Studies on the early diagnosis of pregnancy of dairy cows by EIA-kit of progesterone in milk (Progesterone EIA-kit에 의한 유우(乳牛)의 임신진단(姙娠診斷)에 관한 연구(硏究))

  • This study was carried out to evaluate khe accuracy of the early diagnosis of pregnancy in dairy cows using EIK-kit of progesterone in milk and to compare the progesterone concentrations in milk with those assayed by radioimmunoassay. 1. The progesterone concentrations of the pregnant cows($2.07{\pm}0.54ng/ml$) were significantly higher than those of non-pregnant cows($1.04{\pm}0.19ng/ml$), and thereafter began to inerease and maintained high levels. 2. During 20 to 22 days after artificial insemination, the accuracy of pregnancy diagnosis from EIA-kit of progesterone were 93.7% for non-pregnant cows, and 91.6% for pregnant cows. 3. During 20 to 22 days after artificial insemination, the accuracy of pregnancy diagnosis from milk progesterone concentrations were 100% for non-pregnant cows (<2.7ng/ml), and 92.9% for pregnant cows($${\geq_-}3.2ng/ml$$). The average overall accuracy of prediction for pregnant and non-pregnant cows were 96.5%. 4. Accordingly, the pregnancy diagnosis from EIA-kit of progesterone is thought to be recommendable because this early diagnostic means are simple with accurate result.

Changes of plasma progesterone concentrations during the estrous cycle and its application to early pregnancy diagnosis in Korean native goats (한국재래산양(韓國在來山羊)의 발정주기중(發情週期中) 혈장(血漿) progesterone 농도변화(濃度變化)와 조기임신진단(早期姙娠診斷)에의 응용(應用))

  • A study was conducted to improve the reproductive performance of Korean native goats. The length of estrous cycle and plasma progesterone concentrations during each cycles were determined by both radioimmunoassay and estrus behaviours, and the results were used in the early pregnancy diagnosis. The estrous cycles were classified into the short(l8 days or shorter, average 16.7 days), normal(19 to 22 days, average 20.9 days) and long(23 days and longer, average 23.8 days)cycle. The average length of the 19 estrous cycles was 20.8 days. Plasma progesterone concentrations in 12 normal cycles were the lowest(0.10 ng/ml) at estrus, remained high from 6 to 16 days(range : 4.43~7.93 ng/ml) and drastically decreased thereafter to reach minimal concentrations at the next estrus. Plasma progesterone concentrations were measured for early pregnancy diagnosis at 0, 10 and 20 days after mating in the 12 Korean native goats. Plasma progesterone concentrations in the pregnant goats at 20 days after mating were significantly higher than in the non-pregnant goats(p<0.001). Of the 12 goats, 10 were confirmed pregnancy by both progesterone concentrations and kidding. The accuracy of the pregnancy diagnosis based on plasma progesterone concentrations was 100% for positive as well as for negative.

STUDIES ON THE EARLY PREGNANCY DETERMINATION IN COWS BY USING THE ENZYME-IMMUNOASSY AND RADIO-IMMUNOASSAY IN MILK

  • Milk samples(n = 78) were taken 19d, 20d, 21d, 22d after artificial insemination(AI) for early pregnancy diagnosis by using the Enzyme immunoassay(EIA) kit. The progesterone ($P_4$) concentration in the whole milk was measured on the same day of pregnancy diagnosis. Rectal palpation(RP) was accomplished between 60d and 70d after AI to estimate the ovary condition and pregnancy status. Milk progesterone concentrations measured by Radio-immunoassay(RIA) method, in the pregnant cows at 17d, 19d, 21d after insemination were $17.10{\pm}0.91$, $17.60{\pm}0.46$, and $18.43{\pm}0.79nmol/l$, whereas those in the not-pregnant cows were $6.57{\pm}1.03$, $2.63{\pm}0.29$, and $0.67{\pm}0.08nmol/l$, respectively. When the progesterone concentration was less than 7 nmol/l, the color of the EIA kit was lighter and when the progesterone concentration was ${\geq}16nmol/l$, the color of the EIA kit was darker compared to the standard color. The detection rates of error by judging the color differences were 5.1% and 20.7%, respectively. In the early pregnancy diagnosis by the EIA kit and RIA method, the accuracy rates in the pregnancy of cows were 82% and 87%, and those in not-pregnant cows were 86% and 91%, respectively. For ovarian status estimated by the RIA method and certified by RP, the accuracy rates of the ovarian atrophy, follicular cyst and luteal cyst were 80, 91 and 83% and the progesterone concentrations were 2.51, 2.03, and 26.7 nmol/l, respectively.

  • Prevalence rates reported for malaria in pregnancy in Nigeria vary considerably. The accuracy of results of malaria diagnosis is dependent on training, experience, and motivation of the microscopist as well as the laboratory facility available. Results of training programmes on malaria microscopy have shown low levels of sensitivity and specificity of those involved in malaria diagnosis routinely and for research. This study was done to ascertain the true prevalence of malaria in pregnancy in Lagos, South-West Nigeria. A total of 1,084 pregnant women were recruited into this study. Blood smears stained with Giemsa were used for malaria diagnosis by light microscopy. Malaria infection during pregnancy presents mostly as asymptomatic infection. The prevalence of malaria in this population was 7.7% (95% confidence interval; 6.2-9.4%). Factors identified to increase the risk of malaria infection include young maternal age (<20 years), and gravidity (primigravida). In conclusion, this study exposes the over-diagnosis of malaria in pregnancy and the need for training and retraining of laboratory staffs as well as establishing the malaria diagnosis quality assurance programme to ensure the accuracy of malaria microscopy results at all levels.

The Rare and Challenging Presentation of Gastric Cancer during Pregnancy: A Report of Three Cases

  • Pregnancy-associated gastric cancer is extremely rare. In many cases, it is diagnosed at an advanced stage because the symptoms during pregnancy are generally overlooked. We report three cases of gastric cancer during pregnancy with various outcomes. The first case included a patient with stage IV gastric cancer who received palliative chemotherapy. This patient had a preterm birth and died 7 months after diagnosis. The second case received neoadjuvant chemotherapy during pregnancy and a total gastrectomy was performed after delivery. She then received adjuvant chemoradiotherapy. This patient developed pulmonary metastasis and died of recurrence 41 months after surgery. In the third case, a distal subtotal gastrectomy was performed at week 14 of pregnancy, with no complications. The patient received adjuvant chemoradiotherapy. She is currently without recurrence 14 months after surgery. In patients with pregnancy-associated gastric cancer, treatment decisions are predominantly influenced by clinical stage and gestational age at diagnosis.

Pregnancy Diagnosis of Hanwoo(Korean Native Cattle) by Serum Progesterone Concentration during Early Gestaton (한우에 있어서 혈청 Progesterone 농도 측정에 의한 조기임신진단)

  • Serum progesterone concentrations of pregnant and non-pregnant Hanwoo were compared to evaluate the possibility of early pregnancy prediction. Twenty five female Hanwoo were devided into two groups. Eighteen heads were injected with 980$\mu\textrm{g}$ tiaprost trometamol salt(Iliren:PGF2$\alpha$ analogue) for estrous synchronization and fifteen heads of them were inseminated artifically and seven heads were non-treated. Blood samples were collected from the jugular vein before PGF2$\alpha$ injection and on day 0, 18, 21 after estrous induction. Pregnancy of cattle was confirmed by rectal palpaton after 60 days of artificial insemination. The serum progesterone concentrations at heat showed no significant differences between pregnant and non-pregnant while on day 18 and 21 after estrous induction showed significant differences(p<0.05). The accuracy of pregnancy diagnosis on day 18 and 21 after estrous induction was 85.7 and 71.4% respectively while that of non-pregnancy were 71.4 and 100.0% respectively.

Studies on the Improvement of Precision in Early Pregnancy Diagnosis by Milk Progesterone Test(EIA) in Dairy Cows (Milk Progesterone Test(EIA)에 의한 소의 임신조기판단 정확도 향상에 관한 연구)

  • These experiment was carried out to improve the precision of early pregnancy diagnosis in dairy cattle. Changes in progesterone concentration of milk were measured by Enzyme Immunoassay(EIA) in 73 cows up to 21 days after insemination. The average concentraton of progesterone in milk was 1.9ng/ml at eatrus ; it increased to 17.8ng/ml on day 14, and thereafter it declined to 4.3ng/ml on day 21 in nonpregnant cows. Whereas in pregnant animals, it was maintained and elevated further to 22.2ng/ml on day 21. The accuracy of the pregnancy diagnosis for animals classified as positive (pregnant ; over 10ng/ml and decreasing rate<1.5) and negative (non-pregnant ; under 7ng/ml and decreasing rate>1.5) were 95% and 100% respectively. The samples ranging between 7ng/ml and 10ng/ml were classified as positive (decreasing rate<1.5) and negative(>1.5), which accuracy was 54.5% and 100% respectively. However this range appears to be of the most interest for the veterinary practitioner since cows in proestrus or early interestrus tend to have milk progesterone levels within this value. Causes for the insufficient precision of false pregnancy diagnosis are discussed.

Pregnancy diagnosis in goat by using vaginal cytology and trans-abdominal ultrasonography

  • The present study was conducted in a goat farm for pregnancy diagnosis by using vaginal cytology and B-mode real time ultrasound using 5 MHz probe by transabdominal method. Seventeen pregnant does were used for this study. The objective of the study was to determine the earliest day of pregnancy and describe the chronological characteristics of pregnancy from day 22 to day 40 for vaginal cytology and day 25 to day 60 for ultrasonography of gestation. The differences among the average percentage of cell value in different age of pregnancy were significant (p < 0.05). The average percentage of intermediate cells (81.12%) was also significantly (p < 0.05) higher than superficial (9.41%), keratinized (7.10%) and neutrophil (2.61%) on 22-40th days of pregnancy. In case of real time B-mode ultrasonography, the gestational sac was observed only in three does out of seventeen (17.6 %) at 25-30 days whereas the placentomes and heart beat of the foetus were first detected at 31-35 days in six does (35.3%). The foetal leg buds were first visualized at 36-40 days in four does (23.5%) whereas the foetal vertebral column was first observed at 36-40 days of gestation in only three does (17.6%). In conclusion, vaginal cytology and trans-abdominal ultrasonography can be used for detection of early pregnancy in does.

  • Combined intrauterine and extrauterine pregnancy is rare, occuring 1 in 30,000 pregnancies. The early diagnosis of combined pregnancy is very difficult, and so there is a higher maternal morbidity and fetal loss. Recently the incidence of combined pregnancy is increased. We experienced a case of combined intrauterine and right tubal pregnancy, and this case was presented with a brief review of the literatures.

  • Hyperemesis gravidarum is a severe and intractable form of nausea and vomiting in pregnancy. It is a diagnosis of exclusion and may result in weight loss; nutritional deficiencies; and abnormalities in fluids, electrolyte levels, and acid-base balance. The peak incidence is at 8-12 weeks of pregnancy, and symptoms usually resolve by week 16. Interestingly, nausea and vomiting of pregnancy is generally associated with a lower rate of miscarriage. The cause of severe nausea and vomiting in pregnancy has not been identified. Extreme nausea and vomiting may be related to elevated levels of estrogens or human chorionic gonadotropin. Once the diagnosis is made, treatment consists of mainly supportive care. Until vomiting is controlled, these patients should be maintained as NPO. We treated 1 patient who had hyperemesis gravidarum and visited Kyung-Won Incheon Oriental Hospital by Herbal medication, acupuncture, moxa and nega therapy and got good result from them.
