The purpose of this study is to obtain some informations for the differential diagnosis of ameloblastoma from dentigerous cyst by analysis of the radiographic findings of these lesions. The author studied age and sex distribution, the site of the lesion, tooth behavior and several radiographic features of ameloblastoma and dentigerous cyst. The material consisted of 65 patients of ameloblastoma and 37 patients of dentigerous cyst. The results were obtained as followings. 1) The incidence was highest in 2nd decade (29.2%) and total 65 cases consists of 35 males (53.8%) and 30 females (46.2%) in ameloblastoma. 62 cases were found in lower jaw (95.4%) and the highest site of occurence of ameloblastoma was mandibular molar. region 27 cases. (41.5%) 2) In 65 cases of amelobl!!stoma, 18 cases were seen in association with tooth and 15 cases (83.2%) out of those were associated with mandibular molar teeth. Mandibular molar were most frequently involved in dentigerous cyst (11/31 cases, 29.7%). 3) (a) 23 cases (35.3%) of tooth resorption were found in ameloblastoma and 11 cases (29.7%) of tooth resorption were found in dentigerous cyst. (b) 15 cases (23.1%) of tooth migration were found in ameloblastoma and 10 cases (27.0%) of tooth migration were found in dentigerous cyst. 4) Several radiographic features. (a) Monolocular type ameloblastoma were seen in 23 cases (35.4) and multilocular type of ameloblastoma were seen in 42 cases (64.6%). Monolocular type of dentigerous cyst were seen in 33 cases (89.2%) and multilocular type was seen in 4 cases. (b) Monolocular type ameloblastoma showed 20 cases (87.0%) of scalloped border but 32 cases (97.0%) of dentigerous cyst showed smooth border. (c) 34 cases (81.0%) of ameloblastoma showed honey-comb appearance, soap-bubble appearance or mixed appearance. but all 4 cases of dentigerous cyst showed multicystic appearance. (d) 12 cases (52.2%) of monolocular type ameloblastoma showed slightly increased radiopacity in surrounding bone, and 22 cases (66.7%) of monolocular type dentigerous cyst showed sharp osteosclerotic border. 27 cases (64.3%) of multilocular type ameloblastoma and 3 cases (75.0%) of multilocular type dentigerous cyst showed no changes in surrounding bone.
Much has been written about the possibility that the lining of a dentigerous cyst might undergo neoplastic transformation to an ameloblastoma which is defined as mural ameloblastoma. Mural ameloblastomas are estimated to constitute 5% of all ameloblastomas. 85% of the mural ameloblastomas were associated with dentigerous cyst, all of which were observed before the age of 30. The frequency of recurrence after simple enucleation of the cyst containing this lesion is considerably lower than that for ameloblastomas in general that are treated in a similar manner. Therefore, further and more extensive surgery is not required for mural ameloblastoma. We diagnosed a cystic appearance of a 17 year-old female as dentigerous cyst and undergone marsupialization. For 2 years of period, no significant change was observed and therefore enucleation was undergone and diagnosed as ameloblastoma. We report this case with a review of related literatures.

Ameloblastoma is a common odontogenic tumor originating from the dental lamina, reduced dental epithelium and rests of Malassez, and represents 10% of all odontogenic tumors of the jaw. Unicystic ameloblastoma is normally encountered in young patients, and often occurs in the mandible, and is particularly associated with an impacted tooth. We encountered an unicystic ameloblastoma arising from a dentigerous cyst after the treatment of a radiolucent lesion on the mandible.
The Auther have diagnosed the 9 cases Dentigerous Ameloblastoma from mandible in Korean. 1) All 9 cases were Korean patients. 2) Clinical Diagnosis of 9 cases were cyst but microscopical Diagnos's of all cases were Dentigerous Ameloblastoma. 3) Patients of all cases were sedated with 100mg. Demerol and anesthetized with 2% Lidocain hydrochloride. The surgical procedure was performed in the usual manner.
Ameloblasotma is slowly growing, locally invasive neoplasm with a potentially destructive behavior. The epithelium of ameloblastoma is thought to have an intrinsic growth potential and has been shown to present a higher rate of proliferation as compared to odontogenic cysts with low local recurrence rate. The molecular mechanisms that regulate the cell growth and invasion of ameloblastoma cells are unknown. Bcl-2 protein, which prevent apoptosis, is expressed in immortalized ameloblastoma cell line(AM-1)(Harada et al 1998). Expression of bcl-2 protein occurs in tooth germs, whose epithelial component may act as the histogenic precursor of ameloblastoma. Bax is considered as a main effector of apoptosis. Bax forms homodimers and also heterodimers with bcl-2. p53 tumor supressor gene participates not only in cell proliferation control but also in induction of apoptosis. The objective of the present study was to evaluate the apoptosis related protein expression in odontogenic cyst and ameloblastoma. A total of 10 dentigerous cysts and 16 ameloblastomas were used in the present study. Dentigerous cyst showed negative or slight positive for p53 and bcl-2 but strongly positive for bax, ameloblastoma, on the other hand, strongly positive for p53 and bcl-2 but weekly positive for bax. Bcl-2 was expressed for ameloblastoma mainly in outer layer or whole layer of epithelium and for dentigerous cyst mainly in basal layer. The difference in expression of apoptosis related protein in dentigerous cyst and ameloblastoma might explain the peculiar aggressive growth pattern of ameloblastoma.

Objectives: This study identifies factors for differential diagnosis among lesions by retrospectively comparing panoramic and cone-beam computed tomography images and analyzing the characteristics of lesions associated with impacted mandibular third molars (IMTs). Materials and Methods: A retrospective cohort study was conducted in patients who simultaneously underwent IMT extraction surgery and related benign tumor resection or cyst enucleation at our institution from 2017 to 2021. To compare the characteristics of each group, two comparative analyses were conducted. The first comparison considered the most frequently observed lesions associated with IMTs: dentigerous cysts, odontogenic keratocysts (OKCs), and ameloblastoma. The second comparison involved placing dentigerous cysts, which have a relatively low recurrence rate, into group A and placing OKC, ameloblastoma, and odontogenic myxoma, which have high recurrence rates, into group B. Results: Significant differences in the size of the lesion were found in the order of ameloblastoma, OKC, and dentigerous cyst (P<0.05). The buccolingual width of ameloblastoma differed significantly from that of the other groups, with no significant difference observed between the OKCs and dentigerous cysts (P=0.083). Conclusion: Patient age and lesion size differed significantly among lesion types associated with IMTs, with younger age and larger lesions for OKCs and odontogenic tumors. OKCs are likely to have a larger mesiodistal width than dentigerous cysts. The buccolingual width of ameloblastomas was larger than those of dentigerous cysts and OKCs.
Ameloblastoma is an aggressive but benign epithelial neoplasm of odontogenic origin, and the occurrence of odontogenic epithelium in the wall of a dentigerous cyst is well-known entity. The presence of ameloblastic proliferation in the walls of odontogenic cysts has been reported for many years. Cahn in 1933 described a case in which he considered an ameloblastoma to have originated in a dentigerous cyst, and numerous other cases of ameloblastomatous proliferation have since been reported. In 1977, Robinson and Martinez described a distinct variant of ameloblastoma in which the response to curettage was found to be favorable with a recurrence rate of 25%. The gross and microscopic features indicated that this variant vas associated with a large cystic cavity with either luminal or mural proliferation of ameloblastic tumor cells, and they referred to this variant as unicystic ameloblastoma. Unicystic ameloblastoma occurs most commonly in the second and third decades of life, which is considerably younger than the average age of discovery for the classical ameloblastoma. For the accurate histopathological diagnosis of the unicystic ameloblastoma, the specimen obtained the excisional biopsy, complete enucleation or incisional biopsy from the multiple site of the lesion. This article provides histopathologic evidence of multilocular unicystic ameloblastoma in which ameloblastic tissue was associated with a dentigerous cyst that was found in a 31-year-old female, and complete radiographic, photographic, and microscopic documentation is presented.

Objectives: The purpose of this retrospective study is to find the differentiating characteristics of cystic and cystic-appearing lesions that involve the impacted mandibular third molar by analyzing panoramic radiographs and computed tomography images, and to aid the preoperative diagnosis. Materials and Methods: Eighty-one patients who had a mandibular cystic or cystic-appearing lesion that involved impacted mandibular third molar and underwent cyst enucleation were included in the study. The preoperative panoramic radiograph and computed tomography findings were analyzed in accordance to the histopathologic type. Results: Most of the cystic lesions containing the mandibular third molar were diagnosed as a dentigerous cyst (77.8%). The occurrence of mesio-distal displacement of the third molar was more frequent in the odontogenic keratocyst (71.4%) and in the ameloblastoma (85.7%) than in the dentigerous cyst (19.1%). Downward displacement was primarily observed in each group. Odontogenic keratocyst and ameloblastoma showed more aggressive growth pattern with higher rate of bony discontinuity and cortical bone expansion than in dentigerous cyst. Conclusion: When evaluating mandibular cystic lesions involving the impacted mandibular third molar, dentigerous cyst should first be suspected. However, when the third molar displacement and cortical bone absorption are observed, then odontogenic keratocyst or ameloblastoma should be considered.

Adenomatoid odontogenic tumor(AOT) is an infrequent odontogenic tumor which arise in the jaw. It was considered as a variant of ameloblastoma. The adenomatoid odontogenic tumor is clearly benign and, in contrast to the ameloblastoma, present a very low recurrence. It most often appears in the canine region of the maxilla. The adenomatoid odontogenic tumor is frequently asymptomatic, however it may cause painless swelling. The radiological findings of adenomatoid odontogenic tumor frequently share characteristics of dentigerous cyst and unicystic ameloblastoma. Conservative surgical enucleation and curettage are the treatment of choice. In this case a 10-year-old child was presented with mandibular right lateral incisor in unerupted. Radiographically, the tooth was impacted and a radiolucency was seen in the area. The lesion was enuclated without extraction of the tooth. Bracket was attached on the tooth for orthodontic extrusion installed. Histopathologically adenomatoid odontogenic tumor was revealed.
The dentigerous cyst involving maxilla is rare disease in Otolaryngologic field. A case of 22-year-old male patient involving dentigerous cyst of the maxilla with chronic inflammatory signs has been presented. The occurrence of odontogenic epithelium in the wall of dentigerous cyst is a well known entity. This epithelium usually remains inactive and does not have clinical significance, but these small inactive islands of epithelium may be stimulated, resulting in chronic inflammation, ameloblastoma and squamous cell carcinoma. Therefore correct diagnosis and proper treatment are very important. The authors have recently experienced a case of dentigerous cyst causing a mild disfigulation of face. The cyst was surgically enucleated through sublabial approach.
