Two-body wear behavior of human enamel versus monolithic zirconia, lithium disilicate, ceramometal and composite resin

  • PURPOSE. To investigate and compare the surface roughness (SR), weight and height of monolithic zirconia (MZ), ceramometal (CM), lithium disilicate glass ceramic (LD), composite resin (CR), and their antagonistic human teeth enamel. MATERIALS AND METHODS. 32 disc shaped specimens for the four test materials (n=8) and 32 premolars were prepared and randomly divided. SR, weight and height of the materials and the antagonist enamel were recorded before and after subjecting the specimens to 240,000 wear-cycles ($49N/0.8Hz/5^{\circ}C/50^{\circ}C$). SR, height, weight, and digital microscopic qualitative evaluation were measured. RESULTS. CM ($0.23+0.08{\mu}m$) and LD ($0.68+0.16{\mu}m$) exhibited the least and highest mean difference in the SR, respectively. ANOVA revealed significance (P=.001) between the materials for the SR. Paired T-Test showed significance (P<.05) for the pre- and post- SR for all the materials. For the antagonistic enamel, no significance (P=.987) was found between the groups. However, the pre- and post- SR values of all the enamel groups were significant (P<.05). Wear cycles had significant effect on enamel weight loss against all the materials (P<.05). CR and MZ showed the lowest and highest height loss of 0.14 mm and 0.46 mm, respectively. CONCLUSION. MZ and CM are more resistant to SR against the enamel than LD and CR. Enamel worn against test materials showed similar SR. Significant variations in SR values for the tested materials (MZ, LD, CM, and CR) against the enamel were found. Wear simulation significantly affected the enamel weight loss against all the materials, and enamel antagonist against MZ and CM showed more height loss.

치과용 저작 매스티케이터의 개발 (Development of Dental Chewing Masticator)

  • Dental chewing masticator, which is an essential device for evaluating the wear of dental resin and the interfacial failure between the filling resin and enamel of tooth used in conservative dentistry, was developed. This dental chewing masticator mimics the chewing motion and loading by adapting DC motor and rotary cam system. Chewing loading of 49N was imposed by computer-displacement control, loadcell, LM guide, and spring system. Extracted tooth was fixed into a holding jig, and this jig was mounted with rubber pad on the $15^{\circ}$inclined surface to consider the lateral movement of periodontal ligament. A water bath was installed for providing the environment of inside mouth and for circulating the $5^{\circ}C-55^{\circ}C$ water to evaluate the effect of hydrothermal cycling on the damage of resin filled teeth during long-term chewing activity.

  • The purpose of this study is to report a clinical case of a patient whose vertical dimension of occlusion was lost. There are many reasons for the loss of vertical dimension and one of those can be a tooth wear problem. Thus in this article we will review shortly types of tooth wear: attrition, abrasion, and erosior. And layour the basic philosophy and principles in regaining the esthetics and function of a patient's oral condition with a history of psychological wear, through full mouth restorations.

Comparison of retentive force and wear pattern of Locator® and ADD-TOC attachments combined with CAD-CAM milled bar

  • PURPOSE. The purpose of this study was to investigate changes in retention and wear pattern of Locator® and ADD-TOC attachments on a digital milled bar by performing chewing simulation and repeated insertion/removal of prostheses in fully edentulous models. MATERIALS AND METHODS. Locator (Locator®; Zest Anchors Inc., Escondido, CA, USA) was selected as the control group and ADD-TOC (ADD-TOC; PNUAdd Co., Ltd., Busan, Republic of Korea) as the experimental group. A CAD-CAM milled bar was mounted on a master model and 3 threaded holes for connecting a bar attachment was formed using a tap. Locator and ADD-TOC attachments were then attached to the milled bar. Simulated mastication and repeated insertion/removal were performed over 400,000 cyclic loadings and 1,080 insertions/removals, respectively. Wear patterns on deformed attachment were investigated by field emission scanning electron microscopy. RESULTS. For the ADD-TOC attachments, chewing simulation and repeated insertion/removal resulted in a mean initial retentive force of 24.43 ± 4.89 N, which were significantly lower than that of the Locator attachment, 34.33 ± 8.25 N (P < .05). Amounts of retention loss relative to baseline for the Locator and ADD-TOC attachments were 21.74 ± 7.07 and 8.98 ± 5.76 N (P < .05). CONCLUSION. CAD-CAM milled bar with the ADD-TOC attachment had a lower initial retentive force than the Locator attachment. However, the ADD-TOC attachment might be suitable for long-term use as it showed less deformation and had a higher retentive force after simulated mastication and insertion/removal repetitions.

용융침투법으로 제조한 알루미나-유리 복합체의 내마모 특성 (Wear Behavior of Alumina-glass Composites Prepared by Melt Infiltration)

  • 인공치관용 알루미나-유리 복합체를 in vitro 분위기인 $37^{\circ}C$) 인공타액 하에서 49N~196N의 하중조건 하에서 ball-on-disc 형태로 120rpm의 회전속도로 직경 14mm인 트랙을 $10^{6}$) 횟수까지 내마모거동을 조사하기 위하여 내마모실험을 수행하였다. 하중이 49N,98N 196N으로 증가함에 따라, 마찰계수는 0.025로 일정하였지만 마모율은 2.18${\times}$$10^{-9}$$mm^{3}$/N.m, 3.37${\times}$$10^{-9}$$mm^{3}$/N.m, 2.35${\times}$$10^{-6}$$mm^{3}$/N.m로 증가하였다. 실험결과, 알루미나-유리 복합체의 마모거동은 마모길리 44km까지 전형적인 흡착마모로 치열교정용 세라믹 브라켓으로 적합하였다.

풀지르코니아 수복물의 실용성과 전망 (Practicality and prospect of full-zirconia restoration)

  • 지르코니아 폴리크리스탈은 (Y-TZP) 다른 치과용 세라믹에 비해 기계적 강도와 파절저항성이 우수하다. 지르코니아 기반 세라믹은 캐드캠 시스템을 바탕으로 고정식 수복물 제작에 성공적으로 소개되어 왔다. 이는 고정식 수복물을 위한 금속 구조물의 임상적 대안으로 제시되었다. 지르코니아 수복물의 가장 흔한 합병증은 상부 도재의 파절이다. 다른 옵션은 고도로 반투명한 지르코니아를 전부지르코니아 고정식 수복물로 사용하는 것이다. 본 논문에서는 전부지르코니아 수복물의 장점, 지르코니아 수복물의 임상적용, 지대치 삭제, 대합치 마모, 지르코니아와 레진시멘트와의 접착, 그리고 기공소와의 의사소통에 관하여 보고하였다.

  • 노인 환자 구강의 생리적 변화 중 하나인 생리적 치아 마모는 일반적으로 치료를 필요로 하지 않으나 교합 부조화, 저작기능의 저하, 치수 노출 등 구강 내 문제 발생 시 보철 수복을 필요로 한다. 마모된 치열을 지닌 환자의 보철 수복 시 우선적으로 고려되는 사항들 중 하나가 바로 수직고경에 대한 고려이다. 이에 정확한 진단 및 분석과 예지성 있는 치료계획을 위한 악간관계에 대한 올바른 판단이 요구되며, 노인 환자의 치과 치료를 감안한 단계별 치료접근을 필요로 한다. 본 증례는 93세 남자 환자로 전반적인 치아 마모 및 기존 고정성 보철물의 동요도를 보이고 있었다. 진단 및 평가 후 수직교합 고경을 유지한 상악 구강회복술을 시행하였으며, 기능 및 형태적으로 만족할만한 결과를 보여 보고하는 바이다.

  • The development of dental materials has widened the scope of materials by changes in processing methods. CAD/CAM processing enables the use of zirconia as a dental material. Recent esthetic materials development has been made. For aesthetic purposes, a block for CAD/CAM processing by mixing polymer and ceramic materials are fabricated. However there is no guideline of how these materials should be used in actual clinical practice. Mechanical properties, wear and clinical studies were reviewed.

Evaluation of intaglio surface trueness, wear, and fracture resistance of zirconia crown under simulated mastication: a comparative analysis between subtractive and additive manufacturing

  • PURPOSE. This in-vitro analysis aimed to compare the intaglio trueness, the antagonist's wear volume loss, and fracture load of various single-unit zirconia prostheses fabricated by different manufacturing techniques. MATERIALS AND METHODS. Zirconia crowns were prepared into four different groups (n = 14 per group) according to the manufacturing techniques and generations of the materials. The intaglio surface trueness (root-mean-square estimates, RMS) of the crown was measured at the marginal, axial, occlusal, and inner surface areas. Half of the specimens were artificially aged in the chewing simulator with 120,000 cycles, and the antagonist's volume loss after aging was calculated. The fracture load for each crown group was measured before and after hydrothermal aging. The intaglio trueness was evaluated with Welch's ANOVA and the antagonist's volume loss was assessed by the Kruskal-Wallis tests. The effects of manufacturing and aging on the fracture resistance of the tested zirconia crowns were determined by two-way ANOVA. RESULTS. The trueness analysis of the crown intaglio surfaces showed surface deviation (RMS) within 50 ㎛, regardless of the manufacturing methods (P = .053). After simulated mastication, no significant differences in the volume loss of the antagonists were observed among the zirconia groups (P = .946). The manufacturing methods and simulated chewing had statistically significant effects on the fracture resistance (P < .001). CONCLUSION. The intaglio surface trueness, fracture resistance, and antagonist's wear volume of the additively manufactured 3Y-TZP crown were clinically acceptable, as compared with those of the 4Y- or 5Y-PSZ crowns produced by subtractive milling.

수종 치관수복재료의 마모도에 관한 연구 (A STUDY ON THE WEAR OF DENTAL RESTORATIVE MATERIALS)

  • The purpose of this study was to compare the wear characteristics of Type III Gold Alloy (Degular C : Degussa AG), VMK 95 (Vita Zahnfabrik), Vitadur Alpha (Vita Zahnfabrik), IPS Empress (Ivoclar-Vivadent) and Targis (Ivoclar- Vivadent). Ten samples of each material were abraded against enamel of extracted incisors for 160,000 cycles on the wear machine. The depth of wear scars on enamel was analized with profilometer (Alpha-step 200;Tencor). And the wear of material specimen was determined with micrometer (PB-IB;Mitutoyo Co. Japan). The results obtained were as follows : 1. Type III gold alloy ($2.47{\mu}m$) and Targis ($5.89{\mu}m$) caused less wear of enamel specimens compared to VMK 95 ($75.59{\mu}m$), IPS Empress ($78.60{\mu}m$) and Vitadur Alpha ($78.89{\mu}m$) (p<0.05). 2. The wear of material specimen when opposing enamel was the lowest in type III gold alloy ($3.0{\mu}m$), followed by Targis ($69.8{\mu}m$), IPS Empress ($148.4{\mu}m$). VMK 95 ($298.0{\mu}m$) and Vitadur Alpha ($300.8{\mu}m$ exhibited the most severe wear against enamel (p<0.05).
