A Study on the Facility and Equipment of Laboratory Medicine in General Hospital - Focused on more than 550 bed sized hospitals (종합병원 진단검사의학과 검사실의 시설 설비 현황 조사 - 550 병상 이상 종합병원을 중심으로)

  • Purpose: Though Korean healthcare services have been upgraded, infection and fire had been broken out in general hospitals. And higher concerns about quality assessment made it to clinical laboratory design guideline studies. So, this study investigates the facilities, equipment and personnel of laboratory medicine focusing on more than five hundred fifty bed hospital, and contributes to make guidelines for safety and efficiency in lab. Methods: Questionnaires to supervisor technologist and field surveys to medical laboratories in korean hospitals have been conducted for the data collection. 16 answers have been analysed statistically by MS Excel program. Results: Most of the sample tests such as hematology, clinical chemistry, immunology, transfusion, urinalysis, microbiology and molecular diagnosis are performed by more than 80% in large sized general hospital laboratory. In the test methods, automatic analyzers are used up to 80%, total laboratory automation up to 43% in clinical chemistry and immunology, and manual tests in all sorts of the test. There are placed in single lab or two and three labs above the ground, which are all in semi-open lab. There is some correlation with the number of specimens and the number of lab people depending on the number of hospital beds. Laboratory environment shows that work distance is good, but evacuation path width, visibility, separation of staff area from automatic analyzer, and equipment installations are needed to have more spaces and gears. Most of the infection controls are equipped with mechanical ventilation, air-conditioning, washbasin and wastewater separation, BSC installation and negative pressure lab room. Implications: Although the laboratory space area is calculated considering the number of hospital beds, type of tests and number of staff, hospital's expertise and the samples numbers per year should be taken into account in the planning of the hospital.

A Study on Multi Fault Detection for Turbo Shaft Engine Components of UAV Using Neural Network Algorithms

  • Because the types and severities of most engine faults are various and complex, it is not easy that the conventional model based fault detection approach like the GPA(Gas Path Analysis) method can monitor all engine fault conditions. Therefore this study proposed newly a diagnostic algorithm for isolating and diagnosing effectively the faulted components of the smart UAV propulsion system, which has been developed by KARI(Korea Aerospace Research Institute), using the fuzzy logic and the neural network algorithms. A precise performance model should be needed to perform the model-based diagnostics. The based engine performance model was developed using SIMULINK. For the work and mass flow matching between components of the steady-state simulation, the state-flow library was applied. The proposed steady-state performance model can simulate off-design point performance at various flight conditions and part loads, and in order to evaluate the steady-state performance model their simulation results were compared with manufacturer's performance deck data. According to comparison results, it was confirm that the steady-state model well agreed with the deck data within 3% in all flight envelop. The diagnosis procedure of the proposed diagnostic system has the following steps. Firstly after obtaining database of fault patterns through performance simulation, then secondly the diagnostic system was trained by the FFBP networks. Thirdly after analyzing the trend of the measuring parameters due to fault patterns, then fourthly faulted components were isolated using the fuzzy logic. Finally magnitudes of the detected faults were obtained by the trained neural networks. Because the detected faults have almost same as degradation values of the implanted fault pattern, it was confirmed that the proposed diagnostic system can detect well the engine faults.

Implementation of Maritime Telemedicine System Using Android (안드로이드를 이용한 해양원격진료시스템 구현)

  • The purpose of this study is to design and implement a maritime telemedicine system to provide the best medical service for patients who rescue and happens in sailing ship. For this purpose, INMARSAT and MVAST, which are necessary communication channels for maritime telemedicine, were examined and diversity in maritime communication path was suggested. It was implemented to enable maritime telemedicine by linking smart devices and medical devices for physical examination. The marine telemedicine system adopts the HTTP communication protocol to configure the web server and process the client request. The patient's chart information was entered using an application for AndroidPad and the patient was diagnosed using Bluetooth electronic stethoscope for diagnosis. This system enables appropriate medical treatment to emergency patients at maritime, which ultimately can improve medical services. It is expected to be a more effective medical support system through linkage with various medical equipment.

Aspects of Medical Utilization by Factors for Referrals at Tertiary Hospital - Focused on S University Hospital - (상급종합병원 진료의뢰 요인별 의료이용 양상 - 일개 S대학 병원을 중심으로 -)

  • Purposes: The purpose of this study is to analyze the institutional and personal factors that affect the medical utilization of patients transferred to tertiary medical institutions. Methodology: We retrospectively analyzed the 2 weeks electronic medical records of 1,556 patients, who were referred to the tertiary hospital, from June 15 to 26, 2015. The patient's personal characteristics, referral hospital, referral path, medical experiences and expenses were analyzed for 6 months after the patient's first visit. Findings: The largest proportion (848; 54.5%) of referrals was referred from primary clinic but the referrals of the same tertiary hospital level were one in seven (228; 14.7%) of the patients. Most patients (1,401; 90%) were referred from the clinics and hospitals directly and only one in ten (155; 10%) of the patients utilized the medical referral center. Patients who had been referred from tertiary care institutions had significantly higher medical costs than those referred to primary care (7,560,000 vs 2,333,000 won). The institutional factors including the numbers of visits to outpatient clinic, previous history of hospitalization and operation, consultation to other medical departments and hospitalization fee significantly influenced on medical utility pattern. Personal factors including patient's medical diagnosis and department of disease have a highly correlation with patient's referrals. Practical implications: The medical utilization of medical expenses and experiences is influenced by institutional and individual factors, and it is important to establish a referral system considering the institutional factors of the type of referral hospital.

Management of Maxillary Impacted Canines (매복 상악 견치의 처치)

  • The canine tooth is important both functionally and aesthetically, being positioned between the anterior and posterior teeth. The upper canine has the longest eruption path, forming in the deepest part of the maxillary bone and often experiencing eruption disorders, leading to significant aesthetic and functional issues. Early detection and management of canine impaction are crucial in pediatric dentistry, which focuses on tooth growth and eruption. The prevalence of maxillary canine impaction ranges from 1.1% to 3.0%. In Western populations, palatal impaction is twice as common as labial impaction, while some Korean studies report more labial impaction. Maxillary canine impaction occurs more frequently in women and is associated with various factors such as structural obstacles, pathological conditions in surrounding tissues, developmental abnormalities, and genetic factors. Labial displaced canines are linked to narrow maxillary intercanine width, total dental crowding, and skeletal Class III malocclusion. Maxillary palatal displaced canine impaction is more prevalent in patients with familial agenesis of lateral incisors or conical supernumerary lateral incisors. Understanding these factors aids in early diagnosis and appropriate intervention for canine tooth impaction, ensuring optimal oral health and aesthetics.

Object Detection Based on Deep Learning Model for Two Stage Tracking with Pest Behavior Patterns in Soybean (Glycine max (L.) Merr.)

  • Soybean (Glycine max (L.) Merr.) is a representative food resource. To preserve the integrity of soybean, it is necessary to protect soybean yield and seed quality from threats of various pests and diseases. Riptortus pedestris is a well-known insect pest that causes the greatest loss of soybean yield in South Korea. This pest not only directly reduces yields but also causes disorders and diseases in plant growth. Unfortunately, no resistant soybean resources have been reported. Therefore, it is necessary to identify the distribution and movement of Riptortus pedestris at an early stage to reduce the damage caused by insect pests. Conventionally, the human eye has performed the diagnosis of agronomic traits related to pest outbreaks. However, due to human vision's subjectivity and impermanence, it is time-consuming, requires the assistance of specialists, and is labor-intensive. Therefore, the responses and behavior patterns of Riptortus pedestris to the scent of mixture R were visualized with a 3D model through the perspective of artificial intelligence. The movement patterns of Riptortus pedestris was analyzed by using time-series image data. In addition, classification was performed through visual analysis based on a deep learning model. In the object tracking, implemented using the YOLO series model, the path of the movement of pests shows a negative reaction to a mixture Rina video scene. As a result of 3D modeling using the x, y, and z-axis of the tracked objects, 80% of the subjects showed behavioral patterns consistent with the treatment of mixture R. In addition, these studies are being conducted in the soybean field and it will be possible to preserve the yield of soybeans through the application of a pest control platform to the early stage of soybeans.

  • Continued advances in the field of histo pathology (and cyto pathology) over the past two decades have resulted in dramatic changes in the manner in which these disciplines are now practiced. This is especially true in the setting of a large university hospital where the role of pathologists as clinicians (diagnosticians), undergraduate and postgraduate educators, and researchers has evolved considerably. The world around us has changed significantly during this period bringing about a considerable change in our lifestyles and the way we live. This is the world of the internet and the world-wide web, the world of Google and Wikipedia, of Youtube and Facebook where anyone can obtain any information one desires at the push of a button. The practice of histo (and cyto) pathology has also evolved in line with these changes. For those practicing this discipline in a poor, developing country these changes have been breathtaking. This is an attempt to document these changes as experienced by histo (and cyto) pathologists practicing in the biggest center for Histopathology in Pakistan, a developing country in South Asia with a large (180 million) and ever growing population. The Section of Histopathology, Department of Pathology and Microbiology at the Aga Khan University Hospital (AKUH) in Karachi, Pakistan's largest city has since its inception in the mid-1980s transformed the way histopathology is practiced in Pakistan by incorporating modern methods and rescuing histopathology in Pakistan from the primitive and outdated groove in which it was stuck for decades. It set histopathology in Pakistan firmly on the path of modernity and change which are essential for better patient management and care through accurate and complete diagnosis and more recently prognostic and predictive information as well.

Image Measurement on Influence from Application of Compression Band on Intravenous Urography for Urolithiasis Patient (요로결석 환자의 경정맥 요로조영 검사 시 압박 유무에 따른 영상평가)

  • Intravenous urography (IVU) for urolithiasis is a radiologic examination to diagnosis stone in the ureter path using iodine contrast media, which is radioopacity material. The method includes compression on the upper iliac crest. The compression band prevents outlet of the contrast media through the bladder and enables easier movement to upper urinary tract. This usage depends on the policy of a hospital. Therefore, this study aimed to review and compare the characteristic of progress of contrast media either in compression and non-compression. The retrospective image measurement on 60 cases of intravenous pyelography was conducted at a hospital with the identical type and amount of contrast media as well as criteria for testing. Image measurement was limited to 5 minutes clip, which is optimal for progress of contrast media depending on usage of the compression band. Also, anatomical regions were set as following: "RP" is from renal pyramid to renal pelvis, "PL" is from renal pelvis to lumbar three endplate, and "IU" and "IL" for upper and lower parts from both iliac crests. Analysis has been conducted through the statistical method based on Fisher's Exact Test to find if there are differences of distribution with the anatomical regions with compression or no compression. It has been confirmed that there is no statistical significant difference as the video measurement on 30 cases of compression and non-compression group respectively resulted in P value of 0.580 from left and 0.711 from right (both 0.960). Therefore, it has been concluded that application of a compression band on an intravenous pyelography for urolithiasis patient does not meaningfully affect the progress of contrast media.

Comparison of Hospital Standardized Mortality Ratio Using National Hospital Discharge Injury Data (퇴원손상심층조사 자료를 이용한 의료기관 중증도 보정 사망비 비교)

  • This study was to develop the assessment of medical service outcome using administration data through compared with hospital standardized mortality ratios(HSMR) in various hospitals. This study analyzed 63,664 cases of Hospital Discharge Injury Data of 2007 and 2008, provided by Korea Centers for Disease Control and Prevention. We used data mining technique and compared decision tree and logistic regression for developing risk-adjustment model of in-hospital mortality. Our Analysis shows that gender, length of stay, Elixhauser comorbidity index, hospitalization path, and primary diagnosis are main variables which influence mortality ratio. By comparing hospital standardized mortality ratios(HSMR) with standardized variables, we found concrete differences (55.6-201.6) of hospital standardized mortality ratios(HSMR) among hospitals. This proves that there are quality-gaps of medical service among hospitals. This study outcome should be utilized more to achieve the improvement of the quality of medical service.

THE FORCED ERUPTION OF IMPACTED MAXILLARY INCISOR: CASE REPORT (매복된 상악 중절치의 교정적 견인을 이용한 치험례)

  • A tooth impaction means a state that a tooth does not erupt out of oral mucosa or alveolar bone for many reasons. The reasons for an impaction of the Maxillary central incisor are an odontoma, supernumerary tooth, space loss, prolonged remaining or early loss of a preceding deciduous tooth, abnormalities of crown or root caused by trauma of a deciduous tooth and an ectopic position of a tooth germ. In the case of the impacted maxillary incisor, a rapid mesial movement of a lateral incisor leads a space loss and a midline deviation can be happened. Furthermore, it can cause a cyst. When we treated a patient with an impacted central incisor early, we could see a better prognosis. It means an early diagnosis and an exact treatment are very important. Generally if the impaction is not severe or it is caused by a keratinized covering tissue, a surgical exposure can induce an eruption easily but an orthodontic force is recommended when an eruption does not happen after a surgical method, when the eruption path is too transpositioned to be corrected spontaneously and when an impacted tooth is located so deeply. In the treatment using an orthodontic force, careful considerations about a root length, pulp, and a periodontal tissue can improve the periodontal and esthetic prognosis for the long follow-up results. This case is using an orthodontic traction following a periodic observation and in no expectation of spontaneous eruption. After treatment of this case, I have got some knowledges, so I report this case.
