Corticosteroids are commonly used for pain control in rotator cuff tear. Deregulated $NF-{\kappa}B$ activation is a hallmark of chronic inflammatory diseases and has been responsible for the pathogenesis of rotator cuff tear. The Dexamethasone(DEXA) is a synthetic corticosteroid. The purpose of this study was to examine the exact effect of dexamethasone on $NF-{\kappa}B$ signaling in rotator cuff tear. We measured $NF-{\kappa}B$ expression in four groups: control, $TNF-{\alpha}$-treated, DEXA-treated, and combined treatment with $TNF-{\alpha}$ and DEXA. Tenocytes were isolated from patients with rotator cuff tears and pre-incubated with $TNF-{\alpha}$ (10 ng/ml), DEXA ($1{\mu}M$), or both of them for 10 min, 1 h, and 2 h. Expression of p65, p50, and p52 in the nuclei and cytosol was analyzed by western blotting and immunofluorescence imaging using confocal microscopy. We also evaluated nucleus/cytosol (N/C) ratios of p65, p50, and p52. In our study, the combined treatment with DEXA and $TNF-{\alpha}$ showed increased N/C ratios of p65, p50, and p52 compared with those in the $TNF-{\alpha}$ group at all time points. Additionally, in the DEXA group, N/C ratios of p65, p50, and p52 gradually increased from 10 min to 2 h. In conclusion, DEXA promoted the nuclear localization of p65, p50, and p52, but was not effective in inhibiting the inflammatory response of $TNF-{\alpha}$-stimulated rotator cuff tear.
생쥐의 calvarial osteoblast세포를 분리배양하여 gelatinase생성여부를 골흡수과정에서의 역할을 규명하기 위하여 SDS-PAGE-zymography분석을 한 결과 progelatinase-A를 항속적으로 합성하고 있음을 확인하였다. 생쥐의 osteoblasts를 골재흡수 약물인 PTH, $1,25(OH)_2D_3$, 단핵구배양액 (MCM) 그리고 IL-1으로 자극시키면 gelatinase생산을 촉진하여 콜라겐분해가 증가되었으나, indomethacin과 dexamethasone은 생쥐의 osteoblastic세포의 collagenolysis를 저해하였다. 한편, 골재흡수에 IL-1을 생쥐태아 유래의 장골조직배양 (fetal mouse long bone organ culture)에 처리하자 IL-1 은 골재흡수를 촉진하였다. 더우기, $IL-1{\alpha}$의 농도의존성에 대한 indomethacin과 dexametasone의 영향을 검토한 결과 직선형의 비례커브로 영향을 미쳤다. 이러한 골대사의 지견을 바탕으로 대영전-자하거의 열수추출물의 시험관내 독성검사에서 $1-200\;{\mu}g/ml$의 농도에서는 독성이 없었으며, 또한, $300\;{\mu}g/ml$ 농도에서도 생쥐의 calvarial골에는 독성이 없었다. 대영전-자하거 extract는 PTH (2 units/ml), MCM (5%, v/v), $rhIL-1{\alpha}$ (1 ng/ml) $1,25(OH)_2D_3$ (10 ng/ml)처리에 대해서 그리고 $IL-1{\alpha}$와 $IL-1{\beta}$-유발 collagenolysis에 대해서도 보호효과가 있었다. 대영전-자하거extract을 1시간동안 전처리와 후처리에서 콜라겐분해에 약간의 보호활성이 있었으며 $IL-1{\alpha}$와 $IL-1{\beta}$에 의해 유발되는 콜라겐분해에 보호활성이 보였다. 1시간동안 전처리는 콜라겐분해를 감소시키며, 대영전-자하거 extract는 gelatinase효소를 저해하였으며 PTH, $1,25(OH)_2D_3$, $IL-1{\beta}$ 및 $IL-1{\alpha}$로 유발된 효소활성화가 저해되었다. 즉, 대영전-자하거 extracts는 $IL-1{\alpha}-$와 $IL-1{\beta}$에 의해 촉진되는 골재흡수에 효과적이었으며, 비스테로이드성 항염증제제 (indomethacin 과 dexamethasone)에 의한 골재흡수방지 효과와 유사하였다. 이러한 결과는 대영전-자하거extract가 골다공증치료에 효과적임을 나타내는 것이다.
목 적: 고식적인 치료에 반응하지 않는 기관지폐 이형성증에 대한 치료로서 덱사메타손 구제 치료의 사용현황을 조사하기 위해 본 연구를 시행하였다. 방 법: 2004년 3월부터 2008년 8월까지 서울대학교 어린이병원 및 분당서울대학교병원 신생아 중환자실에 입원하여 기관지폐 이형성증으로 치료를 받은 251례의 미숙아를 대상으로 후향적으로 연구하였다. 산모와 신생아들의 인구학적, 임상적 특성을 살펴보았으며, 덱사메타손 구제 치료의 반응성에 따라 반응군 및 비반응군간의 차이점을 비교 분석하였다. 또한 덱사메타손 구제 치료에 따른 합병증을 조사하였다. 결 과: 93례(37.1%)가 중증도의 기관지폐 이형성증으로 분류되었으며, 모든 덱사메타손 구제 치료는 중증도의 기관지폐 이형성증을 가진 사례에서 시행되었다. 덱사메타손은 고식적인 치료가 실패하여 기관 발관이 불가능한 호흡 곤란을 가진 24례(9.6%)에서 투여되었다. 투여된 사례 중 14례(58.3%)에서 덱사메타손 구제 치료에 반응을 나타냈다. 비반응군은 보다 많은 산소 공급을 요구하였으며 폐동맥 고혈압이 동반되는 경우가 많았다. 반응군은 입원 기간의 단축과 사망률의 감소 소견을 보였다. 고용량의 덱사메타손을 투여하는 것이 저용량에 비하여 보다 효과적으로 기계 환기에서 이탈을 유도한다는 근거는 도출되지 않았다. 패혈증은 덱사메타손 구제 치료 이후에 가장 빈번하게 발생하는 합병증이다. 결 론: 중증의 기관지폐 이형성증에서 폐동맥 고혈압이 발생하기 전에 덱사메타손 구제 치료를 시행하는 것이 치료 효과 측면에서 도움이 될 수 있다. 폐동맥 고혈압이 동반된 경우에는 합병증의 발생 가능성으로 인해 덱사메타손 구제 치료를 하지 않는 것이 바람직하다.
Infectious bovine keratoconjunctivitis (IBK) is the most common ocular disease in cattle, manifesting as corneal opacity, corneal ulcerations and potentially vision loss. The present report describes a 10-month-old Holstein Friesian heifer with IBK treated by systemic tulathromycin, and subconjunctival injection of penicillin and dexamethasone. We investigated changes in the hematological indices and microorganisms related to IBK after treatment. Neutrophils and monocytes decreased during recovery, so it was assumed that these two types of white cells are associated with IBK. Moraxella bovoculi was cleared in the eye, nasal cavity, and oral cavity after treatment. The distribution of M. bovoculi before treatment indicated that a combined systemic and subconjunctival treatment was necessary. The lesioned eye was found to be overwhelmed by Mycoplasma bovoculi, while pathogen abundance was reduced in the nasal cavity and oral cavities. These results suggest that antibiotic treatment can alter the composition and relative abundance of microorganisms.
Current therapeutic modalities for severe oral lichen planus are generally unsatisfactory. Steroid treatment of lichem planus has been reported in the dermatology literature and dental literature, but few reports mention its efficacy for oral lesions. Some cases of severe erosive oral lichen planus revealed good response to this agent. So the author report two cases of erosive oral lichen planus treated with 0.05%. Dexamethasone methyl cellulose mouth wash and two intralesional injections of Triamcinolone acetonide suspension 0.2 mls, one week apart. 1. Subjective improvement was noticed in 4-5 days by two patients. 2. Objective improvement, however, was delayed and became apparent at several weeks 3. Continued improvement was noted in all patients.
Low-dose radiotherapy has been known to have anti-inflammatory activity and been used for treatment of pneumonia together with anti-serum and sulfanilamide. However, it rapidly discontinued after the development of various antibiotics showing outstanding effect. Recently, it was re-considered to treat COVID-19 which has very limited treatment such as remdesivir and dexamethasone. So, several studies of COVID-19 therapy using low-dose radiation were reported very recently. They showed that low-dose radiation of 0.5~1.5 Gy were useful for decreasing the oxygen consumption and hospitalization period of COVID-19 patients without adverse reaction. Radiopharmaceuticals such as [99mTc]Tc-macroaggregated albumin (MAA) also might be used for low-dose radiotherapy. Administration of vitamin D having anti-inflammatory effect would also be helpful for therapy with synergistic effect.
Objectives: Pharmacopuncture is a treatment that medicinal fluid including herbal extract is injected in body under qi/flavor theory and meridian theories. There are a number of studies investigating the efficacy of pharmacopuncture for cervicogenic dizziness but its usage differs in each study. This study aimed to review previous studies of pharmacopuncture treatment for cervicogenic dizziness to navigate the direction of improvement. Methods: Literature review was conducted on studies aimed at pharmacopuncture for cervicogenic dizziness. The randomized controlled studies which assessed the efficacy of pharmacopuncture on cervicogenic dizziness were selected. The studies were searched in Pubmed, RISS, OASIS and CNKI. After selecting eligible studies, the authors read the articles and summarized the points those are necessary in pharmacopuncture treatment for cervicogenic dizziness. Results: Twenty seven studies and 2,709 participants were included. The diverse solutions were used and the most popular were Salvia miltiorrhiza, Angelica gigas, a compound in Gastrodia elata, Panax notoginseng saponins. Some studies investigated the efficacy of conventional medical compounds (lidocaine, vitamin B, dexamethasone) injected in acupoints. Adopting acupoints and combining with body acupuncture have shown the tendency to enhance the cure rate. Pharmacopuncture could improve the efficacy of conventional treatment for cervicogenic dizziness. Conclusion: As the solution and injection method varies, principles of pharmacopuncture for dizziness should be investigated.
Muscle atrophy is defined as a decrease in muscle mass, cross-sectional area, and myofibrillar protein content. Causes inducing muscle atrophy may be inactivity, denervation, undernutrition and steroid. Inactivity may decrease protein synthesis and increase protein breakdown of skeletal muscle. The muscle atrophy due to inactivity was induced by bed rest, hindlimb suspension, cast, total hip replacement arthroplasty, anterior cruciate ligament reconstruction. Denervated atrophy may be induced by the loss of innervation from lower motor neuron. The atrophy was apparent in the lower limb of hemiplegic patients following ischemic stroke and in the hindlimb of ischemic stroke rats. Protein breakdown of skeletal muscle in the undernourished state results in muscle atrophy. The atrophy due to undernutrition was evident in cancer and leukemia patients and in the undernourished rats. Steroids have been used to treat allergies, inflammatory diseases, autoimmune diseases and to inhibit immune function following transplantation. Steroids may induce muscle atrophy by protein breakdown of skeletal muscle. Muscle Physiology Laboratoryat College of Nursing, Seoul National University proved that dexamethasone may induce hindlimb muscle atrophy in rats and exercise and DHEA may attenuate hindlimb muscle atrophy induced by the steroid in rats. Nurses working with patients undergoing steroid treatment need to be cognizant of steroid induced muscle atrophy. They need to assess whether muscle atrophy is being occurred during and after the steroid treatment. Moreover, they need to apply exercise and DHEA to the patients undergoing steroid treatment in order to attenuate the steroid induced muscle atrophy.
A 3-year-old female Thoroughbred racehorse was presented following the accidental oral and skin administration of amitraz. This case report describes the clinical signs and the treatment of this horse. Clinical signs of amitraz toxicosis are associated with the stimulation of alpha2-adrenergic receptors. Amitraz is seldom fatal because the effects can be reversed by alpha2-adrenergic antagonists. The horse displayed typical clinical signs of colic, including pawing, small hard drops, tranquillisation, depression, ataxia, muscular incoordination and impaction colic lasting up to 7 days. The syndrome was accompanied by mild dehydration. The horse survived after persistent symptomatic treatment, including the giving of intravenous fluids, antibiotics, multiple doses of mineral oil per os, nonsteroidal anti-inflammatory drugs and dexamethasone intramuscularly and intravenously.
