Associations of Handgrip Strength and Handgrip Strength Asymmetry With Depression in the Elderly in Korea: A Cross-sectional Study

  • Objectives: Recent studies have suggested that assessing handgrip strength (HGS) asymmetry together with HGS may be helpful for evaluating problems in geriatric patients. This study aimed to identify whether HGS asymmetry, weakness, or both were associated with depression in Korean older adults. Methods: This study included 4274 subjects from the sixth and seventh Korea National Health and Nutrition Examination Survey. Depression was measured using the Patient Health Questionnaire-9. The maximum HGS of the dominant hand was used as a representative value. HGS symmetry was categorized by the ratio of the HGS of the dominant hand to that of non-dominant hand. The odds ratio (OR) for depression was calculated according to the HGS and its symmetry. Results: In total, 240 (12.5%) men and 534 (22.7%) women had depression. HGS or HGS asymmetry showed no statistically significant associations with depression in elderly men. Elevated odds of depression were observed in elderly women with low HGS (OR, 1.93; 95% confidence interval [CI], 1.33 to 2.81) or prominent HGS asymmetry (OR, 1.46; 95% CI, 1.02 to 2.08). There was a positive additive interaction between asymmetric HGS and weakness, as women with low and prominently asymmetric HGS showed higher odds of depression (OR, 3.77; 95% CI, 2.16 to 6.59) than women with high and symmetric HGS. Conclusions: Depression in elderly Korean women was associated with both low and asymmetric HGS. Our findings support the potential value of HGS asymmetry as an indicator of HGS.

Depression and Survival in Chinese Patients with Gastric Cancer: A Prospective Study

  • Aim: Depression is thought to be a predictor of poor survival among cancer patients. In our study, we aimed to investigate the association between depression and survival in patients with gastric cancer. Methods: The subjects were a total of 300 patients aged 20-75 years who had histological confirmed diagnosis of gastric cancer from January 2004 to May 2006. Three months after patients diagnosis, depression was scored using by the Depression Status Inventory (DSI) designed by Willian WK Zung. The follow-up period consisted of a total of 13,643 person-months. A Cox's regression analysis was used to assess the association between depression and survival. Results: The percentage of subjects with depression according to the DSI depression criteria was 31%. Tumor stage and treatment methods were significantly associated with depression of patients. Age (60 years or older), annual income, tumor stage, lymph nodes metastasis and treatment were significantly associated with increased hazard ratio (HR) for gastric cancer survival. The adjusted HR for mortality risk in gastric cancer patients with depression tended to be high (HR=3.34, 95% CI=1.23-5.49) and a significant trend was found (P<0.05). Conclusion: The data obtained in this prospective study in Chinese support the hypothesis that depression is associated with poor survival among gastric cancer patients. Further studies with a large sample and longer term follow-up period are needed.

여성가장의 우울과 지각된 건강상태 (A Study of Depression and Perceived Health Status in Matriarchs)

  • The purpose of this study is to provide the basic data for developing a social policy program and group counseling program that will help matriarchs adapt successfully by investigating the level of depression and perceived health status in them. The data was collected from the period February to July, 2000 which included 155 matriarchs who registered at the 'YWCA', 'Working Women Center' located in Busan. The matriarche's depression was measured through a questionnaire composed of 13 selected items which correspond to depression of SCL-90-R scale and the tool for perceived health status measured by Ware(1976) translated by Yoo Ji Soo, Kim Jo Ja and Park Ji Won (1985). The data was then analyzed using descriptive statistics, t-test, ANOVA and pearson correlation coefficient. The result of which are, as follow: 1. The mean score of depression was 30.8 (SD=${\pm}5.1$) from a range of 13 to 52, and perceived health states was 28.3 (SD=${\pm}4.4$) from a range of 11 to 44. 2.83.3% of the matriarchs have experienced mild depression and above and 3.9% of them experienced high level of depression. 9.0% of the matriarchs perceived their health status low and 78.6% of them have percieved their health status moderately. 3. There was a significant difference statistically in the degree of depression according to the religion(t=2.852, p=.005) and income(t=4.25, p=.000). 4.There was also a negative correlation between depression and perceived health status (r=-.499, p=.000) In conclusion, the majority of matriarchs have experienced depression and perceived their health status below moderately, and there was a significant correlation between depression and preceived health status. Considering these facts therefore, We recommend that this data be used in developing nursing intervention program and in understanding matriarchs. Finally, it is suggested that the nursing assistance for matriarchs should not be limited only to physical support but also psychological and systematic support as well.

청소년 자녀의 건강관련 삶의 질에 대한 부모와 자녀의 인식차이가 청소년 우울에 미치는 영향 (Correlates of Depression and Perception Gap of Adolescents' Health-Related Quality of Life between Parents and Children)

  • Objectives: The objective of this study is to evaluate the correlates of depression and perception gap of adolescents' Health-Related Quality of Life (HRQOL) between Parents and Children. Methods: Research targets are all students and their parents who are enrolled in from 1st~3rd grade of middle school to $1^{st}$ grade of high school which was selected intentionally. The survey was conducted from May $16^{th}$ to June $9^{th}$ in 2014 by research staffs equipped with structured questionnaire. $PedsQL^{TM}4.0$ Generic Core Scales and Korean form of the Kovacs' Children's Depression Inventory were the main tool for measuring the HRQOL and depression. Data of 985 respondents, both of parents and children, was used for analysis of this study. Results: In terms of gender difference of depression, 5.8% of adolescents, 3.2% of male and 8.5% of female (p<0.001), are shown depression symptom. Depression rate of the low level students of academic achievement was 11.0% which is the lowest rate to compare to 4.3% of middle level and 2.6% of high level students (p<0.001). The case that parents overestimated their children's overall HRQOL is 3.699 times more likely to have depression symptom than the case which is in concurrence between parents and children. Especially the overestimation in social, emotional, and academic area of HRQOL promote depression 3.918 times, 3.371 times, and 2.258 times more than the agreement occurred of each areas. In contrast underestimation in physical area of HRQOL reduce depression 0.266 times less than the agreement occurred of the area. Conclusion: The perception gap of adolescents' HRQOL between parents and children is the most influential factor to adolescents' depression among all variables which was evaluated in this study.

Determinants of depression in non-cardiac chest pain patients: a cross sectional study

  • Background: Non-cardiac chest pain (NCCP) is a common patient complaint imposing great costs on the healthcare system. It is associated with psychological factors such as depression. The aim of the present study is determining depression predictors in NCCP patients. Methods: The participants of this cross-sectional study were 361 NCCP patients. Patients filled out questionnaires concerning their sociodemographic, lifestyle, and clinical factors (severity of pain, type D personality, somatization, cardiac anxiety, fear of body sensations, and depression). Results: Based on multiple ordinal logistic regression, lack of physical activity (odds ratio [OR], 1.78; 95% confidence interval [CI], 1.09-2.87), sleep quality (OR, 2.98; 95% CI, 1.15-7.69), being a smoker (OR, 1.33; 95% CI, 2.41-4.03), present pain intensity (OR, 1.08; 95% CI, 1.05-1.11), type D personality (OR, 2.43; 95% CI, 1.47-4.03), and somatization (OR, 1.22; 95% CI, 1.15-1.3) were significant predictors of depression in NCCP patients. Additionally, multiple linear regression showed that being unmarried (β = 1.51, P = 0.008), lack of physical activity (β = 1.22, P = 0.015), sleep quality (β = 2.26, P = 0.022), present pain intensity (β = 0.07, P = 0.045), type D personality (β = 1.87, P < 0.001), somatization (β = 0.45, P < 0.001), and fear of bodily sensation (β = 0.04, P = 0.032) increased significantly depression scores in NCCP patients. Conclusions: Physicians should consider the predictors of depression in NCCP patients which can lead to receiving effective psychological consultations and reducing the costs and ineffectual referrals to medical centers.

유방암 절제술을 받은 여성의 지각된 건강상태, 우울 및 삶의 질의 관계 (Relationships of Perceived Health Status, Depression and Quality of Life of Breast Cancer Survivors)

  • Purpose: The purpose of this study was to describe the degree of perceived health status, depression and quality of life(QOL) and to examine the relationships among these factors in breast cancer survivors. Methods: The subjects were 105 women with mastectomy after breast cancer diagnosis who were in the out-patient department of surgery at SNUH in Seoul. Data was collected using self-report instruments. The instruments included 1-item for perceived health status, Chae and Choe's QOL scale for Korean breast cancer survivors, and Beck Depression Inventory(BDI) as well as socio-demographic and medical information. Data were analyzed with SPSS WIN 10.0 program. Results: The levels of perceived health status and QOL were moderate. Mild depression was found on the BDI. QOL and depression showed a negative correlation(r= -.598), whereas QOL and perceived health status was correlated positively(r=.370). Depression, perceived health status, and time since diagnosis accounted for 47.4% of variance of the QOL. Depression was the most significant predictor of QOL in breast cancer survivors. Conclusion: Breast cancer survivors may experience depression and alteration in QOL. Future nursing research should be directed toward the implementation and evaluation of interventions that promote QOL and decrease depression in breast cancer survivors.

항암 화학요법을 받는 혈액종양 환자의 피로와 우울의 관계 (Relationship between Fatigue and Depression in Patients with Hemato-Malignancy receiving Chemotherapy)

  • Purpose: The purpose of this study was to identify the relationship of fatigue and depression in patients with hemato-malignancy receiving chemotherapy. Methods: The data were collected from December 2005 to November 2006. Study objects were recruited from 4 university hospital in B and U city. Fatigue and depression were measured using the Cancer Fatigue Scale and the Revised Zung's Self-rating Depression Scale, respectively. Results: 1) The mean score of fatigue was $64.64{\pm}21.58$. The mean score of depression was $41.28{\pm}7.62$. 2) The fatigue score was significantly different with age, employment status, and present pain. 3) The depression score was signigicant different by the payment, diet, and present pain. 4) There was a moderate positive correlation between depression and fatigue. Conclusions: Patients with hemato-malignancy receiving chemotherapy experience in fatigue. Decrease in fatigue are associated with decreases in depression. Therefore, nurses must provide planned nursing intervention to reduce fatigue and depression in patients with hemato-malignancy.

우울증에 관한 Sirtuin 1의 역할과 관련된 기전 (Role of Sirtuin 1 in Depression and Associated Mechanisms)

  • 우울증은 높은 유병률과 자살률 증가로 인해 사회적 기능에 부정적인 영향을 미치며, 경제적 부담 또한 높은 질환이다. 우울증은 신경염증, 시냅스 기능장애, 인지 결손과 같은 뇌에서 다양한 현상과 관련이 있다. 임상에서 사용되는 항우울제들은 치료효과가 낮아 빠른 효능을 보이는 항우울제 개발이 시급하다. 현재까지 우울증과 관련된 다양한 유전자, 단백질, 그리고 신호전달계에 대한 많은 연구가 수행되었지만, 우울증의 발생기전은 명확하게 밝혀지지 않았다. Sirtuin 1은 nicotinamide-adenine dinucleotid- (NAD+-) dependent histone deacetylases로써 세포 분화, 세포 사멸, 발생, 자가소화작용, 암 대사에 관여하는 것으로 알려져 있다. 최근의 유전연구들은 Sirtuin 1이 우울증의 잠재적 타겟 유전자라고 제안하고 있다. 또한 전임상 연구에서는 Sirtuin 1의 신호전달기전이 우울행동에 영향을 미친다고 보고 하였다. 본 종설에서는 우울증과 Sirtuin 1에 대한 최신 지식을 제시하였다. 소교세포의 활성, 일주기 생체 리듬, 신경세포 생성, 및 인지기능의 조절에 관여하는 Sirtuin 1이 우울증에 미치는 다양한 영향을 설명하였다. 아울러 Sirtuin 1이 우울증 핵심 기전중의 하나인 신경가소성의 손상에 미치는 영향과 그 기전에 대해서 논의하였다.

Determinants of Depression in Indonesian Youth: Findings From a Community-based Survey

  • Objectives: This study investigated the determinants of depression in adolescents and young adults. Methods: The present study analyzed data from the 2018 Indonesia Basic Health Survey (Riset Kesehatan Dasar; RISKESDAS). The study subjects were adolescents (15-17 years old) and young adults (18-24 years old). In total, 64 179 subjects were included. Univariable, bivariable, and multivariable logistic regression analyses were performed to determine associations between youths' characteristics, risky behavior, chronic disease, parents' health, and youths' depression. Results: The prevalence of depression was 5.1% in adolescents and 5.6% in young adults. The risk factors for depression in adolescents were being women (adjusted odds ratio [aOR], 3.53; 95% confidence interval [CI], 2.89 to 4.30; p<0.001), an ex-smoker (aOR, 2.99; 95% CI, 2.10 to 4.25; p<0.001), or a current smoker (aOR, 2.60; 95% CI, 1.97 to 3.44; p<0.001); consuming alcohol (aOR, 2.00; 95% CI, 1.33 to 3.01; p=0.001), having a chronic disease (aOR, 2.64; 95% CI, 1.41 to 4.96; p=0.002); maternal depression (aOR, 2.47; 95% CI, 1.97 to 3.09; p<0.001); and paternal depression (aOR, 2.63; 95% CI, 1.98 to 3.50; p<0.001). In young adults, the risk factors were being women (aOR, 2.23; 95% CI, 1.82 to 2.75; p<0.001) or an ex-smoker (aOR, 2.47; 95% CI, 1.67 to 3.67; p<0.001), consuming alcohol (aOR, 2.42; 95% CI, 1.89 to 3.09; p<0.001), maternal depression (aOR, 3.12; 95% CI, 2.54 to 3.84; p<0.001), and paternal depression (aOR, 2.80; 95% CI, 2.17 to 3.63; p<0.001). Conclusions: Being women, smoking, drinking alcohol, having a chronic disease, and having a parent with depression were crucial factors associated with youth depression. Mental health screening, prevention, and treatment should involve collaboration among primary healthcare, schools, universities, professionals, and families.

인격과 우울증 치료결과의 상호작용 (Interaction between Personality and Depression Outcome)

  • It is known that the personality is the crucial factor in the treatment outcome of depression. The authors tried to identify the results of such studies and various components which determine the treatment outcome of depression. Nearly 60 papers published between the year 1990 and 2000 about the treatment of depression and personality were reviewed. Among them about 30 papers were selected to compare the research methods, results and discussions. The arguments and critics of the papers were discussed. In the many debates, the authors admitted the fact that premorbid personality trait influences the treatment outcome of depression negatively regardless of treatment method. Subtyping of depression is feasible along the presence of good or bad predictors of treatment outcome for depression. Differentiation of depression and personality seems to have no problem, however test of personality state before the development of psychiatric disorder such as depression is not amenable. For example, cluster A personality trait is often misunderstood as depression in clinical setting. In some cases cognitive behavioral therapy is effective in the treatment of depression accompanying personality disorder. The authors insist that the analysis of personality in the dimensional aspect rather than in the categorical aspect gives more information in the research of personality influencing the treatment outcome of depression. In addition, the reason why we understand the relationship between depression and personality were discussed.
