Porcelain 제작시 고려 사항 (Considerations in Porcelain Fused Metal Restorations)

  • Although various kinds of porcelain restorations, including full porcelain jacked crowns and laminate crowns, are used for Esthetic Dentistry nowadays, the most widely used one so far is porcelain metal restoration. It goes without saying that shade matching in porcelain-metal restoration is much more difficult than in full ceramic veneer restoration because of the metal substructure limitation of the porcelain to metal restoration and then let it go even when shade matching is a little bit unsatisfactory. I think a more satisfactory shade matching can be achieved by developing more elaborate techniques, by selecting proper metal and porcelain and by enabling more detailed communication between the dentists and the dental technicians. Some considerations will be made in an attempt to achieve more successful outputs.

구 보철물 치경부 변색의 심미 재수복 (- Esthetic Retreatment of Old Restorations with Cervical Discolorations -)

  • As patients have concerned the esthetics of dental restorations, the porcelain fused to metal restorations have gain their popularity due to their strength and esthetic appearance for many years. However, metal collar in subgingival level of porcelain fused to metal restoration often causes black shadows. Inadequately positioned metal collar causes plaque depositions, gingival inflammations, black shadows, and discolorations of margin area. Those problems can be avoided by a clear finishing line, well-fabricated provisional restorations with precise margin and fine polishing, collarless porcelain fused to metal restoration, and all ceramic restoration. In this case report, collarless porcelain fused to metal restoration was used to treat cervical discoloration from old restorations.

도재 라미네이트 비니어를 이용한 상악 전치부의 심미적 수복 (Esthetic restoration of upper anterior teeth by porcelain laminate veneer)

  • With the understanding of tooth crown biomechanics and the progress of dentin adhesives, bonded porcelain restorations including a porcelain laminate veneer present an extended spectrum of indications for anterior teeth. Porcelain laminate veneer as a restoration offers the conservative solution that balances the functional and esthetic needs of the anterior dentition. Porcelain's stiffness, its surface characteristics, and the biomechanical strength achieved through bonding to tooth surface enable the restoration of the tooth as a whole supporting occlusal force and masticatory function. Namely, the optical effects inherent in the tooth and the lifelike features of the porcelain make that this restoration approaches the ultimate in esthetic satisfaction for both the dentist and the patient. A 49-year-old female patient with the incisal discoloration of upper central incisors and black triangle between the central incisors was referred to correct her esthetic problems with prosthodontic approach. The patient was satisfied with two porcelain laminate veneers that were made according to prof. Magne and Belser's recommendation.

지르코니아 수복물의 접착을 위한 임상 가이드 (Clinical Guide for Adhesion of Zirconia Restoration)

  • In case of esthetic restorative procedure with zirconia restoration, we have to use resin cement because of not only just for retention but also esthetic reason. In such a clinical situation, we have to consider two bonding interfaces, one is tooth surface to resin cement and the other is zirconia surface to resin cement. There is well established bonding protocol between tooth surface to resin cement, but bonding protocol of zirconia surface to resin cement is still controversial. In scientific point of view, there are two mechanism for bonding of zirconia restoration.. One is mechanical retention and the other is chemical adhesion. However, we have three different options for bonding of zirconia restoration in clinical situation; 1) Tribo-chemical coating with silica and silane coupling agent 2) Zirconia primer with phosphate chemistry 3) Self-adhesive resin cement with phosphate chemistry.

국산 'Hi-Pol' Enamel Bond System의 변연누출에 관한 실험적 연구 (MARGINAL LEAKAGE TEST ON 'Hi-Pol' ENAMEL BOND SYSTEM)

  • The purpose of this study was to evaluate the marginal sealing ability of 'Hi-Pol' Enamel Bond system. Using freshly extracted human teeth and 2% acquous methylene blue, the marginal leakage of dye in restorative materials such as 'Hi-Pol' Enamel Bond system, Hi-Pol, Adaptic, Nuva system, Epolite and Amalgam were investigated at 37℃ and under temperature cycling in range of 4℃-60℃. the results as follows; 1. All filling materials showed some degree of marginal penetration by 2% methylene blue dye. 2. 'Hi-Pol' Enamel Bond system revealed the least marginal penetration by 2% methylene blue as Nuva did at 37℃, but under temperature cycling in range of 4℃-60℃ it showed the greatest marginal penetration. 3. Hi-Pol restoration showed a tendency the similar to the other composite restoration. 4. Nuva system had the most effective marginal sealing ability either at 37℃ and under temperature cycling.

Iatrogenic subcutaneous cervicofacial emphysema with pneumomediastinum after class V restoration

  • Subcutaneous facial emphysema after dental treatment is an uncommon complication caused by the invasion of high-pressure air; in severe cases, it can spread to the neck, mediastinum, and thorax, resulting in cervical emphysema, pneumomediastinum, and pneumothorax. The present case showed subcutaneous cervicofacial emphysema with pneumomediastinum after class V restoration. The patient was fully recovered after eight days of conservative treatment. The cause of this case was the penetration of high-pressure air through the gingival sulcus, which had a weakened gingival attachment. This case indicated that dentists should be careful to prevent subcutaneous emphysema during common dental treatments using a high-speed hand piece and gingival retraction cord.

  • pH varies in the oral environment and on tooth surfaces. The surface of a plaque covered resinous restoration has a low pH. In human dental plaque a number of acids are produced by bacterial metabolism. The predominant acids are acetic, propionic and lactic acids. These acids induced a surface swelling of restorative resin. The swelling cause formation of pores inside the resin matrix from which organic substances can be released, resulting in a mass loss. The purpose of this study was to qualitatively and quantitatively analyse the leached monomers of dental composites after storage in acetate buffer solution as a function of time by means of high performance liquid chromatography(HPLC) / mass spectrometer and further determine whether pH and time has any influence on the degradation behavior of composite restoration.(omitted)

  • CAD/CAM이 치과계에 도입된지도 벌써 꽤 많은 시간이 흘렀음에도 불구하고 현재의 CAD/CAM은 여전히 비효율적이거나 또는 비심미적이다. '효율성' 과 '심미성'이란 두 단어는 지르코이나 보철물에 있어서는 양극에 위치하게 된다. 효율적이기 위해 도재소성없이 monolithic한 지르코니아 보철물을 만들다 보면 비심미적인 경우가 대부분이고, 거꾸로 심미적이기 위해 도재소성 과정을 거치다보면 여전히 사람의 손을 타게되는 비효율성을 피할 수 없기 때문이다. 이 글에서는 지르코니아 보철물의 현주소와 이와 관련된 몇가지 증례들을 보여드리고자 한다.

Comparative analysis of transmittance for different types of commercially available zirconia and lithium disilicate materials

  • PURPOSE. Translucency and colour stability are two most important aspects for an aesthetic dental restoration. Glass ceramic restorations are popular amongst clinicians because of their superior aesthetic properties. In the last decade, zirconia has generated tremendous interest due to its favorable mechanical and biological properties. However, zirconia lacks the translucency that lithium disilicate materials possess and therefore has limitations in its use, especially in esthetically demanding situations. There has been a great thrust in research towards developing translucent zirconia materials for dental restorations. The objective of the study was to evaluate and compare the transmittance of a translucent variant of zirconia to lithium disilicate. MATERIALS AND METHODS. Two commercially available zirconia materials (conventional and high translucency) and 2 lithium disilicate materials (conventional and high translucency) with standardized dimensions were fabricated. Transmittance values were measured for all samples followed by a microstructural analysis using a finite element scanning electron microscope. One way analysis of variance combined with a Tukey-post hoc test was used to analyze the data obtained (P=.05). RESULTS. High translucency lithium disilicate showed highest transmittance of all materials studied, followed by conventional lithium disilicate, high translucency zirconia and conventional zirconia. The difference between all groups of materials was statistically significant. The transmittance of the different materials correlated to their microstructure analysis. CONCLUSION. Despite manufacturers' efforts to make zirconia significantly more translucent, the transmittance values of these materials still do not match conventional lithium disilicate. More research is required on zirconia towards making the material more translucent for its potential use as esthetic monolithic restoration.

Trend analysis of prosthodontic treatment modality between 2005 and 2008 in Seoul National University Dental Hospital

  • PURPOSE. The aim of this article is to analyze the preference for treatment modality of dentists. MATERIAL AND METHODS. Data of 20,038 patients was involved. Data analysis were done by distribution according to the various kinds of prosthesis, including complete denture, removable partial denture, fixed partial denture, implant-supported dental prosthesis as well as distribution according to the professional titles of the dentists finishing the treatment, including resident and professors. RESULTS. The number of cases of dental prosthesis increased year by year. 61.06% of the patients accepted fixed partial denture restoration. The number of patients who accepted implant supported restoration is also increasing year by year. The number of complete denture, implant-supported dental prosthesis finished by professors was larger than that done by residents, while it was contrary for removable partial denture, fixed partial denture, and the difference was statistically significant (P < .05). CONCLUSION. Professors and residents have some difference in the categorization of prosthesis finished. Fixed partial denture and implant-supported dental prosthesis are preferred.
