Purpose: The purpose of this study was analysis of quality of fixed prostheses fabricated by dental CAD/CAM system. Methods: The ten same cases of stone models were manufactured by dental scannable model, and stone models were scanned using the dental scanner for changing digital model. Ten digital models were completed. The design of zirconia core for zirconia based all-ceramic crown was conducted by the dental CAD program. The samples were fabricated using the pre-sintered zirconia block by dental milling machine. Marginal gaps were analyzed using by silicone replica technique at the eight parts for quality analysis of samples. One-way ANOVA was used for statistical analysis(${\alpha}=0.05$). Results: The mean for marginal gaps were $93.2{\sim}99.1{\mu}m$ at the eight parts. One-was ANOVA didn't show significant differences(p=0.089). Conclusion: The fixed dental prostheses fabricated by dental CAD/CAM were within clinically acceptable range.
STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.
Purpose: The purpose of this study was to compare the clinical outcomes of lithium disilicate ceramic pressed zirconia prostheses and monolithic zirconia prostheses and to investigate the complications after two years of follow-up in posterior edentulous site. Materials and methods: A total 17 patients (male: 12, female: 5) were treated with 60 posterior fixed implant-supported prostheses (LP. lithium disilicate ceramic pressed zirconia prostheses: n = 30, MZ. monolithic zirconia prostheses: n = 30). After 24-month, clinical examination of Implant survival rate, marginal bone resorption, probing depth, plaque index, bleeding index, calculus and complications were evaluated. Results: There were no failed implants and all implants were normal in function without mobility. Marginal bone resorption was lower in LP group than MZ group at 12-month (P < .05), and 12-month probing depth and calculus deposit in LP group were significantly higher than MZ group (P < .05). Most common complications in MZ were marginal bone resorptions more than 1.5.mm and 2 chipping occurred in LP group. Conclusion: Within the limitations of the present study, lithium disilicate ceramic pressed zirconia is considered as a predictable treatment option as much as monolithic zirconia in posterior fixed implant-supported prostheses.

PURPOSE. The purposes of this study were to evaluate the marginal and internal gaps, and the potential clinical applications of three different methods of dental prostheses fabrication, and to compare the prostheses prepared using the silicone replica technique (SRT) and those prepared using the three-dimensional superimposition analysis (3DSA). MATERIALS AND METHODS. Five Pekkton, lithium disilicate, and zirconia crowns were each manufactured and tested using both the SRT and the two-dimensional section of the 3DSA. The data were analyzed with the nonparametric version of a two-way analysis of variance using rank-transformed values and the Tukey's post-hoc test (${\alpha}=.05$). RESULTS. Significant differences were observed between the fabrication methods in the marginal gap (P < .010), deep chamfer (P < .001), axial wall (P < .001), and occlusal area (P < .001). A significant difference in the occlusal area was found between the two measurement methods (P < .030), whereas no significant differences were found in the marginal gap (P > .350), deep chamfer (P > .719), and axial wall (P > .150). As the 3DSA method is three-dimensional, it allows for the measurement of arbitrary points. CONCLUSION. All of the three fabrication methods are valid for measuring clinical objectives because they produced prostheses within the clinically acceptable range. Furthermore, a three-dimensional superimposition analysis verification method such as the silicone replica technique is also applicable in clinical settings.
This case report describes a dynamic digital esthetic rehabilitation procedure that integrates a new three-dimensional augmented reality (3D-AR) technique to treat a patient with multiple missing anterior teeth. The prostheses were designed using computer-aided design (CAD) software and virtually trialed using static and dynamic visualization methods. In the static method, the prostheses were visualized by integrating the CAD model with a 3D face scan of the patient. For the dynamic method, the 3D-AR application was used for real-time tracking and projection of the CAD prostheses in the patient's mouth. Results of a quick survey on patient satisfaction with the two visualization methods showed that the patient felt more satisfied with the dynamic visualization method because it allowed him to observe the prostheses directly on his face and be more proactive in the treatment process.
Purpose: This study aimed to assess the quality of dental prostheses printed by digital light-processing (DLP) technology. Methods: Ten experimental models were prepared. The ten specimens that were printed by DLP technology constituted the DLP group. The ten specimens that were produced in the same model by the casting method constituted the control group. The marginal gaps of the 20 specimens produced were measured. These gaps were measured by a silicon replica technique at two abutments of the specimen. Therefore, 20 marginal gaps were measured in each group. An independent sample t-test was performed to compare the marginal gaps measured in the two groups (α=0.05). Results: According to the results of the measurement, there was a significant difference between the mean marginal gap of the control group (78.8 ㎛) and that of the DLP group (91.5 ㎛), p<0.001. Conclusion: Although the mean marginal gaps of dental fixed prostheses produced by the DLP method was higher than the mean marginal gap of those produced by the casting method, it was considered to be within the clinical threshold value suggested by some previous studies.
In the past, restoration of implant crown, ready-made abutment produced by implant manufacturer could only be used. Using straight, angled abutment, there was a limit in adaptation multiple implants. Recently, with the development of implant and CAD/CAM technology, CAD/CAM customized abutment use has become possible which is different from the past when restoration was possible with only prefabricated abutment. Not only it makes emergence profile possible which is similar to natural teeth, but also it makes insertion path possible on CAD in multiple implant restorations. However, on anterior teeth which dental esthetics is very important, another restorations which are formed with natural colored gingiva area could be required. Titanium-based zirconia prostheses which have titanium connection and zirconia structure from 1mm above fixture platform are alternative. Therefore, the purpose of this review is to analyze the characteristics, advantages and disadvantages of the abutment which is used in multiple implant restorations, and to choose right abutment when clinical trials.
Anterior maxillary teeth play an important role in determining a person's first impression and facial profile. Implant surgery in esthetic area requires more careful diagnosis, treatment planning, surgery, and prosthetic restoration than in posterior area. To avoid complications in surgery and prosthetic restoration for implants in esthetic area, accurate diagnosis and appropriate case selection become very important. If you have decided to restore the area with implant prosthesis, you have to know exactly where to place an implant. I will discuss the ideal implant position in terms of mesio-distally, apico-coronally, labio-palatally, and implant angulation. And I would like to point out the selection of fixture diameter & length for anterior implant. Finally, a clinical implant prosthesis case in maxillary central incisor will be shown. In conclusion, for superior esthetic outcome in anterior implant prostheses, we must understand the patient's anatomic condition and know our ability.

Purpose: The purpose of this study was to assess precision of crown prostheses that were fabricated by using 2 kinds of bur or 3 kinds of bur. Methods: The crowns were fabricated by using the first molar of the right maxillary in this study. The abutments that were prepared were scanned by using a scanner and designed by using CAD software. Based on the crown design, NC data were created with CAM software. The created NC data were used while fabricating the crown prostheses by using 5-axis milling machine. Scanning was done for the internal and external surface of the completed crown prostheses and 3-dimensional measurement was conducted for precision assessment. Results: The $RMS{\pm}SD$ value for the external surface of the crown prostheses that was fabricated by using two burs and three burs were $28.5{\pm}4.1{\mu}m$ and $19.1{\pm}2.8{\mu}m$, respectively; and the value for two burs were bigger than that for three burs with statistical significance (p<0.001). The $RMS{\pm}SD$ value for the internal surface of the crown prostheses that was fabricated by using two burs and three burs were $14.9{\pm}1.9{\mu}m$ and $13.3{\pm}2.5{\mu}m$, respectively; and the value for two burs were bigger than that for three burs but with no statistical significance. Conclusion: Based on this study, the prostheses that were fabricated by using 3 bur presented better stability compared to those that were fabricated by using 2 bur and statistically significant difference was found only in the external surface.
Purpose: To investigate the correlation between the amount of salivary metal ions released from fixed prostheses and the period of restoration insertion, and to evaluate the correlation between the type and amount of metal ions in saliva and oral lesions (OL). Methods: Based on the oldest prosthesis, we divided patients into two groups: more than 5 years (n=19) and less than 5 years (n=10). Patients were also divided into two groups by another criteria: the one with the presence (n=15), and the one with absence (n=14) of OL, and the amounts of metal ions were examined. Metal ions-gold (Au), copper, cobalt (Co), chromium (Cr), nickel, zinc, aluminum, palladium (Pd), tin, and platinum (Pt)-were measured using laser ablation microprobe inductively coupled plasma mass spectrometry. Results: Significantly higher quantities of Co, Pt, and Pd ions were released in patients with fixed prosthesis of more than 5 years (p<0.05). Measurement of the average amount of salivary metal ions was performed on patients with and without OL - Significantly higher amounts of Cr, Pd and Au ions were released in patients with OL (p<0.05). Conclusions: Old prostheses (${\geq}5years$) released metal ions, and among them were Co, Pt and Pd ions more than others. Patients with OL showed significantly higher levels of ion release, especially Cr, Pd and Au. There was a positive correlation between the amount of a certain kind of released metal ions, especially Pd, and the development of OL. Salivary Pd ion released from old prostheses could affect the pathogenesis of OL; therefore, long-term follow-up is important.
