근관충전재(根管充塡材)의 매식(埋植)이 골조직(骨組織)에 미치는 영향(影響)에 관(關)한 실험적(實驗的) 연구(硏究) (AN EXPERIMENTAL STUDY OF THE EFFECTS ON THE BONE TISSUE BY IMPLANT OF ROOT CANAL FILLING MATERIALS)

  • The purpose of this study was the observe the toxic effects of root canal sealers in 108 white rats. Experimental animals were divided into control and experimental groups. Theree representative types of materials, such as AH26, Z.O.E. and F.R. were used in this study. Cavities were prepared on the left mandibular area of 108 white rats. Three different sealers were placed in as experiment and bone cavities were left without filling as control. The experimental animals were sacrificed by cervical dislocation at the intervals of 1, 3, 7, 14, 28 and 49 days after filling. Each specimen was fixed with 10% neutral formalin solution, decalcified with 5% nitric acid, embedded in paraffin and sectioned 5-7${\mu}$. in thickness. The paraffin sections stained with Hematoxylin - Eosin were observed through the ordinary light microscope. The results were as follows; 1. Slight toxic effect to surrounding tissue were found in every experimental specimen. 2. AH26 showed the highest inflammatory response, and F.R. showed the lowest inflammatory response which subsided and replaced by fibrosis at 4 weeks after filling. 3. The cavity filled materials, such as implanted root canal sealers, blood clots and necrotic tissue, showed a tendency to be absorbed gradually proportioned to the experimental periods. A small amount of cavity filled materials were observed in the bone cavities after 4 weeks. 4. Fibroblastic proliferation began to produce fibrous capsule around the bone cavity in 2 weeks after filling. Fibrosis was prominent at 4 weeks after filling. 5. Osteoblastic activity of surrounding bone was observed at first in 2 weeks after filling and prominent in 4 weeks after filling. Osteoblastic activity showed an increasing effect as the time prolonged. 6. Surrounding tissue of the bone cavities showed the features of tissue destruction and had very severe inflammatory response at an initial stage. Above-mentioned appeared to be recovered gradually proportioned to the experimental periods.

  • The purpose of this study was to observe the tensile and bonding strength of the joined amalgam restoration. Amalgam alloys of fine-cut (F-type), spherical (S-type), and dispersed type (D-type) were selected in this study, and all specimens were divided into three groups according to the condensation methods as follows. Group I : the control group which condense the same kinds of mixed amalgam into the whole part of the mold respectively. Group II : the group which condense a mix of amalgam into one half of the mold, and then condense a new mix of amalgam into the rest half of the mold 15 minutes later. Group III : the group which condense a mixed amalgam into one half of the mold, and then condense a new mix of amalgam into the rest half of the mold 7 days later. All specimens were stored in incubator at $37{\pm}1^{\circ}C$ for seven days with immersing in saline solution before testing. The tensile and bonding strength of them were measured with Instron Universal Testing machine. The results were as follows: 1. In Group I, the order of tensile strength was F-type, S-type, and D-type. 2. In case of bonding of S-type + S-type, the difference of the bonding strength between Group II and III was not significant. (P> 0.05) 3. The bonding strength of F-type + S-type of Group II was marked the highest in value, and the lowest bonding strength was showed in bonded D-type + D-type of Group III. 4. In case of bonding with the different kinds of amalgam alloy in Group II, the specimen bonded to F-type was marked the highest bonding strength, and the specimen bonded with F-type was marked the lowest one. In Group II, the bonding strength of the specimens bonded with the same kinds of amalgam alloy was presented as the same order as that of Group I. 5. In Group III, the specimen connected with D-type marked the lowest bonding strength of all specimens. In Group III, the bonding strength of the specimens connected with the same kinds of amalgam alloy was the order of S-type + S-type, F-type + F-type, and D-type + D-type.

  • The purpose of this study was to observe the effect of dentin surface conditioners on the dentin surfaces. Freshly extracted human molars were used in this study. They were stored at $4^{\circ}C$ saline solution before experiment. The crown portions of the teeth were cut in various directions by means of wet diamond point to expose dentin which include transverse, vertical oblique, horizontal and oblique cut to the long axis (Fig. 1). Each tooth was then mounted with self curing acrylic resin in brass ring to expose the flattened dentin surfaces. Final finish was accomplished by grinding the dentin specimens with wet No. 180 and No. 600 grit silicon carbide abrasive paper until a 6.0mm in diameter on a dentin surface was exposed without pulp exposure. The specimens were divided into 9 groups according to the modes of dentin treatment procedure. The following surface treatments were applied on these preparation surfaces; Group 1: unetched (control group) after finish with No. 600 silicon carbide abrasive paper. Group 2: etched with 30% phosphoric acid for 60s Group 3: etched with 10-3 solution for 60s Group 4: Cleaned with 5% NaOCl for 30s Group 5: applied Dentin Adhesit Group 6: cleaned with 5% NaOCl followed by applying the Dentin Adhesit$^{(R)}$ Group 7: applied Photo Bond on the unetched dentin followed by applying the Photo Clearfil Bright Group 8: Etched with 30% phosphoric acid followed by applying Photo Bond and Photo Clearfil Bright Group 9: etched with 10-3 solution followed by applying Photo Bond and Photo Clearfil Bright All the specimens were stored in $37^{\circ}C$ under 50% relative humidity for 24 hours before observations. The specimens in 7, 8, and 9 group, omitting the group 1 to 6, were demineralized in 10% HCl for 10s in order to observe the resin tags. All the specimens in each group were then dried at room temperature. The dried specimens were ion coated with Eiko ion coater (Eiko-engineering Co.), and observed in Hitachi S-430 Scanning electron microscope (Hitachi, Co. Tokyo) at 15KV. The following results were obtained as follows; 1. The smear layers were still remained in group 1,2,4,5, and 6. 2. There is no effect of 5% NaOCl and 30% phosphoric acid on the changes of dentin morphology 3. The dentin treated with 10-3 solution, indicating the tubules opened when the smear layer and the dental plug dissolved. 4. In case of applying the bonding agents the resin tag was not formed at the deep area of dentinal tubules, but in case of applying the Dentin Adhesit$^{(R)}$ that was not.

  • The purpose of this study was to examine the fracture strength and characteristics of teeth with MOD cavity preparation. Freshly extracted sound maxillary premolars were cleaned and stored in normal saline solution $37^{\circ}C$ for 72 hours before experiments. The roots of teeth were embedded in a self-curing resin, and the exposed crown were maintained in a vertical position by a modelling wax in a brass ring. The MOD cavities were prepared with No. 57 carbide bur under high speed to a depth of 2.0mm and a width of 2.0mm(Fig.1). All the prepared teeth specimens were divided into 7 groups according to the mode of cavity form and restorative materials (Table 1, 2): Group I, unpreapred, intact teeth as control Group II, prepared cavity without restoration Group III, prepared teeth restored with amalgam Group IV, prepared teeth restored with composite resin (P-10) Group V, prepared teeth with beveled enamel margins restored with composite resin (P-10) Group VI, prepared teeth restored with light-cured composite resin (P-30) Group VII, prepard teeth with beveled enamel margins restored with light-cured composite resin (P-30) After placement of restorations, all of the specimens were stored in water at $37^{\circ}C$ for 72 hours before testing. All of the specimens were tested on the Instron Universal Testing machine (No. 6025) in order to evaluate the strength of fracture. One metal ball 5.0mm in diameter contacting the specimens parallel to the occlusal surface was used to in this study (Fig. 1). The fracture characteristics of the specimens were examined with naked eye and in the scanning electron microscope (JSM-20). The results obtained from this study were as follows: 1. The mean fracture strength was the highest in group VI and that in group II was the lowest. 2. The progress of crack of teeth propagated into the pulp cavity. 3. In case of the group of the restored teeth, the crack occurred to be accompanied with cuspal fracture. 4. The crack of restored teeth was initiated along the pulpo-axial line angle of the cavity.

The effect of different adhesive system applications on push-out bond strengths of glass fiber posts

  • PURPOSE. Over the past years, the adhesion of fiber posts luted with simplified adhesive systems has been a matter of great interest. The aim of this study was to assess the post retentive potential of a self-adhesive resin cement using different adhesive systems to compare the push-out bond strengths of fiber posts. MATERIALS AND METHODS. The post spaces of 56 mandibular premolar roots were prepared and divided into 4 experimental groups and further divided into 2 subgroups according to testing time (n=7). The fiber posts (Rely X Fiber Post) were luted with a self-adhesive resin cement (RelyX Unicem) and one of the following adhesive systems: no adhesive, a total-etch adhesive resin (Single Bond), a two-step self-etch adhesive resin (Clearfil SE Bond) and a one-step self-etch adhesive resin (Clearfil S3 Bond). Each root was cut horizontally, and 1.5 mm thick six root segments were prepared. Push-out tests were performed after one week or three months (0.5 mm/min). Statistical analysis were performed with three-way ANOVA (${\alpha}$=.05). RESULTS. Cervical root segments showed higher bond strength values than middle segments. Adhesive application increased the bond strength. For one week group, the total-etch adhesive resin Single Bond showed higher bond strength than the self-adhesive resin cement RelyX Unicem applied without adhesive resin at middle region. For 3 months group, the two-step self-etch adhesive resin Clearfil SE Bond showed the highest bond strength for both regions. Regarding the time considered, Clearfil SE Bond 3 months group showed higher bond strength values than one week group. CONCLUSION. Using the adhesive resins in combination with the self-adhesive resin cement improves the bond strengths. The bond strength values of two-step self-etch adhesive resin Clearfil SE Bond improved as time passes.

4% lidocaine versus 4% articaine for inferior alveolar nerve block in impacted lower third molar surgery

  • Background: No study has compared lidocaine with articaine, each at a concentration of 4% and combined with epinephrine. The purpose of this study was to compare the effectiveness of 4% lidocaine with that of 4% articaine, with a concentration of 1:100,000 epinephrine added to each, in an inferior alveolar nerve block for surgery on impacted lower third molars. Method: This study was conducted at the Faculty of Dentistry, Mahidol University in Bangkok, Thailand. The randomized, single-blind, comparative split-mouth study was carried out in patients with symmetrically impacted lower third molars, as identified on panoramic radiographs. Each patient underwent surgery for the removal of the lower third molars by the same surgeon under local anesthesia at two separate visits, 3 weeks apart. The onset and duration of local anesthesia, intra-operative pain, surgical duration, and number of additional anesthetics administered were recorded. Results: The subjective and objective onset of action for the local anesthetics showed statistically significant differences (P < 0.05). However, the intra-operative pain, surgical duration, duration of local anesthesia, and number of additional anesthetics administered did not show statistically significant differences. Conclusion: The use of 4% articaine for the inferior alveolar nerve block was clinically more effective in the onset of subjective and objective anesthesia as compared with the use of 4% lidocaine. Based on the pain scores from the visual analogue scale, 4% lidocaine provided more analgesia during the procedure, and patients noted less intra-operative pain than with 4% articaine; however, the difference was not clinically significant.

Evaluation of the postoperative maxillary sinus with computed tomography

  • Purpose: To evaluate the computed tomographic appearances of post-operative maxillary sinuses. Materials and Methods: 33 asymptomatic cases of post-operative maxillary sinus without evidence of any pathologic changes and clinical symptoms were selected. CT images were classified as opacification, soft tissue shadow, anterior wall depression, naso-antral communication, and compartmentalization. The relationships between the CT image and the age of patients at the time of operatation, and between the CT image and the duration of time elapsed since the surgical procedure were evaluated. Results: The most commonly presented radiological characteristics that occurred after the Caldwell-Luc procedure were opacification and soft tissue shadow. Anterior wall depression and naso-antral communication were radiographic indications that a Caldwell-Luc operation had been carried out. The age of patients when they had been first operated on, and the duration between the surgical procedure and the time of evaluation had no effect on the CT appearances of normal changes. In cases involving a longer time interval between the antral surgery and evaluation, the anterior wall depression with bony healing was more commonly observed than soft tissue healing. Conclusion: The radiographic information regarding the normal healing state using computed tomography can distinguish post-operative changes from inflammatory and cystic disease in patients who have undergone a Caldwell-Luc type of radical maxillary antrostomy.

  • Forty extracted human mandibular second molars with C-shaped canal were chosen to study the anatomy of the root canal. The experimental teeth were injected with china ink, decalcified and cleared with Winter green oil, in vitro, to study the number of root, root canal, canal per root, frequency and location of lateral canal and transverse anastomosis. 1. All teedth had one root. 2. Mesial roots with two canals were 25%, and mesial roots with one canal were 75%. All distal roots had one canal. 3. In the roots with two canals, the common apical foramen appeared in 20% and the separte apical foramen appeared in 80%. 4. The frequence of lateral canal was 33.3% and the most frequent region was middle 1/3. 5. All teeth had the transverse anastomosis in all region of the roots.

  • The purpose of this study was to compare Black-pigmented Bacteroides isolated from necrotic pulp with the presence or absence of eight symptoms associated with pulpal necrosis and to identify the proportion of each Black-pigmented Bacteroides species. The canal contents of fourteen traumatically and cariously necrotized teeth were sampled with a special technique and cultured aerobically and anaerobically for growth in blood agar plate and for Black-pigmented Bacteroides on selective blood agar plate. Each Black-pigmented Bacteroides species were identified by Gram's stain, hemolysis reaction, colony color and morphology and biochemical tests. The results were as follows; 1. 60.9 percent of the bacteria isolated were anaerobic and 39.1 percent were aerobic. 2. Four Black-pigmented Bacteroides species were isolated; B. loescheii (74.1%), B. denticola (11.1%), B. intermedius (7.4%) and B. gingivalis (7.4%). 3. Black-pigmented Bacteroides was found to be significantly related to sinus tract formation and exudate.

표백효과에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON EFFECT OF BLEACHING)

  • The purpose of this study was to the observe effects of the various bleaching agents on blood clot. Absorbent papers(N9.5) were coated with O type blood and were dried in shadow for 72 hours. Bleaching agents used were 5% NaOCl 30% $H_2O_2$, saturated solutions of $NaBO_3$ $4H_2O$, $Ca(OCl)_2$ and $Na_2O_2$ at $20^{\circ}C$. After the specimens, blood cloted papers were dipped in 10CC of solution for 1 minute, 5 minutes, 30 minutes, 2 hours and 24 hours, specimens were washed with distilled water and dried. Hue Value/Chroma contrived by Munsell was measured in each specimen. The following results were obtained. 1. Superoxol changes 2.5YR 4.5/2 into N9.5 (white) after 24 hours. 2. All bleaching agents except superoxol change YR into Y.
