Models and Modeling Behavior: A Look at the Critical Thinking Skills of Biology Majors

  • This paper describes the types of models that biology majors use and how they go about making their models in learning key concepts in biology such as the cell membrane, cytoskeleton and cell structure. Initially, a total of 44 biology students from all year levels enrolled in the second semester of calendar year 2008-2009 were asked to make their respective models of the cell membrane, cytoskeleton and cell structure. They were also asked to answer an open-ended questionnaire. Of the 44, only 20 (five from each year level) were randomly selected for a one-on-one interview. Results showed that the student-generated models from all year levels were mostly analogies, some textbook definitions and occasional drawings. In making their model, students first read the text; second, outline similarities in structure and function or both; and third, make the model. Data suggest that models are good diagnostic tools for identifying critical thinking skills of students. In this case, students mostly demonstrate the ability to recognize similarities in structure and function between the concept and their model. Some senior students demonstrated integration and reflective thinking in making their models. Thus, more opportunities for student-generated models must be available if students were to develop integration and reflective thinking in their models.

Voltammetric Determination of Droxidopa in the Presence of Tryptophan Using a Nanostructured Base Electrochemical Sensor

  • A novel carbon paste electrode modified with $Cu-TiO_2$ nanocomposite, 2-(ferrocenylethynyl)fluoren-9-one (2FF) and ionic liquid (IL) (2FF/$Cu-TiO_2$/IL/CPE) was fabricated and employed to study the electrocatalytic oxidation of droxidopa, using cyclic voltammetry (CV), chronoamperometry (CHA) and differential pulse voltammetry (DPV) as diagnostic techniques. It has been found that the oxidation of droxidopa at the surface of modified electrode occurs at a potential of about 295 mV less positive than that of an unmodified CPE. DPV exhibits a linear dynamic range from $5.0{\times}10^{-8}$ to $4.0{\times}10^{-4}M$ and a detection limit of 30.0 nM for droxidopa. Finally this modified electrode was used for simultaneous determination of droxidopa and tryptophan. Also the 2FF/$Cu-TiO_2$/IL/CPE shows excellent ability to determination of droxidopa and tryptophan in real samples.

류마티스 관절염과 운동 (Rheumatoid Arthritis and Exercise)

  • In an effort to obtain preventive, diagnostic and therpeutic medical and exercise training information of rheumatoid arthritis as well as to provide pertinent data to be used in development of exercise program for the management of rheumatoid arthritis, this study was conducted by using literature review. Because RA is a disease that involves both joints and muscles, its activity in the different functional classes influences the patient's ability to exercise. A patient in Functional Class 1 may perform any type of exercise because the disease involvement has not yet reached major importance. Exceptions may include hard physical exercise, running, and individual racket sports. In almost all cases, bicycle exercise is possible. Patients in Functional Class 2 and a few in Class 3 can perform most types of exercise (especially cycling, walking, heated pool exercise and even jogging) in low activity phases of RA. Although a few patients in Functional Class 3 can walk, jog, and perform similar types of exercise, most persons in this class can swim or exercise on a bicycle if the type of exercise, its intensity, and its range of motion are modified according to the patient's anatomic and pathologic conditions. Most patients in Functional Class 4 are not able to carry out complicated movements. We conclude that physical training for persons with RA has physical, psychological, and social consequences that are clearly beneficial. We recommend training be one part of the many involved in the complicated treatment of RA.

악성 림프종에서 FDG PET의 역할 (The Role of FDG PET in Malignant Lymphoma)

  • FDG PET is a functional imaging modality whose ability to detect lesions is directly based on a change of the glycolytic metabolism of targeted tissues, may be advantageous over other techniques. Combined with excellent image qualify, high spatial resolution, and whole body imaging capability, it has become popular as a new approach in the evaluation of patients with various malignancies. Initial staging of nodal and extranodal lymphoma using FDG PET has been proven to be at least equal or superior to conventional imaging modalities. For the assessment of treatment responsiveness, FDG PET has a major impact on the management of patients in differentiating residual lymphoma from treatment related benign changes. Residual FDG uptake after the completion of chemotherapy is a good predictor of early relapse. However, it seems that the absence of FDG uptake in tumor mass may not exclude minimal residual disease causing later relapse. In the early evaluation of treatment response only after a few cycles of chemotherapy, FDG PET may have a promising role in identifying non-responders who could benefit from a different treatment strategy. At present, FDG PET appears to be the cost-effective, diagnostic modality of choice in the management of lymphoma patients. The role of FDG PET based-systems in terms of affecting long-term prognosis and survival benefit should be further elucidated in future prospective studios.

  • Hepatocellular carcinoma is the most common primary tumor in the liver. FDG PET has been applied for staging and treatment planning of hepatocellular carcinoma. It could reflect tumor prognosis because glucose metabolism assessed by FDG PET is known to have correlations with the differentiation and aggressiveness of the tumor. Although the ability of FDG PET to detect well-differentiated or low grade tumors and intra-hepatic lesions is not good, it is expected to playa major role in pre-surgical assessments for liver transplantation because it is useful in detecting extra-hepatic lesions and unexpected distant metastases with a better diagnostic performance than other conventional imaging modalities. Additionally, FDG PET has an advantage to screen other cancers through whole body scanning. As a new tracer for PET, Acetate demonstrates higher sensitivity and specificity to FDG in evaluating hepatocellular carcinoma. It thus seems that simultaneous use of Acetate PET with FDG PET could be helpful in diagnosis, especially detecting extra-hepatic metastases.

Non-classical role of Galectin-3 in cancer progression: translocation to nucleus by carbohydrate-recognition independent manner

  • Galectin-3 is a carbohydrate-binding protein and regulates diverse functions, including cell proliferation and differentiation, mRNA splicing, apoptosis induction, immune surveillance and inflammation, cell adhesion, angiogenesis, and cancer-cell metastasis. Galectin-3 is also recommended as a diagnostic or prognostic biomarker of various diseases, including heart disease, kidney disease, and cancer. Galectin-3 exists as a cytosol, is secreted in extracellular spaces on cells, and is also detected in nuclei. It has been found that galectin-3 has different functions in cellular localization: (i) Extracellular galectin-3 mediates cell attachment and detachment. (ii) cytosolic galectin-3 regulates cell survival by blocking the intrinsic apoptotic pathway, and (iii) nuclear galectin-3 supports the ability of the transcriptional factor for target gene expression. In this review, we focused on the role of galectin-3 on translocation from cytosol to nucleus, because it happens in a way independent of carbohydrate recognition and accelerates cancer progression. We also suggested here that intracellular galecin-3 could be a potent therapeutic target in cancer therapy.

요통과 천장관절기능이상에 대한 고찰 (A Study on Low Back Pain with Sacroiliac Joint Dysfunction)

  • Motion at the sacroiliac joints(SIJ) has been reported in the medical literature since the mid-19th century. However, for many years authorities vie~d that no movement occurred at the sacroiliac joints and therefore were not clinically significant. Recent contributions from both the basic and clinical sciences caused a change in perception of the role of the sacroiloiac joint in low back pain(LBP). Movement within the sacroiliac joint is now generally recognized, although it is only a small amount. Controversy continues as to the type of motion available and the axes of motion, and continues as to the ability of a clinician to identify a significant sacroiliac dysfunction. Dysfunction of the pelvic girdle is complex and not easily understood. It is common to find serveral dysfunctions within the same pelvic girdle. Each needs to be individually diagnosed and appropriately treated. The diagnostic and therapeutic system designed by Philip E. Greenmam, D. O. allows the operator to deal with any combination of physical findings that are found within the pelvic girdle Restoration of pelvic girdle function within the walking cycle is a major therapeutic goal, particularly from the biomechanical postural-structural model.

소아 부비동염 진단 (The diagnosis of pediatric sinusitis)

  • Although most clinicians now agree that sinusitis can afflict children of all ages, appropriate diagnosis remain controversial. Sinusitis is one of the most challenging diagnoses for a clinician, because there is a lack of validated diagnostic criteria for acute rhinosinusitis. Symptoms generally include nasal congestion, purulent nasal discharge, and cough .The physical examination is often unsuccessful in confirming the diagnosis. If purulent discharge is seen oozing from the middle meatus, the diagnosis of acute rhinosinusitis is almost certain. Purulent drainage may also be seen in the posterior pharynx and accompanied by halitosis. Absence of light by transillumination may indicate acute rhinosinusitis. Imaging studies are not necessary to confirm the diagnosis of sinusitis in children younger than 6 years and should be used sparingly in children older than 6 years. Sinus radiographs still have several advantages over CT scanning including the relatively low cost and the ability to obtain films without the use of sedation in younger children. Positive findings of sinusitis on plain film include air-fluid levels, complete opacification, or mucosal thickening greater then 4 mm. The CT scan however, is significantly more sensitive then plain films in detecting these abnormalities. Although recovery of bacteria from a sinus aspiration is considered reference standard for diagnosis in pediatric rhinosinusitis, its routine used by pediatricians is not practical and therefore is not recommended.

Optical Biopsy of Peripheral Nerve Using Confocal Laser Endomicroscopy: A New Tool for Nerve Surgeons?

  • Peripheral nerve injuries remain a challenge for reconstructive surgeons with many patients obtaining suboptimal results. Understanding the level of injury is imperative for successful repair. Current methods for distinguishing healthy from damaged nerve are time consuming and possess limited efficacy. Confocal laser endomicroscopy (CLE) is an emerging optical biopsy technology that enables dynamic, high resolution, sub-surface imaging of live tissue. Porcine sciatic nerve was either left undamaged or briefly clamped to simulate injury. Diluted fluorescein was applied topically to the nerve. CLE imaging was performed by direct contact of the probe with nerve tissue. Images representative of both damaged and undamaged nerve fibers were collected and compared to routine H&E histology. Optical biopsy of undamaged nerve revealed bands of longitudinal nerve fibers, distinct from surrounding adipose and connective tissue. When damaged, these bands appear truncated and terminate in blebs of opacity. H&E staining revealed similar features in damaged nerve fibers. These results prompt development of a protocol for imaging peripheral nerves intraoperatively. To this end, improving surgeons' ability to understand the level of injury through real-time imaging will allow for faster and more informed operative decisions than the current standard permits.

간세포암의 $^{99m}Tc-DISIDA$ 신티그램 소견 ([ $^{99m}Tc-DISIDA$ ] Scintigraphic Findings of Hepatocellular Carcinoma)

  • It is well known that $^{99m}Tc-sulfur$ colloid or phytate hepatic scintigraphy is highly sensitive but not specific. Both $^{99m}Tc-DISIDA$ and bilirubin have been shown to share the same anionic transport pathway in the liver. Hepatocellular carcinoma(HCC) retains the ability to produce bile but has marked limitation to excreting it resulting in accumulation of bile within the tumor cells. Based upon such a fact, $^{99m}Tc-DISIDA$ hepatobiliary scintigraphy is used for the diagnosis of HCC. The present communication deals with our experience of DISIDA scintigraphic exploration of 9 cases of HCC in a retrospective way. We have made an observation on intensity of positive radio nuclide accumulation in the cold area of HCC as it is demonstrated by phytate scintigraphy. In addition we have semi quantitatively analyzed time-activity pattern and the following results were obtained. (1) All of 9 cases showed an increased uptake of $^{99m}Tc-DISIDA$ in delayed scintigrams. Of these 5 cases showed accumulation less than, 3 equal to, 1 more than the surrounding liver tissue. (2) The mean of the first appearing time of $^{99m}Tc-DISIDA$ activity in tumoral region was 2 hours and 20 minutes. (3) DISIDA scintigraphy provides us with positive informations of diagnostic value.
