일차치과의원에서의 조직병리검사 (Histopathologic examination in the primary dental clinic)

  • Biopsy is a critical method for disease diagnosis and treatment planning. It can be applied from simple inflammatory lesions to malignant tumors. But many general dental practitioners are unfamiliar with the basic knowledge and skills required for biopsy. Moreover, biopsy indications and contraindications for certain diseases may differ depending on the type of dental practice environment and the specialty of the dentist. Biopsy education can increase the choices a dentist has during disease diagnosis. Here we will discuss details on biopsy needed to the general dentist.

Influence of reconstruction parameters of micro-computed tomography on the analysis of bone mineral density

  • Purpose: This study was conducted evaluate the influence of reconstruction parameters of micro-computed tomography (micro-CT) images on bone mineral density (BMD) analyses. Materials and Methods: The sample consisted of micro-CT images of the maxillae of 5 Wistar rats, acquired using a SkyScan 1174 unit (Bruker, Kontich, Belgium). Each acquisition was reconstructed following the manufacturer's recommendations(standard protocol; SP) for the application of artifact correction tools(beam hardening correction [BHC], 45%; smoothing filter, degree 2; and ring artifact correction [RAC], level 5). Additionally, images were reconstructed with 36 protocols combining different settings of artifact correction tools (P0 to P35). BMD analysis was performed for each reconstructed image. The BMD values obtained for each protocol were compared to those obtained using the SP through repeated-measures analysis of variance with the Dunnett post hoc test(α=0.05). Results: The BMD values obtained from all protocols that used a BHC of 45% did not significantly differ from those obtained using the SP (P>0.05). The other protocols all yielded significantly different BMD values from the SP(P<0.05). The smoothing and RAC tools did not affect BMD values. Conclusion: BMD values measured on micro-CT images were influenced by the BHC level. Higher levels of BHC induced higher values of BMD.

  • Cutaneous lymphoid hyperplasia (CLH) is a cutaneous pseudolymphoma with a worldwide distribution, equally affecting all races and ethnic groups. Due to its vast array of characteristics, it is most often missed in the differential diagnosis of firm to soft lumps on the head and neck. A systematic approach to the workup and diagnosis along with treatment of such lesions is discussed in this article. A 20-year-old Asian Indian female presented to our Oral and Maxillofacial unit with a lump on the left side of her forehead for 1 month. Local examination revealed a $2.5{\times}3.0cm^2$, well circumscribed swelling over the left para median region that was firm to doughy and non-tender. There was no other significant finding on general examination. Excisional biopsy of the lesion was performed, followed by histopathologic processing. The general etiology, pathogenesis, clinical presentation, differential diagnosis, clinical course, prognosis, treatment, and prevention have been discussed in line with the recent modalities of diagnosis and treatment of CLH. Due to the overlapping clinical and histological characteristics of CLH with many other lesions, it is important to consider this lesion in the differential diagnosis of cutaneous lesions.

디지털 X선 영상을 이용한 치아 와동 컴퓨터 보조 검출 시스템 연구 (A Study of Computer-aided Detection System for Dental Cavity on Digital X-ray Image)

  • Segmentation is one of the first steps in most diagnosis systems for characterization of dental caries in an early stage. The purpose of automatic dental cavity detection system is helping dentist to make more precise diagnosis. We proposed the semi-automatic method for the segmentation of dental caries on digital x-ray images. Based on a manually and roughly selected ROI (Region of Interest), it calculated the contour for the dental cavity. A snake algorithm which is one of active contour models repetitively refined the initial contour and self-examination and correction on the segmentation result. Seven phantom tooth from incisor to molar were made for the evaluation of the developed algorithm. They contained a different form of cavities and each phantom tooth has two dental cavities. From 14 dental cavities, twelve cavities were accurately detected including small cavities. And two cavities were segmented partly. It demonstrates the practical feasibility of the dental lesion detection using Computer-aided Detection (CADe).

Oral Potentially Malignant Disorders among Dental Patients: a Pilot Study in Jordan

  • Background: To determine the prevalence, types, and risk factors of oral potentially malignant disorders (OPMDs) among a group of Arab Jordanian dental patients, and to evaluate their awareness and attitudes toward early diagnosis and treatment. Materials and Methods: A total of 1,041 patients attending a University Hospital for dental care were examined for the presence of OPMDs. Histopathological examination was performed on all cases clinically diagnosed and patients were directly interviewed to evaluate their knowledge and attitudes toward early detection and treatment of oral cancer. Results: The prevalence of OPMDs overall was 2.8%. Lichen planus/lichenoid lesions were the most common lesions (1.8%) followed by leukoplakias (0.48%), chronic hyperplastic candidiosis (0.38%), and erythroplakia (0.096%). Smoking, alcohol, and age (>40 years) were the main identifiable risk factors. Patients with OPMDs displayed a general lack of awareness and negative attitudes towards early diagnosis and treatment. Conclusions: OPMDs among Arab dental patients are relatively uncommon and awareness about oral cancer among Jordanian dental patients is low. Interventions to improve public knowledge about oral cancer and attitudes toward early diagnosis and treatment are urgently indicated.

치과치료에 따른 외래환자의 불안도 수준에 관한 역학적 연구 (An Epidemiologic Study on the Anxiety Level of Dental Ouppatients in Response to Dental Procedure)

  • Fear at the dental treatment is a problem not only for patient but for dentist as well, because the anxious patient often require more item, even for simple procedures. The anxiety level in response to dental procedures was evaluated through a simple questionnaire, Corahs, DAS. 783 female and 790 male outpatients were studied at the Dept. of Oral Diagnosis, PNUH from 1988 to 1989. The obtained results were as follows : 1. The anxiety level in female was significantly higher than that in male, and patients in 10s and 30s showed more anxious than those in other groups. 2. Housewives and pupils revealed higher level of anxiety. 3. The anxiety level in 524 (ICD) was the highest in all diseases, inspite of no difference among various diseases. 4. There was significantly higher level of anxiety in the patient of Orthodontics and Pedodontics than those of other departments. 5. There was no difference in anxiety level between acute and chronic group. 6. Dental phobes were more in female than in male.

A diagnostic dilemma in pediatric osteomyelitis: a case report

  • Infantile osteomyelitis is a rare disease that is infective in nature and may rapidly turn fatal, as the disease is often misdiagnosed due to its varied presenting signs. Early diagnosis may help in avoiding systemic involvement and permanent deformity. The disease presents with signs of orbital involvement, nasal congestion, and emesis, as well as other standard hallmarks of infection. Furthermore, the maxilla is a highly vascular and porous bone and the occurrence of osteomyelitis in an infant maxilla is highly uncommon. In addition, routine blood work is not suggestive of the presence of this disease. Thus, prompt diagnosis of this condition poses a challenge to surgeons due to the confusing array of symptoms combined with the rarity of the disease. One such case of osteomyelitis of the maxilla in a young child is presented. The dilemma encountered by the surgeon during the diagnosis and treatment of the disease is discussed.

  • Selection of potential disease-specific biomarkers from saliva or epithelial tissues through next generation sequencing (NGS)-based protein studies has recently become possible. The early diagnosis of oral squamous cell carcinoma (OSCC) has been difficult, if not impossible, until now due to the lack of an effective OSCC biomarker and efficient molecular validation method. The aim of this study was to summarize the advances in the application of NGS in cancer research and to propose potential proteomic and genomic saliva biomarkers for NGS-based study in OSCC screening and diagnosis programs. We have reviewed four categories including definitions and use of NGS, salivary biomarkers and OSCC, current biomarkers using the NGS-based technique, and potential salivary biomarker candidates in OSCC using NGS.

The influence of different scan resolutions on the detection of proximal caries lesions

  • Purpose: This study was conducted to evaluate the effect of different spatial resolutions of a photostimulable phosphor plate (PSP) radiography system on the detection of proximal caries lesions. Materials and Methods: Forty-five extracted human permanent teeth were radiographed using a PSP system (VistaScan Perio Plus) and scanned at the 4 resolutions (10 lp/mm, 20 lp/mm, 25 lp/mm, and 40 lp/mm) available in the system. Three independent examiners scored the images for the presence and absence of proximal caries lesions using a 5-point scale. The presence or absence of caries was confirmed by histological sections of the examined teeth (defined as the gold standard). Intra- and inter-observer reproducibility was calculated by the weighted kappa test. One-way analysis of variance with the post hoc Tukey test was used to compare the area under the receiver operating characteristic curve for the classifications made with each resolution. Results: For the detection of enamel lesions, the spatial resolution of 10 lp/mm was significantly superior to the other resolutions. However, the spatial resolution did not affect the detection of caries lesions in dentin (P>0.05). Conclusion: Spatial resolution may influence the accuracy of the detection of incipient caries lesions in radiographs with PSP plates. Images with low spatial resolution seem to be more appropriate for this purpose.
