당뇨병성 족부 합병증에 따른 하지 절단술의 위험 인자에 대한 분석 (Analysis of the Risk Factors for Lower Extremity Amputation due to Diabetic Foot Complications)

  • Purpose: To evaluate the possible risk factors of lower extremity amputations in diabetic foot patients. Materials and Methods: The study is based on 37 patients who received lower extremity amputations from April, 1997 to February 2005 due to diabetic foot complications with at least 1 year follow up. As for the control group, 49 diabetes patients who had been treated at the endocrinology department for at least 1 year without any diabetic foot complication were evaluated. As for the possible risk factors, age, gender, duration of diabetes mellitus, body mass index, Hb A1c, blood glucose level, total cholesterol, s-creatinine, C-peptide, smoking, alcohol, hypertension, cardiovascular disease, CVA, retinopathy and neuropathy were investigated. Results: Among the possible risk factors evaluated, age, Hb A1c, smoking, neuropathy and blood glucose level factors showed statistically significant difference between the diabetic amputation and the control group. Conclusion: In reducing the risk of the lower extremity amputations in the diabetic patients due to diabetic foot complications, strict control of blood glucose level and cessation of smoking were found to be utmost important.

한국산 식용 및 약용 식물의 섭취가 당뇨 유발 흰쥐의 혈당, 글리코겐 및 단백질 농도에 미치는 영향 -고본, 누룩치, 모시대 및 산초를 이용하여- (Effects of Edible and Medicinal Plants Intake on Blood Glucose, Glycogen and Protein Levels in Streptozotocin Induced Diabetic Rats)

  • The hypoglycemic effects of four edible plants (Angelicae tenuissimae (A. ten.), Pleurospermum kamtschaticum (P. kam.), Adenophora remotiflora (A. rem.) and Zanthoxylum schinifolium (Z. sch.)) in streptozotocin (STZ) -induced diabetic rats were investigated. Sprague-Dawley male rats weighing 190-230 g were induced diabetes mellitus by the STZ injection (45 mg/kg) into the tail vein and were divided into six groups ; normal, STZ-control and four edible plant groups (A. ten., P kam., A. rem. and Z. sch. groups). Normal and STZ-control groups were fed a AIN-93 diet and four groups of STZ-induced diabetic rats were fed one of each experimental diets containing 10% of the edible plant powder for 4 weeks. Diabetic rats showed the lower weight gain compared to the normal rats. In experimental groups except P. kam., AST activities were close to normal. A. ten. group were lowered ALT activities slightly. The plasma glucose levels of the diabetic experimental groups were significantly decreased at 4th week. The plasma insulin levels in diabetic experimental groups were not significantly different compared to the STZ-control group. The liver glycogen levels in STZ injected rats were significantly lower in compared to the normal rats. However no significant differences were found in response experimental plants intake in diabetic rats. The muscle glycogen were not significantly different among all the groups.

스트렙토토신-당뇨쥐의 유병기간에 따른 혈중지질패턴의 경시적 변화 (Changes in Plasma Lipid Pattern in Streptozotocin-Induced Diabetic Rats: A Time Course Study)

  • This study was carrid out to examine a part of the mechanism for the etiology of diabetic complications. Thirty normal and forty streptozotocin(STZ)-induced diabetic rats were used as the animal models. Animals were sacrificed at the time points of 3 days, 1, 2, 4 and 6 weeks after STZ-injection and time course in body weight and organ weight, the levels of blood glucose, plasma lipid patterns, and atherogenic index were measured during 6 weeks. The STZ-diabetic animals showed 63% survival rate and fsting blood glucose levels of the diabetic animals measured in the range of 230-410mg/dL during the experimental period. The body weigh of diabetic animals decreased significantly throughout the experimental period and the relative weights of organs to body weight were significantly higher than the normal control ones. The enlargement of the kidney in the diabetic animals was especially remarkable. Plasma triglyceride concentration in diabetic rats substancially increased from the first week of onset of diabetes mellitus and maintained higher levels than the control ones throughout the whole experimental period. The plasma total cholesterol level and atherogenic index in the diabetic rats were significantly higher than the normal ones from the third day after STZ injection and showed a gradual increase with the duration of the disease. Throughout the experiment, the diabetic rats consistently showed a slightly lower HDL-cholesterol level compared to the normal animals. From the results of this study, it appears that the significant changes in blood lipid pattern in STZ-diabetic animals start from the first week after STZ injection.

Effects of Multi-Extracts of Mori Folium and of Exercise on Plasma Insulin and Glucose Levels in Streptozotocin-Induced Diabetic Rats

  • This research was conducted to study the effects of the supplementation of multi-extracts of mori folium (MF) and of exercise on plasma insulin and glucose levels in streptozotocin (STZ)-induced diabetic rats. Eight male Sprague-Dawley rats, 4 weeks old, were assigned to each experimental group and were raised in the laboratory for 10 weeks. The animal groups consisted of a normal-control group, a STZ-control group, 3 STZ-induced diabetic groups supplemented ad libitum with various amounts of MF extracts (MF-720, MF-360, and MF-180 groups), and a STZ-induced diabetic group supplemented with MF-360 along with exercise. In the normal-control group, glucose tolerance tests resulted in the peak blood glucose level being achieved in 15 minutes and a fasting blood glucose level being achieved in 60 minutes. In the STZ-control group, the peak blood glucose level was reached after 60 minutes and, even after 90 minutes, blood glucose shown at a significantly higher level compared to the fasting levels. In the groups supplemented with MF extracts, the blood glucose level peaked after 30 minutes of glucose challenge, and returned to the fasting level after 90 minutes; the MF-360 and MF-360+exercise groups showed the best levels of glucose tolerance. Blood glucose levels in the STZ-induced diabetic groups were significantly higher compared to the normal-control group. However, after 7 weeks of supplementation with MF extracts, a significant lowering of blood glucose levels was observed in all groups supplemented with the MF extract. The best effect was observed in the group given MF extract combined with exercise. Compared to the normal-control group, blood insulin levels were significantly lower in all STZ-induced diabetic groups; however, a significantly higher level of insulin was observed in the groups given MF extracts compared to the STZ-control group. This study shows that the supplementation of MF extracts in STZ-induced diabetic rats resulted in increased blood insulin levels and lower blood glucose levels.

Effect of the Dietary Protein Level on Plasma Glucose, Lipids and Hormones in Streptozotocin-Diabetic Rats

  • Atherosclerotic vascular disease is a major cause of the increased morbidity and mortality assciated with diabetes mellitus. The prominent role of nutrition in hypercholesteolemia and atherosclerosis is generally accepted. Diet is a key element in the management of diabetes (type I-IDDM), yet the appropriate diet for patient with diabetes mellitus is not well known. Dietary protein has been shown to have a significant effect on plasma cholesterol levels in both experimental animals and humans. The present experiment was designed to determine the effect of the dietary protein level(20% vs 60%) on plasma glucose concentration, lipids profile, insulin and glucagon levels from non-diabetic and streptozotocin-induced diabetic rats. Results showed that a high protein diet decreased triglyceride concentration in diabetic rats. Also diabetic rats fed a high protein diet were hypocholesterolemic than rats fed a control diet. There were no effects by level of protein on fasting blood glucose concentration and insulin/glucagon ratio. Results from the present study suggest that a high protein diet may be beneficial to control pasma lipids in chemically-induced diabetic rats.

Potential on Hypotriglyceridemic Effect of Chloroform-Methanol Extract of Adlay Diabetic Rats

  • The purpose of this study was to investigate the potential hypolipidemic effect of adlay extract in streptozotocin-induced diabetic rats. Fifty six rats were fed either a control diet or adlay extract diets of : Methanol 1%(Ml%) ; Methanol 2%(M2%) ; Methanol 4%(M4%) ; Chloroform-Methanol 1%(CMl%) ; Chloroform-Methanol 2%(CM2%) ; Chloroform-Methanol 4%(CM4%) for 3 weeks. The amount of extracts added was 1%, 2% or 4% by diet weight respectively. The levels of glucose, total cholesterol(TC), high-density-lipoprotein cholesterol (HDL-C), free-fatty acid(FFA) and triglyceride(TG) in plasma, liver and skeletal muscle were compared. Among diabetic rats, there were no significant differences in the plasma level of glucose and TC regardless of a different extraction procedure or different amount of extracts added. While the plasma 7G level tended to increase with times passed in diabetic control group, was not increased with times passed in CM groups and was significantly lower in CM groups at 3rd week. Compared to the diabetic control group, the levels of FFA tended to be lower in all M groups and were not different in all CM groups. The levels of HDL-cholesterol were not different in all M groups and were significantly lower in all CM groups than diabetic control group. Compared to diabetic control group, liver triglyceride level was lower in M4% group and no significant difference was seen in Ml%, M2% and all CM groups. Muscle triglyceride level tended to be lower in Ml%, M2% and CM2% group and significantly was lower in M4% and CM4% group. Thus, it can be suggested that a CM extract of adllay could have a potential hypotriglyceridemic effect on diabetic subjects. (Korean J Nutrition 31(5) : 921∼926, 1998)

Polymorphonuclear Neutrophil Dysfunctions in Streptozotocin-induced Type 1 Diabetic Rats

  • Since conflicting results have been reported on non-specific immune response in type 1 diabetes, this study evaluates polymorphonuclear neutrophil (PMN) functions in the infection free Long Evan diabetic rats (type 1) by using tests that include: polarization assay, phagocytosis of baker's yeasts (Saccharomyces cerevisiae) and nitroblue tetrazolium (NBT) dye reduction. Polarization assay showed that neutrophils from diabetic rats were significantly activated at the basal level compared to those from the controls (p < 0.001). After PMN activation with N-formyl-methionyl-leucyl-phenylalanine (FMLP), control neutrophils were found to be more polarized than those of the diabetic neutrophils and the highest proportions of polarization were found to be 67% and 57% at $10^{-7}\;M$ FMLP, respectively. In the resting state, neutrophils from the diabetic rats reduced significantly more NBT dye than that of the controls (p < 0.001). The percentages of phagocytosis of opsonized yeast cells by the neutrophils from control and diabetic rats were 87% and 61%, respectively and the difference was statistically significant (p < 0.001). Evaluation of the phagocytic efficiency of PMNs revealed that control neutrophils could phagocytose $381{\pm}17$ whereas those from the diabetic rats phagocytosed $282{\pm}16$ yeast cells, and the efficiency of phagocytosis varied significantly (p < 0.001). Further, both the percentages of phagocytosis and the efficiency of phagocytosis by the diabetic neutrophils were inversely related with the levels of their corresponding plasma glucose (p = 0.02; r = -0.498 and p < 0.05; r = -0.43, respectively), which indicated that increased plasma glucose reduced the phagocytic ability of neutrophils. Such relationship was not observed with the control neutrophils. These data clearly indicate that PMN functions are altered in the streptozotocin (STZ) - induced diabetic rats, and hyperglycemia may be the cause for the impairment of their functions leading to many infectious episodes.

당뇨병 유발에 영향을 미치는 식이 요인 (The Influence of Dietary Factors on the Incidence of Non-Insulin-Dependent Diabetes Mellitus)

  • The purpose of this study was to identify dietary factors related to the incidence of diabetes mellitus in Korea. The study consisted of 165 diabetic patients, male and female, aged 30 to 70 years and 198 healthy persons as controls. Diabetic patients who had been diagnosed with diabetes mellitus for less than five years before the study period were recruited from eight different hospitals located in Seoul, Korea. Socioeconomic status, state of illness, physical activity, food habits and food intake were assessed. Food intake was assessed by food frequency questionnaire method using a 105-food frequency questionnaire developed for diabetic patients. The stress and activity indices of diabetic patients were not significantly different from control, but alcohol consumption and smoking levels were higher in diabetics than controls. Other diseases of male diabetic patients included liver diseases, digestive system diseases, and hypertension, while those of female diabetics were hypertension, neuralgia, arthritis and digestive system diseases. These disease patterns are different from Western countries whose most common complications are hypertension and hyperlipidemia. More irregular and less varied meals were found in the diabetic group compared to the control group, suggesting that diabetic patients have generally undesirable food habits. Otherwise, food and nutrient intake of diabetics did not differ greatly from the control group. It was found that diabetic patients consumed more cereals and less fruit than the control group, and also that male diabetics consumed more alcohol. The carbohydrate : protein : fat energy distribution ratio was 61.7 : 15.8 : 22.5 in male patients and 65.1 : 14.9 : 19.7 in female patients. Discriminant analysis showed that diabetes risk factors differed with sex. In male patients, the important factors were body mass index(BMI), vitamin C intake, family diabetic history and vegetable intake, while in female patients they were BMI, cereals intake, carbohydrate intake, vitamin C intake, stress, food habits and Ca intake. These results show that excessive intake of energy and fat are not the major causal factors in Korean NIDDM. Therefore, the diabetes risk factors of Western countries may not directly apply to Koreans. Mors study is needed to clarify the risk factors of Korean NIDDM.

Streptozotocin 유발 당뇨쥐에 있어서 발효콩을 주원료로 한 기능성 식품의 혈당강하 효과 (Hypoglycemic Effect of the Functional Food Manufactured by Fermented Soybean as Main Materials in Streptozotosin - Induced Diabetic Rats)

  • 본 연구에서는 발효콩을 주원료로 한 기능성 식품이 streptozotocin으로 유발된 당뇨쥐의 체중, 장기무게, 혈당, 혈중 중성 지방, 혈중 콜레스테롤, HDL-콜레스테롤에 미치는 영향에 대하여 조사하였다. 기능성 식품군에서는 당뇨군에 비하여 완만한 체중 감소를 나타내었으며 식이섭취량은 모든 당뇨군에서 현저한 증가를 나타내었다. 식이효율은 모든 당뇨군에서 낮은 수치를 나타내었으나 기능성 식품의 투여로 식이효율이 당뇨군에 비해 유의적인 증가를 나타내었다(p<0.05). 장기의 중량 비교에서 간과 신장의 경우, 모든 당뇨군에서 정상군에 비하여 증가를 나타내었으며 기능성 식품의 투여로 다소 감소를 나타내었다. 심장과 비장의 무게는 모든 실험군에서 유의적인 차이를 나타내지 않았다(p<0.05). 경구 당내성 검사에서는 당뇨군에서 50분대에 혈당치가 최고에 달하였다. 기능성 식품 투여군에서는 30분대에 최고치에 이르렀으며 120분 후에는 현저하게 감소되어 거의 공복전의 수준에 도달하였다. 혈중 포도당의 수치는 기능성 식품의 4주간 투여에 의해 당뇨군에 비하여 유의적인 감소를 나타내었다(p<0.05). 혈청 중 총콜레스테롤, 중성지방, AI의 수치는 묘든 당뇨군에서 정상군에 비해 유의적으로 높은 수치를 나타내었다(p<0.05). 기능성 식품 400 mg/kg을 투여한 투여군에서는 당뇨군에 비하여 각각 16.4%, 15.4% 그리고 48.3%의 감소를 나타내었으나 시료사이의 유의적인 차이는 인정되지 않았다(p<0.05). HDL-콜레스테롤은 기능성 식품 투여에 의해 당뇨군에 비해 다소 증가를 나타내었으며, 400 mg/kg을 투여한 군에서는 정상의 수치에 근접하여 정상군과 유의적 차이가 인정되지 않아(p<0.05) 발효콩을 주원료로 한 기능성 식품이 당뇨 흰쥐의 혈중 콜레스테롤 저하와 지질대사 개선효과가 있음을 알 수 있었다.

당뇨병 환자의 혈당 조절 정도에 대한 인식과 HbA1C 값 비교 (The discrepancy between the estimation for diabetic control and the HbA1C value in diabetic patients)

  • Background : Diabetes mellitus is chronic condition which needs patients' understanding of disease status and effort for disease control. We investigated whether the patients' estimation for blood glucose control was reliable or not comparing with their HbA1C value, Method: Among people who visited one health promotion center in Seoul from 1995 to 2003,those who answered as having diabetes mellitus on self reported questionnaire were identified. Subsequent question was whether his or her diabetes was well controlled or not. We compared these replies with their HbA1C values. When their HbA1C values were below7% with answering not well controlled, we analyzed if there was any variable which can predict the discrepancy. And the same analyze we did for the opposite situation. Result: A total of 1193 patients were eligible. Those who answered as well controlled formed32.9% of the group with HbA1C values above 7%. In the cases with well controlled though significantly associated with this discrepancy, Conclusion: Considerable proportion of diabetic patients' estimation for diabetic control status was not matched with glycemic control guideline. For successful management of diabetes mellitus, more efforts for patient education and communication skills are essential.
