A STUDY ON THE MICROLEAKAGE OF THE IPS EMPRESS CERAMIC CROWN ACCORDING TO MARGIN TYPES AND RESIN CEMENT (변연형태와 레진시멘트에 따른 IPS Empress 도재관의 변연누출에 관한 연구)

  • The purpose of this study was to compare the microleakage of the IPS Empress ceramic crown according to 1) margin types such as rounded shoulder and bevel, 2) margin locations such as enamel and dentine, 3) resin cement types such as Variolink and Bistite. For this study, 80 extracted premolar teeth were used. After 10 experimental dies were prepared for each group, the IPS Empress ceramic crowns were fabricated on the dies and cemented with one of two types of resin cement. After 1,000 cycles of thermal changes were given with the thermocycling unit, each specimen was sectioned with a low speed diamond saw in both the buccolingual and the mesiodistal direction. The microleakage was then measured with a stereo microscope. The obtained results were as follows : 1. Every experimental group showed microleakage. The microleakage of the crowns which had a rounded shoulder on enamel cemented with Variolink was the lowest of all. The resulting rank of the mean microleakage was rounded shoulder on enamel using Variolink(0.601mm), rounded shoulder on enamel using Bistite (0.794mm), bevel on enamel using Variolink (0.826mm), rounded shoulder on dentine using Variolink(1.054mm), bevel on enamel using Bistite(1.087mm), rounded shoulder on dentine using Bistite(1.176mm), bevel on dentine using Variolink(1.258mm), and bevel on dentine using Bistite(1.467mm). 2. The statistically significant differences in the microleakage were found in 4 cases: rounded shoulder on enamel using Variolink and bevel on dentine using Variolink, rounded shoulder on enamel using Variolink and bevel on dentine using Bistite, rounded shoulder on enamel using Bistite and bevel on dentine using Bistite, bevel on enamel using Variolink and bevel on dentine using Bistite. 3. The microleakage of rounded shoulder finish line was lower than that of bevel finish line. The statistically significant differences were found with respect to the type of finish lines. 4. The microleakage of the finish line on enamel was lower than that of finish line on dentine. The statistically significant differences were found with respect to the location of the finish lines. 5. The microleakage of the Variolink cement was lower than that of the Bistite cement. The statistically significant differences were found with respect to the resin cements.

Gross-β Level in Dental Ceramic Materials (치과용 세라믹 재료의 전β 방사능 준위)

  • In all of dental ceramics, these materials contained the radio-isotopes with natural abundance ratio. After dental treatment, remained dental ceramics in the oral cavity is caused for human internal dose. In this study, the gross beta activity levels were measured in dental materials including 22 dental ceramics, 1 resin, and 2 cements for estimation of human internal dose. In dental ceramic samples, the obtained results showed that the gross beta activity level varied between 1.317 ~ 2.935 Bq/g and the gross beta activity level was 2.379 Bq/g. And the same level for dentine, opacious dentine, translucent and enamel were 2.479 Bq/g, 2.491 Bq/g, 2.470 Bq/g and 2.069 Bq/g, respectively. The gross beta activity level of temporary resin and cements were negligible, compare to the same level of dental ceramics. The high gross beta activity level observed in opacious dentine code OD-A4 is 2.935 Bq/g thus mainly ascribable to 40K. The reduction of the radiation level of natural radio-isotopes and the improvement of the dental ceramic materials should be required for internal dose reduction.

AN IN-VITRO WEAR STUDY OF DENTAL PORCELAINS AND HUMAN ENAMEL (치과용도재에 의한 법랑질 마모에 관한 연구)

  • Dental porcelain is one of the materials of choice for restoration where esthetics is of concern, but has a considerable potentials of wear. The wear of enamel is variable when opposed by different porcelain systems and surface conditions, and the exposed dentine and opaque porcelain due to clinical failure is expected to have high potentials of wear. The purpose of this study was to investigate the wear effects of self-glazed, polished incisal porcelain. polished dentine and opaque porcelain against human enamel in the laboratory by use of a pin-on-disk type wear tester. 4 types of dental porcelain($Vita-{\Omega}$, Ceramco-II, Vintage powder $Vita-{\alpha}$ of In-Ceram system) and type IV gold alloy as cotrol group were used for test specimens. Intact buccal cusps of maxillary premolar were used for enamel specimens, and the cusp converged to a point and was devoid of visible abrasion, caries, decalcification. The upper part was the cusp of a maxillary premolar and the lower part was a porcelain specimen. The enamel wear was deter-mined by weighing the cusp before and after each test. Surface profilometer was used to quantitate wear of the porcelain specimens. Vicker's hardness tester was used to evaluate the surface hardness of test specimens. The SEM was used to evaluate the wear surfaces. The results were as follows : 1 Self-glazed porcelain produced more enamel wear than polished porcelain, especially the enamel wear of $Vita-{\alpha}$ self-glazed porcelain was 3.2 times more than that of other groups. 2. Opaque porcelain produced least porcelain wear, $Vita-{\alpha}$ self-glazed porcelain produce greatest porcelain wear, but there was no statistically significant difference between the groups(p>0.05). 3. The enamel wear of dentine porcelain was 3.8 times more than that of polished inisal porcelain(p<0.05), and the enamel wear of opaque porcelain was 1.9 times more than that of polished inisal porcelain, but there was no statistically significant difference between the groups(p>0.05) 4. Overglazed porcelain produced less enamel wear than self-glazed porcelain, and more enamel wear than polished porcelain, but there was no statistically significant difference between the groups(P>0.05). 5. The hardness number of $Vita-{\Omega}$ dentine and Ceramco-II opaque porcelain was larger, but that of Vintage dentine and $Vita-{\alpha}$ self-glazed porcelain was similar to other groups. 6. Examination of SEM photographs revealed that overglazed porcelain had smoother surface than self-glazed porcelain, and self-glazed porcelain had smoother surface than polished porcelain. Much polishing scratches and larger porosities were observed on the opaque porcelain specimen, and much polishing scratchess and small porosities were observed on the dentine porcelain specimen.

THE EFFECTS OF THE FLUORIDE CONCENTRATION OF ACIDULATED BUFFER SOLUTIONS ON DENTINE REMINERALIZATION (유기산 완충용액의 불소농도가 상아질의 재광화에 미치는 영향)

  • The aim of this vitro-study is to evaluate the effects of fluoride on remineralization of artificial dentine caries. 10 sound permanent premolars, which were extracted for orthodontic reason within 1 week. were used for this study. Artificial dentine caries was created by using a partially saturated buffer solution for 2 days with grounded thin specimens and fractured whole-body specimens. Remineralization solutions with three different fluoride concentration (1 ppm. 2 ppm and 4 ppm) were used on demineralized-specimens for 7 days. Polarizing microscope and scanning electron microscope were used for the evaluation of the mineral distribution profile and morphology of crystallites of hydroxyapatite. The results were as follows: 1. When treated with the fluoride solutions, the demineralized dentine specimens showed remineralization of the upper part and demineralization of the lower part of the lesion body simultaneously. 2. As the concentration of fluoride increased, the mineral precipitation in the caries dentine increased. The mineral precipitation mainly occurred in the surface layer in 1 and 2 ppm- specimens and in the whole lesion body in 4 ppm -specimens. 3. When treated with the fluoride solution, the hydroxyapatite crystals grew. This crystal growth was even observed in the lower part of the lesion body which had shown the loss of mineral.

AN EXPERIMENTAL STUDY ON THE EFFECT OF ACID ETCHING OF DENTINE TO PULP TISSUE (상아질(象牙質)의 산부식(酸腐蝕)이 치수조직(齒髓組織)에 미치는 영향(影響)에 관(關)한 실험적(實驗的) 연구(硏究))

  • The purpose of this study was to investigate the pulpal responses to acid etching of dentine. Total 72 class V cavaties' were prepared on the healthy permanent teeth from 6 dogs, and were divided into 4 groups. These were filled with filling materials after acid etching with each etchant for 1 min. Control group ; ZOE filling without acid etching. Group 1: Zinc Oxide-Eugenol cement filling. Group 2: Drying with hot air and Zinc oxide-Eugenol cement filling. Group 3: Scotchbond application and silux filling. Group 4: Silux filling. The dogs were sacrificed after 1 week, 2 weeks, 3 weeks, 4 weeks, 5 weeks and 6 weeks following operation. The specimens were routinely prepared and stained with Hematoxylin and Eosin. Followings were the results obtained through microscopic examination. 1. There was mostly severe pulpal responses in case of Silux filling after acid etching of dentine. 2. The pulpal responses of Silux filling after acid etching and application of Scotch bond group were more severe compared to Zinc Oxide-Eugenol cement filling group, but less severe compared to Silux filling group after acid etching of dentine. 3. The pulpal responses of Zinc Oxide-Eugenol cement filling group were similar to those of Zinc Oxide-Eugenol cement filling after drying with hot air group. 4. There was slight pulpal responses in early stage in case of Zinc Oxide-Eugenol cement filling group, but recovered to normal state soon after.

A Study on the Colors of Zirconia and Veneering Ceramics (지르코니아와 전장용 세라믹의 색상에 관한 연구)

  • Purpose: This study was conducted to investigate the colors of zirconia and veneered ceramics according to the build-up method of veneer porcelain. Methods: The samples were divided into three groups: a group for which a liner was applied to the zirconia surface and dentine coating was built up and burned (Cz1), a group for which the dentine coating was directly built up and burned (Cz2), and a group for which the samples were wash-burned with dentine coating and a dentine coating was built up and burned again (Cz3). The the colors were measured with a Spectrophoto-meter. Results: The average $L^*$ value of the Cz3 group was the highest among the groups at $59.1{\pm}0.5$ (P<0.05), followed by Cz2 at $57.5{\pm}0.8$ and Cz1 at $56.6{\pm}1.3$. The $a^*$ value of Cz3 was negative at $-0.0{\pm}0.0$ and the $a^*$ values of Cz2 and Cz1 were positive at $0.2{\pm}0.1$ and $0.4{\pm}0.0$, respectively (P<0.05). The $b^*$ of Cz1 was the highest at $11.2{\pm}0.6$, followed by Cz3 at $10.8{\pm}0.5$ and Cz2 at $10.5{\pm}0.8$ (P<0.05). Conclusion: Compared to other experimental groups, the Cz3 groups showed the brightest colors with high brightness and relatively low green and yellow levels.

The Study of Age Estimation from Tooth using the Racemization of Aminoacid (아미노산의 라세미화 반응을 이용한 치아로부터의 연령감정에 관한 연구)

  • The need of age estimation for identification was increased by complexity of society, and the tooth was used widely for age estimation because of less individual deviation than the other organ. The age estimation using the tooth had several methods. Recently, the one using the racemization of aminoacid in the tooth was admitted more accurate than the other methods, especially in old age. But, this study was not tried in our country, and I would report the result of experiment about age estimation using racemization of dentine. I selected 40-Whole dentine sample from extracted teeth, those were reserved in natural dried condition for 2 weeks~ 1year and calculated the estimation of age from the ratio of D-aminoacid and L-aminoacid (D/L ratio) using gaschromatography and the results were below. 1. The aminoacids showed apparent K/L ratio in dentine were aspartic acid, serine. 2. The aspartic acid showed the highest racemic rate and its rate was 0.0012$\pm$0.0003/yr. 3. The relation between the actual age and K/L ratio was very positive correlation(r+0.954) in the estimation of age using aspartic acid. 4. The deviation between the estimated age using D/L ratio of aspartic acid and actual age was $\pm$3.32.

Association of Salivary Microbiota with Dental Caries Incidence with Dentine Involvement after 4 Years

  • Salivary microbiota alterations can correlate with dental caries development in children, and mechanisms mediating this association need to be studied in further detail. Our study explored salivary microbiota shifts in children and their association with the incidence of dental caries with dentine involvement. Salivary samples were collected from children with caries and their subsequently matched caries-free controls before and after caries development. The microbiota was analyzed by 16S rRNA gene-based high-throughput sequencing. The salivary microbiota was more diverse in caries-free subjects than in those with dental caries with dentine involvement (DC). Although both groups exhibited similar shifts in microbiota composition, an association with caries was found by function prediction. Analysis of potential microbiome functions revealed that Granulicatella, Streptococcus, Bulleidia, and Staphylococcus in the DC group could be associated with the bacterial invasion of epithelial cells, phosphotransferase system, and ${\text\tiny{D}}-alanine$ metabolism, whereas Neisseria, Lautropia, and Leptotrichia in caries-free subjects could be associated with bacterial motility protein genes, linoleic acid metabolism, and flavonoid biosynthesis, suggesting that functional differences in the salivary microbiota may be associated with caries formation. These results expand the current understanding of the functional significance of the salivary microbiome in caries development, and may facilitate the identification of novel biomarkers and treatment targets.

Desensitizing Effects of a Nd:YAG Laser Irradiation on Hypersensitive Dentine (지각과민치아에 대한 Nd:YAG 레이저 조사의 효과)

  • The purpose of this study was to evaluate the desensitizing effects of a Nd:YAG laser (Sunlase, SUNRISE Technologies, Inc., USA) irradiation on cervically exposed hypersensitive dentine. 45 patients was irradiated with pulsed Nd:YAG laser (1.5 W, 20 Hz, 75 mJ/pulse, 4 minutes) as the experimental group, 27 patients was mock irradiated as the control group. The degree of sensitivity to the thermal and tactile stimuli were determined qualitatively with an evaporative stimulus defined as two times air blast at a distance of 3 mm from each site to be tested and with a mechanical stimulus as a slightly scratching the cervical site with a dental explorer. A qualitative registration of the degree of discomfort was determined according to a numerical pain scale(NPS) in an 11-point scale in which 0= "no pain" and 10="most excruciating pain imaginable". Recordings were assessed before treatment, immediately after, 1 and 2 weeks after treatment. Pain tolerance threshold and pulp vitality were evaluated with electric pulp tester before and immediately after treatment. 1. Desensitizing of hypersensitive dentine with Nd:YAG laser irradiation was more effective than that with mock irradiation. 2. The placebo effect of mock irradiation was recognized for severe sensitive teeth($NPS\;{\geqq}\;6$), but not for moderate sensitive teeth(NPS < 6). 3. Laser irradiation did not affect the pain tolerance threshold and pulp vitality of the hypersensitive teeth. 4. Desensitizing effect of laser irradiation for the hypersensitive teeth had been continuing at least 2 weeks. It was concluded that desensitizing of hypersensitive dentine with a Nd:YAG laser is effective and the maintenance of the positive result was more prolonged than the placebo effect.
