Development and Effects of a Motivational Interviewing Self-management Program for Elderly Patients with Diabetes Mellitus (노인 당뇨병 환자를 위한 동기강화상담 당뇨병 자기관리 프로그램의 개발 및 효과)

  • Purpose: This study was conducted to develop and test the effects of a motivational interviewing self-management program for use with elderly patients with diabetes mellitus. Methods: A non-equivalent control group pretest-posttest design was used. The participants were 42 elderly diabetic patients (experimental group: 21, control group: 21). The motivational interviewing self-management program for elders with diabetes mellitus developed in this study consisted of a 12-week program in total (8 weeks for group motivational interviewing and education and 4 weeks for individual motivational interviewing on the phone). Data were collected between February 13 and May 3, 2013 and were analyzed using t-test, paired t-test, and repeated measure ANOVA with SPSS/WIN 18.0. Results: For the experimental group, significant improvement was found for self-efficacy, self-care behavior, glycemic control and quality of life (daily life satisfaction, influence of disease) as compared to the control group. Conclusion: The study findings indicate that the motivational interviewing self-management program is effective and can be recommended as a nursing intervention for elderly patients with diabetes mellitus.

A study on the Polymorphisms in Salivary Parotid Middle-band Protein of the Patients with Diabetes Mellitus (당뇨병 환자의 이하선 타액내 Pm의 다형현상에 대한 연구)

  • The purpose of this study was to evaluate the polymorphism in parotid middle-band protein(Pm) of the patients with diabetes mellitus. Saliva from the parotid glands was collected from 60 healthy Korean who were live in Kwan-ju and from 33 diabetes mellitus patients who had more than 140mg/dl of fasting blood sugar for on week. In the saliva collected from parotid glands, Pm was analyzed to evaluate the distribution of phenotype using acid-urea starch gel elecrophores is The following results were obtained : 1. The phenotypes of parotid middle band protein(Pm) observed in parotid saliva of the control group(60 people) were Pm(+) in 38 people (63.3%) and Pm(-) in 22 people (36.7%). The gene frequency of Pm+ was0.394, and that of Pm- was 0.606. 2. The phenotypes of parotid middle band protein(Pm) observed in parotid saliva of the diabetes mellitus patient group(33 patients) were Pm(+) n 21 patients(63.6%) and Pm(-) in 12 patients (36.4%). The gene frequency of Pm+ was 0.397, and that of Pm- was 0.693 3. Pm dose not have significant differences between phenotypes on both the control group and the diabetes mellitus patient group.

A Case Report of Patient with Type 2 Diabetes Mellitus Treated with Galgeun-tang (갈근탕을 투여하여 호전을 보인 제 2형 당뇨병 증례 1례)

  • Objectives: The aim of this study was to explore and describe how fasting blood sugar (FBS), postprandial 2 h Glucose (PP2h), and glycated hemoglobin (HbA1c) of a patient diagnosed with type 2 diabetes mellitus can be reduced by treatment with Galgeun-tang herbal medicine. Methods: The patient was administered herbal medicine to reduce serum glucose levels. The prescribed herbal medicines included Galgeun-tang and Galgeun-tang-gami. Results: The therapeutic outcomes were control of blood sugar and glycated hemoglobin (HbA1c) levels and decreased insulin administration. Conclusion: The herbal medicine, Galgeun-tang, appears to be a valid treatment for type 2 diabetes mellitus. Serum glucose (FBS/PP2hrs) and HbA1c were well controlled and insulin administration was decreased. Galgeun-tang was effective in controlling the daily glucose levels in a patient with type 2 diabetes mellitus.

Effects of Self Care and Depression on the Yangsaeng of Diabetes Mellitus Patients (당뇨병 환자의 자가관리, 우울 양상이 양생에 미치는 영향)

  • The purpose of this study was to investigate self care, depression, and Yangsaeng and to confirm the factors related to Yangsaeng of diabetes mellitus patients. The survey was conducted on 116 subjects who consented to Institutional Review Board for diabetes mellitus patients in the area and data were collected by self-report questionnaire. The questionnaires consisted of self care, depression, and Yangsaeng. Date analysis was done by SPSS 22.0 with T-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficient, and multiple regression. Yangsaeng was significantly related to the educational level, partner, income, perceived health status, depression, and admission for diabetes mellitus. Yangsaeng and self care showed positive correlation, on the other hand Yangsaeng and depression showed negative correlation. Yangsaeng was found to be related to education level, self care, and depression through the multiple regression. In conclusion, the Yangsaeng score increased with improving of blood glucose level and depression. Based on these results, we considered various characteristics such as patient's age and illness characteristics, therefore it is necessary to develop a method to measure Yangsaeng.

Factors Influencing Self-Care Behaviors Related to Insulin Therapy in Elders with Diabetes Mellitus (노인 당뇨병환자의 인슐린요법 관련 자가간호행위에 대한 영향요인)

  • Purpose: Self-care behavior is a key dimension of healthcare quality among elderlys with diabetes mellitus. The purpose of this study was to identify factors associated with self-care behaviors related to insulin therapy among elders with diabetes mellitus. Methods: The research was a cross-sectional, descriptive design using questionnaires. The participants were 147 older adult patients with diabetes mellitus receiving insulin therapy. Data were analyzed using descriptive statistics, t-test, ANOVA and multiple regression with SPSS statistics 19.0 program. Results: Participants' self-care behaviors related to insulin therapy were not adequate. Stepwise multiple regression analysis for self-care behaviors related to insulin therapy revealed that the most powerful predictor was self-efficacy. Self-efficacy, family support and frequency of blood sugar test explained 54.4% of the variance in self-care behaviors related to insulin therapy. Conclusion: The results indicate a need to promote self-efficacy, family support and frequency of blood sugar testing with these participants. In addition, strategies to improve self-care behaviors among elderly patients should be considered by health professionals, family members and the elderly patients themselves.

A Case Report of Patient with Type 2 Diabetes Mellitus treated with Traditional Korean Medicine (한방치료를 통한 제 2형 당뇨병 증례 1례)

  • This is a clinical report on a patient with Diabetes Mellitus treated with Traditional Korean Medicine. We applied herbal medication and western medication depending of symptoms of the patient. We observed the change of serum glucose (FBS/PP2hrs) daily and HbA1c. During the treatment and observation, result of serum glucose (FBS/PP2hrs) and HbA1c was well controlled and insulin administration was decreased. Above the results, Traditional Korean Medicine was effective to control type 2 diabetes mellitus.

The Correlation between Dietary Intakes of Calcium and Vitamin D and Osteoporosis, Hypertension and Diabetes Mellitus (식이 칼슘, 비타민 D의 섭취와 골다공증, 고혈압 및 당뇨병의 상관관계)

  • In the light of the increasing concern over the prevalence of osteoporosis, hypertension, diabetes mellitus, we performed this study to review the correlation between the dietary calcium and vitamin D intake and these diseases. To this end, we investigated the effects of dietary calcium and vitamin D on these diseases. We observed that the intake of dietary calcium and vitamin D had a negative correlation with the incidences of osteoporosis, hypertension, and diabetes mellitus. Further, the intake of these two nutrients is expected to improve related mechanisms such as the renin-angiotensin system. Therefore, we suggest that dietary calcium and vitamin D have a beneficial effect on these diseases.

  • Human insulin gene is consisted of the polymorphic region with the repeating units, the regulatory sequence, the structural gene including the intervening sequence, and 3'-flanking region. The polymerase chain reaction, which amplifies the target DNA between two specific primers, has been performed for the amplification of human insulin gene and simple one-step cloning of it into Escherichia coli. Out of 1727 nuceotides compared, only 4 sites were variable: 5'-regulatory region(G2101$\rightarrow$AGG); IVS I(T2401$\rightarrow$A); Exon II(C2411 deletion); IVS II(A2740 dejection). The variations at the G2101 and T2401 were the same as those found in one American allele. The other two variations were observed only in the specific Korean allele. And, the enzyme digestion patterns among normal, insulin dependent diabetes mellitus, and non-insulin dependent diabetes mellitus were the same. On the other hand, PCR method showed the possibility of the quickaccess for the polymorphic region in terms of the restriction fragment length of polymorphism.

Monocyte Attachment and Migration through Collagen IV in Diabetes Mellitus

  • The interactions between monocytes and extracellular matrix proteins have been implicated in atherosclerosis pathophysiology. In the present study we evaluated monocyte attachment and migration through oxidized and non-oxidized collagen IV. Monocyte attachment was tested on microwells coated with either native or oxidized collagen IV. Monocyte migration through collagen IV was examined on transwells. Monocytes derived from patients with diabetes mellitus showed an increased ability to attach and migrate through collagen IV as compared to those derived from healthy volunteers. Moreover, control monocytes attached to oxidized collagen at a higher degree, while they migrated through oxidized collagen at a lower degree, as compared to the native protein. Our results also showed the involvement of the alpha2 integrin subunit in the above phenomena suggesting a modified interaction between monocytes and collagen IV in diabetes mellitus.

Studies on diet theraphy of diabetes mellitus among Koreans (한국인 당뇨병에 관한 조사연구)

  • The incidence of diabetes mellitus among Koreans as shown an increasing tendency recently, probably due to the various factors such as the improving living conditions. The majority of people are ignorant or indifferent to the nature, progress and prognosis of diabetes mellitus in Korea. 107 cases of diabetes mellitus which had been admitted to Severance Hospital between January and August, 1971 were Studied. Of these 107 cases, 22 cases were interviewed thoroughly during their hospital stay and the response to their diet therapy was carefully checked. 1) Of the 107 cases 69 cases were male and 38 cases were female; the sex ratio was 18:1. The age of the onset of the disease was as follows: 2 cases were under 20 years of age; 20 cases (18.7%) were under 40 years of age and 85 cases (79.5%) were over 40 years of age. Juvenile diabetes was less frequent when compared with developed countries. 2) Patients complaints and symptoms on admission, complications of sickness, and duration of sickness until the female discharge were also studied. We found that the incidence of tuberculosis complication in diabetes melltius was alarming (13. 8%). 3) In most cases, the control of diabetes was inadequate and diet practisis by the patients was also very poor even when they had known of the diabetes mellitus for a considerable period of time. During hospitalization 75 cases (70.0%) were controlled by diet and oral medications alone. Only 16 cases (15.0%) needed insulin injection, the remaining 16 cases required both diet control and insulin injection. 4) In general, patients received hospital diet satisfactorily. Only a few cases complained of difficulties with milk intake because of no previous dietary experience or of excessive meat orfish because they preferred vegetables and fruit. 5) Patients responded well to the dietitians interviews in the hospital but follow up study and care were poorly organized after discharge from the hospital. 6) The diet exchange 1ist published by the Korean Diabetic Association was not well received by the patients or the general puplic because it is not inexpensive and detailed instructions were not given at the time of discharge from the hospital.
