TREATMENT OF FUNCTIONAL POSTERIOR CROSSBITES IN THE PRIMARY AND EARLY MIXED DENTITIONS : CASE REPORT (유치열 및 초기 혼합치열기에서 기능성 구치부 반대교합의 치료증례)

  • Posterior crossbites are abnormal buccal, or lingual relationship of a tooth or teeth of the maxilla, the mandible, or both when the teeth of the two arches are in occlusion and involve the molars and premolars. Posterior crossbites are classified as dental, muscular(functional), or skeletal. In an effort to avoid occlusal interferences caused by the inadequate arch width, the patient deviates the mandible laterally upon closure to achieve maximum intercuspation. This is described as functional posterior crossbite. Correction of functional posterior crossbites in the primary & early mixed dentition as early as possible after diagnosis has been recommended, because crossbites do not automatically improve with the eruption of the permanent teeth. Functional posterior crossbites, if left untreated, may have deleterious effects on the development and function of the TMJ. The diagnosis and management of three cases is presented. Each patient with functional posterior crossbites is treated using the bilateral maxillary expansion appliance.

  • The author used cephalometric roentgenogram to observe the longitudinal change by orthodontic treatment for early class III malocclusion in primary and mixed dentition. First, the cephalometric roentgenograms were measured and following results were obtained 1. SNA, SNB, ANB, Gonial angle, and SN to mandibular plane were measured as skeletal pattern and $\underline{1}$ to SN. $\overline{1}$ to mandibular plane and interincisal angle were measured as denture pattern. 2. Angular measurements for the Class III malocclusion were compared with those for the normal occlusion of the same Hellman dental age.

  • 20 children who visited department of pedodontics, S.N.U. Hospital were examined and their study models were obtained. The data obtained from three groups of operator, three method, twice measurement, was analysed. The results of the study were as follow: 1. The Brass-wire method appears to be the least desirable of those examined for space predictions in all three groups of operators. 2. It appears that there is little clinical significance between Moyer's and Segmented-arch method in all three groups of operators. 3. The Moyers' method appears to be more easily conceptualized by inexperienced operators than the other two methods.

  • Maxillary first molar is the key in normal occlusion. Mesial drifting of maxillary first molar result form early loss of second deciduous molar. Mesial drifted maxillary first molar was treated by headgear, Hawley appliance with screw, brasswire, etc. But, these appliance should be necessary for patients cooperation. Recently, several appliance for molar distalizing without patients cooperation has been introduced. We are reporting in this paper about distalizing of mesial drifted maxillary first molar because of early loss of deciduous second molar by open coil jig. Distalization of molar by open coil jig is predictable, rapid, painless method without mecesscity of patient cooperation.

  • The present paper describes 4 clinical cases of Hellman dental age III A group of functional Class III malocclusion in which the horsehoe appliance was applied to obtain normal anteroposterior relationship between jaws. What is the most noteworthy in the result from the use of this appliance is that minimum down and backward rotation during backward repositioning of the mandible is noticed and the excellent retentive property in itself makers allowance for effective orthopedic correction in mixed dentition with anterior crossbite.

  • The purpose of this study was to investigate the treatment effects on anterior crossbite patients using chin cap and labiolingual arch appliance. In the present study, I statistically evaluated measurement values on a cephalogram before and after treatment form twenty anterior crossbite cases. The results were as follows: 1. In the craniofacial patterns, cranial base increased after treatment. 2. In the maxillo-mandibular relationship, ANB and Facial convexity significantly increased. 3. In the denture patterns, occlusal plane, Ul to FH and Overjet significantly increased. On the contrary overbite decreased significantly. Lower incisors inclined lingually. 4. In the soft tissue profile changes, lower lip protrusion was signifacantly improved.

  • This study was undertaken to investigate the exact values of the cephalometric standards of Hellman dental age III B groups of Korean in the reontgenocephalometry. The subjects consisted of 25 males and the same number of females with the normal occlusion and acceptable profile. Each lateral cephalometric head film was taken with the teeth in occlusion and the Focal-film distance was 300cm. Their linear and angular measurements were made directly. The following conclusions were obtained; 1) The author made the tables of standard deviation from the measured values. 2) The degree of the facial convexity of Korean children was larger than that of the white. 3) The labial inclination of the lower central incisor in male was a little greater than that in female.

  • Mandibular permanent lateral incisors showing bilateral transposition and ectopic eruption were seen beside deciduous first molars in a female aged 8yr 8month. Repositioning of the ectopic lateral incisor is difficult not only because of its distal displacement and severe rotation but also because of the potential development of transposition with the erupting canine. This paper is focused on the diagnosis of the ectopic eruption and advocates treatment with active orthodontic management at the early stage of mixed dentition, before the eruption of the permanent cuspid.

  • The purpose of this study is to evaluate the influence of genetics of the dimensions of dental arches in twins. Thirty-four pairs of Korean twins, of which twenty-three were monozygotic and eleven dizygotic, participated in the study. The twins ranged in age from 6 years to 12 years with mixed dentition. The results were as follows: 1) No significant differences were observed between male and female in mean interpair diffenences of monozygotic twins. 2) Mean interpair differences of monozygotic twins were lesser than these of dizygotic twins. 3) Highly significiant zygotic differences were observed in the mean interpair differences of the width of upper and lower dental arch. This suggest that there is a significant component of hereditary variability.

  • Eruption of the teeth can be disturbed by crowding, ectopic eruption path, retention for pathologic condition of follicle and periodontal ligament, local disturbances in the innervation, and delayed eruption for overlying obstacles in the eruption path. Eruption disturbance of permanent posterior teeth is taken with diverse therapeutic approaches according to the patient age, cooperation of patient, tooth position and maturity, degree of impaction, clinical features, and repercussion on the neighboring teeth. However, delayed treatment usually results in less favorable outcomes. Therefore, In order to prevent this situation, periodically radiographic examinations during the early mixed dentition period and early diagnosis of eruption disturbances of permanent posterior teeth are recommended.
