수 종의 생분해성 국소약물 송달제제가 치주염 치료에 미치는 효과 (Effects of Several Biodegradable Controlled-Release Local Delivery Drugs on the Treatment of Periodontitis)

  • The present study was performed to evaluate the clinical effects following local application of 30% minocycline strip(polycaprolactone), 2% minocycline gel(hydrocarbon gel) and 12% minocycline strip(polylactide, Minodent) to augment scaling and root planing in patients with chronic adult periodontitis. Forty teeth with periodontitis were enrolled in the study anddistributed into 4 groups including control group. All patients performed standardized oral hygiene instructions and mechanical debridement at the beginning of the study and then each local delivery drugs were inserted into periodontal pocket in each groups. Examinations regarding plaque index(PI), papillary bleeding index (PBI), probing pocket depth (PPD) were carried out at 0, 2, 4 weeks. All experimental groups showed statistically significant differences between baseline and 2 and 4 weeks in every clinical indices. Especially, 30%minocycline strip and Minodent group showed a significant improvement in PBI at 2 weeks and in PPD at 2 and 4 weeks. In conclusion, highly bio-resorbable Minodent delivered subgingivally as an adjunct to scaling and root planing induces better clinical effects for periodontal health than 2% minocycline gel and control group.

Effects of adjunctive daily phototherapy on chronic periodontitis: a randomized single-blind controlled trial

  • Purpose: The purpose of this randomized single-blind controlled trial was to elucidate the clinical and antimicrobial effects of daily phototherapy (PT) as an adjunct to scaling and root planing (SRP) in patients with chronic periodontitis. Methods: The study was conducted from December 2013 to May 2014 at Ewha Womans University Mokdong Hospital, Seoul, Korea. Forty-one patients with mild to moderate chronic periodontitis were randomly divided into two therapeutic groups in a 1:1 ratio: SRP+PT and SRP (control) groups. All participants underwent full-mouth SRP. PT was performed thrice a day for a month by using electric toothbrushes with embedded light-emitting diodes. Plaque index, gingival index, probing pocket depth (PPD), clinical attachment level (CAL), and bleeding on probing were assessed before (baseline) and four weeks after (follow-up) the treatment. Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Prevotella intermedia, Fusobacterium nucleatum, Parvimonas micra, Campylobacter rectus, Eikenella corrodens, Streptococcus mutans, and Streptococcus sobrinus levels were detected by a real-time polymerase chain reaction at the same points in time. Results: The clinical parameters improved in both the groups. At the follow-up assessment, PPD was significantly decreased in the SRP+PT group (P=0.00). Further, PPD and CAL showed significantly greater changes in the SRP+PT group than in the SRP group (PPD, P=0.03; CAL, P=0.04). P. gingivalis and T. forsythia levels decreased in this group, but no significant intergroup differences were noted. Conclusions: Adjunctive PT seems to have clinical benefits, but evidence of its antimicrobial effects is not sufficient. Long-term studies are necessary to develop the most effective PT protocol and compare the effectiveness of PT with and without exogenous photosensitizers.

Antioxidant profile of whole saliva after scaling and root planing in periodontal disease

  • Purpose: This study compared the total antioxidant status (TAS) and superoxide dismutase (SOD) activity in the saliva of periodontally compromised patients before and after scaling and root planing (SRP) to assess their diagnostic utility. Methods: Severe chronic periodontitis patient (test group) and subjects with no attachment loss, sites showing a 3 mm or more probing depth and a sulcus bleeding index < 10% (control group) were enrolled in this study. Saliva sampling and clinical examination were performed at one week, one month and 3 months after SRP. The TAS and SOD activity in each patient's saliva was measured for the comparative analysis between the groups. Results: In the test group, the TAS decreased directly after SRP. With time, it increased slightly and was relatively unchanged compared to the baseline. In the control group, the TAS also decreased immediately after SRP but increased gradually with time until 3 months. The SOD activity in the test and control subjects decreased immediately after SRP until 1 month. At 3 months, the SOD activity had increased. Both groups had a similar profile of SOD activity. However, the SOD activity of the control group was significantly higher than that of the test group at each point in time (P < 0.05). Conclusions: There was a significant difference in the total salivary antioxidant level between the periodontitis and healthy or gingivitis (control) group during the experiment period. The total antioxidant level in the saliva was higher in the patients with severe chronic periodontitis than the healthy or gingivitis control before SRP. The SOD activity of the periodontitis patients was lower than the control at each time point. These findings conclusively reveal the possible use of saliva as a diagnostic tool for periodontal health.

Effect of scaling and root planing combined with systemic doxycycline therapy on glycemic control in diabetes mellitus subjects with chronic generalized periodontitis: a clinical study

  • Purpose: The purpose of study was to compare glycemic control using glycated hemoglobin levels ($HbA_{1c}$) in diabetic patients with chronic generalized periodontitis (CGP) undergoing scaling and root planing (SRP) with and without systemic doxycycline. Methods: Fifty subjects with type 2 diabetes mellitus ($T_2DM$) and CGP receiving antidiabetic therapy were selected for study. The selected subjects were randomly assigned to two groups (test group [TG] and control group [CG]) comprising 25 patients each. The TG received SRP followed by systemic doxycycline. The CG received treatment with SRP only. The periodontal parameters were recorded at baseline (day zero), and every 1 month for 4 months and included probing depth, clinical attachment level, plaque index, gingival index, and $HbA_{1c}$ level were recorded at baseline (day zero) and at the end of 4 months. Results: A statistically significant effect was demonstrated for the periodontal parameters for both the TG and CG. $HbA_{1c}$ values did not show a statistically significant difference in the treatment group as compared to the CG. Conclusions: The authors concluded that nonsurgical periodontal therapy improved glycemic control in patients with $T_2DM$ in both groups, but no statistical difference was observed with adjunctive systemic doxycycline therapy. A further study with a larger sample size is required.

Slip-Weakening 모델의 확장과 단층 파열속도의 추정 (Extended Slip-Weakening Model and Inference of Rupture Velocity)

  • The slip-weakening model developed by Ohnaka and Yamashita is extended over the breakdown zone by equating the scaling relationships for the breakdown zone and the whole rupture area. For the extension, the study uses the relationship between rupture velocity and radiation efficiency, which was derived in the theory of linear elastic fracture mechanics, and the definition of fmax given in the specific barrier model proposed by Papageorgiou and Aki. The results clearly show that the extended scaling relationship is governed by the ratio of rupture velocity to S wave velocity, and the velocity ratio can be determined by the ratio of characteristic frequencies of a Fourier amplitude spectrum, which are corner frequency, fc, and source-controlled cut-off frequency, fmax, or vice versa. The derived relationship is tested by using the characteristic frequencies extracted from previous studies of more than 130 shallow crustal events (focal depth less than 25 km, MW 3.0~7.5) that occurred in Japan. Under the assumption of a dynamic similarity, the rupture velocity estimated from fmax/fc and the modified integral timescale give quite similar scale-dependence of the rupture area to that given by Kanamori and Anderson. Also, the results for large earthquakes show good agreement to the values from a kinematic inversion in previous studies. The test results also indicate the unavailability of the spectral self-similarity proposed by Aki because of the scale-dependent rupture velocity and the rupture velocity-dependent fmax/fc; however, the results do support the local similarity asserted by Ohnaka. It is also remarkable that the relationship between the rupture velocity and fmax/fc is quite similar to Kolmogorov's hypothesis on a similarity in the theory of isotropic turbulence.

통계적 특징 기반 인공신경망을 이용한 온라인 서명인식 (On-line Signature Recognition Using Statistical Feature Based Artificial Neural Network)

  • 본 논문에서는 키넥트(Kinect)를 통해 얻은 깊이 영상에서 찾아낸 손가락의 끝점으로 임의의 3차원 공간인 공중에 그린 서명을 인식하는 알고리즘을 제안한다. 3차원 공간상에서 서명 궤적의 시프팅(Shifting), 스케일링(Scaling) 변화에 대응하기 위해 X, Y, Z좌표에 관한 각각 10개의 통계적 특징을 사용하였다. 인공신경망(Artificial Neural Network)은 기계학습 중 하나이며, 패턴인식 분야의 복잡한 분류 문제를 해결할 수 있는 도구로 사용되고 있다. 제안한 알고리즘을 실제 온라인 서명인식 시스템을 구현하여 적용하였고, 앞서 추출한 통계적 특징을 인공신경망의 입력값으로 사용하여 학습 과정을 거친 후 4가지 서명을 분류하는 것을 확인하였다.

Applying nano-HA in addition to scaling and root planing increases clinical attachment gain

  • Purpose: This study evaluated the efficacy of treating periodontitis using subgingival nano-hydroxyapatite powder with an air abrasion device (NHAPA) combined with scaling and root planing (SRP). Methods: A total of 28 patients with stage III periodontitis (grade B) were included in this study, although 1 was lost during follow-up and 3 used antibiotics. The patients were divided into a test group and a control group. All patients first received whole-mouth SRP using hand instruments, and a split-mouth approach was used for the second treatment. In the test group, the teeth were treated with NHAPA for 15 seconds at 70% power per pocket. Subgingival plaque samples were obtained from the 2 deepest pockets at the test and control sites before treatment (baseline) and 3 months after treatment. The full-mouth plaque index (PI), gingival index (GI), papillary bleeding index (PBI), bleeding on probing (BOP), probing depth (PD) and clinical attachment level (CAL) were recorded at baseline and at 1- and 3-month post-treatment. Real-time polymerase chain reaction was used to determine the colonisation of Treponema denticola (Td), Porphyromonas gingivalis (Pg), and Aggregatibacter actinomycetemcomitans in the subgingival plaque. Results: From baseline to the first month, the test group showed significantly larger changes in BOP and CAL (43.705%±27.495% and 1.160±0.747 mm, respectively) than the control group (36.311%±27.599% and 0.947±0.635 mm, respectively). Periodontal parameters had improved in both groups at 3 months. The reductions of PI, GI, BOP, PD, and CAL in the test group at 3 months were greater and statistically significant. The total bacterial count and Td and Pg species had decreased significantly by the third month in both groups (P<0.05). Conclusions: Applying NHAPA in addition to SRP improves clinical periodontal parameters more than SRP alone. Subgingival NHAPA may encourage clot adhesion to tooth surfaces by increasing surface wettability.

보성강 유역에 위치한 호수에서의 식물플랑크톤의 시공간적 군집 비교 분석 (Comparison of Spatio-temporal Variations of Phytoplankton Communities in Lakes in the Boseong River Basin)

  • 본 연구는 보성강 유역에 위치한 호수들에서 식물플랑크톤 군집의 시공간적 차이를 비교하고 이러한 군집 구조의 차이에 영향을 미치는 요인들을 확인하고자 2014년 3월부터 2017년 11월까지 분기별 조사를 시행하였다. 조사 결과, 보성호는 다른 호수들에 비해 식물플랑크톤의 다양성이 풍부하고 밀도 역시 높은 수준으로 확인되어 시공간적으로 큰 차이를 보였다. 또한 3월 조사에서 확인된 식물플랑크톤 군집은 다른 조사시기와 우점종을 포함한 규조류와 녹조류의 군집에서 구분되는 특성을 나타냈다. Indicator species analysis를 통해 확인된 각 그룹 간 지표종을 확인한 결과, 계절에 따른 식물플랑크톤 천이와 영양 농도에 따른 다양성 및 밀도 변화를 반영한 것으로 산출되어 호수들의 식물플랑크톤 군집이 시공간적으로 구분되었음을 나타냈다. Non-metric multidimensional scaling을 통해 보성강 유역 내 호수들의 식물플랑크톤 군집 구조에 있어 Secchi depth, 수온, 전기전도도, DO 등이 중요한 요인임을 확인하였고, 영양염을 포함한 다양한 환경요인과 함께 식물플랑크톤 군집 차이에 영향을 끼친 것으로 분석되었다.

상전이가 섭입 슬랩의 좌굴에 미치는 영향과 지체구조적 암시 (Effect of phase transformations on buckling behavior of subducting slab and tectonic implication)

  • 하부 맨틀의 상부에서 관찰되는 섭입된 해양판의 겉보기 두꺼워짐은 과거 연구를 통해 슬랩 좌굴에 의한 것으로 제안되었다. 그러나, 맨틀의 상전이가 슬랩 좌굴에 미치는 영향을 정량적으로 평가하고 이를 규모 법칙으로 검증한 연구는 거의 이루어지지 못하였다. 이 연구에서는 상전이를 고려한 2차원 컴퓨터 섭입 모델링을 수행하여 상전이가 슬랩 좌굴에 미치는 영향에 대해 정량적으로 평가하고 규모 법칙으로 검증하였다. 실험 결과는 410 km 깊이에서 발생하는 감람석-와드슬레이아이트 상전이가 슬랩 좌굴의 발달에 중요한 영향을 미친다는 것을 보였다. 흡열 상전이는 상부 맨틀에서 섭입 슬랩의 침강을 가속시켜 660 km 깊이에 존재하는 불연속면에 빠르게 도달하게 한다. 그러나 660 km 깊이에 존재하는 링우다이트-페로브스카이트+마그네시오우스타이트 상전이는 슬랩 좌굴의 발달에 상대적으로 작은 영향을 미치는데 그 상전이가 섭입 슬랩의 하부 맨틀 침강을 지연시켜 전이대에 섭입한 슬랩을 누적시키기 때문이다. 그럼에도 불구하고 슬랩 좌굴은 규모 법칙을 20% 이내의 오차에서 잘 만족한다. 이처럼 슬랩 좌굴은 맨틀에서 발생하는 보편적인 현상으로써 자바-순다 및 동북 일본 섭입대에서 관찰되는 하부 맨틀의 상부와 전이대에서의 슬랩 좌굴을 잘 설명한다. 또한 백악기 시기 경상 분지가 겪은 주기적인 압축 및 인장이 슬랩 좌굴에 의한 가능성을 암시한다.

확장된 slip-weakening 모델의 응력 강하량과 에너지 수지 특성 및 스케일링 관계 (Characteristics of Stress Drop and Energy Budget from Extended Slip-Weakening Model and Scaling Relationships)

  • The extended slip-weakening model was investigated by using a compiled set of source-spectrum-related parameters, i.e. seismic moment Mo, S-wave velocity Vs, corner-frequency fc, and source-controlled high-cut frequency fmax, for 113 shallow crustal earthquakes (focal depth less than 25 km, MW 3.0~7.5) that occurred in Japan from 1987 to 2016. The investigation was focused on the characteristics of stress drop, radiation energy-to-seismic moment ratio, radiation efficiency, and fracture energy release rate, Gc. The scaling relationships of those source parameters were also investigated and compared with those in previous studies, which were based on generally used singular models with the dimensionless numbers corresponding to fc given by Brune and Madariaga. The results showed that the stress drop from the singular model with Madariaga's dimensionless number was equivalent to the breakdown stress drop, as well as Brune's effective stress, rather than to static stress drop as has been usually assumed. The scale dependence of stress drop showed a different tendency in accordance with the size category of the earthquakes, which may be divided into small-moderate earthquakes and moderate-large earthquakes by comparing to Mo = 1017~1018 Nm. The scale dependence was quite similar to that shown by Kanamori and Rivera. The scale dependence was not because of a poor dynamic range of recorded signals or missing data as asserted by Ide and Beroza, but rather it was because of the scale dependent Vr-induced local similarity of spectrum as shown in a previous study by the authors. The energy release rate Gc with respect to breakdown distance Dc from the extended slip-weakening model coincided with that given by Ellsworth and Beroza in a study on the rupture nucleation phase; and the empirical relationship given by Abercrombie and Rice can represent the results from the extended slip-weakening model, the results from laboratory stick-slip experiments by Ohnaka, and the results given by Ellsworth and Beroza simultaneously. Also the energy flux into the breakdown zone was well correlated with the breakdown stress drop, ${\tilde{e}}$ and peak slip velocity of the fault faces. Consequently, the investigation results indicate the appropriateness of the extended slip-weakening model.
