Gaze, Gesture for the communication, Verbal language, Laughing/Smile, and Normal play behaviors increased more than before the intervention. As the results show, the child's behavior became more positively responsive, playful, and attentive to others. Conclusion: This study suggests that Music Therapy might be an effective intervention for autistic disorder children in order to decrease autistic symptoms and increase joint attention behavior.

  • The representative beat with high SNR could be obtained by the signal averaging, correct and fast detection of significant points and waveform boundary could be obtained by adoption of search interval. All experimental results of waveform boundary were compared with CSE database which had the 5 referees results and 11 ECG measurement programs. All results were within tolerance made by referees, especially the end point of T wave were more close to the referee's results than other 11 measurement programs. The diagnosis parameters that might be used in the Minnsota code criteria were extracted from the representative beat. The diagnostic classification were fulfilled using Minnsota code criteria. Through the comparison on the diagnosis results from designed automated ECG analyzer(YECGA) and the results ECG analyzer manufactured by Fukuda denshi(FCG-2201) in Japan, reliance of the performance on designed system(YECGA) could be validated.

  • The authors developed a comfortable program for routine work of surgical pathology. We used IBM PC(80386) and Foxbase plus program. The main function of this program was automatic coding and concurrent surgical report printing. During gross printing, previous biopsy number and its diagnosis were automatically searched and printed below gross description. The reported datas were stored during surgical report printing simultaneously, and thus the typist's workload became considerably reduced. Search for specific cases could be performed by patient's name, surgical number, hospital number, diagnostic code numbers(SNOMED code microglossary), and certain disease entities on very short duration.

Determination of dosimetric dependence for effective atomic number of LDR brachytherapy seed capsule by Monte Carlo simulation

  • Brachytherapy is a special case of radiotherapy. It should be arranged according to some principles in medical radiation applications and radiation physics. The primary principle is to use as low as reasonably achievable dose in all ionizing radiation applications for diagnostic and therapeutic treatments. Dosimetric distributions are dependent on radioactive source properties and radiation-matter interactions in an absorber medium such as phantom or tissue. In this consideration, the geometrical structure and material of the seed capsule, which surrounds a radioactive material, are directly responsible for isodose profiles and dosimetric functions. In this study, the radiometric properties of capsule material were investigated on dose distribution in a water phantom by changing its nuclear properties using the EGSnrc Monte Carlo (MC) simulation code. Effective atomic numbers of hypothetic mixtures were calculated by using different elements with several fractions for capsule material. Model 6711 brachytherapy seed was modeled by EGSnrc/Dosrcnrc Code and dosimetric functions were calculated. As a result, dosimetric parameters of hypothetic sources have been acquired in large-scale atomic number. Dosimetric deviations between the data of hypothetic seeds and the original one were analyzed. Unit dose (Gy/Particle) distributions belonging to different types of material in seed capsule have remarkably differed from the original capsule's data. Capsule type is major variable to manage the expected dose profile and isodose distribution around a seed. This study shows us systematically varied scale of material type (cross section or effective atomic number dependent) offers selective material usage in production of seed capsules for the expected isodose profile of a specific source.
