  • The aim of this study, in order to reduce the support burden of dementia primary caregiver, within the framework of long-term care insurance system to present its founding model for the development of new services. Previous research as analytical materials and based on the CARE study between 2012 to 2013. Primary caregiver burden of patient are both physical, psychological and economic aspects as well as difficulties in many aspects and had influence on many factors such as age, care burden, economic, health, and social support. It also came high-risk primary caregiver suffer from depression. This primary caregiver to take advantage of the elderly long-term care insurance system implemented at the time of the institutional support necessary for since 2008, presented additional services in this study. Of increasing the small sizes day care center (tentative name) to open an as established in the settlements at a scale of less than nine, within 5 minutes from the residence, limited dementia by expertise in the disease home care services scale model of the current system service It was to improve the quality of existing shares, small group sizes for day care centers and community life apart.

The Music Therapy Effects to Older Person's Emotions after Reminiscence Therapy Through Songs (노래를 중심으로 한 음악치료 회상 요법이 노인의 정서에 미치는 영향)

  • The purpose of this study is to how the music therapy effects to older person's self-esteem feeling and a emotional mood improvement after reminiscence therapy through songs. The object of this research were 10 old men and women, age of 67 to 90 with dementia, and mental and physical dysfunction, but can be self-walking and the stool was made up in social welfare center in the Yuseong-gu, Daejeon-city, The experiment of the research was done from June 18th until July 18th, week 2 each 50 minutes the total 12 times. This study was an experiment, the treatment of a same group before and after treatment. Before the first session I check the self-esteem and emotional mood. And after the final 12th session, post-therapy examination was conducted the same tests. The tools of Self-esteem test is the self-esteem scale developed by Rosenberg(1965) and Jun Byung-se(1974), which was translated. Emotional state check tools developed by MoNair(1992), which was translated by Yun Jae-ryang, and it can also apply to the Korean older person has been modified by Shin yon-hee. The results of changing mood state to 3 factors are as follows: The score of anxiety-depression factor changes from 50.42($\pm$14.15) to 35.10($\pm$10.60), and this is statistically significant.(p<.05). The score of anger factor changes from 11.20($\pm$4.39) to 6.90($\pm$2.51), and this is statistically significant.(p<.01). The score of vitality factor Changes from 30.00($\pm$4.27) to 23.90($\pm$5.09), and this is Statistically significant.(p<.05). For this research, I selected older person's favorite songs which have a positive lyrics and bright melody. And This has significance in terms give positive effects to older person's emotion which influence on their self-esteem and better mood positively. In the clinical treatment for the elderly to improve the positive sentiment and give practical assistance on the basis of this research is done to the future, And I look forward to the future development of a variety of music therapy.

A Study on the Fee-Based Model Development of Day Care Centers for the Elderly (유료 노인 낮보호 시설 모형개발에 관한 연구)

  • The aim of this study is the development of a fee - based model day care center for the elderly by inquiring into the current condition of facilities in America and in Korea, and in surveying the opinion of domestic elderly about day care facilities. A field trip to U.S. day care services was held between July 5 and July 15 in 1997, and an on-the-spot study for domestic facilities took place during March in 1998. Our research reveals that the overall supply of day care facilities can not meet future demand in terms of quality and quantity. Therefore a model must be created for day care centers of a that consists of a director from a professional group. an adequate environment, and a standardized in order to offer a qualified public health service linked to the home and community in Korea. The director of a day care center is a critical variable in determining the quality of service. Professional skills related to the needs of the elderly and the person's quality of service should be considered in appointing director for the center. This study belleves that a professional nurse should be the director of a day care center. The operating environment of a day care facility should be made up of considerable space comparable to the number of residents, should be in a comfortable and safe location, and should have equipment that provides a qualified, safe service to the elderly. Our model is designed for 20 persons and allocates 4 Peng per person. This model is comprised of a reading room. a craft room, a health room, a room for physical therapy, a dining room, a staff office, and a multi -purpose room connected to other rooms. Day care service should be a comprehensive service program meeting the multidimensional needs of the elderly. A comprehensive service program needs a team of various professionals made up of the elderly family, participants, nurses, social workers, physical therapists, nutritionists, and medical doctors. The program will also include health care service, physical therapy, speech therapy. diet, occupational therapy, transportation service, health and an education program, etc. In conclusion, a model of a day care center is developed with the following components: a professional director and an environment and program, that considers the physical, mental, and social characteristics of the elderly. A model should also motivate self-reliance self-fulfillment in the elderly in order to fulfill their health needs and to prevent isolation from society and mental depression. Furthermore, This facility will be a beneficial factor in reducing a family's burden on caring for the elderly that includes unnecessary hospital expenses. The following is a suggestion based on results this study: A service program should be developed to fit the conditions of the elderly in Korea by specifically analyzing the needs of the elderly.

Debris removal efficiency depend on different ultrasonic irrigation protocols (초음파 세정 프로토콜에 따른 근관 내 잔사 제거의 효율성 비교)

  • Purpose: The purpose of this study is comparing the efficiency of debris removal of passive ultrasonic irrigation depend on different protocols. Materials and Methods: Forty-eight mandibular premolars were randomly divided into 2 groups depend on vibration power (Weak and Strong, n = 24). And then two groups were subdivided into 3 groups depend on the number of times that PUI used (1, 2 and 3, n = 8). After standardization, teeth were split into two halves. On the wall of one half of root canal, three depressions were cut at 1, 3 and 5 mm from the apex and in the same way two depressions were cut on the other half of root canal at 2, 4 mm from the apex. Each depression was filled with mixture of dentine and NaOCl. After irrigation, images of the root canal wall were taken, and then the amount of remaining dentine debris was evaluated. Results: There was no significant difference between weak and strong power of vibration groups. Weak vibration groups did not show significant difference depend on the different number of times PUI used. But in the strong vibration groups, twice and three times used groups show better removal efficiency. Conclusion: The removal efficiency of dentine debris is not significantly different depend on the power of vibration. And multiple use of PUI could have better irrigating effects at the apical third area in the strong vibration group.

A Study on the Health as Perceived by the Aged (노인들이 지각하고 있는 건강수준에 관한 연구)

  • The purpose of this study was to develop a nursing model for the aged. This study was conducted to measure the physical, psychological and mental health status of the aged and to identify relationships between these health levels and various variables. The data were collected from 172 aged(over 60 years old) by interviews. The tools used for this study was a structured questionnaire which was developed and revised by the researchers. The tool consisted of 22 items on physical health assessment, 7 items on psychological health assessment, 9 items on mental health assessment and 10 items on an ADL evaluation. The major results of the study were as follows ; 1. In physical health, the aged complained of visual disturbance(60.5%), incontinence of urine (55.2%), back or muscle pain(73.3%), dizziness(70.3%) and diarrhea or constipation(44.2%). In psychological health, most aged people felt anger when they got some order from an other person(80.2%). Also, they had depression (69.8%) and felt like dying(64.0%). The ADL level was mostly normal for daily life. 2. The aged man was more healthy than the aged woman in physical and mental health especially the 60-64 year old group. The physical, mental health and ADL level had a positive correlation with age(p>.004), but psychological health had a negative correlation with age. 3. The physical, psychological, mental health and ADL level was positively interrelated with each other. 4. The physical, mental health, & ADL level lowered with inceasing age. Physical & psychological health worsened rapidly from 65-69 years, but for 10-15 years, their health level was preserved. Psychological health level was high in 80-84. 5. The physical health was affected by edu cational level, sex and pocket money (R=.4029, 16.24%). The psychological health was affected by the supportive style and pocket money (R=.5128, 26.30%). And the mental health was affected by education level, age, support ive style, sex and job(R=.4377, 19.16%). As seen above, we suggest the intervention of the young for the old to cope with their life and to maintain their healthy late adulthood. Also, if they have received psychological support in the institution, they will maintain healthy life condition. For further studies should be a search for variables that affect aged health, and should contribut to a nursing program better suited for the aged.

Health-related quality of life in elderly patients with diabetes mellitus according to age: based on Korea National Health and Nutrition Examination Survey (노인 당뇨병 인지자의 연령에 따른 삶의 질 수준 분석: 국민건강영양조사를 이용하여)

  • Purpose: Although the importance of life satisfaction in patients with chronic diseases has been emphasized, limited studies have been performed on the health-related quality of life (HRQoL) among elderly patients with chronic diseases. This study examined the HRQoL among Korean patients with diabetes aged 65 years or older. Methods: This study used data from the Korea National Health and Nutrition Examination Survey 2008-2016. The analysis included data of 2,353 elderly patients with diabetes aged 65 years or older who were aware that they had type 2 diabetes. Each participant was categorized into one of three age groups (65-69 years, 70-74 years, and ≥ 75 years). The EuroQol-5 Dimensions (EQ-5D) scale was computed to estimate the HRQoL. Multivariate linear regression analysis was used to compare the demographic characteristics, lifestyle characteristics, nutrient intake, and EQ-5D scores between the age groups. Results: The intake levels of most nutrients were similar in the three age groups, but the total energy intake levels were significantly lower, and the sodium intake levels were significantly higher in the older age groups (p < 0.001). In addition, all items of the EQ-5D index, except for anxiety/depression, tended to decrease with age. In particular, the oldest age group (≥ 75 years) showed significantly lower levels on most items and the EQ-5D index than those in the other age groups (p < 0.001). Conclusion: Elderly patients with type 2 diabetes may have low levels of life satisfaction, which worsens with increasing age. Therefore, it is important to develop and manage personalized guidelines and medical systems so that elderly patients have a higher quality of life for the remainder of their lives. The results of this study are expected to be used as basic data for the systematic management of diabetes in elderly Korean patients.

  • Enterprise which is rapidly growing in Korea is hesitating because of economic depression lately. Large enterprise have been successfully growing with Korea economic growth for fifty years. But many of them have already gone bankrupt and face the crisis of management, many people look away from them. Large enterprise come to face the time which they strongly need the improvement of their image against the devalation of their one. It is more important to use external periodicals than excess PR advertisement for the improvement of the image. External periodicals is aimed at readers who is outside of company and company PR advertisement, the purpose of issuing external periodicals is to improve the image of enterprise by arranging organization, draw attention from people (PR) and produce profit by acceleration marketing. People is much interested in external periodicals(free) under the situation of IMF because they can easily get information from that. So the role of external periodicals become more important for enterprise PR. This paper shows the importance of the edit design in external periodicals. We analysed the edit design (five group) per composition factor, suggest the improvement plan. ·The distinction of cover. ·Clear, beautiful typographic arrangement. ·The specialty of Photo and illustration. · Characteristic & standard layout. Now external periodicals is getting important because it shows the idea and character of company to people (PR). So it need characteristic plan and creative layout with the impotance by the understanding of the executive in company and also more does creation & originality by edit designer day by day. External periodicals should be not only the improvement of the image but also leading information paper which sensitively cope which an age by news and editing which can match people's need.

Nurse's Experience of Changing Role in the Hospice Unit of Medical Ward (호스피스병동 간호사의 역할 변화 경험)

  • Purpose: This study was designed to investigate various role changes of nurses who have cared both medical cases and hospice patients and what they experienced. Methods: Focus group interviews were done 3 times and participants were 12 nurses who have worked in the hospice unit of medical ward. Results: Role changes in 4 areas such as holistic care, end-of-life care, care fur rare givers, and coordination of hospice team were reported by the participants. What they felt were as follows : fear, confusion, maturation, increasingly labor, regret, accomplishment, sympathy and depression. Conclusion: Although hospice care in general medical ward added extra tasks, it helped clinical nurses recover professional identity and led to growth of nursing by acquiring new knowledge and skill in hospice care.

  • Business corporations have become specialized and on the basis of various interests try to strengthen their competitiveness through a cooperative system of purchase, distribution, service and IT technology. And an advanced Internet-based electronic commerce has witnessed explosive growth and a business-to-business (B2B) electronic commerce. Through E-marketplace. business corporations achieve such diverse utilities as expenditure curtailment, process reduction and prime cost reduction. And with business depression worldwide and soaring prices of materials, many business corporations consider the introduction of comprehensive purchase of MRO (maintenance, repair and operation) materials. The Purpose of this study was to examine the definition of B2B E-marketplace and the utilities of MRO commerce correctly focusing on an empirical analysis of cases of MRO supply and purchase business corporations. The indications acquired from this research results are as follows. Firstly, as for MRO integrated purchase under the characteristics of the market environment, it had to use the supplier obligatorily through the integrated purchase on the level of CEO's decision-making or the group rather than the voluntary will of the purchasing department or the procurement department. Secondly, as for the present B2B E-marketplace MRO purchase, the purchasing agency business has formed the mainstream, but it is aiming at the diversification of the benefit by supplementing/correcting the business model such as category type, mediation business type, MRO-type advancing in the store inclusive of the purchasing agency business, keeping pace with the potential market of MRO. Thirdly, as for analysis of the products, the purchaser wants more various products and assortment of goods than those of the present time, and also to be provided with the precise product information. Especially, as the importance of the product sourcing becomes high, the whole energy has been bestowed on acquirement of the excellent suppliers. Fourth, as for use of B2B E-marketplace MRO integrated purchasing system, there are the purchasing companies complaining the inconvenience even until now, and there is the demerit spending the long time during the image move and the process treatment. It shall try to shorten the searching time and the process treatment time as the system is centered on the purchaser. In order to enhance the efficiency of MRO E-marketplace purchase, followings must be considered: First, because the importance of product sources ever increases, an excellent supplier must be secured earnestly. Second, the time of process must be reduced focusing on purchasers. B2B E-marketplace will increase and diversify electronic commerce continuously. Through MRO E-marketplace, business corporations will reduce expenditure, achieve a transparent and speedy trade, and purchase products of fine quality, thus establishing a most effective market. In addition, in this study the investigator brings focus into MRO which has not been clearly discussed in the academic and business world so far and intensively highlights an indirect material-oriented expenditure curtailment effect. By taking all the aspects of supplier, purchaser and practical economical value into account, the investigator presents a strategic direction for the successful comprehensive purchase of B2B MRO.

Relationship between Metabolic Syndrome and Cognitive Functions in Patients with Chronic Schizophrenia (만성 조현병 환자에서 대사증후군과 인지기능과의 연관성)

  • Objectives : The aim of this study was to investigate the relationship between metabolic syndrome and cognitive functions among chronic schizophrenia patients. Methods : The survey participants were 105(40 metabolic syndrome and 65 non-metabolic syndrome) chronic schizophrenia patients at Yang-san Hospital. Each score of cognitive test(A Korean version of the Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet, stroop test), Positive and Negative Syndrome Scale, Hamilton rating scale for Depression were assessed. Statistical analysis of the relationship between various tests of A Korean version of the Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet and metabolic syndrome were performed using ANCOVA and logistic regression. Results : Schizophrenia patients with lower score on construction praxia were likely to be included in Metabolic syndrome group. However, there were no significant relationships in other tests between the two groups. Conclusions : This study found metabolic syndrome caused cognitive decline in chronic schizophrenia patients, especially Construction praxia. This study could be a basis to show metabolic syndrome has to be treated appropriately in schizophrenia patients and suggests the necessity of following longitudinal designed study.
