  • An immediate complete denture is considered as restoration for lost natural teeth, which is fabricated following the extraction of the remaining teeth. Current esthetics and function can be retained by using immediate denture without edentulous period. However, the major disadvantages of immediate denture relate to the difficulties associated with taking accurate definitive impression and predicting the results of immediate denture. In this case report, the Campagna tray technique was used to take the final impression in a 49-year-old male patient presented with all remaining teeth diagnosed as hopeless teeth. Surgical templates were used for alveoloplasty after extraction. The immediate complete dentures were then delivered. The clinical assessments of immediate dentures showed good esthetic and functional outcomes. The patient showed high level of satisfaction.

  • Porcelain is considered to be one of the materials of choice for restoration where esthetics is of concern. But porcelain surface without final glazing treatment may induce undesirable results such as inflammatory response on adjacent soft tissues due to plaque accumulation and increased wear of opposing teeth. Therefore, rough porcelain surface must be smoothened by final glazing treatment or chairside polishing procedure. The purpose of this study was to compare the surface roughness among self-glazed, overglazed and polished porcelain with various polishing kit, and to detect which phase of polishing is optimal in clinic. Specimens were fabricated with Vita VMK porcelain. The surface treatment of each group was performed as follows. Group 1 : overglazing treatment Group 2 : self-glazing treatment Group 3 : polishing with the Truluster Polishing System for Porcelain(Brasseler, U.S.A.) Group 4 : polishing with the Exa Cerapol Adjustment kit (Edenta dental products, Switzerland) followed by finishing with diamond-filled polishing paste Group 5 : polishing with the Shofu Porcelain Adjustment kit (Shofu inc., Japan) followed by finishing with diamond-filled polishing paste. At each polishing steps, the measurement of Ra and Rq values were performed, and the surface was examined by scanning electron microscope. The results were as follows : 1. Overglazing treatment brought smoother surface than self-glazing treatment. 2. Polishing systems without porcelain polishing paste did not make better result than self-glazing treatment. 3. Polishing system with porcelain polishing paste made similar result to overglazing treatment. 4. Applying diamond-filled polishing paste after using polishing system which has no porcelain polishing paste produced surface as smooth as overglazing treatment does.

  • Tooth transposition is the phenomenon in which two teeth in the dental arch are exchanged. The tooth most frequently involved in transposition is the permanent maxillary canine, especially with the first premolar. The etiology of transposition is still an enigma. Interchange of tooth germs, migration during eruption, genetic factor, local pathologic factors may be suggested as possible etiologic factors. There are three main options for treatment, which are the extraction of one of the transposed teeth, the alignment in the transposed position, the orthodontic movement of the teeth to their correct positions. If complete transposition occurred especially in mandible, alignment of teeth in the transposed position followed by occlusal reduction and esthetic restoration is usually recommended. The presented two-case report will describe the maxillary canine to the first premolar and to lateral incisor transposition with associated peg lateralis and retained primary canines. Although alignment the transposed teeth in original position is ideal treatment, it companies many complication as root resorption, alveolar bone destruction, gingival deheisence, etc. Therefore the treatment procedures in these case was relative cost-benefit effective method to both clinician and patients.

  • Statement of problem : Chronic implant screw loosening remains a problem in restorative practices. Some implant manufactureres have introduced abutment screws with treated material, surfaces and macrostructures in an effort to reduce potential loosening. Purpose : This study evaluated the materials and loading cycles on detorque value after dynamic continous fatigue test in the sinulated conditions of posterior single restoration. Material and method : Fourteen of each of the following abutment screws - titanium alloy, gold alloy, gold-tite, and titanium alloy modified - were used in test. SEM is used to verify macrostructures of each screws. $ZrO_2/Al_2O_3$ composite abutment was tightened on $4{\times}10.0mm$ titanium external implant at 30 Ncm. Cyclic loading machine delivered dynamic loading forces between 20 and 320N for 100,000, 200,000, 300,000, 500,000, and 1,000,000 cycles at frequencies 14Hz. Torque and detorque value after loading was measured. Results : All measued screws had different screw length and thread form. Titanium modified screw had greater detorque value than others before and after cyclic loadings(p<0.05). All abutment screws had no significant change in mean percentage of detorque value after loading to initial value after less than 500.000 cyclic loadings, but significant lower value after 1,000,000 cycles(p<0.05). Conclusion : Within limintations of this study all abutment screws may be loosend after about 1 year use. Annual check-up is nessasary to prevent screw loosening.

  • Purpose: This study is to evaluate the efficacy of the autogenous tooth bone graft material, clinically and radiologically, as related to implant installation. Methods: In oral and maxillofacial surgery department of Ajou University Hospital, guided bone regeneration (GBR), implant placement combined with GBR, sinus graft, implant placement combined with sinus graft, and defect filling were performed in 46 patients, using autogenous tooth bone. Among these, 66 implants were inserted with autogenous tooth bone. Implant stability quotient (ISQ) was measured by Osstell Mentor (Integration Diagnostics, Goteborg, Sweden) on 39 implants on the operation date and 4 months later, and on 21 implants 9months on the average at the final setting of restoration. Twenty-eight implants with GBR and sinus graft (GBR group: n=14, sinus graft group: n=14) were evaluated radiologically to measure the resorption of grafted autogenous tooth bone after loading. Results: The average initial stabilization of the installed implants was 67 ISQ, and the average secondary stabilization at 4 months later was 76. The average bone loss of GBR group as measured 8.0 months after application of prosthesis loading was 0.29 mm and the average bone loss of the sinus graft group as measured 7.6 months after application of prosthesis loading was 0.66 mm, respectively. In the histological assessment, formation of the new bone and continuous trabecular bone pattern was identified around autogenous tooth bone. Conclusion: Based on these results, we concluded that autogenous tooth bone is an excellent bone graft material that can substitute the autogenous bone.

  • Recently as the esthetic concerns about dental prosthesis have been increased, the nonmetallic post and core systems have been introduced clinically to improve the color and the low optical transmittance of conventional casting metal post and core systems. The purpose of this study was to compare and analyze the optical transmittance and the shade changes of all-ceramic crowns with two nonmetallic post and core systems. The experimental groups were classified as follows : Total 27 specimens(9 samples in each group) were evaluated. Group I : Natural teeth as a control group Group II : CosmoPost and $Empress^{(R)}$ core (Ivoclar-Vivadent, Liechtenstein) Group III : $LIGHT-POST^{TM}$ and $LIGHT-CORE^{TM}$ (Bisco, U.S.A.) In all group, all-ceramic crowns were fabricated with the same shade of IPS $Empress^{(R)}$ II (Ivoclar-Vivadent, Liechtenstein) after abutment preparation, and then two kinds of spectrophotometers, UV 3101 PC (Shimadzu, Japan) and CM 503i (Minolta Co. Ltd. Japan), were used to measure the optical transmittance and the color value. The following results were obtained : 1. The optical transmittance of each group without all-ceramic crowns, was siginificantly decreased in order of Group III, I and II (p<0.05). 2. The optical transmittance of each group with all-ceramic crowns cemented, were siginificantly decreased in order of Group I, II and III (p<0.05). 3. In comparison with the shade of all-ceramic crowns, there were no significant differences between groups regardless of the cementation. 4. In comparison with the shade changes in each group after the cementation of all-ceramic crowns, there were no significant differences between groups. From the above results, it is thought that the selection among materials used in this study doesn't influence greatly the esthetic restoration, because the differences of optical transmittance were so small that those were clinically insignificant.

  • Excessive tooth wear can lead to decrease in occlusal vertical dimension and can cause pathological changes in the oral environment and masticatory system. When recovering occlusal vertical dimension and occlusion, accurate diagnosis and analysis are essential. This clinical case describes a 75-year-old woman with severely worn dentition due to loss of the posterior support. Full mouth rehabilitation with occlusal vertical dimension increment was planned. Clinical and radiographic examinations, occlusal vertical dimension evaluation, and diagnostic wax-up were performed and patient adaptability was evaluated using provisional restorations. As for definitive restoration, considering economic condition of the patient, removable partial denture was fabricated and solitary implants were placed in the mandibular left and right posterior region to increase support and retention of the removable partial denture. During one year of follow-up, functional and esthetic outcomes were observed satisfactory.

  • Loss of multiple teeth by multiple dental caries, traumatic influence or hereditable disease can cause many problem in children. Using removable appliance can be used for treatment of this problem. But conventional removable appliance has limits of retention by wire-clasp, non esthetic appearance, difficulty in pronunciation by its volume. Flexible denture using $Valplast^{(R)}$ system could be a excellent partial denture for restorarion in child because of various advantage such as retention from soft tissue, natural esthetic aspect, biocompatibility, durability to compare with conventional acrylic appliance. The present case report describe properties and consideration of flexible denture using Valplast $system^{(R)}$ in comparison with conventional acrylic appliance and report successful restoration of child using Valplast $system^{(R)}$.

  • Supernumerary tooth is developed from overgrowth of dental laminar and is mainly observed in the maxillary anterior area, named 'mesiodens. Mesiodens cause many problems such as malposition of permanent anterior tooth, diastema, formation of cyst, and eruption to oral or nasal cavity. Extraction, therefore, is generally recommended choice of treatment. This case report describes that the patient is a 9 years and 8 months old boy, whose unilateral maxillary incisor was missing due to trauma, and he had a mesiodens. We practiced autotransplantation of the mesiodens to the missing maxillary central incisor area and then restored with prosthetics, substantially kept out alveolar bony resorption and provided esthetic appearance. Mesiodens was too small to be used as an abutment because its length is totally 14mm, so that poor prognosis was anticipated due to bad crown/root ratio. However, we observed clinically acceptable result during the observation period of 8 months after operation.

  • This study analyzed the longevity of preformed metal crowns (PMCs) in first permanent molars and evaluated factors influencing their survival during a long-term follow-up period. In all, 115 first permanent molars treated with PMCs between June 2008 and June 2018 were retrospectively analyzed. The overall combined success rate for the study group was 84.3%. The 5-year survival rate was 82.8%. Multivariate Cox regression analyses identified distal cavities and mandibular PMC placement as risk factors for restoration failure. Careful placement of PMCs at the final try-in stage augments the longevity of the crown.
