완전 무치악 환자에서 CAD-CAM 기법을 이용한 상악 총의치 및 하악 임플란트 피개의치 수복: 증례 보고 (A case of digital maxillary complete denture and mandibular implant overdenture fabricated by CAD-CAM technique)

  • 컴퓨터 지원 설계 및 제조 기법(CAD-CAM)은 치과 영역에서 급속도로 발전해온 분야로, 다양한 보철 치료 영역에 적용되고 있다. 이 중 절삭형 방식(Subtractive Manufacturing)으로 분류되는 절삭가공 기법을 이용한 의치 제작은 상용화된 디지털 총의치 제작 방식 중 하나이며, 동시에 첨가형 방식(Additive Manufacturing)으로 분류되는 레이저 소결 혹은 용융 기법을 활용하여 피개의치를 위한 금속구조물을 보다 효율적으로 제작할 수 있게 되었다. 본 증례에서는, CAD-CAM을 통해 절삭형 방식으로 제작한 상악 총의치와 3D 금속 프린팅을 이용한 금속구조물과 절삭형 방식으로 제작된 인공치를 접목한 하악 임플란트 피개의치를 제작하였다. 이를 통해 기능적, 심미적으로 적절한 임상적 결과를 얻었으므로 이를 보고하는 바이다.

Mechanical Properties and Microstructure of the Leucite-Reinforced Glass-Ceramics for Dental CAD/CAM

  • The computer-aided design/computer-aided manufacturing (CAD/CAM) system was introduced to shorten the production time of all-ceramic restorations and the number of patient visits. Among these types of ceramic for dental CAD/CAM, they have been processed into inlay, onlay, and crown shapes using leucite-reinforced glass-ceramics to improve strength. The purpose of this study was to observe the mechanical properties and microstructure of leucite-reinforced glass-ceramics for dental CAD/CAM. Two types of leucite-reinforced glass-ceramic blocks (IPS Empress CAD, Rosetta BM) were prepared with diameter of 13 mm and thickness of 1 mm. Biaxial flexural testing was conducted using a piston-on-three-ball method at a crosshead speed of 0.5 mm/min. Weibull statistics were used for the analysis of biaxial flexural strength. Fracture toughness was obtained using an indentation fracture method. Specimens were observed by field emission scanning electron microscopy to examine the microstructure of the leucite crystalline phase after acid etching with 0.5% hydrofluoric acid aqueous solution for 1 minute. The results of strength testing showed that IPS Empress CAD had a mean value of $158.1{\pm}8.6MPa$ and Rosetta BM of $172.3{\pm}8.3MPa$. The fracture toughness results showed that IPS Empress CAD had a mean value of $1.28{\pm}0.19MPa{\cdot}m^{1/2}$ and Rosetta BM of $1.38{\pm}0.12MPa{\cdot}m^{1/2}$. The Rosetta BM sample exhibited higher strength and fracture toughness. Moreover, the crystalline phase size and ratio were increased in the Rosetta BM sample. The above results are expected to elucidate the basic mechanical properties and crystal structure characteristics of IPS Empress CAD and Rosetta BM. Additionally, they will help develop leucite-reinforced glass-ceramic materials for CAD/CAM.

  • 이전 단일 구조 지르코니아는 색조의 단조로움 등 심미성의 문제로 전치부 사용에서 어려움이 있었다. 이에 전치부 증례의 지르코니아는 코핑으로만 주로 이용되어 최종보철물은 추가적인 도재 피개가 필요했었다. 이에 따라 도재 파절 등의 문제가 존재하였으나 점차 단일 구조 지르코니아 자체의 투과도와 색조가 개선되어 도재 피개 없이도 사용이 가능해졌다. 본 증례는 63세 남성환자로 단일 구조 지르코니아를 이용하여 전악 보철 수복을 시행해 기능적으로나 심미적으로 만족스러운 결과를 얻었기에 이를 보고하는 바이다.

The influence of saliva pH value on the retention and durability of bar-clip attachments

  • PURPOSE. The aim of this study was to compare the durability and retention of 4 types of attachments placed over computer aided design/computer aided manufacturing (CAD/CAM) titanium bars when subjected to different pH conditions. MATERIALS AND METHODS. Four commercially available attachments were investigated: Hader Yellow, Hader Red, Ackerman Gold and Ackerman Stainless Steel. These attachments and Ackerman CAD/CAM titanium bars were placed in 2 vessels containing different artificial saliva solutions (pH 7/pH 4) at $37^{\circ}C$ for one month to simulate corrosion conditions, and they were then subjected to mechanical testing (5400 cycles of insertion and removal). RESULTS. The results revealed that there were significant differences in the average values of insertion/removal force due to the pH (F (1, 24)=9.207, P<.05) and the type of attachment (F (3, 24)=11.742, P<.05). CONCLUSION. More acidic pH values were found to have a negative influence on the retention capacity of the attachments.

Endocrown restorations for extensively damaged posterior teeth: clinical performance of three cases

  • The restoration of endodontically treated teeth (ETT) with more than one cusp missing and thin remaining walls is challenging for the general practitioner. The use of posts combined with full coverage restorations is a well-established approach, yet not following the minimal invasive principles of adhesive dentistry. Endocrowns are indirect monoblock restorations that use the pulp chamber of the ETT for retention. In this study the fabrication of 4 endocrowns and their clinical performance will be discussed. Two clinical cases include computer-aided design/computer-aided manufacturing manufactured molar endocrowns (one feldspathic ceramic and one hybrid composite-ceramic restoration) and the other two are dental laboratory manufactured resin composite premolar endocrown restorations. The modified United States Public Health Service criteria were used to assess the clinical behavior of the restorations at different follow up periods. Endocrown restorations present a satisfactory clinical alternative, either by the use of resin composite or glass ceramic and hybrid materials. Specific guidelines with minimal alterations should be followed for an endocrown restoration to be successful. Due to limited evidence regarding the long term evaluation of this restorative technique, a careful selection of cases should be applied.

  • We are currently living in an era where the use of computer-aided design/computer-aided manufacturing has allowed individualized orthodontic treatments, but has also incorporated enhanced digitalized technology that does not permit improvisation. The purpose of this systematic review was to analyze publications that assessed the accuracy and efficiency of the $Invisalign^{(R)}$ system. A systematic review was performed using a search strategy to identify articles that referenced $Invisalign^{(R)}$, which were published between August 2007 and August 2017, and listed in the following databases: MEDLINE, Embase, Cochrane Library, Web of Knowledge, Google Scholar, and LILACS. Additionally, a manual search of clinical trials was performed in scientific journals and other databases. To rate the methodological quality of the articles, a grading system described by the Swedish Council on Technology Assessment in Health Care was used, in combination with the Cochrane tool for risk of bias assessment. We selected 20 articles that met the inclusion criteria and excluded 5 due to excess biases. The level of evidence was high. Although it is possible to treat malocclusions with plastic systems, the results are not as accurate as those achieved by treatment with fixed appliances.

  • 고기능성 폴리머 중 하나인 Polyetherketoneketone (PEKK)는 좋은 생체적합성과 우수한 물성을 가져 치과 영역을 포함한 다양한 분야에서 사용되고 있다. 이러한 PEKK를 프레임워크로 하는 임플란트 지지 고정성 보철물에 관한 성공적인 증례들이 많이 보고되어 왔으나, 보고된 증례에 대한 장기간의 관찰과 합병증에 관한 고찰은 부족한 상태이다. 이에 본 증례보고에서는 PEKK 프레임워크를 composite resin, lithium disilicate crown, high-impact polymethyl methacrylate PMMA로 각각 veneering하여 임플란트 지지 고정성 보철 수복을 한 세 가지 증례와 각각의 6년간의 경과관찰 결과를 소개하고 이를 고찰해 보고하고자 한다.

치과용 복합레진으로 수리된 CAD-CAM hybrid 수복물의 전단결합강도 (Shear bond strength of dental CAD-CAM hybrid restorative materials repaired with composite resin)

  • 목적: 본 연구에서는 치과용 CAD-CAM (computer aided design-computer aided manufacturing) hybrid 수복재료인 LAVA Ultimate와 VITA ENAMIC을 광중합 복합레진을 사용하여 수리할 때 표면처리방법(grinding, air abrasion with aluminum oxide, HF acid)과 접착재료(Adper Single Bond 2, Single Bond Universal)의 종류가 두 재료 사이의 전단결합강도에 어떠한 영향을 미치는지 알아보고자 하였다. 재료 및 방법: LAVA Ultimate와 VITA ENAMIC 시편을 30일간 $37^{\circ}C$의 인공타액(Xerova solution)에 보관하여 시효처리를 실시한 후 각각 SiC paper grinding한 것, grinding 후 air abrasion처리를 추가한 것, grinding 후 HF 처리한 것으로 분류하고 각각 no bonding, Adper Single Bond 2, 또는, Single Bond Universal 도포로 세분하여 9개의 group, 총 18개의 subgroup으로 나누어 실험을 실시하였다(N=10). HF 처리group에서는 도재시편을 대조군으로 추가하였다(N=10). 표면 처리 후 광중합 복합레진(Filtek Z250)을 각각의 시편에 부착하고 이를 1주일간 실온의 물에 침적시켰고 이후 전단결합강도를 측정하고 파절양상 및 표면처리 효과를 SEM으로 확인하였다. One-way ANOVA를 이용하여 group 간의 유의성을 분석하였고 사후 분석으로 Scheffe test를 실시하였다(${\alpha}=.05$). 결과: 실험 결과 접착재료 처리를 한 group들이 접착재료 처리를 하지 않은 group에 비해 모든 표면처리에서 더 높은 전단결합력을 나타내었으며, 표면처리만 시행한 group에서는 aluminum oxide air abrasion이 전단결합력의 증가에 약간의 영향을 미치는 것으로 나타났으나 통계적 유의성은 보이지 않았다. 결론: LAVA Ultimate와 VITA ENAMIC의 두 재료를 광중합 복합레진을 이용하여 수리를 실시할 경우 각각의 재료에 적합한 표면처리방법과 접착재료의 선택에 대한 연구가 더 필요할 것으로 사료된다. 특히 LAVA Ultimate의 경우 접착재료의 사용은 추천된다고 사료되었다.

Marginal fit of anterior 3-unit fixed partial zirconia restorations using different CAD/CAM systems

  • PURPOSE. Few studies have investigated the marginal accuracy of 3-unit zirconia fixed partial dentures (FPDs) fabricated by computer-aided design/computer-aided manufacturing (CAD/CAM) system. The purpose of this study was to compare the marginal fit of zirconia FPDs made using two CAD/CAM systems with that of metal-ceramic FPDs. MATERIALS AND METHODS. Artificial resin maxillary central and lateral incisors were prepared for 3-unit FPDs and fixed in yellow stone. This model was duplicated to epoxy resin die. On the resin die, 15 three-unit FPDs were fabricated per group (45 in total): Group A, zirconia 3-unit FPDs made with the Everest system; Group B, zirconia 3-unit FPDs made with the Lava system; and Group C, metal-ceramic 3-unit FPDs. They were cemented to resin dies with resin cement. After removal of pontic, each retainer was separated and observed under a microscope (Presize 440C). Marginal gaps of experimental groups were analyzed using one-way ANOVA and Duncan test. RESULTS. Mean marginal gaps of 3-unit FPDs were $60.46{\mu}m$ for the Everest group, $78.71{\mu}m$ for the Lava group, and $81.32{\mu}m$ for the metal-ceramic group. The Everest group demonstrated significantly smaller marginal gap than the Lava and the metal-ceramic groups (P<.05). The marginal gap did not significantly differ between the Lava and the metal-ceramic groups (P>.05). CONCLUSION. The marginal gaps of anterior 3-unit zirconia FPD differed according to CAD/CAM systems, but still fell within clinically acceptable ranges compared with conventional metal-ceramic restoration.

Comparative fracture strength analysis of Lava and Digident CAD/CAM zirconia ceramic crowns

  • PURPOSE. All-ceramic crowns are subject to fracture during function. To minimize this common clinical complication, zirconium oxide has been used as the framework for all-ceramic crowns. The aim of this study was to compare the fracture strengths of two computer-aided design/computer-aided manufacturing (CAD/CAM) zirconia crown systems: Lava and Digident. MATERIALS AND METHODS. Twenty Lava CAD/CAM zirconia crowns and twenty Digident CAD/CAM zirconia crowns were fabricated. A metal die was also duplicated from the original prepared tooth for fracture testing. A universal testing machine was used to determine the fracture strength of the crowns. RESULTS. The mean fracture strengths were as follows: $54.9{\pm}15.6$ N for the Lava CAD/CAM zirconia crowns and $87.0{\pm}16.0$ N for the Digident CAD/CAM zirconia crowns. The difference between the mean fracture strengths of the Lava and Digident crowns was statistically significant (P<.001). Lava CAD/CAM zirconia crowns showed a complete fracture of both the veneering porcelain and the core whereas the Digident CAD/CAM zirconia crowns showed fracture only of the veneering porcelain. CONCLUSION. The fracture strengths of CAD/CAM zirconia crowns differ depending on the compatibility of the core material and the veneering porcelain.
