일본 연하장애 어린이의 치과적 접근 (DENTAL APPROACHES OF CHILDREN WITH DYSPHAGIA IN JAPAN)

  • I participated in Academic Exchange Program(Action plan II) between KADH(Korean Association for Disability and Oral Health) and JSDH(Japanses Society for Disability and Oral Health) for 2 months from 3rd July 2012 to 2nd september 2012 in the Department of Hygiene and Oral Health, School of Dentistry, Showa University at Tokyo, Japan. I have observed their operation process and learned what dysphagia is and how it is consulted and taken care of as a therapy for patients with eating and swallowing disorders for two months in The department of special needs dentistry at Showa University Dental Hospital, Jonan Branch of Tokyo Metropolitan Kita Medical Rehabilitation Center for the Disabled, Smile Nakano Center, Tokyo metropolitan center for persons with disabilities in Lidabashi for one week, Eating and swallowing functional therapy workshop for disabled children, Tokyo metropolitan Tobu medical center for Persons with Developmental/Multiple Disabilities located in Minamisunamitchi for one week and on The 17-18th JSDR(Japanese Society of Dysphagia rehabilitation) in Sapporo. Through Action Plan II program, I learned how precious eating, drinking and swallowing with ease are and observed how they do and what they do as a dentist or a dental hygienist in Japan for dysphagia patients. Therefore, I want to present the dental approaches of children with dysphagia in Japan, based on my experience for two months.

  • The main purpose of this paper is to analyze the modes of therapeutic intervention. The emphasis is on the neurophysiological perpective arising out of neurological principles and developmental concepts. The obtained results are as follows. 1. The important hypostheses predicted that the group intervened by neurodevelopmental approach would improve motor function better than the group done by traditional approach and it was proved that neurodevelopmental approach was more effective in gross motor region(P<.01) 2. In the comparison of type of involovement, neurodevelopmental intervention group in spastic type showed improvements in the region of gross motor.(P<.001) 3. In the comparision of degree of disorder, neurodevelopmental intervention group showed improvement of motor function in all the gross motor region in the mild, moderate and severe case.(P<.001) 4. In the comparison of ages of intervention beginning, the group of child between 25-36, 49-60 and 61-72 months(P<.001) intervened by neurodevelopmental approach showed improvements of motor function. 5. In the comparison of intervention duration, neurodevelopmental intervention group showed improvements of motor function in gross motor region according to intervention durstion(P<.001)

20대 남녀별 구르기 형태의 분석 (Description of Rolling Movement Between the Gender in the Twenties)

  • The purpose of this study were 1) to describe the rolling movements of the twenties, 2) to identify developmental sequences of three body regions and 3) to evaluate the influence gender might have on the movement patterns used for rolling. Fifty males(mean 23.2 years of age) and fifty females(mean 21.1 years of age) performed the 10 trials of rolling from a supine to a prone position while being videotaped. Individual videotaped trials were classified using the described categories for upper extremity, lower extremity and head-trunk component. The most common combination of movement patterns described. The results of this study were as follows : 1. Only $16\%$ of males and $12\%$ of females demonstrated a same combination of movements during rolling. 2. Gender differences were found in the incidence of movement patterns of each body region. 3. This study determined if head-trunk anion might develop in advance of limb action. 4. This study determined if upper extremity action might develop in advance of lower extremity action. The variability of adults' rolling movement provides physical therapists with numeous movement combinations that might be used when teaching patterns to rolling.

측두하악 관절 장애의 평가 (Clinical Assessment of Temporomandibular Joint Dysfunction)

  • The Temporomandibural joint(TMJ) is one of the most frequently used joint in the body as $1,500{\sim}2,000$ times per day for the activities of chewing, swallowing, talking, yawing and sneezing. The TMJ are formed by condylar process of mandible and mandible fossa of temporal bone, separated by an articular disc. This articular disc divides into two cavities as upper cavity and lower cavity. The gliding movement occurs in the upper cavity of the joint, whereas hinge movement occurs in the lower cavity. The movements that are allowed at the TMJ are opening, closing, protrusion, retraction and lateral movement. A cause of TMJ dysfunction are capsulitis, internal derangement, osteoarthritis, rheumatoid arthritis, infection and inflammation near the joint, trauma on joint, ankylosis, subluxation or dislocation of joint, injury of articular disc, myositis, muscle contracture or spasm, myofascial pain dysfunction syndrome, dyskinesia of masticatory muscles, developmental abnormality, tumor, connective tissue disease, fibrosis, malocclusion, swallowing abnormality, wrong habits such as bite nail or hair, bruxism, psycological stress and Costen syndrome etc. Assessment of TMJ dysfunction consist of interview, observation, functional examination, palpation, reflex test, joint play test, electromyography and radiologic examination and behavioral and psycological assessment etc.

  • Orthopaedic manual therapy(OMT) includes knowledge in arthokinematic, biological, behavioral, biomechanical, patholology, anatomy, and clinical science. it allows proper approach to patient evaluation and program development and relieves joint and soft tissue lesions. The OMT has sports injurys, traumatic, developmental, and neuromusculoskeletal disorders of acute and chronic states, and prevention of dysfunction as well as the development, improvement, restoration, or maintenance of joint and muscular mobility, stability, relaxation, strenth, endurance, balance, and functional skills. The purpose of this article has to define and clarify the change and development of basic concepts of orthopaedic manual therapy. OMT is ancient in origin. The book of ' Manipulation Past and Present ' contains pictures recording various types of joint mobilization by Hippocrates. Recently, there are many kinds of manual therapeutic maneuvers to assess and relieve joint and soft tissue lesions developed by James Cyriax, John McM Mennell, Brian R Mullign, Robin McKenzie, Freddy Kaltenborn, Geoffrey Maitland, and Ola Grimsby.

Effects of Knowledge of Result Versus Knowledge of Performance on Treadmill Training on gait ability in Stroke Patients: A Randomized Clinical Trial

  • Purpose: This study compared the walking ability of chronic stroke patients following either treadmill training with knowledge of the result (KR group) or treadmill training with knowledge of the performance (KP group). Methods: Nineteen patients with chronic stroke were recruited from a rehabilitation hospital. The patients were divided into two groups: a KR group (10 patients) and a KP group (9 patients). They received 30 minutes of neuro-developmental therapy and treadmill training 30 minutes, five times a week for three weeks. The gait parameters were measured before and after training using the Optogait system. Results: After the training periods, the KR group showed significant improvement in gait speed, cadence, step length of the unaffected limb, stance time of the affected limb, and functional gait assessment compared to the KP group (p<0.05). Conclusion: The results showed that treadmill training with KR was more effective in improving the gait speed and cadence, step length of the unaffected limb, stance time of the affected limb, and functional gait ability than the treadmill training with KP. Therefore, to improve the walking ability of stroke patients, it is necessary to consider treadmill training with KR. If it can be combined with conventional neurological physiotherapy, it would be an effective rehabilitation for stroke patients.

미국과 한국의 AAC 서비스 전달체계 비교 분석 (A Comparative Analysis on the AAC Service Delivery System between the U.S. and Korea)

  • 본 논문에서는 국내의 효과적인 AAC 서비스 전달체계 수립방안 논의를 위해 미국과 한국의 AAC 서비스 전달체계 현황을 비교 분석하였다. 비교 분석을 통해 본문에서 제안한 'AAC 서비스 전달체계 모델'은 현재 정부나 각 기관에서 운영하고 있는 AAC 서비스 전달체계 관련 공적급여의 방식을 최대한 유지하면서 AAC 지원체계의 사각지대가 발생하지 않도록 현재의 방식에서 부족한 부분을 추가 보완하는 형태로 구성하였다. 제언 및 시사점으로는 현재 공적급여에서 제한적으로 지원하고 있는 AAC 보조기기 품목을 대폭 확대하고 전국에 설치되어 있는 보조기기센터, 특수교육 지원센터, 발달장애인 지원센터, 언어치료실 등을 활용한 인프라 구축을 통해 사각지대가 없는 보편적인 AAC 서비스의 제공이 필요하며. 관련 협회 및 학회 차원에서 AAC 서비스를 제공할 수 있는 전문가 연수 및 보수교육과 같은 인력 관리에도 많은 노력이 필요하다.

A phenomenological study on the meaning of presence of elderly participants in an exercise-music program

  • Objective: The reason why it is important to look at the benefits of exercise-music experience perceived by the elderly is that the satisfaction of life in old age is a meaningful reflection of the subjective perception of the life of the elderly. This is because it must be understood as the center. Design: A qualitative study. Methods: Participants are senior citizens attending senior citizens' college located in Eumseong city. Those who have taken an exercise-music song program for at least one semester served as the participants. Individual interviews were conducted by selecting 10 people through purposeful sampling. The collected data were analyzed by applying Colaizzi's method among phenomenological methods. The exercise-music program was conducted for 12 weeks, for a total of 50 minutes for each time (10 minutes for introduction, 35 minutes for music, 5 minutes for finishing). Results: As a result of the study, the experience of the exercise-music program was derived into 6 categories and 13 subjects. These 6 main categories are 'Changes from the developmental crisis', 'Freedom from the confines of others', 'Positive emotions found through songs', 'Feeling of peer and fellowship relationships', 'Changes in self-awareness', It appeared as a 'Change in the way of life'. Conclusions: In conclusion, the experience of the exercise-music program brought about a positive changes in life of doing one's best by improving confidence and confirming presence. Therefore, this study suggests activation of senior college programs to find a sense of presence for each level by using exercise-music programs with good accessibility.

Effects of Walking Training according to Rhythmic Auditory Stimulation Speed Control Balance of Stroke Patients

  • Purpose: In this study, based on the error augmentation, we performed walking training with increased rhythmic auditory stimulation speed on the affected side (IRAS) and walking training with decreased rhythmic auditory stimulation speed on the unaffected side (DRAS). The purpose of this study was to verify whether motor learning was effective in improving balance ability. Methods: Twenty-eight subjects with chronic stroke were recruited from a rehabilitation center. The subjects were divided into three groups: an IRAS group (10 subjects), a DRAS group (9 subjects), and control group (9 subjects). They received 30minutes of neuro-developmental therapy and walking training for 30minutes, five times a week for three weeks. Static and functional balance ability were measured before and after the training period. Static balance was measured by balancia software. Functional balance was measured by the timed up and go test (TUG) and the berg balance scale (BBS). Results: After the training periods, the IRAS group showed a significant improvement in TUG, BBS, area 95% COP, and weight distribution on the affected side when compared to both the DRAS group and control group (p<0.05). Conclusion: Based on the results of this study, it is possible to consider error augmentation methods of motor learning if rhythmic auditory stimulation is applied to stroke patients in clinical practice. If the affected side is shorter than the unaffected side, the affected side should be adjusted to the increased rhythmic auditory stimulation speed, which is considered to be an effective intervention to improve balance ability.

감각통합치료가 발달장애 아동의 기능적 쥐기에 미치는 영향 (The Effects of Sensory Integration on Functional Grasp in Developmental Disabled Child)

  • Objective : The experiment consists of measuring functional grasp ability of a child suffering from sensory defense disability of the hands as the child is treated with sensory integration therapy. Methods : We treated two-year-old child with developmental disability to set up the reversal design(ABAB design). We used treatment equipments, free activities, and toys for sensory integration therapy. Results : A child's functional grasp ability did not show anything before the sensory integration therapy, however, after the sensory integration was applied during the first treatment(Treatment 1), the five minutes before the treatment, it averaged 1.87 times(ranges=0 to 5 times), the five minutes after the treatment, it averaged 1.25 times(ranges=0 to 8 times) showing an increased. After ending the sensory integration therapy, the functional grasp ability averaged 2.5 times before the therapy and 4 times after the therapy, which showed a totally different tendency, when we reapplied the treatment during the second treatment(Treatment 2), it averaged 2.4 times(ranges=1 to 4 times) before the treatment, and it averaged 5.3 times(ranges=4 to 6 times) after the treatment, which showed a big improvement. Conclusion : The sensory integration therapy brought an increase in functional grasp ability along with improvement in daily life functions of the child. For this result, there should be more research to see more effects and results to help those in need.
