당뇨교육이 당뇨환자의 역할행위 이행에 미치는 영향 (The Effect of Patient Teaching on Compliance with Sick Role Behavior of diabetic Patients)

  • This study examined the effect of patient teaching on Compliance with sick role behavior in diabetic patients. The purpose was to improve diabetic patients Compliance by D. M. patient teaching. The study objectives were to determine the effect of patient teaching on Compliance with sick role behavior, and factors influencing compliance with sick role behavior of diabetic patients. The Subjects, consisting of 52 diabtic patients diagnosed in the C. and Other hospitals in K. city were divided into experimental and Control groups. Data were gathered from July 25th to September 3rd, 1988 through interviews by questionnaires, measurment of blood sugar level by Reflolux. D. M. Patient teaching was defined as informational intervention of social support by the nurse. A booklet representing patient education and questionnairs were developed by the investigator, and were tested for Content validity, and reliability by Item Analysis: Cronbachs $\alpha$ for any instrument to measure variables was patient Compliance .83, perceived health belief .65, diabetic knowledge .70. Analysis of data were done by paired t-test, t-test, Anova, Pearson correlation and Stepwise multiple regression, The results of the study may be summarized as follows : 1. The effect of patient teaching on Compliance with sick role behavior, on diabetic Knowledge and health belief was Confirmed by significant differences between the experimental and the control group before and after the experiments. (P=000 P=006, P=004). 2. Factors influencing compliance with sick role behavior of diabetic patient were diabetic patient teaching (P<.005), perceived health belief(r=.5597, P<.005), blood sugar(r=.3205, P<.01), diabetic knowledge(r=.2876, P<.05).

소양인 당뇨병성 말초신경병증 치험 1례 (A Case study of a Soyangin Patient with Diabetic neuropathy)

  • Objective We report a case about a Soyangin patient with diabetic neuropathy. His symptoms were alleviated with Sasang Constitutional medicine. Methods A Soyangin patient with diabetic neuropathy was treated by herbal medicine, acupuncture, pharmacopuncture according to Sasang Constitutional Medicine. The progress was evaluated with numerical rating scale(NRS) and blood sugar test(BST). Results Sasang Constitutional Medicine seems to be effective for the symtoms of the patient with diabetic neuropathy in this case. Conclusions This case study shows that Sasang Constitutional Medicine may be an effective treatment for Soyangin's diabetic neuropathy.

  • Diabetes is a controllable disease, not a curable disease. If Diabetic patients do not regularly monitor themselves, they run risk of diabetic complications that cannot be reversed or treated. Especially, if the patients have high blood suger level, a cardiovascular disease, or poor blood circulation, there immune system will not function, and thus making the patient more susceptible to infection. Eventually, the patient will have to undergo amputation. The case is a report about a Diabetic patient with foot ulcers. The patient was treated by Taeumin Bopyewontang and the foot ulcers were improved.

  • One of the serious long-tenn complications of diabetes mellitus is the diabetic foot lesion. Diabetic foot lesions are a major cause of morbidity, mortality, disability in patients with diabetes and frequent cause of lower-extremity amputations. The treatment of diabetic foot lesions is based on the strict control of diabetes mellitus. A patient with diabetic foot lesions was treated with Cheunhwasangamibang and improvement was seen. Results in this case support a role for Cheunhwasangamibang in controlling diabetes mellitus, and are here reported to encourage further study.

  • Diabetic foot ulcer is a serious complication which result from long-standing diabetes. Especially, severe infected diabetic foot ulcer results in unwanted lower extremity amputation. The diabetic patient is considered the relative contraindication for microsurgery because of the severe peripheral vascular disease. Recently, microvascular free tissue transfer technique applied to diabetic foot ulcer. It is well known that free tissue transfer provides immediate soft tissue coverage and control of infection. So it is possible that preservation of the lower extremity through free tissue transfer. A retrospective study of diabetic patients who had infected foot ulcer from 1999 to 2000 with foot defects reconstructed with free tissue transfer were reviewed. Thirteen patients were studied with mean follow-up of 12.7 months. There were two deaths during follow-up period. There were two failures after free flap surgery. All eleven survived patients were ambulatory. There was no recurrence of ulcer. No patient need amputation above the ankle joint. We have found that free tissue transfer for infected diabetic foot ulcer is very effective surgical technique. Careful patient selection and regular follow-up is important.

  • The purposes of this article are to review the pathogenesis, prevention, and management of amputation due to diabetes mel1itus complications, and to report one case who had lost his toes due to diabetes mellitus. A primary cause for hospital admission of the patient was foot ulcer. Since many amputations in diabetic patients are precipitated by such ulcers, a program for active prevention and optimal treatment of diabetic foot lesions might decrease the risk of amputation. Diabetic foot ulcers and, ultimately, amputation can stem from a variety of pathways. The combination of peripheral neuropathy, peripheral vascular disease and infections is the harbinger of the final cataclysmic events of gangrene and amputation. As the physical therapist is often involved in the treatment of diabetic patients, the therapist should be aware of the followings: the patient's type of diabetes and the severity of the diabetes, the complications of the disease, the effects of exercise, the importance of wearing proper shoes and education to patients about appropriate diabetic foot care.

  • 1. Objectives Besides Diabetes Mellitus, its complications are matter of concerns as well. Its following complications are cardiovascular disease, glomerulopathy, retinopathy, neuropathy and diabetic gangrene. These complications can cause overall damage in major organs. In traditional oriental medicine it has been perceived that gangrene is mainly caused by "hwa(火)" but under sasang constitutional medicine its has been diagnosed as Taeumin superficial-disease(太陰人 表病) and Taeumin Jowiseungchung-tang(調胃升淸湯) was prescribed. As a result, significal improvments was noted and thefore we report the outcome. 2. Methods We have given Taeumin Jowiseungchung-tang(太陰人 調胃升淸湯) to a patient for treating diabetes complications. 3. Results This patient was treated with Sasang Constitional Medicine and acupuncture for about three weeks. After treatment, ulcer and whole body edema were improved considerbly. 4. Conclusions We have dianosed the patient as Taeumin superficial-disease(太陰人 表病) by undergoing several diagnostic method used in sasang constutional medicine. Therefore we have given Taeumin Jowiseungchung-tang(太陰人 調胃升淸湯) to the patient and has obtained satisfactory results in curing several diabetic complications especially ulcer due to diabetes.

  • The study was performed to investigate the inhibitory action on the growth rate of Candida albicans under the various immunological conditions, Candida albicans was grown in broth media containing 0.1ml. (in total broth volume 4.0ml.) of normal human serum, diabetic patient serum, albumin solution, artifical bulla content and nutrient solution as control subject. The inhibitory action of Candida albicans was indicated by mesurement of transparency rate with use of the spectrophotometer. The results are as follows: 1. Normal human serum shows inhibitory effect on the growth of the candida distinctly. 2. Albumin solution reveals almost similar to that of the effect of normal human serum. 3. Artificial bulla content which obtained by irradiation of ultraviolot ray after application of 1% 8-Methoxy-psolaren cream and the diabetic patient serum shows reduction of inhibitory effect as compared with that of the normal human serum. 4. It is estimated that the titer of negative effect of diabetic patient serum is not related with the variation of immunoglobulin titer in patient serum.

  • Purpose: The purpose of this study is to identify the relationship among clinical competence of diabetic diet education on standardized patients, self-efficacy, communication skill, learning satisfaction, and professional values of nursing students. Methods: The present study was a descriptive research. The participants were 144 junior nursing students in a university. The clinical competence on diabetic diet education was assessed by trained evaluators when nursing students performed diabetic diet education using the standardized patients. The students' self-efficacy, communication skill, professional values, and learning satisfaction were measured by themselves using the questionnaires. Results: The clinical competence on diabetic diet education showed positive correlations with communication skill and learning satisfaction, while the competence was not correlated to self-efficacy and professional values. Conclusion: The results of this study suggest that clinical competence on diabetic diet education may be more related to communication skills and learning satisfaction than self-efficacy and professional values. Promoting communication skills and learning satisfaction using a standardized patient may increase nursing students' competence on diabetic diet education.

  • Amputation of diabetic foot ulcer and infection is a critical modality for saving a patient's life from life threatening infections or ischemic limbs. However, it can cause serious handicaps or complications, such as lifetime shortening and re-amputation of the other limb. The minimal amputation is the main goal of amputation in diabetic patients. However, insufficient amputation can have a harmful effect on patients. The decision of amputation is very difficult and should be made using multidisciplinary approaches. All aspects of the patient's situation, including vascular status, degree of infection, and medical conditions should be considered. The foot surgeon should keep in mind the notion that proper amputation can lead to a new life for diabetic foot patients.
